The Iberian Journal of Clinical and Forensic
Neuroscience publish papers on a broad range of topics
of general interest to those working on the
neuroscientific field.
The Journal of Clinical and Forensic
Neuroscience publishes theory-driven patient studies,
studies about forensic field applied to neuroscience as
well as basic studies in the large neuroscience area.
The journal publishes group and case studies addressing
fundamental issues concerning the brain functional
relations with behavior, epidemiology, basic science, etc.
The journal is dedicated to a fast and proficient turn-
around of papers, targeting to complete reviewing in
under 60 days. Submissions should be made in a word
format document (editable) to
(Luis Maia, Editor in Chief), or
Papers will be valued by the Editorial Board and referees
in terms of scientific value, readability, and importance
to a wide-ranging circulation.
Papers should be submitted following APA norms (APA
Publication Manual published by the American
Psychological Association), and preferentially
presented in written English language. Abstracts
should be available in English, Portuguese and Spanish
idiom as well as key words.
The Journal operates in a policy of anonymous peer
review.
Each submission should follow the procedure of sending
by email two documents: a) a Title page with all
information about authors’ affiliations and indications of
the correspondent author; b) a second document,
containing the entire article, WIHTOUT ANY
IDENTIFICATION of the authors, to assure the blind
review process.
After the effective publication of articles, authors assume
that the copyright are totally trespassed to Iberian

Editorial
Strengthening of a new era in Neuroscience in
Portugal and Worldwide
With the launch of this scientific periodic we
intended to be accessible to all readers (scientists or
not) and we are aware that the project proposed is
of great magnificence.
We do it because it is not understandable that in
Portugal (in particular) and in Iberian Peninsula, as
well as even worldwide, there are many reasons to
refer to the initiatives of the Portuguese level
research projects and subsequent disclosure might
abound.
Much less can it be said when mentioning the
private initiative of a group of colleagues in the
public and private sector representation and
without support of a public institution (such as a
university, institute or foundation).
With this number of the Iberian Journal of Clinical
and Forensic Neuroscience we intend to strenght a
new cycle.
We are a humble group, supported by a more
humble editor (Edicts Prometáicos), however we
realize our value. What we want to assign to us, and
at the same time, how much we can work with and
for you, demonstrate captivating our colleagues and
friends ... even more.... We are so humble and
small, but at the same time as small as what our
eyes can reach: the infinity!

Abstract
This is study about the application of an Integrated Therapeutic Program based on natural Herbal &
alternative and Complementary medicine for the overall improvement of the healing process in patients with
Anterior Circulate Ligament Injury (ACLI), in which we did a qualitative analysis of the efficiency of this
therapeutic program by comparing the results of ACLI Quality of Life Questionnaire, Brief Pain Inventory,
Squad Test and Standing Stork Test, applied to patients with ACLI that attended the main hospitals and private
physiotherapy clinics in Cape Verde and the patients who received the treatment by the proposed therapeutic
program.
Some of the results that most stood out in the research were the significant difference between the
measurements as well as a better improvement in the results of the experimental group than control group.
Also was no significant difference between the experimental group and their teammates without ACLI.
Keywords: Anterior Circulate Ligament Injury, Brief Pain Inventory, ACL quality of Life Questionnaire, Squats

Resumo
Este é um estudo sobre a aplicação de um programa terapêutico integrado baseado em medicinas naturais e
complementares para a melhoria global do processo de cura em pacientes com lesão do ligamento anterior
cruzado (LLAC), no qual fizemos uma análise qualitativa da eficiência deste programa terapêutico,
comparando os resultados do questionário da qualidade de vida da lesão do ligamento anterior cruzado,
inventário breve de dor , teste de agachamento e teste de cegonha em pé, aplicada a pacientes com LLAC que
frequentaram os principais hospitais e clínicas de fisioterapia privadas em Cabo Verde e os pacientes que
receberam o tratamento pelo programa terapêutico proposto.
Alguns dos resultados que mais se destacaram na pesquisa foram a diferença significativa entre as medições,
bem como uma melhoria nos resultados do grupo experimental em relação ao grupo controle. Também foi a
diferença significativa entre o grupo experimental e seus companheiros de equipa sem LLAC.
Palavras-chave: Lesão do ligamento anterior cruzado, inventário breve de dor, questionário da qualidade de
vida da lesão do ligamento anterior cruzado, teste de agachamento e teste de cegonha em pé.
Resumen
Se trata de un estudio sobre la aplicación de un programa de tratamiento integral basado en la medicina
natural y complementaria a la mejora global del proceso de curación en pacientes con lesión del ligamento
cruzado anterior (Llac), en el que se realizó un análisis cualitativo de la eficacia de este programa terapéutico,
la comparación de los resultados del cuestionario de calidad de vida de la lesión del ligamento cruzado
anterior, breve inventario del dolor, la prueba y la prueba de la cigüeña de pie en cuclillas aplicado a pacientes
con LLAC que asistieron los principales hospitales y clínicas de fisioterapia privada en Cabo Verde y los
pacientes que fueron tratados por el programa terapéutico propuesto.
Algunos de los resultados que se destacaron en la encuesta fueron la diferencia significativa entre las
mediciones, así como una mejora en los resultados del grupo experimental en comparación con el grupo
control. También había una diferencia significativa entre el grupo experimental y sus compañeros de equipo
no Llac.
Palabras clave: Lesión del ligamento cruzado anterior, breve inventario de la calidad del cuestionario de dolor

Introduction
Scientific and technical advances have among
its objectives the preservation of life and
human health, as well as finding various
means to these ends is always present.
Among the means to achieve these
purposes, the Physical activity with
therapeutic purposes and Herbal & Natural
Alternative and Complementary (HNAC) won
at present a great therapeutic importance.
Despite the enormous scientific advances in the
medical field, science has shown us that
there is much to know and learn about the
human body and its regenerative ability and
healing. Every day appear new cures and
new approaches about the rehabilitation
process of certain diseases.
Contrary to the methods commonly used in
most physical therapy clinics in Cape Verde
for treating Anterior Circulate Ligament
Injury (physical agents such as ultrasound,
Therapeutic Laser and Physical Exercises and
the use of drugs), this research aims to
present an Integrator Therapeutic Program
based in HNAC, as is the use of acupuncture,
seat baths, 5 Tibetans and exercise with
therapeutic purposes for the overall
improvement of the healing process in
patients with Anterior Circulate Ligament
Injury (ACLI) in Cape Verde.
The anterior circulate ligament (ACL), which has
a huge importance in stabilizing the knee
joint, is often injured, both athletes and non-
athletes, with the consequent functional
disability and pain (Cirstoiu, C., et al. 2011;
Farshad, M., et al. 2011). The morbidity is
wide spectrum of injury may, in an extreme
situation, causing the early termination of
sports careers, but on the other hand,
surgical reconstruction and rehabilitation
can be so effective that allow the athlete to
return to the level of competition pre- lesion
(Tjoumakaris, F. P., et al. 2012). This bonding
is important not only in preventing the
previous trans-location and internal rotation
of the tibia but also provides rotational
stability to the joint (Silvers, H. J. and B. R.
Mandelbaum 2007; Cimino, F., et al. 2010).
The evaluation of the lesion should be made
as early as possible and the diagnosis is
based on symptoms (pain, swelling and
sensation of unsteadiness), physical
examination (Lachman test, anterior drawer,
pivot shift and Jerk or pivot-shift reverse)
imaging and any exploratory arthroscopy
(Cirstoiu, C., et al. 2011; Cimino, F., et al.
2010). The inability of the patient perform
hyper-extension and the description of a
"click" at the moment of injury are two
characteristic symptoms (Cimino, F., et al.
2010). A physical examination performed

correctly has a sensitivity of 82% and
specificity of 94% (Cimino, F., et al. 2010).
The lesions may result from contact or non-
traumatic injury situations, the latter being
the most frequent mechanism, with an
overall incidence of 70-80% (Shimokochi, Y.
and S. J. Shultz 2008; Ageberg, E. and T.
Friden 2008). The non-contact injuries result
from sudden downturns, changes in the
direction of movement, disruption of
rotational movements and support after a
jump, increasing the risk if the foot is in
closed-loop position (Cirstoiu, C., et al. 2011;
Yoo, J. H., et al. 2010).
During such movements, anterior forces in tibial
and internal / external rotation, the valgus
position of the knee, hip and knee in full or
nearly full extension and contraction of the
quadriceps muscle can cause large tension
level ACL (Shimokochi, Y. and S. J. Shultz
2008; Zhang, Y., et al. 2011). The ACL contact
injuries are determined by the knee position
and the nature of the external force can not
be the target of preventive exercises except
the avoidance of risk situations (Yoo, J. H., et
al. 2010).
In ACLI, the sensory system is disturbed with
loss of mechanic-receptors and decrease of
the afferent feedback, resulting in defects in
neuromuscular function of the thigh muscles
(Ageberg, E. and T. Friden 2008; Konishi, Y.
and T. Fukubayashi 2010). After unilateral
lesion, both knees are affected due to
physical inactivity, changes in gait deficits in
sensory feedback and modification of central
motor programs (Ageberg, E. and T. Friden
2008). The instability of the knee can result
in a changed position of this articulation in
relation to the hip and the foot, resulting in
postural changes, not only in the knee as
well as the adjacent joints (Trulsson, A., et al.
2010). Briefly, ACLI results in atrophy and
muscle weakness (especially at the femoral
quadriceps and calf muscle triceps), poor
knee function (with decreased physical
activity and increased risk of osteoarthritis)
and a consequent decrease in the quality of
life and short-long-term (Frobell, R. B., et al.
2010; Hasegawa, S., et al. 2011).
Following the back pain, knee injuries are the
most common neuromuscular problems at
the level of primary health care, with a
prevalence of 48 per 1,000 patients / year
(van Grinsven, S., et al. 2010). About 66% of

patients with complete ACL rupture have
meniscus cartilage-associated lesions
(Silvers, H. J. and B. R. Mandelbaum 2007).
Most injuries occur during sports activities,
especially in individuals between 15 and 25
years of age, with an incidence 4-6 times
higher in female athletes, but with a larger
absolute number of injured male athletes
(van Grinsven, S., et al. 2010; Barrett, A. M.,
et al. 2011). The inter-gender difference is
due to anatomical, environmental factors,
hormonal, bio-mechanical and
neuromuscular, and only the latter can be
influenced by the application of preventive
programs (Silvers, H. J. and B. R.
Mandelbaum 2007; Yoo, J. H., et al. 2010).
On average, an athlete is between 6-9
months out of competition, and half of the
cases result in premature end of sporting
career and in 36% there is a significant
reduction in activity level (Silvers, H. J. and B.
R. Mandelbaum 2007; Kvist, J., et al. 2005).
Osteoarthritis of the knee occurs ten times
more in cases with a history of ACLI,
occurring in about 90% of these situations, it
is not preventable through ACL
reconstruction (Yoo, J. H., et al. 2010;
Mihelic, R., et al. 2011; Micheo, W., et al.
2010). The time between the injury and the
reconstruction, the patient's age and the
concomitant injury to the meniscus are risk
factors for the development of osteoarthritis
(Mihelic, R., et al. 2011). Cartilage
degenerative changes manifest themselves,
in average 7 years after injury (Mihelic, R., et
al. 2011).
Prevention is a key component in reducing the
impact of ACLI in athletes, particularly
women, have an important role in different
variables, as it reduces the costs for the
treatment, the absence of competition time
and the residual disability (Yoo, J. H., et al.
2010). If prevention programs do not
provide objective improvements in sports
performance, athletes are not usually
motivated to integrate.
Treatment of a patient with ACLI may be
surgical or non-surgical, and a decision is
made according to the patient's age, the
activity level of the individual and the
preference of the patient, although not the
exact criteria are defined for that decision
(Cook, C., et al. 2008; Thomee, P., et al.
2008). When the patient is a young athletes,
as a rule, surgical reconstruction method is

chosen in order to restore knee kinematics,
reduce the risk of further injury as well as
the progression of degenerative lesions
(Legnani, C., et al. 2011).
Social reasons and psychological obstacles, such
as fear of re-injury and pain, decisively
influence the return to activity, is common,
despite the objective recovery of joint
stability, a lack of confidence in the injured
knee and the fear of re-injury (van Grinsven,
S., et al. 2010; Kvist, J., et al. 2005). Patients
who use negative strategies for dealing with
pain and with the worst living conditions
have shown the worst results in terms of
surgery and rehabilitation (Thomee, P., et al.
2007). In this context, self-determination is
crucial to the success of recovery, being
decisive in the effort expended, the
persistence in the face of difficulties, the
nature of patterns of thinking and emotional
reactions (Peter Kelder, 1999). It is necessary
to take into account the patient's
expectations and adjust them to reality, with
particular emphasis young people, because
usually have overly high expectations
(Mascarenhas, R., et al. 2012).
Treatment of ACLI, as indeed other lesions in
Cape Verde, takes place generally in the
base of conventional medicine, it naturally
without taking into account the so-called
traditional medicine, connected to popular
tradition of curing methods "homemade"
and unconventional still very practiced in
rural areas and has been advised against, as
a rule.
Materials and methods:
Our sample consists of 26 patients in both sexes
(13 Male and 13 Female) that are part of our
experimental group and 52 patients who are
part of our control group. The 52 patients
who are part of our control group are from
both sexes and are divided into 4 categories
(1st = Male Control Group; 2nd = Female
Control Group; 3rd = Male Teammates of the
Experimental Group and 4th = Female
Teammates of the Experimental Group). The
sample is also divided into two age
categories (1st from 18 to 25 and 2nd from
26 to 35 years old)
Were included in this research every athletes
patient (that are part of any sports team in

Cape Verde), with ACL Injury that began to
attend physiotherapy treatments from 3 of
the main Hospitals and 17 of the main
private clinics in Cape Verde that started in
November 2014 and continued until April
2015 at least. Were considered an exclusion
criteria, patients who had any kind of heart
problems, any type of malignant tumor, as
well as patients over the age of 35 years or
who were not athletes
To know whether the treatment was effective
and to see if there was significant
differences between the two therapeutic
models adopted, was applied in two
moments (1st-3 weeks after they started the
treatments and 2nd after 5 months of
treatment) to the experimental group (those
who did the treatment for ACL Injury
through therapeutic program proposed by
this research) and control group (patients
with ACL Injury that made treatment in the
main hospitals and private clinics from Cape
Verde) the ACL Quality of Life Questionnaire,
the Brief Pain Inventory, the Squad Test and
the Standing Stork Test. By the other hand,
once our experimental group was composed

It is a descriptive analysis in which used

For the Standing Stork Test, having the variables
"1st Male Standing Stork Test (SST1M)" and

and the Standing Stork Test to some of team-
mates of our experimental group the criteria to
establish the significance was identical to the
above mentioned except that in this case we
used only data obtained in seconds tests
applied to the experimental group and the data
obtained in a single test done to team-mates. In
this investigation we will take into account for
the Squad Test only the Good and Excellent
cases and for the Standing Stork Test only the
Above Average and Excellent cases Table VII)

Regarding ACL Quality of Life Questionnaire,

Results and discussion:
As regards the Brief Pain Inventory to find out
the level of Pain Severity, we calculated by
adding the scores for questions 2, 3, 4 and 5
and then dividing by 4. This gives the severity
score out of 10. And to know the level of Pain
Interference we calculated by adding the scores
for questions 8a, b, c, d, e, f and g and then
dividing by 7. This gives an interference score
out of 10. (Table IX and X).

