particular ward type. It is my hope to be able to use this to inform the ideal equipment ratio per bed for each type of ward and then compare this with my own hospital to determine whether or not we are over or underequipped. I am not interested in being able to identify any medical equipment and require only numbers of particular equipment types. It is likely that terminology may be differ so please interpret these are broad headings thatencapsulate most of the equipment within the particular ward type.I do not require information from other areas of your hospital, just in the ward types that I have listed. I have made a spreadsheet for you to complete with the information to return if that makes the reporting easier.

What I would like is a total inventory count of the amount of each Equipment Type you have for all of your wards in the following categories:• General Medical Ward• General Surgical Ward• Paediatric Ward Please do not include any other areas such as Critical Care, Theatres, SCBU. Please record the number of total number of beds in each Ward Category.

The Equipment Types I have chosen are as follows:• Point of care monitor – Stand-alone Point of Care Monitoring used for spot checking such as GE Dinamap (minimum specification Pulse Oximetry, NIBP)• Temperature type devices – Stand-alone thermometer use at the point of care such as Genuis 3, • Physiological monitor – Stand-alone ward monitoring which includes ECG, Respiration, Pulse Oximetry, NIBP, [IBP]) including any telemetric monitoring reporting to a local central station.• Infusion Pump– Stand-alone volumetric infusion pump of any kind that plugs into a mains power supply such as Carefusion GP• Syringe Pump– Stand-alone syringe pump of any kind that plugs into the mains power supply such as Carefusion CC• Falls Alarm type device – any stand-alone falls alarm which does not form part of another piece of equipment such as a bed.• Patient Scales – any stand-alone device that is capable of weighing a patient at Class III or better, that is not part of another piece of equipment such as a bed or a hoist• Patient Hoist – any hoist capable of lifting a patient from a bed. Please do not include standing or bath hoists.• Defibrillator – any device capable of defibrillating a patient, including AEDs.

On behalf of Sky News, I am trying to obtain an answer to the following questions.

I would appreciate a response as detailed as possible please.

"Simon Stevens and Amanda Pritchard wrote to all NHS trusts and CCG Chief Executives on the 29th of April to advise that employers, on a precautionary basis, should conduct risk assessments for staff at greater riskduring the COVID-19 pandemic and to act accordingly.Has your organisation completed risk assessments on all Black Asian and Minority Ethnic (BAME) staff?ANDWhat precautions, if any, have you put in place to protect BAME staff once they've been risk assessed?"

During these unprecedented times our new working home environments have presented us with an opportunity to gauge a better understanding of our market and product penetration. We are looking to establish thefollowing information...

Urology/GI · Number of ERPC suites · Number of annual colonoscopy procedures · Number of annual Gastroscopy procedures

Cardiology· Number of resting ECG's undertaken per week · Number of ECHO's undertaken per week · Number of stress tests undertaken per week · Number of holter monitors undertaken per week

• We appreciate that these maybe rough average numbers and Pre Covid-19 due to reduced clinic capacity as a result of dealing with the current epidemic. Further to this information helping us establish market share potential, it is to note that we are a preferred supplier into your trust and this data may also be valuable in terms of stock forecasting which may perhaps help usimprove our service level and product development going forward.

Could you please tell me for the calendar year 2019:

1) How many complaints through the patient advice and liaison system (PALS) your Trust received from patients alleging they had been discriminated against or badly treated by a member of staff because of their beingoverweight or obese

Please do this by searching for complaints containing any of the words "weight", "fat", "obese", "overweight" and "bariatric" and filtering these to find those that refer to fat-shaming comments made by a member of staff.

2) Can you select the first five such complaints from 2019 and provide me with the following details:

a. What type of staff member was the complaint levelled against? E.g. healthcare assistant, junior doctor, consultant, cleaner, kitchen staff

b. Please quote the words allegedly used by the hospital staff or summarise the offending action

c. Please tell me what, if any, action was taken by your trust in response to these five sample complaints from the beginning of 2018
