Getting old is inevitable, but there are many choices in what to do when a loved one can no longer take care of

themselves. Whether there is a need for full-time assisted living or just some-one who can help out a couple times a week, there are many facilities and op-tions to choose from in Fresno.

It can be a struggle for children of ag-ing parents when it comes time to make decisions about who can best care for their parents. With so many choices and levels of care to choose from, read on for information on some local options and assistance with the decision-mak-ing process.

Twilight Haven has three levels of care for seniors. It has affordable housing apartments on site for seniors who are able to care for themselves. Seniors can come and go as they please, join in on the various activities, buy meal plans and do not have to worry about maintenance problems or utilities be-cause it is included in their rent. It also offers transportation to those who need it.

There is an efficiency assisted living op-tion for those who just need mild as-sistance, with levels one through four based on the amount of care one needs. It is an apartment setting, in which resi-dents can bring their own furniture.

People have the option of private or semi-private rooms in the assisted living wing. There are skilled 24-hour licensed nurses on staff for medical care and it has the option of hospice (and patients don’t have to move to another location).

“The biggest reason people should check out Twilight is because of the continuing care. Residents can start out independent and don’t have to move if they need more care down the road,” said Sharon Bailey, administrator at Twi-light Haven.

lived there for the last five years. After her mom passed away last year, her fa-ther moved to the assisted living wing.

She decided to look into Twilight be-cause her aunt and uncle were there, and after her dad had a stroke, her par-ents told her they needed help. The couple didn’t have the money to pay for 24-hour care in their home, so the Hol-lesens sold their home to pay for their care at Twilight.

“The staff treated my parents like kings and queens,” Chute said. “I can’t say anything negative about Twilight. They went above and beyond to make my parents feel at home.”

Chute says it was not easy to have to put her parents in a home, but she is still working and couldn’t take care of them. It was a big transition at first, but once her parents got adjusted, both of them really liked it there.

Bel Haven Care, a senior assisted living community in Fresno, has three types of rooms: a shared room with two people per room sharing a bath, semi-private with each person having a separate room but share a bathroom between the living space (two separate doors), and private rooms for one person with their own bathroom.

Bel Haven Administrator Russell Milnes says care is their primary focus. One would hope the care is good at every facility, but

unfortunately that is not always the case. He doesn’t want families to worry about loved ones.

“The families are entrusting their loved one to us and expect us to be watchful of changes and address their daily needs,” Milnes said.

Price and location are its other areas of interest since price is a strong motiva-tor when choosing a facility. Bel Haven is in the lower range, providing a good dollar value.

It also offers respite care, allowing the family time to rest, take a vacation or just have a break. And when families are able to enjoy their activities, they can rest as-sured that their loved ones are also in-volved in fun activities – an essential part of Bel Haven. It provides a full range of activities seven days a week. Milnes says though every assisted living is under the same licensing rules, each facility can decide how much care it is willing to offer. For example, some won’t take people that require a two person

transfer or lift and some won’t accept people with advanced dementia or hos-pice residents.

“At Bel Haven, our goal is to do whatever we can to avoid a resident having to go to skilled nursing or move because they are on hospice,” he said. “When someone moves in, we want Bel Haven to be the last place they have to move to.”

Sunrise Senior Living in Fresno also of-fers a wide array of options for seniors. From assisted living and memory care services to respite and short-term care, it provides the assisted living services that residents need.

“When you step into the warm and invit-ing environment at Sunrise of Fresno, you

can’t help but feel at home,” said Lauren Bentley, corporate communications at Sunrise Senior Living.

Fresno residents can also check out Golden Living Centers. There are four loca-tions in Fresno, but the big-gest location with the most options is located on Fresno Street. It offers long and short term care, inpatient rehab services, two Alzheim-er’s units and respite care.

The short term care and re-hab services it offers can help with a fam-ily member that has been injured and needs physical, occupational, speech or joint replacement therapy. After the injury gets better then the patient can return home. It also has different units for patients who have diseases including Alzheimer’s and dementia so if it advanc-es they can stay in the location and just move to a different unit.

Golden Living Assistant Executive Direc-tor Kevin McGuire says every situation

is different and it offers care depend-ing on what someone needs. He says sometimes short-term care or respite care becomes long-term care because the family determines it is the best place for their loved one.

“A lot of people can’t do it alone and we are here to assist,” McGuire said. “We want families to be part of our family.”

Another option is Amdal In-Home Care, which provides Personal Care Atten-dants (PCAs) who assist seniors in many different ways. In-home care is a flex-ible option. Care is scheduled as often as family members need and there is no minimum number of shifts required.

