Both OPIE and Futura already have tools to streamline this process, and more improvements are on the way. Here are the details: IN OPIE To efficiently document all components on the Delivery Receipt, you must be using OPIE Purchasing & Inventory, and must add those components from the OPIE patient chart using the Componen- try, Consumables, or Parts Order forms. IN FUTURA If you are on the Futura Hosted version, the easiest way to document components is to request your FREE OPIE Purchasing & Inventory account. This account can be integrated with Futura version 6.6, and enables you to select and order compo- nentry from the transaction in Futura. We are in the process of rolling out a new proof of delivery form in both Futura and the Practitioner Portal that will then auto- matically pull in the componentry details from OPIE Purchasing & Inventory. If you are not on the hosted version of Futura, or have not yet taken ad- vantage of the free OPIE Purchasing & Inventory option, you can still add the componentry information manually to the service line notes in Futura. If taking this approach, you will need to contact Futura support to obtain an updated Proof of Delivery form that will populate the componentry information from those service lines notes. In the near future we will release additional functionality that automatically pulls the serial numbers into the appropriate OPIE form or Futura transaction and automatically populates on the proof of delivery. We are also working with some of our larger Integrated Supplier partners to have serial numbers auto- matically populated from their system to OPIE Purchasing & Inventory when the items are shipped. In the near future, many of your serial numbers will be able to flow all the way from the supplier to your proof of delivery with no additional effort required! We Continue Improving So You Can Too!

New Componentry Features for Proof of Delivery EARLIER THIS YEAR, CMS ISSUED a reminder regarding requirements to document componentry information on the Proof of Delivery and, since that time, a number of customers have reported claim denials as a result of not having this information. Both OPIE and Futura already have tools to streamline this process, and more improvements are on the way. Here are the details:

IN OPIETo efficiently document all components on the Delivery Receipt, you must be using OPIE Purchasing & Inventory, and must add those components from the OPIE patient chart using the Componen-try, Consumables, or Parts Order forms. From there, simply hit the “Component Info” button on the Delivery Receipt, select the components that were actually delivered, and all of the information will be Automatically Populated.

ICD-10 Transition Info

OPIE + Futura Pave the Way to a SmoothICD-10 Transition

OPIE and Futura developers have been hard at work implementing software en-hancements to assist with the upcoming ICD-10 transition on October 1.

As stated on a CMS factsheet, “The transition to ICD-10 is occurring because ICD-9 produces limited data about patients’ medical conditions and hospital inpatient procedures. ICD-9 is 30 years old, has outdated terms, and is inconsistent with current medical practice. Also, the structure of ICD-9 limits the number of new codes that can be created and many ICD-9 categories are full.” (CMS, 2014)

You may have already noticed the changes implemented in OPIE last year before ICD-10 was postponed:n OPIE Practice Management allows

you to choose from a separate ICD-10 code set.

n OPIE Dex Diagnosis Codes feature a new tab to house the ICD-10 codes.

n Logic is built into the Prescription so that you cannot accidentally mix ICD-9 and ICD-10 codes.

n OPIE Billing automatically transfers the code set designation to the claim so that it is properly identified on the paper and electronic claims.

WE HAVE MORE EXCITING UPDATES COMING SOON!

NEW OPIE FEATURESn Functionality has been created so that

you can update your system with the ICD-10 codes well in advance of the October transition date. This will allow plenty of time for any internal testing.

n We have created a new payer level designation so that you can identify any payers that might still be using ICD-9. Some non-HIPAA covered entities such as Workers’ Comp and auto insurance payers are not required to make the switch, and it is not yet

known if they will begin to use ICD-10 after October 1. OPIE is ready to support both code sets for as long as both are necessary.

n Any new prescriptions created in OPIE beginning on October 1 will default to ICD-10 unless the payer has already been designated as an ICD-9 payer.

n In the weeks leading up to October 1, bear in mind that any prescriptions received that you plan to deliver after October 1 will need to have ICD-10 codes.

n You will have the ability to modify the website used for crosswalking in OPIE’s Administrative Tools. At this time, based on user feedback, there are no plans for internal crosswalking tools within OPIE.

n In OPIE Billing we have added a validation at submission that will verify the Dx codes against the designated code set and warn you if you have codes on your claim that do not exist in the code set.

n Capabilities of the Review and Submit functionality have been ex-panded so that it will better support the needs of outlying nonstandard payers such as hospitals, charitable institutions, nursing homes, and non-HIPAA covered payers.