Such as we can observe in table VI in both tests
and in both sexes the treatment effect was
better in the Experimental group. And it is to be
noted that all patients in the Experimental
Group reached in the Standing Stork Test
results of Good Effect or Excellent Effect. The
same didn't happen in the Control Group case,
because from the 13 Female only 6 reached
Good Effect and none has achieved Excellent
Effect. With regard to 13 Males from the
Control Group only 4 reached Good Effect and 4
Excellent Effect.
The Male A Squats Test (refers to results
achieved by male groups from both the
Experimental Group and the Control Group in
patients with age between 18-25 years of age),
5 patients in the Experimental Group 4 had
Extremely Effect and 1 Very Good Effect while
any element of Control Group found in the
categories mentioned.
On Male B Squats Test (refers to results
achieved by both male groups of the
Experimental Group and the Control Group in
patients with age between 26-35 years old), of
8 patients in the Experimental Group 3 had
Extremely Effect and 3 Very Good Effect as that
of the 8 patients in the Control Group nobody
obtained Extremely Effect and 3 obtained Very
Good Effect.
In the Female A Squats Test (refers to results
achieved by both the Experimental Group and
the Control Group in patients with age between
18-25 years old), it can be concluded that both
groups improved considerably their results
although there isn't a great difference between
them. Once, Of 6 Experimental Group patients,
4 had Extremely Effect and 2 Very Good Effect
as that of the 5 patients in the Control Group 4
achieved Extremely Effect and 1 obtained Very
Good Effect.
The Female B Squats Test (refers to results
achieved by both the Experimental Group and
the Control Group in patients with age between
26-35 years old), it can be concluded that the
Females of the Experimental Group all 7 are in
the categories mentioned having 1 achieved
Extremely Effect and 6 Very Good Effect, while
that of the 8 patients in the Control Group
nobody obtained Extremely Effect and 6 have
obtained Very Good Effect.
In the table VII, when comparing the result
reached in the Standing Stork Test and Squats
Test between the Experimental Group and their
Teammates, we noticed that there was a

difference between the two groups, but we
don't consider to be significant because both
were between Good and Excellent, just as there
is a balance between the categories. Which
leads us to assume that the treatment applied
to our experimental group had positive effect
In the table VIII and IX, in the ACL Quality of Life
Questionnaire in both groups virtually there
was an improvement in quality of life after both
therapeutic models adopted. Since in the vast
majority of the questions there were cases of
Very Significant and Extremely Significant.
Although both groups have improved while
quality of life reflected in the questionnaire, we
can also note that the values of significance in
the Experimental Group are superior to the
Control Group. In the case of questions 25, 27
and 33 of this questionnaire did not show
significant improvements since it is groups of
athletes who have received psychological
counselling from the first moment. Being no
evident differences between the two moments
of the application questionnaire.
Table IX and X, the Brief Pain Inventory values
show that there was a considerable
improvement as the manifestation of pain in
both groups since the first moment of
application of the questionnaire until the
second time. Taking the Experimental group
better improvement performance in minimizing
the effects of pain. Which reinforces the
position that the proposed program influences
positively in the healing process of patients with
Previous Circulate Ligament Injury attending the
Hospitals and Private Physiotherapy clinics in
Cape Verde.
Table XI and XII, shows us the frequency of ACL
injury in major private therapy clinics and
hospitals and in Cape Verde, their form of
treatment and diagnosis. As we can observe in
Cape Verde the ACL injury is a common injury
and the main methods of diagnosis are Drawer
test, Lachman test and Pivot Shift Test.
Even though Radiography and Magnetic
Resonance because are efficient as a diagnosis
method, they are very expensive and they can
be only found in hospitals. In the case of
Magnetic Resonance only one clinic has it. The
questionnaire revealed that not all the
institutions knew and applied the therapeutic
methods used in this investigation and that not
everyone who knew these methods they knew
where they were.

Perspective and acknowledgements:
High incidence of ACLI and consequent
morbidity associated, as well as the absence of
consensus on the approach to this problem,
justified the preparation of this updated
literature review in order to systematize various
aspects related to the prevention and
rehabilitation of ACLI.
There is in Cape Verde any integrator program
of HNAC, nor is it a widespread recognition of
their reason for being.
It turns out, however, that despite the
enormous efforts and Cape Verde advances in
the field of physiotherapy unfortunately
patients with this injury are required in many
cases, to carry out the treatment (surgery and
physiotherapy) abroad.
Taking into account the clinical importance of
this injury, the uncertainty as to the best
approach to it and the lack of any integrator
program in Cape Verde of HNAC, it is justified to
make an literature updated and review of this
subject, as well as create and apply a
therapeutic program based on HNAC for the
rehabilitation of ACLI, enabling clinician's to a
global and systematized comprehension.
Conclusions:
The program based on Herbal & natural
alternative and Complementary
Medicine is characterized by a focus on
the improvement of healing process in
patients with Anterior Circulate
Ligament Injury attending the Hospitals
and Private Physiotherapy clinics in
Cape Verde.
Indicators assessing the physical
condition of the patients studied
improved clearly, without complications
arose in the patients in the study, which
confirms the accuracy of the stated
hypothesis.
Recommendations:
After analyzing the results obtained in the
research process as well as the conclusions at
which we have arrived in a general way, we
submit for consideration the following
recommendations:
Propose to the national group of
therapeutic Areas the possibility of
including the proposed program in the

rehabilitators, working in the different
centers and areas where they deal with
ACL injuries, with the goal to learn how
to work with the program.
Disseminate the results obtained in this
research so that they are known by
people who suffer from this injury and
other interested.

The impact of tics disorder in the development of a child

Abstract
In this paper, we present a clinical case of a 10-year-old boy with a chronic tic disorder, and
symptoms of excessive worry and repetitive behaviours, that lead us to think in the existence of
comorbid diagnoses of generalized anxiety disorder and obsessive-compulsive disorder. We reflect
on the impact of having a tic disorder in the child development, and the intervention needed to
promote well-being and adjustment. We also reflect on the task that clinicians have in
differentiating the normal, transient and developmentally appropriate expression of fear, worry,
and repetitive behaviours, from other pathological symptoms. Within the principles of cognitive
behavioural therapy and narrative therapy, we will present a proposal of intervention that aims to
psycho-educate about tics and emotions; help to relieve symptoms or achieve symptom control;
develop strategies to cope with stress and anxiety; enhance the child sense of competence, and
help parents deal with the children symptoms, promoting his development.

Resumo
Neste artigo, apresentamos um caso clínico de um menino de 10 anos de idade, com um tique
nervoso crónico e sintomas de preocupação excessiva com comportamentos repetitivos, que nos
levam a pensar na existência de diagnóstico de comorbidade de transtorno de ansiedade
generalizada e perturbação obsessivo compulsivo. Refletimos sobre o impacto de ter um tique
nervoso no desenvolvimento da criança, bem como a intervenção necessária para promover o

bem-estar e de ajuste. Refletimos também sobre a tarefa que os médicos têm em diferenciar a
expressão normal, transitória e adequadas ao desenvolvimento de medo, preocupação e
comportamentos repetitivos, de outros sintomas patológicos. Dentro dos princípios da terapia
cognitivo-comportamental e terapia narrativa, iremos apresentar uma proposta de intervenção
que visa psico-educar sobre tiques e emoções; ajudar a aliviar os sintomas ou conseguir o controlo
dos sintomas; desenvolver estratégias para lidar com o estresse e ansiedade; aumentar a sensação
de criança da sua competência, e ajudar os pais a lidar com os sintomas da criança, promovendo o
seu desenvolvimento.
Palavras-chave: desordem motora crônica - tique, ansiedade, preocupação, comportamento

Resumen
En este trabajo, se presenta un caso clínico de un niño de 10 años de edad con un trastorno de tic
crónico y síntomas de preocupación excesiva y comportamientos repetitivos, que nos llevan a
pensar en la existencia de diagnósticos comórbidos del trastorno de ansiedad generalizada y
desorden obsesivo compulsivo. Se reflexiona sobre el impacto de tener un trastorno de tic en el
desarrollo del niño, y la intervención necesaria para promover el bienestar y el ajuste. También se
reflexiona sobre la tarea que tienen los médicos en la diferenciación de la expresión normal,
transitorio y apropiado para el desarrollo del miedo, preocupación, y conductas repetitivas, de
otros síntomas patológicos. Dentro de los principios de la terapia cognitivo-conductual y la terapia
narrativa, vamos a presentar una propuesta de intervención que tiene como objetivo psico-educar
acerca de los tics y las emociones; ayudar a aliviar los síntomas o lograr el control de síntomas;
desarrollar estrategias para lidiar con el estrés y la ansiedad; mejorar el sentido infantil de la
competencia, y ayudar a los padres a tratar con los síntomas de los niños, y la promoción de su
desarrollo.

Introduction
Tics are stereotyped, rapid, recurring motor
movements and/or vocalizations that are
nonrhythmic, involuntary, and sudden in onset
(Coffey & Shechter, 2005; Hoekstra, Lundervold, Lie,
Gillberg, & Plessen, 2013). They occur frequently in
childhood and adolescence, especially from the
ages of 6 to 14 years old. Children with tics often
experience accompanying problems that may have
more impact on their well-being and quality of life
than the tics itself. The presence of tics is associated
with a range of internalizing and externalizing
difficulties, as well as problems in peer
relationships. It has been found the existence of
comorbidity between tic disorders, that are
neuropsychiatric disorders, with other psychiatric
disorders, as generalized anxiety and obsessive-
compulsive disorder, between others (Cardona,
Romano, Bollea, & Chiarotti, 2004; Hoekstra et al.,
2013). .
Anxiety disorders are common among
children with a prevalence of 4% and 20%. The
presence of anxiety disorders in children is a strong
predictor of anxiety disorders in adulthood
(Peterson & Wainer, 2011)], thus needing to be
carefully diagnosed and addressed. Therefore it’s
important to differentiate the normal, transient and
developmentally appropriate expression of certain
symptoms from pathological ones. For instance,
during middle childhood, fear, anxiety and ritualistic
behaviours are frequent experiences. They are
particularly intense, decreasing most rapidly
between 7 to 10 years of age. However, sometimes
they become more intense, impairing the normal
functioning of the child.
We will present a study case of a 10 year old
boy, that has a chronic motor tic disorder, with
other comorbidity symptoms, as exc
Excessive worrying and ritualistic behaviours,
that lead us to think in the possible comorbidity
with generalized anxiety and obsessive compulsive
disorder, or in the possible early onset of those.
Also we will reflect on the need for an early
diagnose and intervention, considering the course
of some disorders, that cause impairment in the
daily functioning, in mental health and child
development, giving possible strategies to better
intervene within this set of symptoms. The
therapeutic intervention proposed is based in the
cognitive-behavioural and narrative therapy
principles, aimed at helping him cope with the
symptoms presented and to build a new and more
adaptive narrative towards the problems faced. We
will underline the stories that the child has been
saying to himself, before and during the therapeutic
process, which has an influence in the way he sees,
remembers, and faces the future.
This is the study case of Alan, who in the
early beginning of the evaluation process stated
needing help to achieve his three big wishes: “I
have a wish to not have tics, stop worrying and
never die”.

Alan’s history
Alan is a 10-year-old boy, shy, quiet and a
good student. Lives with both his parents and his
younger sister.
Alan’s parents seek for psychological
counselling due to several difficulties, namely lack
of motivation towards school and other activities,
an increase of tics presented as well of anxiety
symptoms, like worries, and also the beginning of
night rituals. These difficulties appeared in the last
four months. Parents also state that Alan doesn't
have the confidence to defend himself, being
described as an insecure child.
He participates in several activities / groups.
Frequently he assumes a leader position, given by
the teachers, due to his calm temperament.
However, he feels overwhelmed in fulfilling this
position.
He didn’t present any particular problems in
his development. Parents only reported the
presence of tics since he was 7 years old. These tics
often appear and disappear, changing throughout
time. Also he has difficulties on sleeping when
anxious. And since 3 years ago, he sleeps with his
father, while his sister sleeps with the mother.
From the analysis of family history, anxiety is
a common feature in Alan’s mother and her
brother, who performs ritualistic behaviours since
childhood.
Assessment and case conceptualization
A comprehensive, detailed history was
essential to assess Alan’s psychological well-being
and adjustment, and to better understand the
symptoms presented, as well the application of
psychological diagnostic tests among other
exercises and observation.
We collected information from Alan’s parent’s
though semi-structured interviews and also we got
in touch with Alan’s teacher. The following
instruments were applied: the Child Behaviour
Checklist (CBCL 4-18) for ages 4-18
(Achenbach,1991), a parent-report questionnaire
on which the child is rated on various behavioural
and emotional problems; the Coloured Progressive
Matrices, CPM, (Raven, Raven, & Court, 2009), an
instrument used to assess non-verbal intelligence
and especially logical reasoning; the Family
Drawing Test of Corman (1985), used to address
the perception of the child about family
relationships; and Roberts-2 (Roberts & Gruder,
2005), providing a measure of the child social
understanding, as expressed in free narrative,
reflecting both developmental and clinical concerns.
The results of Alan’s CBCL 4-18 were not
clinically significant, although presenting some
elevations in the internalization scales, as well in
the thought problems scale. Parents stated that he
can’t get his mind off certain thoughts / obsessions,
given as an example “when he gets nervous he
cannot stop thinking”; and also “he repeats certain

acts over and over / compulsions” (i.e. “he has to do
some sequence of gestures at night, like a ritual”).
The higher values achieved in the Coloured
Progressive Matrices showed that Alan has very
good abilities in logical reasoning. In the Corman
Family drawing test, he doesn’t draw himself, only
his sister and parents. The mother is the character
more invested, both in the drawing and in the
interview. He identifies himself with the father,
described as the saddest, because “he thinks his
daughter doesn’t like him, since she is always with
the mother”. The sister is negatively invested due to
her bad behaviour.
Alan’s profile in Roberts-2 was not clinical,
although with elevations in the clinical scales of
anxiety, depression and rejection. Nonetheless, he
has also good adaptive scales that seem to be
helping him cope with these emotions. He has high
abilities in identifying problems, a good indicator of
his cognitive development that is congruent with
the values in CPM. He seems being able to ask for
help. He revels some assertiveness and auto-
affirmation. He has also the ability to find age
appropriate solutions for the problems, exception
made to a situation related to the fear of dying.
Alan’s narratives are creative. He had no
problem projecting himself. In the narratives he
tries different scenarios until finding a solution. The
narrative themes are always motives of anxiety /
worries, like losing his mother; his parents getting
separated; academic worries like not understanding
something or losing an academic year; colleagues
stop liking him, being poor; and not corresponding
to the expectations of authoritarian figures,
characteristics that are congruent with the high
values achieved in the limit setting scale.
Other exercises as drawings, games and
written tasks were used to better establish a
therapeutic relationship and understand his worries
and other key aspects about his socio-emotional
development. He showed having a stable
behaviour, attention, and collaboration in the
sessions, verbal understanding and expression. He
is very perfectionist and rigorous in his work (i.e.
drawings) and self-cautious. He doesn’t present
difficulties in identifying and acknowledging
emotions. While expressing emotions and thoughts
the tics usually arise. He cannot control touching his
face and moving his neck and other superior
muscles members. In other activities the tics seem
to disappear. He says that when he is anxious he
feels a bigger tension in his body (neck,
shoulders, back, legs, foot, and wrists).
He states having an intense worry and fear
about dying. He has also other fears that he liked to
manage such as sleeping alone, losing someone,
friends making fun of him or disliking him, not
having good grades or not knowing the answer to a
question.
The three big wishes that Alan has are: stop
having tics, stop worrying about things and never
die.

Analysis and description of the problem
Alan presents motor tics since he is 7 years
old. Those tics involve multiple muscle groups (like
shoulder shrugging, head turning, touching his face,
moving other superior members) and also involve
other movements with a purpose, like touching
objects. Last year, in school, he had tics as knocking
things, books, his own head and also spitting,
especially when facing evaluating situations. In the
current year they had disappear on this context.
Other tics are still present at home, even when he is
relaxed. These tics get intensify in stressful
situations (i.e. new situations, moments of
evaluation, or while expressing his thoughts or
emotions). Sometimes Alan tends to avoid these
new situations, feeling insecure (i.e. situations like
meeting new people, starting new activities, sharing
his opinion and asking questions).
Parents often call Alan’s attention for
stopping the tics. Alan can hold off them for a short
period of time. However, he only feels relief when
he carries out these movements, thus being
described as a response to an inner urge. Alan
wishes the tics to stop, especially because of his
colleagues. He is afraid that they start making fun of
him, not respecting him. This fear is intensified
since Alan is anticipating going to a new and bigger
school.
Since four months ago, he started feeling
demotivated towards school, decreasing grades,
being more worried and showing more irritable
behaviour than usual, especially with the parents
and sister. Also he started presenting repetitive
behaviours, carried out in a rigid and compulsive-
like manner (rituals). Accordingly to the parents “he
does some sequence of gestures at night, as a ritual,
before sleeping. He has a particular way of dressing
his pyjama, he has to wash his teeth more than one
time, to reassure that they are well washed; he
knocks with his hand in the furniture and opens and
closes plenty times the doors of the wardrobe. After
that he gives a goodnight kiss to his mother,
knocking her two times in the arm. He does the
same with his sister, and also to his father, but with
him, he says he only needs to knock one time”.
When he does this ritual he feels calmer, whereas
when he doesn't do that he starts to feel agitated,
needing to perform that sequence again. Exception
was made to one time he was able to sleep away
from the parents, and managed to cope with the
distress felt.
Alan’s symptoms in a developmental
perspective
Alan, as a 10-year-old boy, started crossing
the bridge between childhood and adolescence.
This period is packed with physical, cognitive, social,
emotional and moral changes. Logical thinking and
reasoning is a hallmark on this period (Muris,
Merckelbach, Meesters, & Brand, 2002).. Alan has
very good abilities in these domains, and appears to
have a big sense of justice and a strict moral code,
showed also in the narratives created: "The boy
went home saying that he had won a trophy (...)
Then he felt so guilty. He told to the juries the truth

and then they took the trophy away from him (...).
He was very sad, but he knew he was doing the right
thing” (Alan’s Roberts-2).
In this developmental stage the world starts
to be seen as a more complex place. Changing
demands at school sometimes show up some of the
child weaknesses. Until this year Alan perceived
himself as academically competent. However he
started having difficulties in addressing all the
compelling demands of the present scholar year. As
soon as he started having these difficulties, the
anxiety and the worries increased, negatively
influencing his self-image and self-efficacy. This is
well expressed in Alan’s narrative in Roberts-2 test:
“Once upon a time, there was a boy who loved
school and was very concerned about it. He never
had bad grades (...) But one day he could not
manage to bring the homework done. The teacher
got very upset, because he usually was the best
student.”
In this period the social relationships become
also more challenging and more competitive. Alan
struggles with the fear of others make fun of him,
due to his tics.
Since self-image is a learning product,
childhood influences are very important. We
continually take in information and evaluate
ourselves in multiple domains such as physical
appearance, performance, and relationships. For
instance, parents are a mirror that reflects back an
image of us. Also our experiences with others, like
teachers, friends, and other family members, add
more images to that mirror. Therefore,
relationships reinforce what we think and feel
about ourselves. With a positive self-image, we
recognize our assets and potentials. With a negative
self-image, we focus on our faults and weaknesses.
So it’s important to see what reflection is Alan
getting from adults, family and friends, tic related
and otherwise. Also it seems important to help him
preserve his self-image and self-esteem. What he
thinks about himself affects how he feels and
interacts with others and the world. Alan during the
last months was feeling less competent in school;
physically impaired by tics; and sometimes feeling
that colleagues do not respect him.
Throughout this developmental stage it’s
expected that children wish to do things more
independently. Nevertheless they face the fear of
not being able to do it. Thus it is normal the
tendency to check out situations, as a way to
make sure they are safe. So during middle
childhood, fears, worries and ritualistic behaviour
are particularly intense, decreasing most rapidly
between 7 to 10 years of age (Laing, Fernyhough,
Turner, & Freeston, 2009; Muris, Merckelbach,
Gadet, & Moulaert, 2000).
Fears are concerns regarding danger or threat
to survival. While fears with imaginary themes as
ghosts and monsters prevail in early childhood,
realistic fears involving bodily injury / physical
danger increase with age, culminating in fears of
social and medical situations (Laing, et al., 2009). In

Alan the fears are about death, night and other
social fears.
Worrying, is much more future orientated,
anticipating threats with the purpose of preparing
for those threats (Verstraeten, Bijttebier, Vasey, &
Raes, 2011). As children mature, the worries are
increasingly overshadowed by competency and
social evaluative concerns. The most common
worries (harm befalling a loved one, health, social
relations, and school performance) are apparent
throughout childhood and adolescence (Laing, et
al., 2009). Alan seems to have similar worries. The
next narrative gives an example of one of those
worries. “Once there was a family who had lost the
mother. After the mother died the father of the boys
started selling fabrics, but the father was not able to
sell them, and they were poor. But one day the
youngest son (...) started selling fabrics to people
that he found in a party, thus coming back home
with lots of money. Then the boy told his father
what he had done and the father was very pleased”.

