How it works: 1. Start your journey at Headquarters - registration table in the waiting room. 2. Pick up and complete your official Doc Force 5 Registration Card. 3. Follow the path to the main stage meeting “Doc Force 5”™ and the rest of their
team. Play games and collect punches as you go. Be sure to collect 10 punches along the way to be eligible for grand prize drawings. 4. Collect all 6 of the Doc Force 5 trading cards along the way. 5. Get balloon animals throughout the day in the main stage area. 6. Turn in the Doc Force 5 registration card by the main stage to be entered in the
Grand Prize Drawings by no later than 4:00 pm CST October 11, 2014. 7. Spin the prize wheel to win DOC FORCE 5 Team cards and other prizes. 8. Grab a bite to eat at our food vendors conveniently located in the parking lot 9. HAVE FUN

Physician Biographies
DAVID BROTTMAN, MD
Dr. B has been practicing in the Arlington Heights area
since 1991. He was born in Chicago and grew up in
Schaumburg, He attended college at The University of
Illinois in Champaign where he received a B.S. in Honors
Biology. He attended medical school at Loyola Stritch
School of Medicine in Maywood. He received his
residency training at The Children’s Hospital of Michigan
in Detroit. “Dr. B” was on faculty at The University of
Chicago before coming out to Arlington Heights.
M. MEDALINE SANTIAGO, MD
Dr. Santiago was born in the Philippines and has lived in
Chicago, Illinois since eight months of age. She attended
Northwestern University for both her undergraduate
and medical degrees. Dr. Santiago completed her
residency training at the Ronald McDonald Children’s
Hospital at Loyola University in Maywood, Illinois. She
has been practicing pediatrics in the Chicago area since
2001
JACQUELINE SCHWARZ, DO
Dr. Jackie was born and raised in Indianapolis. She
attended Indiana University for her undergraduate
degree and Indiana University – Purdue University for
her master’s degree. Her medical degree was received
from West Virginia School of Osteopathic Medicine.
Dr. Schwarz completed her residency training at the
Advocate Children’s Hospital at Lutheran General in
2013

DAVID L. BAUM, MD
Dr. Baum was born and raised in Ohio. He attended Ohio
State University for his undergraduate degree and
Radford University for his Master’s degree in clinical
psychology. Dr. Baum worked in the field of clinical
psychology for 6 years. He then received his medical
degree from the University of Cincinnati College of
Medicine. Dr. Baum completed his residency training at
the Akron Children’s Hospital. Dr. Baum had worked as a
pediatrician in the Akron, Ohio area since 2008
TIFFANY SCOTT, MD
Dr. Scott was born and raised in Michigan. She attended
Michigan State University for her undergraduate
degree and received her medical degree from the
University of Michigan. She completed her residency
training at the Cincinnati Children's Hospital. Dr. Scott
has been working as a pediatrician in the North Carolina
area since 2011. She is married and has one child.

Extra Rules for Mom's and Dads: 1. All children under 12 years of age must be accompanied by a parent or guardian at
all times. 2. Individual Doc Force Five Cards can be picked up only from physicians. After the
event, Physicians will be giving out their individual Doc Force Five Cards at appointments only, while supplies last. 3. Only Registration Cards receiving 10 punches are eligible for Grand Prizes. 4. Grand Prize Drawings will be held at 4:00 PM 10-11-14. Winners need not be present
to win. All decisions made by the judges are final. Contestants agree to hold Arlington Pediatrics Ltd harmless for any occurrence associated or related to this event or prizes given at the event.

APL Gives Back Through Community Service Projects The staff members at Arlington Pediatrics Ltd. feel strongly about giving back to our community. We feel fortunate to have careers that enable us to help children and parents directly but we also believe in helping the greater community as well. This is a concept we try to instill in our patients. Below is a list of current charities and non-profits we support. Please join us in helping our community. Just Hang It Up - www.JustHangItUp.org Launched in September of 2013, JHIU hang tags were created to deter the use of cell phones while driving. Dr. B was frustrated with the staggering statistics on adolescent cell phone use while driving. He was tired of receiving calls from emergency departments and parents that his patients had been victims of car crashes related to distracted driving. He wanted to do something. Brian Wright, CEO of Healthcare Mar-keting Professionals who works with APL, was impacted by two major car accidents, occurring almost a year to the date of each other, caused by distracted drivers. Thus, over lunch one day, Dr. B and Mr. Wright were discussing this issue. They created the Just Hang It Up hang tag and are now recruiting groups, corporations, individuals and religious organizations to get everyone to “Just Hang It Up”. The Irwin Brottman Memorial Classroom Opened in the spring of 2013, the IBMC was created as a center for classes and support groups hosted by APL. Our physicians have always wanted to create a center for learning for the community. Classes and support groups meet two to three times per week. APL currently offers the following support groups: lactation support group, parents who have suffered a fetal loss, parents of children with developmental delays and parents with children who have diabetes. APL offers a variety of classes, which are updated on this site and in our monthly newsletters. We often advertise these classes in the office and via email alerts. Recent classes included child rearing , kite building, CPR, gun safety and toilet training. If you would like more information, have an idea for a class or would like to establish a support group, contact the office at 847-398-0400 APL Community Service Grants Each year APL offers eight (8) $250 community service grants. This offer is open to any APL family. Each fall, APL announces the application deadline for these grants and selects 8 applications that are worthy of the award on a first come first serve basis. The physicians at APL want to promote participation in community service activities by its patients and are excited to help fund the start of their life time of giving to others. APL Community Service College Scholarship Program Each spring, two (2) $1000 college scholarships are awarded to the two APL high school seniors who have given the most back to their communities. High school seniors who have been patients in the practice for greater than one year are eligible. Applications for these scholarships are released each fall. Services, Classes and Breast Pump Rentals We offer the convenience of renting hospital grade breast pumps to mothers. The Medela Symphony is the model that is available. At APL Lactation Services, all of our kits are sterile to provide you the security of knowing you are giving the safest milk to your baby. Our professionally trained certified Lactation Consultants will assist you on the correct use of all of your pump equipment.