NEW FUTURA FEATURESIn Futura, we will have your ICD-10 codes input and ready to go in advance of the October 1 transition date. Please call Futura support at 800.840.6057, Option 1, if you need them right away for testing.n Futura has a great internal crosswalk

function. Once you click the radio button for the crosswalk, you can input any ICD-9 code and it will show you a list of possible correlating ICD-10 codes.

n Any Futura transaction with a date of service of October 1 or later will prompt you for an ICD-10 code and warn you if the claim has ICD-9 codes. You will still be able to override this if necessary.

n Futura will prompt you if you acciden-tally choose an ICD-10 code prior to the October 1 transition date.

n Futura transactions will now allow a total of eight diagnosis codes instead of four.

n Futura solutions for the ICD indicator on electronic submissions and fixes for the paper claim submissions are in the works and will be delivered to Futura users soon!

We have been conducting testing with OPIE and ZirMed for the last few months. Several OPIE sites have par-ticipated with successful results. Please call OPIE Support if you are interested in participating in testing with ZirMed. If you are a ZirMed user, you may have recently received a warning regarding 5010 transmission standards. Please be assured that ZirMed and OPIE have this issue well in hand and that ZirMed will continue to convert to the required 5010 standard before transmitting to your payers.

Since October 2014, I have been back in Gainesville for

less than 14 days. My mission has been to focus my attention on OPIE and to person-ally redefine my work and life balance after 20 years of concurrently

building and working in multiple companies. It was a tough decision to leave my practice behind, but one that I believe will have the most positive impact for the entire profession.

After an unexpected break resulted in a four-month stay in a 450 sq. ft. microhome in North Carolina due to the harsh winter snow of Buffalo, New York, covering the RV, the journey continued again in April 2015. Since I began I’ve put over 10,000 miles on the 23-foot Volkswagen Rialta, visited dozens of O&P clinics on the East Coast, across the central and South-

western states, learned a lot about how people are using our software in practice, and have made some incredible memories spending time outside of the office with many of you, your families, and friends.

I’ve stayed in some of the nation’s most beautiful national parks, a spectrum of

RV campgrounds ranging from one extreme to the other, plugged-in and slept in some of the finest office park-ing lots and home driveways in the field, drove miles to find a working Wi-Fi signal in order to make in-ternational work-related Skype calls, broadcast webinars from mountain tops, canyons, and deserts, and assem-bled presentations in the mobile office under the moonlight to the sounds of crickets and coyotes singing. I’ve realized that more than half the stuff I took with me I don’t need or haven’t used, and I have experienced firsthand

the richness of the simple life.The nomadic lifestyle is quite

amazing—every day is completely different and exciting in some way, but it has its challenges as well. As an example, I recently made it to California, so East Coast conference calls require me to be up and involved in calls before 5 a.m. several days a week. I’m a night owl. Creativity for me has always flowed after the rest of the world is asleep. This is a challenge!

Secondly, since I am awake after the call, I typically keep on going—the West Coast works till 5 p.m., and so do I while I am there—and some-times beyond that—so when should I turn off the switch? Why can’t I? Why don’t I? I’ve realized that old habits are hard to break, no matter where you park your RV. That elusive life-work balance we all seek is still quite difficult to find.

The nice thing though is that when I finally do stop and come up for air, I look around and am amazed at what an incredible country we live in as I bike, hike, climb, float, paddle, or simply start a campfire to wind down. I promised myself that I will start slowing things down in July and try a new approach so that there is not such a rush to get to the next place. I’ll let you know how that goes, and in the mean time I’ll keep updating you with some pictures that punctuate my days via our FACEBOOK PAGE.

As business owners and practicing clinicians who are primarily focused on the urgent and important

demands of our day-to-day responsi-bilities, we may not have a full picture of how the changes underway across healthcare will impact O&P. One thing that we are all very aware of is how easily reimbursement dollars can now be lost. Gone are the days of being able to provide all patients with the best interventions for optimal clinical outcomes without spending twice as much time managing the mysterious maze of documentation requirements needed for authoriza-tion and reimbursement.

We are in the dawn of the new age of the O&P profession, and focusing on data, clinical outcomes, and proving the value of the care we provide as clinicians through improved documentation will help your organi-zation remain viable into the future. One of the areas where we can make significant strides is in our approach to clinical decision-making processes when evaluating and caring for our patients. Payers in all of healthcare are pushing for better documentation to support patient treatment plans. OPIE and the OPIE clinical education team have been promoting and encouraging an approach to documenting our care in a way that positions our profession for long-term success. This approach integrates the concepts of clinical outcomes that you have been learning at Academy and AOPA meetings for years, and the goal is to make it part of your daily practice in a practical way.