The worries could have an adaptive
function and resemble problem solving, directed at
avoiding or coping with negative outcomes.
However sometimes worrying thoughts can become
excessive, with the danger constantly being
rehearsed (Szabó & Lovibond, 2004, Verstraeten et
al., 2011).
The worrisome thoughts become
prominent in children after age 7 because after this
period the understanding of multiple possibilities
increases and children can consider large number of
possibilities via deductive reasoning (Muris et al.,
2000; 2002). This can be linked to Piaget´s Theory of
cognitive development. At age 7, children enter the
concrete operational stage. They are now able to
decentration. They can focus their attention on
several attributes of an object or event
simultaneously and understand the relationships
among dimensions or attributes (Muris, et al., 2002,
Szabó & Lovibond, 2004). Because of Alan’s ability
to reason about future possibilities, to consider
multiple outcomes, and to elaborate potential
negative consequences, worrying thoughts
increased. In fact, in Alan’s Roberts-2 test he
rehearses possible aversive events and outcomes,
and at the same time searching for ways to avoid or
to face them.
Its important to state that the problem
solving skills are still developing until the reach of
the formal-operational stage. In this period
children are increasingly able to use flexible and
abstract reasoning, to test mental hypotheses, and
consider multiple outcomes for events (Szabó &
Lovibond, 2004; Verstraeten et al., 2011). The
development delays in normal problem solving or
coping processes are thought to play an important
role in the causality or maintenance of excessive
worrying. Thus being an area that should be
addressed (Szabó & Lovibond, 2004).
Excessive worrying is a common feature in
several anxieties and mood disorders and is often
associated with other disorders (Szabó & Lovibond,

2004). It doesn’t lead to engage in adaptive
problem solving at the same level as individuals
who worry less (Silverman, Greca, & Wasserstein,
1995).
Therefore, Alan looks to be at some risk of
developing anxiety disorder, due to the excessive
worrying, creating non realist and catastrophic
situations, that for now and due to his abilities he is
managing to solve. However, worrying intensively
could prevent him from deploying adaptive coping
resources, which could lead to the development
and maintenance of the anxiety (Szabó & Lovibond,
2004; Verstraeten et al., 2011).
The behavioural etiology of anxiety disorder
is based on the paradigm of learned avoidance. The
cognitive etiology is based on the non-realistic
perceptions and in the interpretation of the
threatening facts. The self-dialogue of anxious
children is full of predictions of failure and danger.
Anxious children are usually pessimists about the
events magnitude and normally they have an error
in the information processing - catastrophization.
They overestimate the external events and
underestimate their abilities in confronting with
them (Peterson & Wainer, 2011).
Some studies suggest that anxieties are
superimposed on developmental fears and
contribute to a radicalization of those fears, but also
that conditioning, modelling, and informational
experiences contribute to the manifestations of
common anxiety phenomena in childhood (Muris et
al. 2000).
In this way, the ritualistic behaviours
presented by Alan may represent efforts to relieve
tension and anxiety. The study of Laing et al. (2009)
reported that worry intensity is, in fact, a stronger
predictor of the propensity to perform ritualistic
behaviour, more than fear.
Rituals are seen as typical of childhood,
decreasing significantly around age 6 and continue
declining throughout childhood and adolescence
(Laing, et al., 2009). Young pre-schoolers frequently
demand uniformity, constancy and attention to
details. Later in childhood, rituals can take more
complex forms, rule-based games, or behaviours
intended to ward off or “undo” negative
consequences, as stated by Laing et al. (2009).
Rituals have an adaptive function at this time,
providing increased feelings of self-efficacy in an
environment perceived as out of control,
sometimes being used as a way to control anxiety
(Laing, et al., 2009). However, in Alan the rituals
are not yet decreasing, and instead they seem to be
becoming more secretive and isolated to some
contexts. Thus attending to Alan’s increased
reasoning and cognitive abilities, and his greater
understanding of the contingencies of daily life, the
performance of ritualistic behaviours may become
an increasingly maladaptive reaction to anxiety as
childhood progresses, and so needing to be
addressed.
Diagnostic hypotheses
Alan presents motor tics, anxiety symptoms,
as worrying (excessive concerns about different life

aspects), and obsessive-compulsive symptoms /
rituals (repetitive behaviours that are carried out in
a rigid and compulsive-like manner). These
symptoms are causing clinically significant distress
and are accompanied by lack of motivation.
There is enough evidence to support a
chronic motor tic disorder diagnose. Also, attending
to the symptoms of excessive worrying and rituals
we could think in generalized anxiety disorder and
obsessive-compulsive disorder as comorbid
diagnostic hypotheses or the early onset of them.
Chronic motor tic disorder
Accordingly to DSM-IV-TR (2000), Alan
presents a chronic motor tic disorder, with multiple
motor tics - sudden, rapid, recurrent nonrhythmic,
stereotyped motor movements. It does not fill the
criteria for Tourette’s disorder. Alan’s tics occur
many times a day, nearly every day, or
intermittently throughout a period more than one
year. He didn’t had a period of more than three
consecutive months without tics, which excludes
the transient tic disorder diagnose. Alan presents
the tics at least 3 years long, which is causing
marked distress and some impairment in important
areas of the functioning (i.e. school relationships),
criteria aligned with DSM-IV-TR (2000).
Tics occur frequently in childhood and
adolescence, especially from the ages 6 to 14 years
old (Cardona, Romano, Bollea, & Chiarotti, 2004).
Accordingly to Coffey and Shechter (2005),
males are at least three to four times more likely
than females to manifest tics disorders. The tics are
sometimes enhanced by stress and excitement. The
tics are experienced as irresistible, but can be
suppressed for varying periods of time..
The findings of Cardona et al. (2004) suggest
that an elevated percentage of individuals could
present a tic disorder (and some obsessive
compulsive symptoms) without any clear pattern of
comorbidity. However, other studies have
suggested a wide range of behavioural and
emotional disturbances co-occurring with tic
disorder, also in children with milder tic symptoms,
such as attention deficit hyperactivity disorder,
obsessive compulsive disorder, mood disorders as
separation anxiety disorder and generalized anxiety
(Cardona et al., 2004; Coffey & Shechter, 2005).
Accordingly to the findings of the same study some
types of behaviours and psychopathologies seem to
be related to the duration of the tic disorder,
instead of the tic disorder itself or the children’s
age (Cardona et al., 2004)].
Generalized anxiety disorder (GAD)
Excessive and uncontrollable worrying was
introduced in the DSM-IV (1994), as the cardinal
diagnostic feature of generalized anxiety disorder,
in both adults and children. To be diagnosed with
GAD the child or adolescent must experience
excessive worry that impair daily functioning and
continues for at least six months (Donnelly, &
McQuade, 2005), time criteria not entirely filled in
Alan’s case.
Pathological worries of children with GAD
tend to include more domains of concerns (such as

family members health, school performance, social
relationships), be associated with a greater distress,
more difficult to control, and occur more frequently
than those of healthy children (Donnelly, &
McQuade, 2005; Silverman, Greca, & Wasserstein,
1995), which seems to be Alan’s case.
Obsessive-compulsive disorder (OCD)
The essential feature of OCD includes the
recurrence of obsessions and or compulsions severe
enough to be time consuming (more than an hour
per day) and cause marked impairment or
significant distress. Obsessions are recurrent and
persistent thoughts, urges, impulses, or images that
are experienced as intrusive and inappropriate and
which cause anxiety and distress. Compulsions are
repetitive behaviours or mental acts that the person
feels driven to perform in response to an obsession.
Those compulsions are aimed at preventing some
feared event or situation, or in order to get
something “just so” (Donnelly, & McQuade, 2005).
To hypothesize the OCD diagnose, firstly it is
important to verify if the behaviours presented are
whether a ritual or a complex motor tic. Tics are
usually not heralded by preceding thought or
obsession (Donnelly & McQuade, 2005). In Alan’s
we verify preceding thoughts / worries, that lead us
to think in this diagnose. Also to think in this
diagnose it’s important to distinguish between
ritualistic behaviours non associated with
compulsions that are common in young children
(Donnelly & McQuade, 2005), from those associate
with compulsions, that are accompany by the
experience of anxiety or distress while performing
them. In Alan’s case the ritualistic behaviours are
accompanied by feelings of distress.
Tic-related OCD occurs more often in males
than in females, with males outnumbering females
by 3:2 ratio. It has an earlier age at onset than non-
tic-related OCD, especially a pre-puberal onset. The
mean age of onset is 10.3 years. The symptoms are
presented 5-8 years before they come to clinical
attention (Cardona et al., 2004; Donnelly &
McQuade, 2005; Hanna, Piancentini, Cantwell,
Fischer, Himle, & Etten, 2002).. Genetic studies
have shown a shared underlying susceptibility
between tics and childhood-OCD (Hoekstra et al.,
2013).
Prognostic
Accordingly to Coffey and Shechter (2005),
tics may persist into adulthood. Nevertheless the
tics severity often decreases significantly in
adolescence. This doesn’t mean full remission
and well-being. However is not the severity of tic
disorders, but the comorbid between tic, OCD and
other psychopathologies that could influence the
development of emotional and behavioural
difficulties in childhood / adulthood, thus leading to
an enhance of distress, maladjustment, as well as
problems in peer relationships (Cardona et al.,
2004; Hoekstra et al., 2013). Most patients with
very mild symptoms need only monitoring,
education, guidance, and support. Moderate to
severe symptoms usually should be treated with
medication, especially if the symptoms significantly

interfere with adaptation to family, school, peer
relationships, or interfere with the developmental
progress (Coffey & Shechter, 2005)]. If needed and
if there is impairment in school or with academic
performance, clinicians should be available to
provide consultation, guidance, and education to
teachers.
As described by Laing et al. (2009) young
children reporting high levels of anxiety, excessive
worry, and ritualistic behaviours may be more likely
to remain susceptible to these experiences
throughout ontogeny. Hence when left untreated,
the presence of anxiety is associated with later
mental health difficulties (adult anxiety disorders,
depression, substance use problems), as stated by
Kendall, Settipani and Cummings (2012). Thus is
crucial to early intervene.
Therapeutic Intervention
The majority of tics do not need treatment.
The treatment of tic disorder justifies it self when it
has consequences at social level and personal level.
Still, Alan is facing some other difficulties managing
symptoms of anxiety and others. Those difficulties
are a risk factor for the early onset of other
psychopathologies. Also Alan has other risk factors,
like behavioural inhibition; shyness; perfectionism;
lack of confidence, between others. Nevertheless,
he has also protective factors such as his cognitive
abilities, the problem-solving ability that is still
developing, and the family support. Likewise he is
willingly to participate in the therapy activities; he
self-disclosed about his worries and is mentally
engaging with the therapeutic materials, which are
good variables to the coping process. Therefore it’s
important to exist a psychological intervention, and
only if needed add some medication.
Therefore, Alan’s therapeutic intervention
should aim at psychoeducating about tics and
emotions; helping him relieve symptoms or achieve
symptom control (such as worry and obsessive
compulsive symptoms); developing strategies to
cope with stress and anxiety; enhancing Alan’s
developmental progress and his sense of
competence; helping him to construct a new
narrative that enable him to face the problems
faced.
Also the intervention should aimed at helping
parents to deal with the symptoms, promoting
Alan’s development and self-efficacy, and helping
parents cope with their own biases cognitions that
are feeding Alan’s anxious symptoms.
Psycoeducating the child and family about
tic disorders is essential, as also to psychoeducate
teachers and peers, when possible. Accordingly to
Antunes (2009), tics are not the true problem, thus
the problem is in the people who berates the tics,
the colleagues that make fun, the family members
that say “stop with that”. It’s urgent to provide
patients and parents with current information
about the causes of tics (genetic factors and brain
neurochemical imbalances) and emphasize that
they are not signs of psychological or emotional
illness, a common misperception. Learning about
the importance of genetic factors and clarifying that

the patient’s symptoms are not voluntary often
relieves a sense of guilt in both patient and parents
(Coffey & Shechter, 2005; Shprecher & Kurlan,
2009). This is especially important since Alan’s
parent often ask him to stop the tics. The best way
to deal with the tics is ignoring them (when socially
acceptable). Also it is important to recognize the
moments when the tics appear mostly, as a way to
better find strategies to cope with them.
Psycoeducating about anxiety and teaching
how to recognise the signs of anxiety, is another
step in the process. Other will be to teach strategies
to manage anxiety in different situations, and also
to diminish the tendency to worry. The reduction of
anxiety symptoms is associated with decrease in
negative automatic thoughts and increase of
anxiety control, accordingly to Kendall, Settipani
and Cummings (2012). Worried children may
benefit from a focus on increasing positive problem-
solving skills (generating alternative solutions to
problems, playfulness, and effectiveness of
solutions) and problem-solving beliefs, like
confidence and perceived control (Broeren, Muris,
Bouwmeester, Heijden, & Abee, 2011; Parkinson, &
Creswell, 2011; Verstraeten et al., 2011).
Since we cannot forget that patients with tic
disorders have some personal and emotional
vulnerability (Coffey & Shechter, 2005), helping
Alan restore or maintain his self-esteem and
promote his self-efficacy in coping with emotions
are core elements of the intervention. Thus the
reduction in negative self-talk is crucial, mediating
treatment gains (Kendall, Settipani, & Cummings,
2012). Also it’s important to promote Alan’s
autonomy at different levels (i.e. sleeping), helping
him to continue his normal course of development.
Theoretical frame for intervention
Attending to the developmental stage of
Alan, his cognitive and verbal abilities, his
involvement towards the therapeutic process, it
seems that cognitive-behavioural therapy and
narrative therapy are appropriate choices. It’s
important to work both at an individual and family
level.
Narrative therapy aims at deconstructing the
non-functional narratives of Alan and elaborate new
ones, more adaptive and flexible, as stated by
Gonçalves and Henriques (2005). The writing of
letters is a useful tool (White & Epston, 1990).
Therapeutic letters provide a bridge between
sessions and a mean to anchor new stories that
promote personal agency, continuing the
meaning-making that occur in a therapeutic
conversation (Paré & Rombach, in press). Thus the
use of externalization techniques and the
identification of unique outcomes constitute
important elements in the narrative process.
Cognitive-behavioural therapy (CBT) is an
effective intervention towards anxiety and
obsessive-compulsive symptoms. It’s used to treat
60% to 65% of youth anxiety disorder, showing
meaningful reduction in anxiety symptoms
(Donnelly, & McQuade, 2005; Kendall, Settipani, &
Cummings, 2012). Cognitive-behavioural therapy

examines distortions in the ways of looking at the
world and ourselves. It also aims to identify
automatic negative thoughts that contribute to
anxiety, thus developing coping mechanisms
(Verstraeten et al., 2011). Also behavioural
paradigms are indicated for a number of difficulties
in tic disorder patients. They can be used to help
“de-stimulate” and contain some of the symptoms
when needed (Coffey & Shechter, 2005).
Relaxation techniques, such as deep
breathing, guided imagery, and use of relaxation
tapes, can be useful for the anxious or stressed tic
disorder patients. Habit reversal, using competing
response to oppose motor tics, may have utility in
the treatment. Opposing muscles are contracted
following the urge to have a tic. This competing
response theoretically prevents the emergence of
the tic. Since emotional conflicts and stress
frequently increases symptom intensity and
frequency, time-limited withdrawal from stressful
situations can be beneficial, as stated by Coffey and
Shechter (2005).
Graduate exposure and response prevention
is useful in treating anxiety and obsessive
compulsive symptoms. Thus, after the identification
of fears, obsessions and compulsions, it will be
helpful to make a stimulus hierarchy and then
exposure tasks (Donnelly, & McQuade, 2005).
Besides the individual intervention following
these principles, it is important to intervene with
family. The family system seems to have slowed or
altered his development, due to the focus on Alan’s
difficulties (tics, worries and rituals). They are
struggling with fears about Alan’s future and how to
help promote his development.
Parent factors can influence both the
development and maintenance of anxiety in youth
by predicting danger, restricting independence,
preventing child distress, and modelling avoidance
(Kendall, Settipani, & Cummings, 2012). Alan’s
parents seem to be feeding the fear of night, and by
doing enhancing others anxious symptoms as the
night rituals, not promoting the development of
Alan’s coping strategies.
Therefore besides the traditional parental
involvement in treatment, which usually focus on
parental management skills of tics and OCD
symptoms and addresses family accommodation of
symptoms, it is useful to incorporate cognitive
approaches to parent training. This approach
involves teaching parents how to identify
rationale and flexible cognitive appraisals and
cognitive processes, which may have an influence
on the development of children’s own cognitive
biases and OCD severity (Farrell, Waters, & Zimmer-
Gembeck, 2012). Note that Alan’s mother admits
being too anxious, having difficulties managing her
own anxiety.
So the parent involvement in the therapeutic
process could augment treatment response via
helping children practice their coping strategies
(acquired in CBT and narrative therapy), as pointed
out by Kendall, Settipani and Cummings (2012).