“We allow parents to ‘age-in-place,’ which means they can often remain safely at home with some assistance,” said Amdal Director of Marketing, Jeff Hills. “Often we are providing much needed relief to family caregivers.”

Most of their clients wish to remain in their home for as long as possible. All caregiver’s time is devoted to the needs of just one person. But it also provides care in assisted and indepen-dent living facilities. All of its caregivers are screened for professional care experience, perform

criminal and DMV background checks and test their ability to provide care, as well as require TB tests. In addition, it provides ongoing training and all care is overseen by a care coordinator as-signed to the client.

“Often the parent is more receptive to care from someone who is not their immediate relative,” Hills said. “Some-times families utilize our services to ‘buy some time’ while they evaluate their options.”

Newspaper headlines tout the benefits of using bariatric surgery to cure Type 2 diabe-tes – even in people who are

not morbidly obese. That’s old news to Drs. Kelvin Higa and Keith Boone, bar-iatric surgeons at Fresno Heart & Surgical Hospital, who’ve seen these results again and again in their patients and have been studying the effect for several years.

“Many people think that bariatric sur-gery is cosmetic, but that is not true,” said Dr. Boone. “Losing weight helps with general looks, but we perform bar-iatric surgery to ‘cure’ or improve medi-cal problems that are caused or made worse by obesity. Diabetes is the num-ber one reason for proceeding with bar-iatric surgery. In our patients who have diabetes and have the Roux-en-Y Gas-tric Bypass, 85 percent leave the hospital right after surgery off of all of their diabe-tes medicines. The diabetes and obesity epidemics are so interconnected that we now have a new word called ‘diabesity.’”

Dr. Boone continued, “In addition to diabetes, we see resolution or im-provement in many other medical

problems including hypertension, hy-perlipidemia, sleep apnea, reflux dis-ease (GERD), asthma, and arthritis of weight bearing joints.”

“Surgery is the most effective treatment for Type 2 diabetes and morbid obesi-ty,” emphatically agreed Dr. Higa, who has participated in several international conferences on this subject. He often demonstrates bariatric surgical tech-niques overseas – especially in countries with socialized medicine where the gov-ernment picks up more of the costs of health care.

“Many countries find it more cost effec-tive to treat diabetes with surgery,” Dr Higa explained. “Take away all the emo-tional, pre-conceived ideas and preju-dice regarding obesity and what you’re left with is a growing number of patients who do not have access to the most ef-fective treatment for type 2 diabetes – surgery. This has been proven through randomized, controlled studies.”

At this year’s International Diabetes Surgery Task Force summit in Italy, at-tendees issued a consensus statement

pronouncing bariatric surgery to be a “legitimate approach” for diabetes treatment. Dr. Higa, who participated in this summit, said research supports a much more positive position. “Even the best medical practices cannot match the results we obtain with surgery.”

The Associated Press reported last July that New York-Presbyterian/Weill Cornell Medical Center has been con-ducting a study on this issue, doing gastric bypass surgery on patients with diabetes, who because they are not ex-tremely overweight, wouldn’t normally qualify for the weight-loss procedure. One of the hospital’s patients, Chris-tina Iaboni, turned to bariatric surgery after trying unsuccessfully to control her diabetes through daily shots of in-sulin and other medications.

“I didn’t care if I lost any weight. I just wanted the diabetes to go away,” the 45-year-old Connecticut mother of two teenagers told the Associated Press. She’s 50 pounds lighter since her surgery last fall, she’s stopped taking all her dia-betes medications and her blood sugar is almost normal, she reported. >>

Most insurance plans cover bariatric sur-gery as a last resort – only after diet and exercise plans have failed. And then guidelines, which often follow federal Medicare regulations, say surgery candi-dates must be morbidly obese with a body mass index over 40, or a BMI over 35 plus a weight-related medical problem like diabetes, sleep apnea or high blood pres-sure. Generally those who are 100 lbs over-weight are considered morbidly obese. These limits, set by the National Institutes of Health in 1991, may have to be changed in light of new evidence regarding bariatric surgery’s affect on diabetes.

“In addition to discussions on lowering the criteria to be eligible for bariatric sur-gery,” said Dr. Boone, “there are some new procedures in experimental phase for patients who are not overweight or obese, for the treatment of diabetes.”

The Associated Press reported that Dr. Philip Schauer of the Cleveland Clinic is among those pushing the BMI envelope. For a study, he’s recruiting 150 overweight and obese diabetics with BMIs between 27 and 43. Some will have surgery and their progress will be compared to those who manage their diabetes with medi-cine. The goal is to see which group can achieve complete remission. “These pro-cedures can cause long-term remission and restore someone to normal blood sugar levels without medication,” Schau-er said to the Associated Press.

