Affinity Trust (SCORE) Innovative opportunities service for adults with learning disabilities Run by national charity Affinity Trust, SCORE, is based in Hereford and supports adults with a learning disability to have the opportunity to make friends and keep active. We offer community activities such as drama, fitness classes, swimming and photography. We consult with the people we support about new activities, and actively seek out new organisations with which to develop exciting projects.

Affinity Trust supports over 650 people around the UK to live in their own homes and get out into the community as happily and independently as possible.

Halo Leisure At the helm of fitness and leisure in Herefordshire. Halo Leisure provides leisure services for its major partner Herefordshire Council. It has centres in Hereford, Ledbury, Ross, Leominster and Bromyard.

Working with GPs and other health care professionals, Halo also delivers the LIFT referral scheme. LIFT stands for Lifestyle Improvements for Today and is Herefordshire’s Exercise Referral Scheme.

The Haven Since 2004 we have provided emotional and practical support for people who have had a diagnosis of breast cancer (at any time), and also their families. We offer a free programme including counselling, Breast Care Nurse, specialist therapists, support groups and activity groups (more details on our website – address below). Herefordshire Citizens Advice Bureau Herefordshire Citizens Advice Bureau provides free, independent, confidential and impartial advice to everyone on their rights and responsibilities in relation to the following: Welfare Benefits, Finance, Money and Debt, Tax, Employment, Family/Relationships, Immigration, Housing, Consumer Utilities, Education, Health, Travel The bureau is currently running a pilot project with GP surgeries and health professionals to refer patients for advice.

Herefordshire Joint Visiting Team We provide a visiting service for older and vulnerable people who need help with claiming benefits or paying for care services. Herefordshire Signposting Team 'The award winning Signposting Scheme is all about connecting vulnerable people to services quickly, to improve and help prevent the need for more costly interventions at a later stage. Herefordshire Vision Links HVL is an independent local charity supporting people with sight loss in Herefordshire. Our aim is to provide early help and long term support for the blind and partially sighted community of Herefordshire that can make living with sight loss a little easier. Locally HVL is part of an organisation developing the UK Vision Strategy and implementing a plan locally - 2009 – 2014 for Herefordshire.

Herefordshire Voluntary Organisations Support Service (HVOSS)

HVOSS is a new generalist and specialist organisation to meet the support needs of the voluntary and community sector across Herefordshire. Home-Start Herefordshire Is an independent charity which offers support, friendship and practical help to young families help to families with children 0-19 years of age and who are under stress and finding it hard to cope. What does Home-Start do? Home-Start recruits and trains volunteers, who are all parents themselves, to visit families regularly at their home each week spending 2 to 3 hours with them. Our volunteers are from all walks of life. They provide a listening ear so that parents can talk about their problems and difficulties and explore ways to resolve them. Some families need more practical help which we can offer as well.

Services for Independent Living Services for Independent Living (SIL) is a user led organisation providing services that enable disabled and older people to live independent lives in their own homes within their own community.