Small changes in Alans’ narratives during the
therapeutic process
Alan’s therapeutic intervention is on-going. It
counts with 9 sessions, and it is being guided by the
above principles. Some improvements were seen;
especially in Alan’s confidence and the way he faced
some fears. One of the key elements of the
intervention was the writing of therapeutic letters,
mostly as homework.
In one of the letters written he stated: “Hello
horrible fears. I want to say that you don't scare me
too much now, because I ‘am trying to eliminate
you. My psychologist and I found a way of erasing
you, step by step, every day. The method is paying
off (...). It is working better than I though. So I’m
going to tell you about that: We fill 2 balloons and
draw in them the face of the fears. After blowing up
the balloons, we pick the remaining’s and glue them
in a cardboard. We write by pencil the name of the
fears, and every time they get less intense, I will
erase letter by letter their name. So, goodbye
horrible fears.”
In another session, Alan brought the
following letter:
“Hello horrible fear. You don’t scare me so
much now, because I’m being able to control you.
For me, you are a child’s fear, and I'm getting
bigger. So you don't scare me as before”.
During one session, Alan wrote a coping card
towards a fear that was not allowing him to go to
the bathroom in the upper floor of the house: “To
face this fear I need to disobey him. If it still stands
in front of me, I need to tell him to get out of my
way. Still, if he doesn't leave I can go around him,
and if that doesn't work I need to tell him that if he
doesn't get out of my way I will urinate there, and
that definitely will put anyone away, fear or
person.”
By the time we finish writing this article, Alan
could identify his biggest achievements, since the
beginning of therapeutic process. “I did it! I don’t have to
knock with the hand in the shoulder of my parents and
sister to go to sleep! Now, I also don't fear that a
creature get out from the mirror. I don’t have anymore
fear of water or sea creatures, and also the fear of going
alone to the upper floor of my house disappear (...) I did
it!”
The therapeutic change is taking place. Thus our
goal is helping Alan towards consolidating these changes
and increasing the unique outcomes.
Final reflections
Tics are like sobs. They cannot be
controlled.
Alan lives constantly with tics, especially in
stressful situations. Alan’s biggest wish is to not
have tics, because he is afraid of his colleagues
making fun of him, nor respecting him. This issue
must be taken into consideration and reflected
about, thus being necessary to psychoeducate
Alan’s, his family and society towards a better
knowledge about what it means to have a tic
disorder.
Moreover as clinicians we should be aware
that young school age children with tics are more

vulnerable to the development of emotional and
peer problems over time. This is not due to the tic
disorder itself, but the misconceptions about the
disorder and also the comorbid psychopathologies,
that lead to difficulties in adjusting to the
environment, thus being essential to address these
comorbidities. Besides the tic disorder, Alan
presented intense worry and some obsessive-
compulsive symptoms, which he wishes to face.
Sometimes making an accurate diagnose is
difficult, due to the thin line that separates normal
from pathological symptoms. Accordingly to
Donnelly and McQuade (2005), clinicians need to
maintain a high level of suspicion for anxiety
disorders when evaluating children. They have high
prevalence and often mimic or are comorbid with
other childhood disorders. Also children need to be
evaluated within a bio psychological framework, in
which genetic vulnerability, biological etiologies, life
experience, social and family contexts, and
developmental phase should be attended to better
understand the expression of pathological
symptoms. Given the uniqueness of each child and
the complex interplay among the internal and
external variables that drive anxiety and other
symptoms, a multimodal approach to diagnosis and
treatment is critical (Donnelly & McQuade, 2005). In
this study case, we suggested a behavioural
cognitive and narrative approach, as it has been
found to be effective, especially, in anxiety
disorders and obsessive-compulsive symptoms.
Therefore its important to intervene in early
life, as a way to develop adequate and more flexible
responses to anxiety, so it can be possible to deal
with the complex environmental demands of later
childhood. This will result also in a decrease in
ritualistic behaviour with age or other unhealthy
behaviours (Laing, et al., 2009)]. Thus being said,
the key aspects of the therapeutic intervention
process are to help Alan to learn how to manage
emotions not falling into intense worrying or
avoiding situations; increase his positive problem-
solving skills and believes; reduce negative self-talk;
promote his autonomy and mastery.
According to Bronfenbrenner, the
development of a child is influenced by the varied
systems of the child’s environment and also by the
interrelationships among those systems. The
environment influences the child and the child
influences the environment. So in this process,
family is not the only one responsible for the
“construction”, but it puts “bricks that are decisive”.
Therefore it’s crucial to have a supportive family
environment in which a child can comfortably
approach their parents to let them know about the
problematic situations or the symptoms they face
(Shprecher & Kurlan, 2009).
Alan started the therapeutic process with the
statement: “I have a wish to not have tics, stop
worrying and never die”. The process of change
takes time, and is being made step by step, with
many achievements and setbacks. He needs to learn
how to live with tics, and how to cope with the other

symptoms presented, like anxiety and obsessive-
compulsive symptoms. The writing of therapeutic
letters is being extremely useful in the therapeutic
process, whereas he is using it to re-write his own
history. Thus, Alan is creating new narratives, more
adaptive and flexible, about how he feels towards
himself and about the future.

Abstract: The present work is an exploration of multiple sclerosis, which is one of the major
neurodegenerative diseases that affect young adults. Different aspects related to the formation and
evolution of the disease, characteristics of different types, etiological factors, differential diagnosis and risk
factors are also reported. Although some of the aspects are explained and measured. The highlight of the
central issue is the neuropsychological intervention in the disease, where are considered different models,
typology and intervention areas, from rehabilitation and changes in lifestyle until the use of
pharmacological measures.

Resumo
Este trabajo es una exploración de la esclerosis múltiple, que es una enfermedad neurodegenerativa que
afecta a gran adultos jóvenes. Están dirigidos diferentes aspectos del proceso de formación y evolución de
la enfermedad, las características de diferentes tipos, factores etiológicos, diagnóstico diferencial y factores
de riesgo. Aunque se explican algunos aspectos de la evaluación, el objetivo de este trabajo es la
intervención neuropsicológica en esta enfermedad, que se ocupa de diferentes modelos, tipos y áreas de
intervención, desde la rehabilitación y los cambios en el estilo de vida con el uso de medidas
farmacológicas.

Introdução
A Esclerose Múltipla (EM) é uma doença
crónica auto-imune que atinge o sistema
nervoso central, caracterizada por inflamação,
desmielinização, gliose e neurodegeneração. A
sua etiologia é multifactorial com maior
incidência de factores genéticos e ambientais
(Compston & Coles, 2002 cit in Pedro, Pais-
Ribeiro & Pinheiro, 2011; Lutton, Winston &
Rodman, 2004), afectando com maior
frequência adultos jovens e nas últimas
décadas maioritariamente o sexo feminino
(Alonso & Hernán, 2008). É uma doença
progressivamente incapacitante e
imprevisível, pelo que se estima que
aproximadamente 50% das pessoas
necessitem de um objeto auxiliador de
marcha 15 anos após o início da doença (Irani,
2005; Marrie, 2004).
A Esclerose Múltipla pode caracterizar-se
pela presença de lesões na substância branca,
lesões que resultam de inflamações, da
desmielinização, destruição axonal,
astrocitose e/ou atrofia tecidual (Rodrigues,
Nielson e Marinho, 2008). Estas lesões,
também denominadas de placas, localizam-se
predominantemente na substancia branca, no
tronco encefálico e na medula espinal,
podendo agregar-se e originar placas de
grades dimensões (Daroff, Fenichel, Jankovic
& Mazziotta cit in Fernandes, 2012; Alonso &
Hernán, 2008).

Este processo de desmielinização está
dependente da activação periférica de
linfócitos T que expressam moléculas de
adesão na superfície, permitindo o atravessar
da barreira hematoencefálica. No sistema
nervoso central, estes linfócitos promovem a
proliferação de linfócitos T citotóxicos, a
síntese de anticorpos e a activação da
microglia, o que resulta na destruição da
bainha de mielina e dos oligodendrócitos
(Comabella & Khoury, 2012). Após este
episódio, a velocidade de transmissão do
impulso nervoso diminui, resultando daí as
manifestações clínicas (i.e. os sintomas) da
EM. A reversibilidade dos sintomas pode ser
total ou parcial dependendo de vários factores
como a redistribuição dos canais de sódio ao
longo do segmento desmielinizado e/ou a
capacidade de remielinização dos
oligodendrócitos (Comabella & Khoury, 2012;
Disanto, Morahan, Barnett, Giovannoni &
Ramagopalan, 2012; Menge et al., 2008).
A progressão da EM depende do nível dos
processos de inflamação dos neurónios e do
ritmo da desmielinazação, estando assim
definidos 4 subgrupos da doença: Evento
Clínico Isolado; Esclerose Múltipla Forma
Surto-Remissão (EMSR); Esclerose Múltipla
Progressiva Primária (EMPP) e Esclerose
Múltipla Progressiva Secundária (EMPS). O
subgrupo de forma benigna também chamado
de evento clínico isolado (CIS – Clinically
Isolated Syndrome), é caracterizado pela
ocorrência de um único surto, não se
verificando manifestações dos sintomas
durante vários anos (Compston & Coles, 2002
cit in Pedro et al., 2011; Alonso & Hernán,
2008).
O subgrupo de forma surto-remissão
(EMSR), frequentes no primeiro estádio da
doença em cerca de 85 a 90% dos indivíduos
com EM, resulta de um processo inflamatório
agudo na mielina dos neurónios afectados
originando uma disfunção neurológica aguda,
seguida por períodos de recuperação variável
de uma restituição integral de funções a um
aumento cumulativo de defeito neurológico
que pode evoluir para EMPS (Mitchell et al.,
2005 cit in Pedro et al., 2011; Lutton et al.,
2004).
O subgrupo de EMPP ocorre quando há
desmielinização lenta, discreta mas

progressiva, constatando-se uma instalação
gradual dos sintomas e da perda de funções,
não existindo surtos. O subgrupo de EM
progressiva secundária, também denominada
de forma progressiva com surtos, inicia-se de
recidivas-remissão com a instalação de uma
perda progressiva de funções (Compston &
Coles, 2002 cit in Pedro et al., 2011; Menge et
al., 2008).
No que concerne ao seu diagnóstico
diferencial, a EM pode apresentar sintomas ou
características semelhantes às de outras
doenças desmielinizantes como a
neuromielite ótica e encefalomielite
disseminada aguda, doenças que envolvam o
sistema nervoso central como a vasculite
sistémica e neoplasias ou outras lesões que
afetem as estruturas cerebrais (Nylander &
Hafler, 2012).
Existem alguns factores de risco que
parecem estar associados à EM como: a
residência em locais de latitudes elevadas, o
que parece ser explicado pelos défices em
vitamina D característicos nessas regiões;
história familiar de EM; infecções virais (e.g.
vírus herpes humano, vírus varicela zóster); e
stress físico e emocional (Owens, Gilden,
Burgoon, Yu & Bennett, 2011; Marrie, 2004;
Lutton et al., 2004).
Avaliação neuropsicológica
No que diz respeito à sintomatologia, podem
identificar-se o envolvimento do sistema motor,
sensorial, visual e autónomo. As manifestações
mais comuns incluem fadiga, as alterações
cognitivas, as dificuldades na resolução de
problemas, a dor e desconforto motor, a fraqueza
muscular, parestesias, a ataxia, alterações de
equilíbrio e dificuldades na marcha (Compston,
2004 cit in Fernandes, 2012; Gondolfi et al., 2014;
Schuhfried, Mittermaier, Jovanovic, Pieber &
Paternostro-Sluga, 2005).
Várias metodologias podem ser utilizadas para a
avaliação da Esclerose Múltipla como exames
neurorradiológicos, análise do líquido
cefalorraquidiano (LCR) e potenciais evocados
(Daroff, Fenichel, Jankovic & Mazziotta, 2012 cit in
Fernandes, 2012), sendo que o diagnóstico é
realizado com base na revisão de 2010 dos critérios
de McDonald, através do exame clínico, de testes

laboratoriais e disseminação espacial e temporal
(Polman et al., 2011 cit in Fernandes, 2012).
Os potenciais evocados permitem avaliar a
função das vias aferentes e/ou eferentes e o
aumento das respetivas latências que traduzem as
consequências da desmielinização na condução do
impulso eléctrico (Daroff, Fenichel, Jankovic &
Mazziotta, 2012 cit in Fernandes, 2012). A eletrose
das proteínas do LCR é utilizada especialmente para
a exclusão de diagnósticos alternativos (Lutton et
al., 2004).
Também a ressonância magnética, sendo um dos
métodos de avaliação responsável pelo diagnóstico
cada vez mais precoce, tem um papel fundamental
na avaliação desta patologia por recolher
informação acerca da substancia branca (Compston
& Coles, 2002 cit in Pedro et al., 2011). Sabe-se
porém que podem também existir pequenas
alterações na substancia cinzenta, particularmente
no córtex cerebral e nos núcleos da base. Por
norma as lesões são pequenas, de forma circular ou
ovalada e são maioritariamente observadas nas
regiões periventriculares, justacorticais e
infratentoriais (Saharaian & Eshaghi, 2010).
Também uma avaliação da memória (Introzzi,
Juric, Andrés & Richard’s, 2007), dos processos
cognitivos, das funções executivas e da existência
de alguma psicopatologia (Olivares-Pérez et al.,
2009) pode revelar-se fundamental para o
delineamento de um processo de intervenção mais
eficaz e integrado.
Reabilitação neuropsicológica
A patogénese da Esclerose Múltipla envolve
interacções complexas entre o sistema imunitário e
o sistema nervoso central o que conduz a uma
necessidade de intervenção em diferentes alvos
terapêuticos. Entende-se que o objetivo principal da
intervenção é a diminuição do risco de ocorrência
de novos surtos e da progressão da incapacidade
neurológica, sendo que a intervenção sintomática é
igualmente importante, uma vez que permite o
treino das funções, conduzindo a uma maior
qualidade de vida (Bem-zacharia, 2011; Lutton et
al., 2004; Rodrigues et al., 2008).
Diversos autores propõem que os programas de
intervenção devem integrar componentes físicos,
psicológicos e sociais, com o intuito de ajudar o
individuo a adquirir estratégias de auto-organização
e autocontrolo. Os componentes físicos são
baseados na organização biológica, os componentes
psicológicos integram a construção, manutenção e
realização das funções cerebrais que atuam na
direcção, organização e avaliação das interacções

individuais, e por último os componentes sociais
estão relacionados com vários ambientes que
acabam por influenciar o dia-a-dia do individuo,
estimulando-o nas diversas vertentes (Ford &
Urban, 1998 cit in Pedro et al., 2011).
Assim, o tratamento dos sintomas inclui por
norma: medidas não farmacológicas, como a
reabilitação e as alterações no estilo de vida e
suporte psicossocial (Rodrigues et al., 2008); e
medidas farmacológicas e/ou procedimentos
cirúrgicos (Bem-zacharia, 2011).
No que diz respeito à reabilitação e às alterações
no estilo de vida, sabe-se que é cada vez mais
comum que os pacientes com Esclerose Múltipla
mantenham as suas funções durante períodos mais
prolongados de tempo, o que consequentemente
lhes possibilita estilos de vida mais ativos
(Rodrigues et al., 2008). É neste seguimento que se
revela importante a implementação de programas
que visem o melhorar ou o manter do nível de
actividade física, de forma a facilitar a
funcionalidade (Pedro et al., 2011).
Existe uma multiplicidade de modelos teóricos
que podem ser aplicados com o intuito de
promover um estilo de vida mais saudável e um
aumento da qualidade de vida como a Teoria da
Aprendizagem Social (Bandura, 1986), o Modelo de
Crenças da Saúde (Janz & Becker, 1984), a Teoria da
Ação Planeada (Ajzen & Fishbein, 1980 cit in Joyce-
Moniz & Barros, 2005) e o Modelo Transteórico da
Mudança Comportamental (Prochaska, Di Clemente
& Norcross, 1992). Todos estes modelos reforçam a
importância da motivação para a adopção do
comportamento (Miller & Rollnick, 1991 cit in Pedro
et al., 2011).
Um outro modelo vastamente utilizado em
pacientes com Esclerose Múltipla é o Modelo de
Auto-regulação de Maes e Karoly (2005). A
aplicação deste modelo em pacientes com esta
patologia é frequente, visto que o mesmo envolve a
definição pelo indivíduo de metas pessoais e
comportamentais que orientam o delineamento
de estratégias, permitindo um constante feedback
importante para a monitorização do processo
terapêutico e para a auto-avaliação.
O indivíduo é percebido por estes autores como
um sistema auto-regulado que cria e mantem a
dinâmica da organização interna e de interacção
externa, assim o aparecimento de uma doença ou
situações súbitas de surtos são consideradas
situações de desestabilização do sistema, situações
que vêm alterar esta dinâmica, existindo por
conseguinte a necessidade de a reorganizar (Maes