More and more physicians are making the same assertions about a cure for diabetes. This past spring at the Interna-tional Diabetes Surgery Task Force sum-mit in Italy, attendees issued a consensus statement pronouncing bariatric surgery to be a “legitimate approach” for dia-betes treatment. Last year, 220,000 people had obesity surgery, which can cost between $14,000

and $26,000, according to the American Society for Metabolic and Bariatric Sur-gery. But diabetes treatment can also be expensive – costing an average of $11,711 a year for total health care for those with the disease, according to the American Diabetes Association. Even so the association said there’s not enough evidence to generally recommend sur-gery for diabetics with a BMI lower than 35 outside of an experiment.

When her cough wouldn’t go away last February, Anetta “Jane” Edwards’ primary care physician

ordered an X-ray, which confirmed her worst fear – lung cancer. Next came a CAT scan and a lung biopsy that left her in the ICU in a south Valley hospital for days.

When Edwards was transferred to Community Regional Medical Cen-ter for a higher level of care, doctors were hoping her cancer was small enough to be treated by the area’s only CyberKnife – a knifeless, pain-less high-tech surgical procedure where patients undergo incisionless treatments. But Jane’s cancer was fast-growing so she was referred to Community Regional’s Lung Nodule Program – one of only a handful of its kind throughout the country.

The Lung Nodule program provided Edwards an expedited treatment ap-proach designed to give a faster di-agnosis and treatment for those with lung cancer.

This cutting-edge program works in an expedited manner where patients are evaluated by a team of specialized cli-nicians – all targeted to occur within a week of referral. This can make a big difference for patients like Edwards, who benefit by having reduced wait-ing time from detection to treatment, which ensures a greater likelihood of positive outcomes.

Prior to last year, the average patient with lung cancer in the Central Valley waited almost three months for a diag-nosis and treatment plan. Part of this delay is the complexity of decisions and the need for multiple specialists to be involved in the decisions.

“One year ago, a group of interested spe-cialists from both UCSF Fresno and the private practice community came togeth-er to develop a coordinated program to provide care for patients with lung cancer or at risk for lung cancer,” said Dr. Mi-chael Peterson, chief of medicine, UCSF Fresno. “This program puts the patients at the center and brings the doctors to them.”

Dr. Peterson says this has already re-sulted in much shorter times between presentation and treatment. Patients can remain close to home with their sup-port systems and still receive the highest level of coordinated care.

Now after her third round of chemo, Edwards had positron emission tomog-raphy (PET) to see if her tumor was shrinking with her treatment therapies. A PET scan enables physicians to more accurately diagnose and identify cancer, heart disease and brain disorders.

“I’m having a PET scan and then I’ll know if the tumor has shrunk,” said Edwards. “Depending, I’ll get more chemo or ra-diation … or go home!”

With an upbeat personality, Edwards made great friends with caregivers like registered nurse Carolyn Pierce, who has been by her side during her chemo treatments.“You have to have a positive attitude to get through cancer,” Edwards says. “It’s very important.”

Edwards said her treatment – even though she’s from out of town – is very convenient because it’s all in one place and she doesn’t have to go from place to place throughout Fresno.

“I know where to go and everything is right here,” Edwards said. “The people are great and compassionate – they know what I’m going through.”

The program also helped Edwards get diagnosed and treated faster. While she was in Fresno, Edwards stayed in a motorhome outside of California Cancer Center for the week during her treatments. Before she came to the program she had to wait for different doctor’s appointments and go to sev-eral locations.

A few weeks after her treatments and PET scan, Edwards got a call from Kathy Norkunas, a nurse navigator with the Lung Nodule Program who works with patients like Edwards.

“It’s great news!” Edwards said. “Kathy told me my tumor has shrunk so much you can hardly see it. I got goose bumps on my arms when she told me.”

Edwards still had to have 33 days of radi-ation for three to five minutes per day as a follow up to her chemo. She also had six more weeks of chemo, but just for one day a week, and not nearly as strong of a dose as she had before. She will wait for a couple months and then have an-other PET scan to recheck the tumor.

“They saved my life,” she said of her caregivers and treatments. “It’s amazing, just amazing.”

Over the last year Edwards credits her caregivers, treatments and posi-tive attitude for helping her through her ordeal.

“When you have cancer, you have it 24 hours a day,” she said. “You can’t get away from it.”

Edwards said planning her 50th wed-ding anniversary in April with husband Ron really helped her get through her chemo treatments.