As you well know, one of the most universally accepted forms of clinical

documentation is the SOAP (subjective, objective, assess-ment, and plan) note method. This is a structured format for patient notes that helps to ensure you provide thorough detail regarding each patient. If you follow the SOAP method as you write your notes, you will cover the patient’s story, your objective observations of the patient, a clinical assess-ment, and your plan of action.

The biggest challenge is fo-cusing your notes to ensure you clearly show medical necessity for your proposed treatment plan. The payer has to be satisfied with your content, and if you do not prove medical necessity, the payer will refuse reimbursements. We also need to be able to demonstrate the effective-ness of our care after the fitting and throughout continued follow-up. This is where the objective criteria come into play to help us prove the value of our professional O&P intervention for the patient as part of the multi- disciplinary care team.

That’s where SOOGAP comes in—SOAP + OG (outcomes and goals). The documentation will be sig-nificantly enhanced by incorporating “patient reported” as well as “clinically relevant” goals supported (when possible) by appropriate outcome measures for baselining the patient’s condition and progress throughout the episode of care. These goals and outcomes will be the key drivers of our ongoing assessments, also incor-porating subjective and objective data. Instead of writing narrative notes that tell a story, you will be collecting data

from the patient beginning on day one and tracking his or her prog-ress based upon your recommended treatment plan. Each time you see the patient and track your notes in the SOOGAP format, you are not only more likely to objectively show how your orthotic or prosthetic care has helped the patient, but you are also clearly demonstrating to the healthcare team and payers the value that we as a profession are providing to the patient’s overall health and well-being.

If you make this one change in your practice, it will quickly show its worth, as it has the potential to directly affect your authorizations, reimbursements, and appeals. More importantly, it sets up your practice to successfully navi-gate the world of value-based payment systems where you have to prove the value that you provide beyond just filling a doctor’s orders. Making any change in your practice is difficult, but adaptability is part of making your organization sustainable in the new healthcare environment.

The first-ever OPIE Con—a Futura and OPIE Software user conference and training event—convened at the Georgia Tech Hotel and Conference Center, May 14-16.

With over 170 attendees spread over five educational tracks, the event offered a little

something for everyone and was unlike anything previously provided in the O&P profession.

The event featured two keynote speak-ers, Peter Thomas, JD, and Kelley Hundt, MBA, who spoke on “The Changing World of Healthcare and Where O&P Fits in the Future” and “Empowering Staff to Make Pos-itive and Lasting Changes in Your Practice” respectively. Each morning after the key topic speech, attendees broke off into their specific educational track—a one-day OPIE Kickstarter lab, a three-day clinical course, two days of billing training in an on-site

computer lab, an intensive three-day Futura immersion, or a three-day adminis-trative and compliance training.

Connecting and collaborating is another big part of OPIE Con. From icebreakers to group activities during the morning keynote, to ending each day with a fun reception—conference goers are never bored and they walk away having built relationships with their colleagues.

“Jump to patient” from the patient tab on a claim is an

efficient way of navigating between modules.

Right clicking on a claim in the task window allows you to

“mark complete” the claim without opening and clicking on

each individual task (very helpful when working collections).

The colored icons when viewing claims by patient have a

meaning:

n A green “$” means money on the claim is to be collected.

n A red “A” means auth/pre-auth claim is not delivered.

n A gray “$” means the claim has been paid in full.

How can I quickly change which branch or office I am logged into?

OPIE has a Quick-Switch Button in the top right corner of the Task pane. This button allows users to quickly change which branch they are logged into. The logged-in branch controls what appears at the top of your printouts.

The title bar at the very top of the OPIE window indicates which branch you are currently showing.

A few of my patients prefer e-mailed appointment reminders. Is this something I can do in the OPIE Graphic Scheduler?

Yes! An e-mail can be sent directly from the appointment dia-log in the Graphic Scheduler. You will just need to ensure that the setting to have the e-mail icon show on the appointment details screen is activated and there is an e-mail address for the patient in OPIE.

To activate this setting, click on the Settings button in the Graphic Scheduler, go to the User Settings tab, and check the Show e-mail icon on appts. setting.

The e-mail icon (pictured on the Appointment Details window) will appear in the lower middle of the Appointment Details screen. The patient’s phone number is also listed on the right side of the Appointment Details screen.

OPIE will generate a generic appointment reminder that you can modify with text of your choosing.