& Karoly, 2005). Neste sentido, a auto-regulação
surge como uma abordagem facilitadora para o
indivíduo, enfatizando o seu papel ativo na
alteração dos seus pensamentos, emoções e
comportamentos em prol do alcance dos seus
objectivos de vida, objectivos esses que por norma
tendem a coincidir com os objectivos terapêuticos
(Maes & Karoly, 2005; Vohs & Baumeister, 2004 cit
in Pedro et al., 2011).
No que diz respeito às funções mnésicas e
portanto aos processos de codificação e
organização/armazenamento e recuperação da
informação, estudos demonstram que a memória
episódica, a memória semântica e a memória a
curto prazo e parecem estar afetadas em pacientes
com esta patologia (Gold & Leiguarda, 1992 cit in
Introzzi et al., 2007; Drake, Carrá & Allegri, 2001).
Alguns estudos indicam que este défice se deve
principalmente a falhas nos processos de
recuperação da informação (Coolidge et al., 1996 cit
in Introzzi et al., 2007), no entanto estudos recentes
apontam para a influência dos processos de
codificação como principal responsável pelos
défices mnésicos (Drake et al., 2001).
Também outros processos cognitivos,
relacionados com uma desconexão entre as
estruturas corticais e subcorticais como o tálamo e
os gânglios da base parecem estar afectados na
Esclerose Múltipla, o que facilmente se entende
pela desmielinização subcortical e degeneração
axonal, (Gold & Leiguarda, 1992 cit in Introzzi et al.,
2007; Comi, Rovaris, Leocani, Martinelli & Filippi,
2000).
As funções neuropsicológicas mais
frequentemente afectadas são a
atenção/concentração, a velocidade de
processamento, os défices de memória e as funções
executivas (Drake et al., 2001). Pelo que se revela
fundamental intervir na reabilitação destas funções
através das diversas técnicas e teorias auxiliares
que estão ao dispor dos neuropsicólogos (Introzzi
et al., 2007).
No que diz respeito a perturbações
psicopatológicas em comorbilidade com a EM,
encontram-se por vezes diagnósticos de processos
depressivos e ansiogénicos associados à doença. Se
isso se verificar no processo de avaliação é essencial
que também nestes processos incida o processo de
intervenção neuropsicológica, uma vez que estes
processos psicopatológicos podem agravar o ritmo,
nível ou gravidade da patologia (Sánchez-López,
Olivares-Pérez, Nieto-Barco, Hernández-Pérez &

Barroso-Ribal, 2004; Simioni, Ruffieux, Bruggimann,
Annoni & Schluep, 2007).
Nos primeiros anos da doença, a ocorrência de
surtos, com duração mínima de 24 horas, é uma
condição espectável dentro da patologia. Estas
manifestações podem estar presentes durante dias
e semanas sendo seguidas de períodos de remissão,
onde o paciente atinge uma recuperação parcial ou
total (Irani, 2005). A frequência destes surtos é
irregular, mas por norma não excede os 1 ou 2 por
ano. Na prática clínica, os surtos são utilizados para
definir a actividade da doença e a eficácia do
processo terapêutico (Irani, 2005; Cohen & Rudick,
2011).
A técnica de neuroprotecção, estratégia
terapêutica que tenta impedir ou atrasar a perda
neuronal e portanto a evolução da doença, através
do uso dos canabinóides como moléculas
promissoras com potencial para reduzir a
excitotoxidade, o influxo de cálcio e as lesões
oxidativas, diminuindo os processos inflamatórios, é
uma das técnicas muito utilizadas na Esclerose
Mútipla (Cohen & Rudick, 2011; Gomes & Lima,
2008). Desta forma, os canabinóides representam
um papel importante dada a excitotoxidade e
neurodegeneração característica desta patologia,
atuando no retardo da progressão da doença e na
protecção dos oligodendrócitos (Pertwee, 2002).
Neste sentido a ressonância magnética, para
além de ser utilizada para a realização do
diagnóstico e prognóstico, desempenha também
um papel fundamental no acompanhamento
evolutivo da Esclerose Múltipla (Saharaian &
Eshaghi, 2010; Irani, 2005) pela informação
referente às anormalidades na substancia branca.
Tem contribuído também para a utilização de
medicamentos que têm ajudado a modificar o
desenvolvimento progressivo da doença, atuando
no retardamento da incapacidade (Compston &
Coles, 2002 cit in Pedro et al., 2011; Gomes &
Lima, 2008).
Relativamente às medidas farmacológicas, o
principal objectivo do uso dos fármacos é a
intervenção na progressão da doença e na
acumulação de incapacidade neurológica
(Compston, 2004 cit in Fernandes, 2012). O
primeiro tratamento modificador surgiu na década
de 90, com a aprovação do primeiro interferão beta
(IFN-B) pela FDA (Food and Drug Admnistration).
Actualmente existem várias terapêuticas aprovadas

ou em estudo pela European Medical Agency (EMA)
para esta patologia, como a betaferon, avonex e
copaxone (fármacos de primeira linha), tysabri e
gilenya (fármacos de segunda linha) e novantrone
(que está associada a alguns efeitos secundários
graves mas que muitas vezes são utilizados fármaco
de terceira linha) (H-peter, Vermersch & Olsson,
2011).
O tratamento farmacológico dos episódios de
disfunção neurológica, é realizado com 1 grama de
metilprednisolona, administrado diariamente por
via intravenosa ou dividido em doses durante 3 a 5
dias, dependendo da gravidade. Nos surtos mais
graves e com prognóstico de recuperação mínima
tende a administrar-se corticoides durante 7 dias,
sendo que a plasmaferese é também considerada
em muitos casos (Bem-zacharia, 2011). Sabe-se que
a selecção dos fármacos deve ser individualizada e
depende de vários factores, como a preferência e a
tolerância de cada doente e curso clínico e
imagiológico da doença. Deve ter-se em conta
também o modo de administração, a tolerabilidade
e o perfil de segurança do agente farmacológico
(Compston, 2004 cit in Fernandes, 2012).
Conclusão
Sendo a Esclerose Múltipla uma das doenças
neurodegenerativas mais prevalentes em adultos
jovens (Alonso & Hernán, 2008), é de extrema
importância repensar esta patologia nas suas
diferentes vertentes. Desta forma, vários aspectos
devem ser considerados e integrados na avaliação e
particularmente na intervenção com estes
pacientes, desde os aspectos neurológicos até aos
possíveis diagnósticos de psicopatologia (Drake et
al., 2001; Introzzi et al., 2007).
A intervenção neuropsicológica com estes
pacientes deve então fazer convergir os
componentes biológicos, neurológicos e
neuroanatómicos, centrando-se nas principais
estruturas envolvidas, da mesma forma que deve
trabalhar na reabilitação das funções para a
adaptação do sujeito à vida diárias, procurando um
alívio dos sintomas e maioritariamente com vista no
objectivo primordial de retardar o avanço da
patologia e o aumento da incapacidade (Bem-
zacharia, 2011).
Assim e dada imprevisibilidade e instabilidade
característica desta patologia, releva-se
fundamental a criação de uma boa relação
terapêutica com o neuropsicólogo. Para a
promoção da confiança e motivação no paciente
para o processo terapêutico é da responsabilidade

do neuropsicólogo expressar empatia, provocar a
discrepância para induzir estimulação, evitar a
argumentação cíclica, minimizar as resistências e
promover a auto-eficácia (Miller & Rollnick, 1991 cit
in Pedro et al., 2011).

Alzheimer

Neuropsicologia do
envelhecimento
As mais variadas investigações
das neurociências permitiram verificar
que o desenvolvimento
neuropsicológico dos indivíduos
decorre por estágios, no percurso da
sua via. Sendo que ao longo deste
percurso se vão transformando
gradualmente, de um estádio para o
outro. É de salientar que neste
processo também se verificam
diferenças entre os indivíduos,
derivado das características de cada
um, das experiencias de vida, bem
como, do ambiente em que vive. Pode-
se considerar esses estádios
equiparados às etapas evolutivas
principais: desenvolvimento,
maturidade e envelhecimento. No
último estádio, a forma como o cérebro
funciona depende do que o individuo
fazia nos outros estádios prévios, ou
seja, da estimulação que dava ao
cérebro (Dusjardin & Lemaire, 2008).
O cérebro comanda todas as
atividades do corpo, para o
cumprimento e funcionamento pleno
das tarefas, conta com a atividade dos
seus duzentos bilhões de neurônios,
tornando-se essencial que estes
neurónios sejam preservados para que
possam realizar um normal
funcionamento. Na última fase do
desenvolvimento neuropsicológico,
ocorre uma deterioração
geneticamente programada, sendo que
há um envelhecimento celular e uma
escassez na capacidade das células de

se renovar e regenerar. Este tipo de
decadência para além de ser inevitável
é também ditado por regras biológicas
(Plancher, Nicolas & Piolino, 2008;
Rocha, 2012).
Assim na fase do
envelhecimento ocorrem mudanças em
diversos âmbitos neurobiológicos e
neurofisiológicos (sinapses diminuídas,
lentidão do fluxo axoplasmático,
decréscimo na plasticidade),
neuroquímicos (alterações na
circuitária 0colinérgica; ao nível das
monoaminas) e estruturais (neocórtex,
complexo hipocampal, núcleos da
base). Relativamente à morfologia do
cérebro do indivíduo idoso, em especial
de um idoso com envelhecimento
patológico, verificam-se diferenças
quando comparado com o cérebro de
um indivíduo jovem, ou seja, redução
do tamanho e do peso cerebral,
alargamento ventricular e dos sulcos e
afinamento dos giros. (Melo, 2008;
Santos, Andrade & Bueno, 2009).
Estas alterações biológicas em
elevadas quantidades, desencadeiam
apoptose, radicais livres, mudanças
protéicas e outros danos secundários,
levando assim à presença de défices
físicos, cognitivos e comportamentais
no idoso. O cérebro do indivíduo idoso,
apresenta giros mais finos e separados por
sulcos mais profundos e abertos, existindo
uma menor espessura das regiões corticais,
ou seja, uma diminuição do número de
neurônios e sinapses. Verifica-se uma
existência de sintomas psicológicos e
físicos, tais como, os lapsos de memória,
uma lentificação do raciocínio, episódios
passageiros de confusão, tremor,
dificuldade de locomoção, insônia noturna
com sonolência diurna e falta de equilíbrio
(Dusjardin, et al., 2008).
No que concerne a demência
nos idosos, esta abrange défices no
âmbito social, ocupacional, em funções
cognitivas e em atividades instru-
mentais da vida diária. A sua principal
característica prende-se com o declínio
da memória associado a défice de, pelo
menos, uma outra função cognitiva
(linguagem, gnosias, praxias ou funções
executivas) com intensidade suficiente
para interferir no desempenho social
ou profissional do indivíduo
(Schlindwein-Zanini, 2010).
À medida que envelhecemos
ocorrem algumas alterações em
diferentes áreas da nossa cognição e de
nosso comportamento, seguindo uma
ordem temporal. Deste modo poderia
ser esperado que ocorresse primeiro,
alterações em tarefas viso-espaciais e
viso-construtivas do que em tarefas

verbais. Os idosos com mais de 65 anos
de forma geral queixam-se de
dificuldades com a memória e outras
habilidades cognitivas comparadas com
seu próprio desempenho em anos
anteriores (Ávila, Miotto, 2003).
Ponderando sobre a prevalência
de doença de Alzheimer e outras
demências que afetam a cognição, a
queixa sobre a memória é
compreensível e requer atenção
precauciosa. Salienta-se ainda que os
défices nas funções cognitivas em
idosos podem proporcionar,
dependendo de seu grau, perda da
independência, aumento da taxa de
mortalidade e baixa na qualidade de
vida. Desta maneira, compreende-se o
esforço científico que tem sido
atribuído aos variadíssimos estudos
para se conseguir a melhor
compreensão do declínio cognitivo dos
idosos saudáveis (Ávila, et al., 2003;
Plancher, et al., 2008).
No que diz respeito aos últimos
estudos realizados no âmbito das
neurociências têm-se reunido
evidências de que o comprometimento
cognitivo leve seria uma condição de
alto risco para o desenvolvimento da
doença de Alzheimer. Os estudos pós-
morte encontraram nos cérebros de
uma grande percentagem de idosos
com comprometimento cognitivo leve
as alterações morfológicas compatíveis
com a doença de Alzheimer (Santos, et
al., 2009).
Alzheimer
A doença de Alzheimer, é uma
doença cerebral degenerativa que afeta
principalmente os idosos. Caracteriza-
se por uma perda gradual de memória
e de outras funções cognitivas,
resultando em défices nas atividades
de vida diária, sociais e ocupacionais do
indivíduo (Cunha, Cunha, Silva & Couto,
2011; Hwang, Cha & Cho, 2015).
A neuropatologia da doença de
Alzheimer carateriza-se por dois
mecanismos críticos que determinam a
morte neuronal, sendo que num
primeiro momento verifica-se a
formação de placas amilóides pela
gamasecretase e beta secretase, e num
segundo momento dá-se a
hiperfosforilação da proteína, que leva
à formação de emaranhados
neurofibrilares dentro dos neurônios. A
ocorrência deste procedimento leva à
atrofia cerebral, num momento inicial
em áreas do lobo temporal, mais
propriamente no hipocampo e córtex

entorrinal (áreas relacionadas com o
processamento de memória recente) e
também a atrofia do núcleo de
Meynert, bem como dos núcleos
septais, no prosencéfalo basal. Estes
núcleos são responsáveis pela
formação de acetilcolina, que consiste
num neurotransmissor mediador da
atividade cognitiva. Após estas
ocorrências o processo
neurodegenerativo vais ocorrendo de
forma progressiva, acometendo todo o
córtex cerebral, delimitando o declínio
das demais funções cognitivas, bem
como também os distúrbios de
comportamento. Deste modo, as
mudanças neuropatológicas ocorrem
antecipadamente ao diagnóstico clínico
da doença de Alzheimer. Porém a
capacidade cerebral pode ser
compensada até o momento em que a
função cognitiva comece a apresentar
falhas evidentes, até que o paciente se
encontre na fase severa (Azevedo,
Landim, Fávero & Chiappetta, 2009;
Lalanne & Piolino, 2013, Oliveira, 2010).
Na doença de alzheimer a
atrofia cerebral dá-se
predominantemente na região do lobo
temporal e parietal, sendo depois
estendida ao lobo frontal. Neste
sentido, os emaranhados
neurofibrilares, aparecem no córtex
frontal com o progresso da doença. No
entanto, constata-se uma gravidade
neuropatológica na região frontal
(Ávila, et al., 2003; Ladislau, Guimarães
& de Souza, 2015; Lee, Gambete,
Gayraud, Barkat-Defradas, 2013).
Apesar da memória e da
linguagem terem recebido mais
atenção nas investigações
neuropsicológicas, as funções
executivas também aparecem com
particular vulnerabilidade desde o
início da doença. Há evidências de que
alguns pacientes apresentam maior
grau de défices nas funções executivas
do que outros (Ávila, et al., 2003).
Fases da Doença de Alzheimer
Existem 3 fases na Doença de
Alzheimer com aproximadamente 2 a 3
anos cada. Na fase leve, ou inicial,
ocorre um défice na memória recente e
remota, levando a alterações na
personalidade, estágios de
irritabilidade, hostilidade, apatia e
frustração. Ao nível da comunicação, o
individuo apresenta desordens
respetivas ao conteúdo da linguagem,
associadas a défice no raciocínio
linguístico e disfonia; possui
dificuldades em encontrar a palavra