“I’m planning a two-week cruise to Ha-waii,” she said. “You can’t start too soon for that.”

What is TMJ?TMJ stands for Temperomandibular Joint. It’s your jaw-joint. Everybody has a TMJ. If your TMJ isn’t working properly, then you have TMD – Temperomandibular Joint Disorder. Everybody does not have TMD.

What does TMD have to do with my migraine headaches?Everything! When you move your jaw you can feel all the muscles of your head, neck and shoulders involved in that movement. If your TMJ isn’t positioned correctly, then all of those muscles are overworked just to do the normal things that your jaw does – eating, speaking and swallowing. The position of the TMJ is controlled by the contact points of your teeth. So, if the teeth contacts are not correct, then your TMJ isn’t positioned correctly, and the head and neck muscles never get a chance to rest (not even when you sleep). This starts a chain of events that leads to migraine headaches.

Why do I still get headaches after I had my bite adjusted?Probably for a couple of reasons.The bad tooth contacts usually occur during a 3-8 hundredths-second time interval during the opening and closing of our jaw. If your bite was adjusted using only ink paper, then it was adjusted with a tool ( the ink paper) that doesn’t tell when the teeth touch, only where (and is actually 70-80% wrong in telling where!).

Also, the overworked head and neck muscles learned incorrect muscle memory. So the bite was probably adjusted based on incorrect muscled movement - even if you wore a mouth guard before the adjustment.

What do you do differently?I first address the incorrect muscle memory using four different types of massage therapy – manual massage, ultrasound massage, micro-current nerve stimulation and cold-laser muscle treatment. I balance the temporary mouth guard using a computer-assisted bite analyzer called a T-Scan.

I follow by treating your TMD using computer-assisted precision. Every aspect of the therapy is precise and objective. The muscles’ activity is monitored using EMG (electromyography). The TMJ noise (it should be silent) is monitored using an accelerometer. Your bite is virtually slowed into hundredth-second intervals using the T-Scan, so you and I can both see exactly when, where and how heavily each tooth is contacting. The movement of your jaw is also precisely tracked in every direction using EGN (electrognathography).

Tell me a bit about yourself and why you chose your specialty?

I am extremely interested in people and have always wanted to contribute to society. After a bit of exploring in my undergraduate years, I realized that medicine would be the best route to accomplish my dreams. The first time I saw cataract surgery in medical school, I couldn’t imagine a more rewarding field than ophthalmology, where one could help restore the most important human sense. Now I enjoy doing this on a daily basis, but I also carry out frequent surgical missions overseas where I provide care to underserved patients and help train local doctors. I have the privilege of being a part of Eye Medical Center’s group of specialists since 2008.

Who is a good candidate for LASIK surgery?

Most people over the age of 21 who have a stable prescription with healthy corneas are good candidates;

however, when I evaluate patients for LASIK, I look at many factors very closely including the patient’s goals. Personal communication with the surgeon is crucial, especially for patients over forty who have developed presbyopia, or trouble with near focus. Laser Refractive Surgery in the hands of a trained specialist is very safe with over 25 years of evidence demonstrating excellent outcomes and patient satisfaction.

What are some refractive procedures besides LASIK?

Some individuals with very high prescriptions may be better candidates for Intraocular Lens Implants rather than Refractive Surgery with a laser. These lenses provide excellent vision, even better than contact lenses. In some cases we can even help patients with diseased corneas, such as those with keratoconus (a condition in which the cornea is abnormally shaped). We use Intacs, a polymer ring, to help stabilize the cornea, reduce astigmatism, and allow patients to wear contacts with improved vision & comfort.

I have heard of new intraocular lenses for patients with cataracts that can help restore youthful vision. Is this true?

Yes, today third & even fourth generations of these “Multifocal” or “Accommodating” lenses provide people with both far & near vision after cataract surgery. There are also special “Toric” lenses that help eliminate astigmatism after surgery. In other cases, a combination of cataract & laser corneal surgery can overcome both astigmatism and presybyopia.

What are some other innovative procedures that you offer patients?

I have been doing “DSAEK” (sutureless corneal transplant) with great success since coming to Fresno. This surgical technique allows for very rapid visual recovery & reduces complications like rejection and infection. I will soon be doing the first artificial corneal transplant in Fresno, using the “Boston K-Pro” device. This prosthetic cornea restores vision in patients who have had multiple corneal transplant failures. Other novel therapies that I use include serum tears for patients with severe dry eyes and amniotic membrane ocular reconstruction for ocular cancers, severe pterygium, & corneal scars or melting.
