ACCEPTANCE TO THE HEALTH PHYSICS FOR URANIUM RECOVERY COURSE (F-104) (FSME-12-043)
Purpose: To provide the list of students selected for the NRC Health Physics for Uranium Recovery Course (F-104).
Background: NRC provides the list of students and instructions to the States to help ensure that States with candidates on waiting lists will have an opportunity to fill vacated slots that may open up after this notification letter has been sent.
Discussion: Enclosure 1 is the list of students from the States selected to attend the July 9-13, 2012 Health Physics for Uranium Recovery Course (F-104). This course is to be held in Moab, Utah. Please provide the list of students and the instructions (Enclosure 2) to each individual from your program that is on the list. Enclosed for your information is a tentative schedule for the course (Enclosure 3). Students attending this course will be paid travel and per diem by the U.S. Nuclear Regulatory Commission (NRC). Students should make their travel arrangements through Carlson Wagonlit Travel at 1-866-250-2160 immediately and then submit their travel information needed for NRC to issue their travel authorization (Appendix A) to Brenda Usilton (Fax 301-415-3502). Please go to the following website to download the Travel Application Form http://nrc-stp.ornl.gov/training.html and then send it to [email protected] or fax it to 301-415-3502.
We ask that you inform us of any cancellations 30 days prior to the course starting date or as soon as you are aware that the student cannot attend the course.*

INSTRUCTIONS TO STUDENTS
ACCEPTANCE: This is to advise you that those individuals in Enclosure 1 have been accepted for participation in the training course (F-104), "Health Physics for Uranium Recovery Course." This course is scheduled to be presented at the Moab Valley Inn, 711 South Main, Moab, Utah 84532. If you have any questions on the logistics of the course please contact John Hultquist. COURSE: This course will be conducted beginning at 8:00 a.m. and end at 4:00 p.m. each day except on Friday when the course will end at 1:00 pm. Enclosed is a tentative schedule for the course (Enclosure 3). On Friday, July 13th there will be an optional tour. Tour participants for the Denison Mines mill site tour in Blanding, Utah need to be bring work boots, preferably steel-toed shoes, long pants and shirts with sleeves (t-shirts are acceptable). This clothing should be a type that the participant does not mind getting dirty while touring the Denison Mines mill site. Individuals sensitive to sunlight should plan to bring a hat, sunglasses and sunscreen. Moab will be hot in July, probably high 90s or low 100s. Cellular phones and similar devices with audible capability should be disabled while classes are in session.
LODGING AND TRAVEL: You should plan to arrive on Sunday, July 8, 2012, and depart on Friday, July 13, 2012. If you plan on taking the optional tour then you will be able to go home on Saturday, July 14, 2012. No rental cars will be authorized for travel. If traveling by air, you need to contact Carlson Travel at 1-866-250-2160 for airline reservations. The per diem for Moab, Utah is 104/56/160. Please complete the Travel Application Form http://nrc-stp.ornl.gov/training.html and return it to Brenda Usilton at [email protected] or fax it to 301-415-3502. If you have any questions regarding the travel form please contact Brenda on 301-415-2348. A block of 20 rooms has been reserved for this course. Reservations can be made by calling Moab Valley Inn directly by using 435-259-4419 or 800 831-6622 and informing the desk clerk you wish to reserve a room with the Division of Radiation Control on July 8, 2012. The group code is DORC7912. Course participants can also go to the web page and book a room from the block by using the group code. The link below will take participants directly to the booking engine for the group. Please go to our website http://nrc-stp.ornl.gov/training.html to receive a copy of the travel instructions and voucher for reimbursement. It has been determined that it is easiest to fly into Grand Junction, CO and then take a shuttle for approximately 2 hrs to Moab, UT. If you have the ability to rent a car through your State agency that will allow you more travel flexibility than that provided by shuttle services. Attached for your information, as separate documents is the shuttle service to Moab, Utah, Airport information and Visit Request Form that is required to be filled out.