correta ou lembrar nomes de objetos
ou pessoas. No que concerne as
habilidades visuoespaciais, estas
também se apresentam alteradas, o
paciente faz construções incorretas e
desordenação topográfica. Para além
destas incapacidades, o paciente
também não é capaz de solucionar
problemas, uma vez que se apresenta
confuso e com dificuldade em tomar
decisões. Relativamente ao sistema
motor geralmente encontra-se com um
funcionamento normal, com alguns
sinais extrapiramidais. É de ressaltar
que nesta fase e apesar destas
limitações iniciais, o paciente possui
consciência e perceção das suas
dificuldades fazendo frequentemente
recurso a estratégias para compensá-
las (Azevedo, et al., 2009; Grande,
2013; Thivierg 2013).
Na fase moderada, ou
intermediária, constata-se uma
acentuação do ligeiro défice de
memória e aprendizagem. Como um
maior comprometimento da memoria
verifica-se também mudanças de
personalidade, indiferença, hostilidade,
julgamento social pobre, baixa
afetividade. A comunicação, também
apresenta um comprometimento mais
acentuado relativamente à fase
anterior, sendo possível verificar na
comunicação do paciente uma
desorganização ao nível do conteúdo e
alguns défices estruturais que
prejudicam a coerência. Também se
confirma uma desorientação espacial,
construção pobre e dificuldades
percetivas. O período de fala é mais
fluente, porém menos coerente,
apresentando agitação, derivado ao
sistema motor da fala apresentar
tremores (Azevedo, et al., 2009; Soto,
Andrieu, Gares, Cesari, Gillette-
Guyonnet, Cantet, Vellas &
Nourhashémi, 2015).
Na fase grave, ou final, é
quando se verifica um maior
comprometimento global das funções
intelectuais, levando o paciente a um
estado de dependência total. Neste
momento a sua personalidade mostra-
se totalmente desorganizada.
Relativamente à comunicação esta
encontra-se deteriorada com ecolalias,
perseveração e mutismo. O paciente
encontra-se incapaz de solucionar de
solucionar os seus problemas e de
realizar de forma independente as
atividades de vida diárias (higiene
pessoal, alimentação, entre outras). Ao
nível do tónus muscular apresenta
rigidez na região dos quadris e postura

em flexão, características da síndrome
da imobilização (Azevedo, et al., 2009).
Os pacientes com doença de
alzheimer após um estágio amnéstico
inicial irão começar a apresentar um
défice na atenção dividida, antes
mesmo do comprometimento da
linguagem e das funções viso-espaciais.
Este défice origina dificuldades em
atividades da vida diária apresentadas
pelos pacientes desde o início da
doença. Estas dificuldades manifestam-
se no acompanhamento de conversas
que envolvem mais de uma pessoa, ou
em lugares com muito barulho.
Acredita-se se as mesmas são
decorrentes de défices de função
executiva, como é o caso da atenção
dividida ou da memória operativa. Esta
dificuldade agrava-se
consideravelmente quando a situação
social é complexa ou nova. É de
salientar que alguns pacientes que
ainda estejam na fase inicial da doença
podem apresentar sinais destas
dificuldades (Ávila, et al., 2003;
Démonet, 2013).
No início do percurso da doença
os três subtipos de funções atencionais
(seletiva, dividida e amplitude) não
estão afetados da mesma maneira.
Assim a atenção dividida e aspetos da
atenção seletiva são particularmente
vulneráveis, apresentando-se com
algumas limitações. Porém a amplitude
atencional (memória imediata)
encontra-se relativamente preservada
(Ávila, et al., 2003; Grande, 2013).
No que concerne o processo de
avaliação é de ter em conta que pesar
de a atenção fazer parte das funções
executivas, existem testes específicos
para medirem a atenção, sendo
diferentes dos que avaliam as funções
executivas. Sendo que estas num
momento de avaliação estão mais
relacionadas com provas que requerem
a solução de problemas, com
planeamento, monitoração, estratégia,
e também atenção (Hwang, et al.,
2015).
Com a alteração da volição,
componente das funções executivas,
verifica-se que mais de 70% dos
pacientes com doença de alzheimer
leve apresentam apatia e passividade
sendo que em estas características
estão presentes em 90% dos pacientes
com a doença em estádio grave. Os
estudos neuropsicológicos e de
neuroimagem revelaram associação
destes défices de comportamento com
disfunção do sistema frontal. Nos
mesmos estudos também se observou

que 25% dos pacientes com alzheimer
apresentavam desinibição e
comportamentos sociais inapropriados
(Ávila, et al., 2003).
Mais recentemente, diversos
estudos investigaram a capacidade de
consciência ou insight nestes pacientes,
referente aos seus défices cognitivos.
Esta dificuldade de consciência
relativamente às próprias dificuldades e
limitações, nomeou-se de anosognosia.
Num estudo que visou comparar
dois grupos de idosos, onde um seria
constituído por idosos com alzheimer
no estádio leve e o outro por idosos
sem a doença, verificou-se que o grupo
de idosos com alzheimer apresentou
défices mais acentuados de memória
episódica e também prejuízos viso-
espaciais. Relativamente às funções
executivas, estes apresentaram
prejuízos mais leves no início da
doença. O grupo demonstrou prejuízo
grave das funções executivas e
memória episódica de evocação com
maior preservação de memória de
reconhecimento nos estágios iniciais
(Ávila, et al., 2003; Thivierg, 2013).
Para se diagnosticar a doença de
Alzheimer é necessário que, além do
comprometimento da memória, ocorra
pelo menos mais um défice da função
cognitiva, como linguagem, atenção
seletiva e dividida, e funções
executivas, e essas informações só
podem ser obtidas por meio de
avaliações diretas com o paciente ou
com entrevistas realizadas com o
cuidador (Azevedo, et al., 2009; Cunha,
et al., 2009).
Ainda não existe tratamento
que possa curar ou reverter a
deterioração causada pela doença de
alzheimer. As formas de intervenção
disponíveis visam aliviar os défices
cognitivos e as alterações de
comportamento, através do uso de
medicamentos, e melhorar a qualidade
de vida do paciente e de sua família,
com uma abordagem multidisciplinar.
Com a farmacoterapia, o tratamento da
doença recebeu grande impulso nos
últimos anos, depois da introdução dos
medicamentos anti-colinesterásicos
associados a uma intervenção
terapêutica, levando ao melhoramento
da qualidade de vida e algum progresso
no recardo do declínio cognitivo
(Bottino, Carvalho, Alvarez, Avila,
Zukauskas, Bustamante, Andrade,
Hototian, Saffi & Camargo, 2002).

Reabilitação
neuropsicológica
A reabilitação neuropsicológica
é um processo ativo com o intuito de
capacitar as pessoas com défices
cognitivos causados, por lesão ou
doença, que apresente um bom nível
de funcionamento social, físico e
psíquico. A intervenção para além de
tratar os défices cognitivos, também
sugere tratar as alterações de
comportamento e das emoções que se
encontram em padrões de
funcionamento desadaptado (Filipe,
2009).
A reabilitação neuropsicológica
faz-se por um meio integrado de
tratamentos, cognitivo,
psicoterapêutico (individual e familiar),
medicamentoso e atividades de
inserção social e profissional. Para que
com a implementação da reabilitação
neuropsicológica tenhamos resultados
mais eficazes e eficiente, torna-se
primordial que haja uma avaliação
neuropsicológica com diagnóstico o
mais precocemente possível. Deste
modo, podem ser estabelecidas
medidas capazes de minimizar,
temporariamente estabilizar ou atrasar
a progressão deste processo
degenerativo, como o tratamento
medicamentoso e o neuropsicológico
(Thivierge, Jean & Simard, 2014;
Santos, et al., 2009).
No que concerne a recuperação
de funções cognitivas depende tanto da
plasticidade dos neurónios, ou seja, é a
capacidade do cérebro de recuperar
uma função através de reprodução
neural e interações sinápticas, quanto
de plasticidade funcional, ou seja, é a
capacidade de recuperação de uma
função por meio de estratégias de
comportamento alteradas. É de
salientar alguns fatores que poderão
condicionar esta recuperação, um deles
será a idade, a função envolvida na
perda, mas também por fatores como
comprometimento cerebral bilateral,
presença de co-morbidades, níveis pré-
mórbidos de funcionamento cognitivo
e também outros fatores que
influenciam o individuo,
nomeadamente os fatores sociais,
econômicos e culturais (Ergis, 2012;
Santos, et al., 2009).
Assim, a reabilitação cognitiva
objetiva o restauro funcional, isto é, o
fortalecimento de padrões de
comportamento cognitivo aprendido e

estratégias compensatórias,
representados pelo paciente de novos
padrões que lhe permita lidar com as
disfunções persistentes. O
neuropsicólogo deve para cada
paciente traçar um plano de
tratamento de reabilitação cognitiva
que deverá ser orientado pelo
neuropsicólogo e apoiado por equipa
multidisciplinar. As suas prioridades da
intervenção visam que o paciente
consiga ser independente nas
atividades de vida diária, consiga
melhorar o desempenho acadêmico ou
profissional e consiga desenvolver
competências de habilidades
interpessoais e socioemocionais. É
necessário salientar que, o sucesso da
reabilitação cognitiva depende do
paciente, ou seja, este deverá estar
consciente do benefício de cada
exercício e motivado para o processo
(Santos, et al., 2009).
Reabilitação
neuropsicológica na
Alzheimer
No caso das demências é
indicada a combinação de tratamentos
farmacológicos, reabilitação
neuropsicológica e apoio aos familiares
do paciente. No caso de idosos
saudáveis com queixas quanto à queda
do desempenho de tarefas do dia-a-dia,
estes também podem beneficiar de
treinos cognitivos. Deste modo, será
treinado o uso de auxílios internos e
externos para otimizar a memória e a
atenção, sendo estas as queixas mais
transversais nos idosos (Ávila, et al.,
2003; Oliveira, 2009).
É de salientar que estas
terapias, disponíveis para a doença de
Alzheimer, não impedem o curso da
doença, porém os tratamentos podem
intervir atrasando a manifestação
clínica da mesma. Para
compreendermos as implicações da
doença e da intervenção podemos
recorrer às descobertas feitas pelas
neurociências. A plasticidade cerebral
possibilita um contributo essencial para
a reorganização e evitamento de um
declínio acentuado da doença. A
doença da alzheimer destrói alguns
neurônios, comprometendo assim a
atividade de uma ou mais funções
cognitivas. Porém, o cérebro devido à
sua plasticidade neuronal, tem a
capacidade de reorganizar novas
sinapses em outras regiões para vir a
desempenhar funções parecidas que

eram efetuadas nos locais lesionados,
isso se houver interações entre os
processos fisiológicos e as influências
ambientais. Sendo que o
neuropsicólogo ao estimular o paciente
vai fazer com que existam modificações
no sistema nervoso central,
nomeadamente na produção de
neurónios, permitindo assim verificar
alguns resultados com a intervenção
neuropsicológica. É por existirem estas
modificações estre cérebro e corpo que
se justifica a intervenção
neuropsicológica (Azevedo, et al., 2009;
Oliveira, 2010).
É um tratamento biopsicossocial
que envolve pacientes e familiares
como um todo, onde se propõe ensinar
a pacientes, familiares e/ou cuidadores
estratégias compensatórias,
propiciando a melhoria das funções
cognitivas e da qualidade de vida.
Assim o objetivo da reabilitação
cognitiva é permitir quer aos pacientes,
quer aos familiares conviverem,
lidarem, reduzirem ou superarem as
deficiências cognitivas resultantes de
lesão neurológica aumentando assim a
qualidade de vida. Deste modo, a
reabilitação implica maximizar funções
cognitivas por meio do bem-estar
psicológico e na capacidade de
realização de atividades de vida diária.
Para além destes objetivos a
reabilitação neuropsicológica também
procura a diminuição dos défices que
contribuem para o afastamento e
isolamento social e dependência (Filipe,
2009).
O tratamento multidisciplinar
tem como objetivo complementar o
tratamento farmacológico na doença
de alzheimer. A literatura descreve
grande variedade de métodos de
intervenção para melhorar ou manter o
desempenho cognitivo na doença.
Entre as principais técnicas que
envolvem trabalho multidisciplinar
destacam-se: treinamento cognitivo,
técnica para melhor estruturação do
ambiente, orientação nutricional,
programas de exercícios físicos,
orientação e suporte psicológico aos
familiares e cuidadores (Bottino, et al.
2002).
Antes de se iniciar qualquer
programa de reabilitação, é necessário
definir o perfil cognitivo de cada
paciente, delineando os aspetos da
cognição que estão preservados e os
possíveis deficits existentes. Além
disso, é muito importante adequar o

tratamento proposto ao nível
intelectual e cultural do paciente. Um
programa pessoal de intervenção
comportamental deve atuar sobre
múltiplos fatores, nomeadamente: na
adaptação e modificação do ambiente;
na modificação das interações Sociais;
no tratamento psicoterapêutico; nos
cuidados somáticos; apoio à família e
no tratamento em instituições (Oliveira,
2010).
Um tratamento que possa curar
ou reverter a deterioração causada pela
demência, os tratamentos disponíveis,
atualmente, buscam minimizar
sintomas cognitivos e comportamentais
por meio de medicação e técnicas
cognitivas de reabilitação; melhor
estruturação do ambiente; e também
por meio de grupos informativos para
pacientes e familiares (Bottino, et al.,
2002).
Adaptação e modificação do ambiente
O ambiente onde o idoso vive
deve ser adaptado de modo a que ele
mantenha a máxima autonomia e a
mínima dependência possível. Para tal
algumas modificações podem ser feitas
na sua casa, nomeadamente a
utilização de sinalizações (riscos no
chão e marcas nas paredes), dever-se-á
também atender à iluminação
adequada derivado das modificações
óticas do idoso. A orientação
visuoespacial do idoso também merece
atenção do técnico, pode ser bastante
útil para o mesmo fazer-se uma
desobstrução de passagens, remoção
de degraus, relógios e calendários de
tamanho satisfatório, entre outras
mudanças que sejam pertinentes para
o paciente. Para além do espaço
devemos atender à organização do
vestuário e objetos relativos à sua
higiene pessoal, para que estes sejam
idealizados no sentido de promover um
melhor manuseio dos mesmos. Deste
modo torna-se essencial um programa
personalizado onde sejam ponderadas
todas as alterações a serem corrigidas
no vestuário e na forma como este
deve ser disponibilizado ao doente
(Camões, Pereira & Gonçalves, s/d).
Estratégias compensatórias
As estratégias compensatórias
são também usualmente utilizadas com
estes pacientes. Esta metodologia pode

ser utilizada quando os défices de
memória não podem ser diretamente
enfrentados, podendo assim recorrer-
se a ajudas externas. Neste sentido, o
paciente poderá utilizar agendas,
blocos de notas, despertadores,
sinalizações, para que consiga
contornar os problemas mnésicos
(Santos, et al., 2009). No que concerne
a eficiência desta abordagem
demonstra-se limitada, pois alguns
pacientes apresentam défices em
outras áreas da cognição, que os
impedem de aprender a usar estes os
recursos, nomeadamente a iliteracia.
Contudo, estas estratégias são mais
eficazes quando o paciente ainda está
em fases precoces da doença, ou
quando o cuidador o auxilia no uso dos
recursos (Bottino, et al., 2002).
Modificação das interações sociais
Esta intervenção foca-se na
postura das pessoas para com o idoso,
com o intuito de favorecer sempre a
autonomia, evitar o paternalismo sem
deixar de estar atento às dificuldades
do doente, com o desenvolvimento de
competências no paciente. Uma das
estratégias para promover a autonomia
refere-se à estimulação da atenção a si
próprio, pois permite preservar a
cognição. Nesta estratégia o paciente
será treinado para a realização de
várias atividades, nomeadamente, na
alimentação, na higiene pessoal, em
tomar banho, em se vestir, em pentear-
se entre ostras. A deterioração
cognitiva impede o controlo dos
esfíncteres e o paciente passa a
apresentar incontinências (fecal e
urinária), neste sentido esta
intervenção visa o treino do paciente
através de um programa com exercícios
perineais para fortalecer a musculatura
envolvida na continência fecal e
urinária, mas, como ocorre na maioria
dos casos, não há condições
neurológicas e cognitivas para tais
exercícios. Assim o cuidador assume
um papel relevante tendo que ter
conhecimentos sobre as características
fisiológicas do paciente. (Camões, et al.,
s/d).
Reabilitação da memória
Défices na memória são os
primeiros sintomas de doença de
Alzheimer e também os que causam
maior frustração e incapacidade do
paciente, comprometendo a realização