(Remember: HIPAA Rules require a specific patient authorization for allowing a provider to send them e-mails containing PHI. The authorization must state that the patient understands that e-mail communications are not secure.)

OPIE’S AUTOPOSTING FUNCTIONALITY is currently rolling out to all customers. If you haven’t yet received a notification about autoposting being available to your facility, you are welcome to contact OPIE Sup-port and we’ll be happy to include you in the next wave of updates.

With OPIE autoposting, electronic remittance files are automatically downloaded from ZirMed and matched up to the appropriate claims in OPIE Billing. You have the opportunity to review each claim as it is posted, with clear indicators that show whether there are any discrepancies from what was expected.

Early users of the autoposting functionality have reported a dramatic savings in the amount of time that it takes to post insurance payments, so we are very excited to deliver this functionality to all customers!

Q:

With the new Proof of Delivery functionality, you can add

information to the notes section in the service line, such as

the manufacturer, model number, serial number, or a detailed

product description. Alternatively, Futura hosted customers can

select components using the free OPIE Purchasing & Inventory

module. Both methods can then automatically populate the

Proof of Delivery.

Q: How can I manage all of the active reminders in the system surrounding each patient?

Futura has a great tool that can help with this: The Navigator. You can see an example of it below.

The Navigator in Futura allows users to manage all of their active reminders from the tickle file, calls from the call log, and appointments (if the user is a specialist).

You can use the key combination Ctrl+Alt+N to show or hide the Navigator at any time.

Is it possible to attach notes from a call to a patient record?

Yes! There is a Call Log feature available in Futura. The Call Log tool allows you to document phone calls. It gives you the ability to enter notes regarding the call, and there are several options at the bottom that will initiate a reminder in the system to ensure you follow up with each patient.

In addition, there is an E-mail Copy button that allows you to e-mail a copy of the call notes to yourself and the call recipient or the patient.

Once you are finished with the call, click the Done with Call button and the system will prompt you to select whether or not you want to save the call entry to the patient’s record. All call logs are saved in the patient notes section of the record.

Things are heating up!

The OPIE Choice Network is moving a variety of projects forward at an ag-gressive pace, and we are creating even more tangible results for our network members!

The OPIE Choice Network is focused on working with members to make their practices and the O&P profession as a whole more sustainable and better able to adapt to the changes underway in the healthcare arena.

Some of our most recent progress includes:

1. Business Intelligence DashboardsOPIE is now in beta testing with a handful of O&P practices for a set of tools that will quickly show you where your business stands today. Our goal is to give you fingertip access so you can easily monitor the detailed informa-tion from OPIE Reports, and you can adjust how you lead your organization much more quickly.

There will be dashboards focused on financials, patient satisfaction, referrals, sustainability, operations, a broad-based business overview and more. For members of the Choice Network, many of these new metrics will include a comparison measure that reflects aggregated data from all members of the Choice Network, so you have the ability to benchmark yourself against real-world data.

This is a first for the O&P profes-sion, and the initial feedback has been

very positive! As we move toward full rollout, we have a series of webinars planned to provide training for you and the rest of your team.

2. Payer RelationsWe are continuing to work with legal counsel and HealthAgree to develop the legal infrastructure that is neces-sary for the OPIE Choice Network to move into Payer Relations. There are extensive legal hurdles that need to be ad-dressed to allow us to collaborate on contract negotiations, but we anticipate having our initial plans ready in the next few weeks for your feedback.

Over the past month, we conducted almost 60 phone calls with Choice Network members, and we are more convinced than ever that a strong collaboration on Payer Relations can greatly benefit our members and improve future positions with payers.

Our initial focus will be on state and/or regional payer negotiations as we start these Payer Relations efforts. Our ability to bring real-world data to the discussion opens doors regard-ing how to work with ACOs, how to create the first pay-for-performance contracts in the O&P field, and the possibility of creating narrow net- works where regional insurers will incentivize their beneficiaries to utilize O&P practices that meet minimum standards.

3. Group Purchasing Organization (GPO)The hard work of our GPO Product Selection Committee continues to produce results! We now have GPO contracts in place with College Park (for K2 feet), Streifeneder (for

mechanical knees), and WillowWood (for liners). Our committees are continuing negotiations with several other manufacturers, and we are very optimistic that we will soon have GPO contracts awarded in the following categories—K3 feet, mechanical knees, vacuum, and componentry.

The collective data of Choice

Network Members from the OPIE Purchasing & Inventory system has allowed us to provide the most accurate data to the Product Selection Commit-tee, and has ultimately been a powerful force in moving suppliers forward with these new offers.
