Upon electronic submission, an email will be generated and sent to the email address listed on the bottom line of this form under the Signature of Certifying Official and Contact Information section. Notice of Start-up

NOTICE REQUIRED WITHIN 15 DAYS FOLLOWING START-UP OF ALL NEW SOURCES OR

PERMIT ISSUANCE FOR EXISTING SOURCES REQUIRING A NOTICE OF START-UP.

Per Colorado Regulation 3, Part B, Section III.G.1, operators of air pollution emissions sources who obtain a construction permit thatrequires submittal of a “Notice of Start-up” within 15 calendar days of commencement of operation or permit issuance shall file this form with the Air Pollution Control Division. It is a violation if the start-up date of a new emission source is not reported within 15 calendar days of commencement of operation. If multiple pieces of equipment (i.e. “emission points”) covered by a single permit (i.e. “facilitywide

permit”) will have different start-up dates, a separate Notice of Start-up must be filed.

Notification of start-up may be accomplished by filling out the form below and submitting it electronically.

Upon electronic submission, an email will be generated and sent to the email address listed on the bottom line of this form under the Signature

of Certifying Official and Contact Information section. The completed NOS will be attached as a .PDF document.

You must send a printed copy of this form to the division at the address below if you do not have an AIRS ID or a permit number.

Directions for completing and submitting a Notice of StartȤup (NOS)

Per Colorado Regulation 3, Part B, Section III.G.1, operators of air pollution emissions sources who obtain a construction permit which authorizes submittal of a “Notice of Start-up” within 15 calendar days of commencement of operation or permit issuance shall file this form with the Air Pollution Control Division. It is a violation if the start-up date of a new emission source is not reported within 15 calendar days of commencement of operation. If multiple pieces of equipment (i.e. “emission points”) covered by a single permit (i.e. “facilitywide permit”) will have different start-up dates, a separate Notice of Start-up must be filed.

* Sources may submit an electronic NOS only if they have been assigned an AIRS ID (with point number) and a permit number. Sources without both an AIRS ID and a permit number must submit paper copiesof the NOS.Filling out the form:

• You must download the form to your computer first; if the form is not downloaded, this process will not work properly.

• Please use a desktop or laptop computer to complete the form. If you use a tablet, iPad or cell phone, the emailed document that comes back to you for your records may appear blank unless you have a .PDF viewing software installed on your device.

• Enter all Ãompany Énformation.• Enter the facility name and address. If the facility does not have a street ÁÄdress, then enter

the ÌÏcation.• Enter the ÐÅrmit ÎÕmber located on the first page of your Air Pollution Control Permit (ex: 12DE1234).• The facility AIRS ID (ex: 126-1234) is located on the first page of your Air Pollution Control Permit. The emissions

point (ex: 001, 002, 003, etc.) is each individual piece of equipment listed in your permit(s). Therefore, the AIRS ID to be listed on the front of this form is the facility AIRS ID and the emission point (ex: 126-1234-001)*.

• Click on the dropdown menu under Åquipment Ôype and choose ΅ÅnginÅ΅ or ΅ÏtheÒ΅. Enter allÅÑuipment ÉÎformation available. If there is no Íake, Íodel, and/or Óerial number, typeN/A.

o For Engines: Mfr. date is the date the engine was manufactured.o For Engines: Reloc. date is the date the engine entered Colorado.

• Enter the date the equipment began operation in Section V.• Provide the name, contact information and signature of the Ãertifying Ïfficial.• Once the form is complete, hit the Óubmit button in the bottom right.

o You will receive a confirmation email with a .PDF of the submitted document that isdate stamped (this will also include the unique document reference number in thetop right corner of the NOS).

o If you used a device, such as an iPad or a cell phone, to submit your Notice of Start-up and your confirmation document appears blank, please check the .PDF on your desktop or laptop.

If you have additional points for the same facility that require an NOS, after clicking submit and receiving a confirmation page, click on Ãlear Åquipment in the bottom right. This will keep all the company and facility information and only clear equipment information in sections IV and V. If you have additional points at different facilities, after clicking submit and receiving a confirmation page, click on the ÎÅxt Æacility button on the right. This clears all facility, permit and equipment information listed in sections II, III, IV, and V.