das atividades de vida diárias e sua
qualidade de vida. Com isso, o paciente
torna-se cada vez menos
independente, aumentando a
necessidade de cuidados dos familiares
e do cuidador (Avila, 2003; Bottino, et
al., 2002).
Uma das principais formas de
reabilitação da memória fundamenta-
se em trabalhar com a modalidade
específica da memória que ainda se
encontra intacta, para esta possa
compensar a modalidade que está
deficitária. Outros métodos têm como
objetivo trabalhar as habilidades
residuais da modalidade de memória
que está deficitária. Assim visa-se
melhorar as capacidades mnésicas do
paciente por meio de técnicas
específicas ou estratégias, e não em
modificar a habilidade que o paciente
possui de memorização. Vejamos o
seguinte exemplo: se um doente tem
maior facilidade para memorizar a
partir de pistas visuais, a reabilitação
não vai treiná-lo a utilizar pistas
verbais, mas ensinar-lhe estratégias
para melhorar sua performance, como
é o caso da utilização de pistas
multissensoriais. Porém, muitas das
técnicas utilizadas pelos idosos tornam-
se ineficientes, quer seja derivado ao
progresso tecnológico e científico, mas
também derivado a muitas recordações
que antes eram acessíveis facilmente,
hoje necessitam ser anotadas (Camões,
et al., s/d).
Terapia Ocupacional
Esta intervenção cognitiva pode
ser realizada de forma individualizada
ou em grupo, é feita com o intuito de
lentificar o processo degenerativo no
estágio inicial, sobretudo em interação
com estratégias medicamentosas. A
terapia individual permite direcionar o
tratamento conforme a necessidade do
paciente, já a terapia em grupo
proporciona um contato entre os
pacientes, o que favorece habilidades
comunicativas. Para que se obtenha um
maior sucesso terapêutico deve-se
fazer um uso combinado das
intervenções. Para além do processo
terapêutico, e não menos importante é
a avaliação, pois permite ao
profissional verificar quais as
componentes que estão deficitários e
quais as que estão preservados,
possibilitando decidir em que direção o
tratamento será realizado. Esta

intervenção é possível fazer-se em
pacientes com prejuízos cognitivos de
leves a moderados, sendo que recorre
a estratégias compensatórias, uma vez
que os pacientes ainda têm
capacidades para aprender (Corrêa &
Silva, 2009).
Terapias de Orientação da
Realidade
A terapia de orientação da
realidade tem como princípio expor
dados de realidade ao paciente de
forma organizada e sistemática, com o
intuito de criar estímulos ambientais
que facilitem a orientação e levando
em consideração que a realidade não
consiste apenas em orientação
temporal. Assim esta terapia tenta-se
comprometer com o indivíduo no
trabalho de adaptação às interações
sociais e também para o melhoramento
da comunicação através linguagem
clara ou não verbal. Visa também o
treino de habilidades cognitivas,
proporcionando atividades adequadas
às suas dificuldades. Os pacientes que
beneficiam desta estratégia
apresentam melhora significativa na
orientação verbal, na atenção e
interesse no ambiente e no
desempenho em escalas de interação
social e funcionamento intelectual
(Bottino, et al., 2002).
Reeducação comportamental ativa
Esta estratégia é idêntica à
apresentada à anterior, tratando-se
também de uma forma de orientação
da realidade. O primordial objetivo é
que o paciente consiga se adaptar e
uma consiga uma maior autonomia,
permitindo uma diminuição dasua
sensação de inferioridade e
dependência. Esta estratégia visa
promover uma intensificação das
interações sociais e objetais no
ambiente para se conseguir o máximo
de independência do paciente. Num
momento inicial dever-se-á fazer um
inventário das atitudes que se deseja
modificar. Posto isto deve-se investigar
as modificações ambientais e pessoais
de modo a obter os objetivos finais.
Com o decorrer da terapia visa-se que o
paciente aumente a troca verbal, a
frequência de contactos sociais e o
incremento da autonomia da marcha e
mobilidade (Camões, et al., s/d).

Terapia de Reminiscência
A terapia de reminiscência, que é uma
variação da terapia de orientação para
a realidade, tem como objetivo
estimular o resgate de informações por
meio de figuras, fotos (livro de
memórias), músicas, jogos e outros
estímulos relacionados à juventude dos
pacientes. Essa técnica tem sido muito
utilizada para resgatar emoções vividas
previamente, gerando maior
sociabilização e entretenimento como
parte da terapia (Bottino, et al., 2002).
Intervenção com as famílias dos pacientes com alzheimer
A intervenção com as famílias é
de extrema importância, pois a
qualidade de vida de pacientes com
demência depende, daqueles que são
responsáveis por seu cuidado. Deste
modo, a maioria das abordagens
terapêuticas inclui trabalhos com
grupos de familiares e cuidadores. Esta
terapia visa ajudar a família a lidar
melhor com sua sobrecarga emocional
e ocupacional gerada pelo cuidado
intensivo. O plano de intervenção com
a família deverá assentar nos seguintes
pontos: informações sobre a doença e
os recursos disponíveis na comunidade;
aconselhamento sobre como lidar com
situações do dia-a-dia e a possibilidade
de trocar experiências e falar de seus
sentimentos com pessoas que vivem
situações semelhantes (Bottino, et al.,
2002; Turró-Garriga, Soler-Cors, Garre-
Olmo, López-Pousa, Vilalta-Franch, &
Monserrat-Vila, 2015).
Conclusão
Há múltiplos fatores associados
ao processo de envelhecimento,
nomeadamente fatores moleculares,
celulares, sistêmicos,
comportamentais, cognitivos e sociais.
Estes interagem e regulam tanto o
funcionamento típico quanto o atípico
do indivíduo que envelhece. É
fundamental que o neuropsicólogo/
psicólogo, assim como os próprios
idosos, os seus familiares e cuidadores
tenham uma visão integrada destes
fenômenos.
Muitos avanços têm sido feitos
no sentido de apoiar medidas que
proporcionem um funcionamento
saudável nesta faixa etária, bem como
de intervir nos fenômenos associados
ao envelhecimento patológico, como é

associada ao tratamento
farmacológico.
A reabilitação neuropsicológica
é um tratamento biopsicossocial que
envolve os pacientes e seus familiares
levando em conta as alterações físicas e
cognitivas dos pacientes, o ambiente
em que vivem, os fatores subjetivos e a
sua biografia. A reabilitação
neuropsicológica é um processo ativo
que visa capacitar pessoas com défices
cognitivos causados por lesão ou
doença, para que essas adquiram um
bom nível de funcionamento social,
físico e psicológico. Dessa maneira, a
reabilitação visa maximizar as funções
cognitivas bem como o bem-estar
psicológico, da habilidade em
atividades de vida diária e do
relacionamento social. Além disso,
também busca a diminuição dos défices
que ocasionam afastamento e
isolamento social, dependência e
discriminação.
Em doentes com demência, as
melhorias conseguidas são
temporárias, pois as alterações das
áreas cerebrais que suportam as
funções cognitivas são progressivas e
os resultados de qualquer intervenção
terapêutica serão cada vez menos
eficazes, o que não significa que a
intervenção não seja necessária, mas é
importante sabermos as limitações e
que existem questões por responder,
como por exemplo por quanto tempo o
paciente vai conseguir manter aquela
função.
Antes de se iniciar qualquer
programa de reabilitação, é necessário

tratamento proposto ao nível
intelectual e cultural do paciente. Um
programa pessoal de intervenção

adaptação e modificação do ambiente;
na modificação das interações sociais;
no tratamento psicoterapêutico; nos
cuidados somáticos; apoio à família e
no tratamento em instituições. O
desenho do programa de intervenção
deverá ser explicado ao doente e aos
familiares, bem como os seus objetivos.

alguns resultados com a intervenção
neuropsicológica e a sua pertinência. É
por existirem estas modificações estre
cérebro e corpo que se justifica a
intervenção neuropsicológica.
Algumas das técnicas que
podem ser utilizadas para intervir na
estimulação da memória com idosos
poderão ser: dizer 3 palavras (bola,
avião, casa) pedir ao paciente que as
decorre, durante alguns minutos falar
com ele e depois pedir que as tente
recuperar. Para o treino da memoria é
bastante comum, levar o nome de uma
música que consideramos que o
paciente a deve ter ouvido quando era
mais novo, vamos treinar com ele a
recuperação dessa memória. Numa
fase inicial da doença é possível
trabalharmos com o paciente para que
este decorre uma musica nova.
Também poderá ser trabalhado com
ele, pois nem todos gostam de músicas,
as suas histórias de vida, tendo em
consideração que estas podem ser
memórias falsas. É de salientar também
que quando tentamos trabalhar a
estimulação da memória podemos
evocar memórias negativas e o
terapêutico deve ter disponibilidade
para abordar essas emoções negativas.
Para além dessas que referi, mas
menos dinâmicas para se trabalhar com
os idosos, poderá ser contar uma
história e depois pedir que seja ele a
contar a história. Também podemos
dizer 10 palavras que ele deverá
decorar e ao nosso sinal poderá dize-
las. Relativamente a esta última tarefa
existem formas diferentes de procurar
decorar as palavras, contudo a
reabilitação no idoso, não visa o
ensinamento dessas estratégias, mas
sim a estimulação da habilidade /
competências para memorização.

Resumo: O presente trabalho retrata a esquizofrenia e algumas abordagens da
reabilitação neuropsicológica desta perturbação. Primeiramente encontra-se uma breve
descrição do que é a reabilitação neuropsicológica, dando ênfase ao conceito de
plasticidade cerebral. Em seguida, surge uma definição de esquizofrenia e,
posteriormente, as abordagens de reabilitação em esquizofrenia, tendo como principal
enfoque a funcionalidade do sujeito. Deste modo, o presente artigo faz alusão a
conceitos como a cognição social e a intervenções de restruturação/remediação e de
compensação, bem como treino de habilidades sociais em esquizofrénicos e estratégias
compensatórias. Tendo em conta as alterações que tal patologia provoca na estrutura e
organização familiar é também pertinente abordar o papel da família/cuidadores.

Abstract:
This work depicts schizophrenia and some approaches to neuropsychological
rehabilitation of this disorder. First we present is a brief description of what is
neuropsychological rehabilitation, emphasizing the concept of brain plasticity. Then
comes a definition of schizophrenia and later rehabilitation approaches in schizophrenia,
the main approach to the subject functionality. Thus, this article refers to concepts such
as social cognition and interventions of restructuring / remediation and compensation,
as well as social skills training in schizophrenic and compensatory strategies. Taking into
account the changes that this disease causes in the family structure and organization is
also relevant to address the role of the family / caregivers.

Resumen: Este trabajo representa la esquizofrenia y algunos enfoques para la
rehabilitación neuropsicológica de este trastorno. En primer lugar se presenta una breve
descripción de lo que es la rehabilitación neuropsicológica, haciendo hincapié en el
concepto de plasticidad cerebral. Luego viene una definición de la esquizofrenia y la
rehabilitación después se acerca en la esquizofrenia, el enfoque principal para la
funcionalidad del sujeto. Por lo tanto, este artículo se refiere a conceptos tales como la
cognición social y las intervenciones de reestructuración / remediación y compensación,
así como el entrenamiento en habilidades sociales en las estrategias compensatorias de
esquizofrénicos. Teniendo en cuenta los cambios que esta enfermedad provoca en la
estructura familiar y la organización también es relevante para abordar el papel de los
familiares / cuidadores.

Introdução
Reabilitação neuropsicológica
De acordo com Wilson, 2003 (cit in. Rodrigues,
2013), a reabilitação neuropsicológica pode ser
definida como um conjunto de intervenções que
têm como objetivo proporcionar uma melhoria dos
processos cognitivos, emocionais e sociais de
qualquer indivíduo que tenha sofrido lesão
encefálica ou que seja portador de alguma
perturbação psiquiátrica. Esta reabilitação
possibilita uma consciencialização do paciente a
respeito das suas capacidades remanescentes, o
que conduz a uma alteração na auto-observação
(D’Almeida et al., 2004 cit. in Pontes & Hübner,
2008), tratando-se, assim, de um processo ativo que
visa a melhoria do funcionamento social, físico e
psiquico (Mc Lellan,1991 cit. in Ávila, 2003).
Deste modo, a reabilitação implica a
maximização de funções cognitivas, através do
bem-estar psicológico, da habilidade de atividades
de vida diárias e do relacionamento social (Clare &
Woods, 2001 cit. in Ávila, 2003), procurando a
diminuição dos défices que levam ao afastamento e
isolamento social, dependência e discriminação
(Kitwood, 1997 cit. in Ávila, 2003). E, por isso, a
avaliação na reabilitação cognitiva deve incluir
tanto medidas neuropsicológicas quanto
comportamentais para identificar vantagens e
debilidades cognitivas e definir as dificuldades de
vida diária. Significa, então, desenvolver as áreas
básicas da função mental: atenção, linguagem,
memória, capacidade viso-espacial (Camara et al.,
2009), objetivando-se com a estimulação cognitiva
o resgate das funções comprometidas, o estímulo
das áreas bloqueadas e o desenvolvimento de

potenciais remanescentes do paciente (Camara et
al., 2009).
Considera-se, portanto, a reabilitação
neuropsicológica como um tratamento
biopsicossocial que envolve os pacientes e os seus
familiares considerando as alterações físicas e
cognitivas dos pacientes, o ambiente em que vivem,
os fatores subjetivos e a sua biografia (Ávila, 2003),
sendo mais ampla do que a reabilitação cognitiva,
uma vez que além do interesse em melhorar as
capacidades cognitivas, também enfatiza os aspetos
emocionais, psicossociais, comportamentais e
físicos, que possam estar deficitários após a lesão
cerebral (Wilson, 2008 cit. in Gindri et al., 2012).
Mesmo face a esta diferença conceitual e de
alcance, muitas vezes reabilitação neuropsicológica
e cognitiva são consideradas sinónimas.
Face ao exposto, existem dificuldades em
desenvolver estratégias padronizadas para cada
perturbação neuropsicológica, devido à diversidade
da população afetada e às características dos
défices observados (Wilson, 1991 cit in Hamdan &
Riechi, 2011). Assim sendo, a reabilitação
neuropsicológica necessita de uma larga base
teórica, uma vez que não existe um modelo ou
teoria que abranja as mais variadas perturbações
neurológicas e neuropsiquiátricas (Abrisqueta-
Gomez, 2006 cit. in Pontes & Hübner, 2008).
Plasticidade cerebral
Todo o processo de reabilitação neuropsicológica
fundamenta-se na crença de que o cérebro humano
é um órgão dinâmico e adaptativo, capaz de se
reestruturar em função de novas exigências
ambientais ou das limitações funcionais impostas
por lesões cerebrais, (Kandel, 1998, 1999 cit. in
Haase & Lacerda, 2003). Assim, quando falamos em
reabilitação psicológica é fundamental abordar os
conceitos de plasticidade cerebral e de princípio da
transferência (Meinzer et al, 2008 cit.in Rodrigues,
2013), uma vez que é a plasticidade neuronal o
fundamento científico da reabilitação (D’Almeida,
Pinna, Martins, Siebra & Moura, 2004 cit. in Maia,
Correia, & Leite, 2007). Esta diz respeito à
capacidade do cérebro para se remodelar em
função das experiências do sujeito, reformulando as
suas conexões em prol das necessidades e dos
fatores ambientais (Meinzer et al, 2008 cit.in
Rodrigues, 2013), o que explica o facto de certas
regiões do cérebro poderem substituir as funções
afetadas por lesões cerebrais (Mateer e Kerns, 2000
cit.in Rodrigues, 2013). A recuperação da função
perdida depende de alguns fatores, como a idade
do paciente, a área lesionada, o tempo de
exposição aos danos, a natureza da lesão, a
quantidade de tecidos afetados, os mecanismos
de reorganização cerebral envolvidos, bem como
os fatores psicossociais e ambientais (Rodrigues,
2013). Dados de algumas pesquisas (e.g., Gauggel,
Konrad, Wietasch, 1998, Kertesz, 1993, Meier,
Strauman e Thompson, 1987 cit. in Haase &
Lacerda, 2003) indicam que, de um modo geral, o
sexo feminino, a juventude, o nível educacional, o
estatuto sócio-económico, a menor severidade da
lesão, a preservação da capacidade de insight, a
ausência de transtornos psicopatológicos ou adição
a álcool/ drogas etc., são fatores correlacionados
com melhor sucesso após lesão cerebral adquirida.
De acordo com Dennis (2000, cit.in Haase &
Lacerda, 2003) a neuroplasticidade pode ser
concebida e avaliada a partir de uma perspetiva

estrutural (configuração sináptica) ou funcional
(modificação do comportamento). E, segundo Kleim
e Jones (2008 cit. in Gindri, 2012), existem dez
princípios da plasticidade cerebral relacionada à
experiência:
1. A utilização das redes neurais é essencial para a
manutenção da funcionalidade.
2. O treino de determinada função cerebral
provavelmente irá fortalece-la.
3. A plasticidade cerebral pode ser restrita à função
treinada, dependendo da abordagem terapêutica
utilizada.
4. O treino repetido de tarefas é necessário para
que a plasticidade ocorra.
5. A intensidade do treino deve também ser
observada para a indução da plasticidade.
6. O tempo é uma variável importante na
plasticidade cerebral, pois a intensidade e a duração
do resultado comportamental e do que acontece
fisiologicamente no processo de plasticidade é
maior quanto mais próximo do período pós-lesão
for iniciada a reabilitação cognitiva.
7. O programa de reabilitação deve ser envolvente,
ou seja, deve ter significado para o paciente, para
que as informações sejam mais bem codificadas.
8. A idade do indivíduo em processo de reabilitação
tem influência na plasticidade, sendo que quanto
mais jovem o indivíduo, mais facilitada será a
estimulação da plasticidade cerebral.
9. O fenómeno da transferência ocorre quando a
estimulação de certo substrato neural promove
melhoras em outras vias neurais, ou seja, quando o
treinamento de uma habilidade funciona como um
facilitador para a melhora de outras habilidades, ou
seja, quando um domínio cognitivo, tarefa ou
habilitação não treinada melhora como o resultado
da reabilitação noutro domínio, tarefa ou
habilidade (Dahlin et al., 2009 cit in Rodrigues,
2013).
10. O fenómeno da interferência é caracterizado
quando algumas áreas do cérebro são estimuladas a
partir da interação com o ambiente ou de maneira
artificial e impedem que outras áreas se beneficiem
do processo (Kleim & Jones, 2008 cit. in Gindri,
2012).
Estes princípios vêm reforçar a diferenciação entre
plasticidade cerebral positiva e negativa. Segundo
Vance e al. (2010, cit.in Rodrigues, 2013) a
neuroplasticidade positiva ocorre quando as
mudanças morfológicas e funcionais cerebrais
levam ao aumento da reserva cognitiva. Já, a
neuroplasticidade negativa ocorre quando tais
mudanças cerebrais adaptativas conduzem à
diminuição da reserva cognitiva. Sendo que,
entende-se por reserva cognitiva o conjunto de
recursos de processos cognitivos e redes neurais
subjacentes ao desempenho em tarefas que
possibilitem ao indivíduo, após uma lesão cerebral,
melhor manejar as consequências e possíveis
limitações decorrentes do acometimento
neurológico (Stern, 2009 cit. in Rodrigues, 2013),
inúmeros fatores contribuem para a reserva
cognitiva, tais como alta e qualificada escolaridades,
alta frequência de hábitos de leitura e escrita,
sistemática e prolongada realização de atividades
físicas, entre outros.
É importante salientar que nem sempre é possível
restaurar a função cognitiva prejudicada, mas é
possível compensá-la, encontrando maneiras de

minimizar os problemas quotidianos (Pontes &
Hübner, 2008).
Esquizofrenia
Considera-se a esquizofrenia como um distúrbio
psiquiátrico sério, clinicamente caracterizado por
experiências e crenças anormais, distúrbios
emocionais, afetivos e comportamentais e
funcionamento social debilitado (Moller & von
Zerssen, 1995 cit. in Pantelis, Stephen & Maruff,
2002). Pacientes esquizofrénicos apresentam
alterações no desempenho em uma grande
variedade de testes neuropsicológicos, sendo que
diversos estudos clínicos sugerem que as alterações
cognitivas podem ser observadas desde o início dos
sintomas da esquizofrenia (Meltzer, 1996 cit. in
Adad, Castro & Mattos, 2000).
Estudos sugerem que pacientes com esquizofrenia
apresentam alterações mais pronunciadas em
relação a determinados setores do funcionamento
cognitivo. Os processos de atenção e memória, bem
como as funções executivas, parecem ser afetadas
de forma diferenciada em grande parte dos
indivíduos acometidos pela doença (Gray,
Feldon,Rawlins, Hemsley & Smith cit. in Adad et al.,
2000).
No que concerne à sua origem, a esquizofrenia é
considerada uma doença multifatorial, marcada por
numerosos e múltiplos mecanismos fisiopatológicos
primários, decorrentes da interação de diversos
fatores genéticos e ambientais. Esta confluência de
fatores acaba por afetar o sistema nervoso central
(Gejman & Sanders, 2012; Minzenberg & Carter,
2012; Elissalde et al., 2011; Kulkarni, Hayes &
Gavrilidis, 2012; Rajji, Ismail & Mulsant, 2009 cit. in
Araújo, 2012).
Os défices apresentados por pacientes
esquizofrénicos, alguns já presentes antes da
eclosão da doença, parecem constituir um dos
principais determinantes da funcionalidade dos
indivíduos. Mais recentemente, têm sido descritos
défices da cognição social, que pode ser entendida
como uma dimensão separada e independente de
neurocognição ou cognição não social, podendo
constituir uma variável mediadora entre a
neurocognição e a funcionalidade (Vaz-Serra, Palha,
Figueira, Peixoto, Brissos, Casquinha, Damas-Reis,
Ferreira, Gago, Jara, Relvas & Marques-Teixiera,
2010).
Assim, os défices no domínio da cognição social em
doentes com esquizofrenia traduzem uma
insuficiência social tão marcada e relevante que,
para alguns autores, tais défices definem a
esquizofrenia (Brune, Abdel-Hamid, Lehmkamper &
Sonntag, 2007 cit in. Vaz-Serra, Palha, Figueira,
Peixoto, Brissos, Casquinha, Damas-Reis, Ferreira,
Gago, Jara, Relvas & Marques-Teixiera, 2010). Os
domínios mais frequentemente afetados são: a
resolução de problemas cognitivo-sociais, as
aptidões sociais e as aptidões de comunicação.
Estima-se que cerca de 1% da população mundial
sofra de perturbação esquizofrénica, todavia, hoje
sabe-se que o paciente pode ser autónomo por
muitos anos se for submetido a tratamento
medicamentoso, psicológico, familiar e relacional
(Salgado, 2008 cit. in Moreira, 2012).
Reabilitação na esquizofrenia
A APA (Associação Psiquiátrica Americana) resume
os passos para o tratamento de pacientes com
esquizofrenia da seguinte forma: Estabelecer e
manter a aliança terapêutica; Monitorar e

acompanhar o paciente, prestando atenção aos
sintomas prodrómicos de recaída; Promover
educação sobre a esquizofrenia e seu tratamento;
Determinar a necessidade de medicação e de
intervenções psicossociais, e elaborar um plano de
tratamento; Reforçar a adesão ao plano de
tratamento; Incentivar a compreensão e a
adaptação psicossocial, e buscar uma adaptação
social compatível para cada caso; Ajudar a
reconhecer precocemente as recaídas, promover as
mudanças no tratamento e identificar fatores que
precipitam ou perpetuam os surtos; Empregar
esforços para aliviar o stress familiar e melhorar o
funcionamento familiar; Facilitar o acesso do
paciente aos diversos serviços e coordenar os
recursos destinados à saúde mental (APA, 1997 cit
in. Shirakawa, 2000). Portanto, além do tratamento
psicofarmacológico, os tratamentos psicossociais
são imprescindíveis na abordagem global do
paciente com esquizofrenia. Vários programas têm
sido desenvolvidos e avaliados para atenuar os
défices cognitivos da esquizofrenia. Esses
programas empregam métodos variados, como
exercícios práticos, treinamentos realizados em
computador, estratégias pedagógicas, estratégias
compensatórias e grupos de discussão (McGurk,
Mueser, Feldman, Wolfe & Pascaris, 2007;
Dickinson, Tenhula, Morris, Brown, Peer, Spencer,
et al. 2010).
Ao longo de várias décadas, o prejuízo funcional da
esquizofrenia tem sido tratado por intermédio de
programas de reabilitação psicossociais (Kurtz et al.,
2001 cit. in Monteiro & Louzã, 2007), no entanto, os
resultados nesta área têm sido pouco expressivos,
apontando para a possível existência de algum
outro fator a interferir nos efeitos dessas
intervenções. Atualmente, os défices cognitivos são
reconhecidos como fatores centrais desses
resultados. Considerando o fato de que estes
programas são baseados nos modelos de
aprendizagem e de que os défices cognitivos
limitam a aquisição de novas informações e
habilidades, pode-se supor que quanto mais graves
forem os défices cognitivos, principalmente aqueles
relacionados à atenção, à memória e às funções
executivas, menor é a probabilidade dos pacientes
com esquizofrenia tirarem beneficio de tais
programas. Além disso, os sintomas negativos estão
associados ao menor benefício das intervenções em
reabilitação psiquiátrica (Sharma e Antonova, 2003
cit. in Monteiro & Louzã, 2007). Estudos
experimentais com ênfase no treino cognitivo
sistemático têm revelado alguns resultados
positivos com pacientes esquizofrénicos (Kurtz et
al., 2001 cit in. Monteiro & Louzã, 2007).
Resultados em estudos de reabilitação cognitiva
em três áreas: funções executivas, atenção e
memória indicaram que os pacientes obtiveram
melhora na execução do teste de classificação de
cartas (Wisconsin Card Sorting Test), observada por
intermédio das mensurações de erros
perseverativos, categorias completas e nível
conceitual de respostas, tendo como base as
intervenções específicas, como instruções
ampliadas e reforço das contingências. Melhoria da
habilidade atencional e da memória foi observada
por meio do treino de sustentação da atenção e
procedimentos de codificação. Além disso, o treino
dessas habilidades levou a uma melhora do

reconhecimento dos sinais sociais (Monteiro &
Louzã, 2007).
Estratégias Compensatórias
O modelo compensatório comumente utilizado na
reabilitação médica tem sido considerado o mais
apropriado para minimizar o impacto dos défices
cognitivos na vida diária (Bellack et al., 1999).
Embora a restauração da função (ou parte da
restauração) seja possível após acometimentos
neurológicos, na maioria dos casos de défices
cognitivos, a reabilitação não alcança essa meta.
Nestes casos, Wilson (2000 cit in. Gindri, 2012)
indica que se ensine ao paciente o uso de
estratégias e técnicas compensatórias como
alternativa para alcançar objetivos, com a finalidade
de melhorar o desempenho de comportamentos
que estejam dependentes de funções cognitivas
deficitárias. O paciente passa, então, a ser
beneficiado por recursos externos a sua cognição,
que para atuarem recrutam também funções mais
preservadas do encéfalo lesado. O uso de
estratégias compensatórias é uma técnica
amplamente utilizada na reabilitação
neuropsicológica, atuando principalmente nos
défices mnemónicos, atencionais e executivos.
Inclui a utilização de aparelhos eletrónicos,
agendas, blocos e quadros de anotações,
calendários, placas de sinalização, entre outros.
Essa técnica torna possível que pacientes que
apresentam défices cognitivos para aprender e
relembrar informações consigam novamente
organizar suas atividades diárias (Sohlberg;
McLaughlin; Pavese; Heidrich & Posner, 2000 cit in.
Gindri, 2012; Wilson, Emslie, Quirk & Evans, 2001).
Intervenção de Restituição/Remediação
Nos programas de reabilitação cognitiva para a
esquizofrenia, é fundamental estabelecer a
diferenciação entre intervenções de
restituição/remediação e intervenções de
compensação (Vieira, 2013).
Nas intervenções de restituição/remediação, os
sujeitos aprendem a executar tarefas que já faziam
antes, de forma semelhante ou não, com o
propósito de otimizar a forma como reproduzem
essas atividades (Abrahamson & Lazaroff, 2007 cit
in. Vieira, 2013). Aqui inclui-se intervenções como a
Terapia Psicossocial Integrada, Cognitive
Enhancement therapy, Neurocognitive
Enhancement Therapy, Treino de Remediação
Cognitiva, Neuropsychological Educational Aproach
to Rehabilitation, Attention Process Training,
Attention Shaping Procedure e Thinking Skills for
work program.
Intervenção de Compensação
Nas intervenções de compensação, os sujeitos
aprendem estratégias para poderem realizar
atividades de uma forma alternativa ao habitual,
com a finalidade de produzirem comportamentos
similares aos dos sujeitos saudáveis, de forma
inconsciente e automática. Estas estratégias não
têm a finalidade de restaurar um défice cognitivo
mas sim compensá-lo ou superá-lo, por meio das
competências cognitivas intactas ou suportes
ambientas (Palha & Marques-Teixiera, 2006;
Krabbendam & Aleman, 2003 cit in. Vieira, 2013).
Destacam-se enquanto estratégias para a
intervenção em esquizofrenia, o Errorless Learning
e o Cognitive Adaptation Training (Abrahamson &
Lazaroff, 2007 cit in. Vieira, 2013).

Para Caballo (2006 cit in. Felipe, 2009) o
comportamento socialmente hábil refere-se a
conteúdos, que são expressões do comportamento,
e suas consequências, que são os reforços sociais.
Espera-se que este tipo de comportamento gere
mais reforço positivo que castigo. Assim, torna-se
importante avaliar o comportamento das pessoas e
as reações que seu o comportamento gera no
outro. Sendo assim, o comportamento socialmente
hábil é um conjunto de comportamentos de um
indivíduo num contexto interpessoal que expressa
sentimentos, atitudes, desejos, opiniões ou direitos
desse indivíduo de modo adequado a situação,
respeitando esses comportamentos nos demais, e
que geralmente resolve os problemas imediatos da
situação enquanto minimiza a probabilidade de
futuros problemas.
Dentro das intervenções terapêuticas desenvolvidas
para facilitar o processo de reinserção social de
sujeitos esquizofrénicos, destaca-se o treino de
habilidades sociais, que tem como objetivo
aumentar a competência social em habilidades
verbais e não-verbais presentes numa interação
social (Wallace, Liberman, MacKain, Blackwell &
Eckman, 1992 cit in. Scemes, 2012), partindo do
pressuposto que as habilidades sociais e a
competência social funcionam como fatores
protetores contra o stress e promovem o aumento
da resiliência, da interação social e da qualidade de
vida (Kopelowicz; Liberman & Zarate, 2006). Deste
modo, as habilidades são a matéria-prima da
competência social e abrangem toda a gama de
performances socias humanas: verbais, não-verbais
e comportamentos paralinguísticos (Kopelowicz,
Liberman, & Zarate, 2006), que definem a nossa
competência como ser humano pela capacidade de
nos comportarmos de forma adequada em
situações sociais (Lucca, 2004 cit in. Felipe, 2009).
Alguns itens importantes para o treino das
habilidades sociais passam por: iniciar, manter e
encerrar conversação, fazer pedidos, responder a
pedidos, pedir a alguém para mudar um
comportamento indesejável, responder a críticas e
fazer e receber elogios (Caballo, 2003; Falcone,
2002; Del Prette & Del Prette, 2001ª cit in. Felipe,
2009).
Bandeira, Machado e Pereira (2002 cit in. Felipe,
2009) clarificam que no processo de reintegração
social de pessoas esquizofrénicas, quando
realizados programas de treino em habilidades
sociais, aumenta a autonomia o que contribui para
redução da sobrecarga nos familiares cuidadores.
De acordo com Tarrier (2005 cit in. Felipe, 2009) há
evidencias que defendam a eficácia da intervenção
familiar na redução de recaídas na esquizofrenia
e aumento de habilidades e motivação para
enfrentar posteriores problemas. Posto isto,
verifica-se a importância de uma análise funcional
para uma intervenção se uma vez que, a análise
funcional permite identificar o que mantém o
comportamento e assim, alterar as condições que o
influencia (Felipe, 2009).
Papel da família/cuidadores
Conhecer os significados atribuídos à esquizofrenia
e à atenção pelos cuidadores é fundamental para
que se possam adotar estratégias em saúde que
considerem as singularidades, as biografias e as
raízes culturais destes sujeitos (Almeida, Schall,
Martins & Modena, 2010). A esquizofrenia, provoca
uma rutura na organização da dinâmica familiar,

fazendo com que a pessoa enferma e seus
cuidadores repensem suas atividades diárias, papéis
e conceitos (Gomes, Mendonça e Pontes, 2002).
Nesse momento é recorrente que os cuidadores
experienciem situações novas, levando a uma
reorganização das formas de pensar, sentir e agir. O
quotidiano do cuidado e a interferência nas
atividades e na vida do cuidador trazem uma
sobrecarga que pode assumir diferentes graus de
acordo com as variáveis envolvidas na relação
cuidador/paciente (Barroso, Bandeira e
Nascimento, 2007; Grandón, Jenaro e Lemos, 2008
cit in. Almeida, Schall, Martins & Modena, 2010).
As intervenções definidas como programas de
tratamento psicopedagógico das famílias têm a
finalidade de diminuir as tensões presentes no
ambiente familiar e de melhorar o funcionamento
social do portador, assim têm os seguintes objetivos
principais: Reduzir o custo da doença (por exemplo,
uma família bem orientada contribui para reduzir as
possibilidades de recaídas e reinternações);
Orientar a família sobre o programa
medicamentoso e de atividades da vida diária;
Ajudar o portador a colaborar com o tratamento e a
assumir responsabilidades, dentro do possível;
Verificar se está a haver resposta ao tratamento e
comunicar as mudanças que surgirem; Estar
informado sobre os sinais e os sintomas precoces
de recaídas; Encorajar o portador a tornar-se
independente, para suprir as necessidades básicas;
Familiares muito exigentes devem ser orientados a
serem menos exigentes; Familiares ausentes devem
ser orientados para serem mais participantes
(Shirakawa, 2000).
Considerações Finais
Tendo em conta que a esquizofrenia é considerada
uma patologia clínica grave, que apresenta
sintomas que afetam a vida do sujeito, a
intervenção neuropsicológica passa por ajudar o
paciente a tornar-se mais funcional. As
intervenções focam-se, além do tratamento
farmacológico, em programas psicossociais que
auxiliem o sujeito a melhorar as suas capacidades
nos relacionamentos interpessoais, bem como
resolução de problemas. Visa-se, assim,
fundamental, o uso de estratégias compensatórias
que possam permitir ao paciente um quotidiano
mais próximo do normativo possível. O papel dos
cuidadores é fundamental neste processo, dado à
incapacidade de funcionalidade de alguns
pacientes. É de ressaltar que, tendo em conta a
existência de resultados contraditórios em alguns
estudos presentes na literatura, o processo de
reabilitação tem de ter sempre em conta as
particularidades do sujeito e do seu ambiente.
