President’s Message

For more than 45 years, MobileMed has always strived to address unmet health needs in Montgomery County. Along the way, the County has experienced

significant demographic shifts, economic cycles, and of course the Affordable Care Act (ACA). These major changes in the landscape ultimately led the Board of Directors to the important strategic decision to pursue designation as a Federally Qualified Health Center. It’s the right path for MobileMed to build on our track record of success.

Therefore, I know that I speak for the entire Board of Directors when I express how gratifying it is that MobileMed has achieved FQHC status. This grant award provides an extraordinary opportunity to expand MobileMed’s impact in the community. Even with the ACA, disparities persist in healthcare access and outcomes. A great many individuals have been able to enroll in expanded Medicaid, but are still in need of culturally competent primary care providers to help them navigate the complicated healthcare system. At the same time, tens of thousands of low-income residents are ineligible for insurance coverage and will continue to rely on local safety net programs. MobileMed is committed to serving this diverse population, offering a

medical home where patients can receive not only top-notch primary and preventive care services, but also the enabling services that they need to improve their health status.

The FQHC award is in part a testament to the strong foundation we’ve established, and in part affirmation of our relevance for the future. MobileMed’s success going forward will continue to depend on the generous support of the community. MobileMed is fortunate to be a part of a community that believes in our mission and our vision and consistently demonstrates its commitment in meaningful ways. The community has provided MobileMed with essential financial and in-kind contributions and complementary programs that we effectively leverage in order to achieve excellent results. Making a difference in the primary care setting is the most compassionate, prudent, and cost-effective pathway for good health.

MobileMed’s FY2015 HighlightsFortifying our sustainability.We were awarded a New Access Point grant, designating us as an FQHC (Federally Qualified Health Center). As the first FQHC based entirely in Montgomery County, we will be able to greatly expand our community impact.

Serving those in need.We provided primary and preventive healthcare services to nearly 5,000 low-income, uninsured and underinsured residents of Montgomery County. We delivered over 14,000 medical visits in multiple languages (English, Spanish, French, Korean, Chinese, Amharic and Farsi) at our seven primary care and two specialty clinics located throughout the County. Mobile medical vans complement our fixed sites, ensuring primary care access for our diverse community.

Enhancing our primary care model. Through our diabetes initiative begun last year, we significantly improved our annual retinopathy and foot screening rates by integrating optometry and podiatry services on-site.With grant support from the Healthcare Initiative Foundation, we significantly increased our patients’ colorectal cancer screening rates. As part of our integrated behavioral health program with Aspire Counseling, we significantly increased the screening rate among all new patients for depression and anxiety.

Leveraging collaboration resources for outreach and care.We now link with community health promoters at several social service agencies to make it easier for their clients to be referred and scheduled for primary care appointments. Uninsured patients continue to benefit greatly from our generous hospital partners -- Adventist HealthCare and Suburban Hospital -- that donate essential diagnostic lab, radiology, and cardiac testing. We have also initiated a promising transitions-of-care program that reduces hospital readmission rates by connecting to primary care.

Providing a rewarding patient experience.In our most recent patient satisfaction survey, 96% of our patients said they were treated respectfully and efficiently. The MobileMed clinical team encourage patients, particularly those with chronic conditions, to play an active role in managing their own health, supplemented by educational materials, coaching and case management.

Executive Director’s Year-End SummaryWho could have imagined that what started out as two doctors hanging up sheets to create exam rooms in a Kensington church basement would

evolve into today’s MobileMed? Well, maybe it’s not so surprising, given this organization’s unwavering commitment to its mission and values and given our extraordinary community relationships. The FQHC award represents not a culmination, but rather a validation that we’re on the right path to make an even bigger difference.

Our FY15 Highlights, which summarize some of our tangible achievements, understate the profound impact that MobileMed has on thousands of patients. Primary care has become a more complex and comprehensive endeavor; MobileMed works hard to keep up with best practices in prevention and chronic disease management. Through it all, our incredible staff and volunteers build trusting, vital relationships with our patients that can change lives. I’m often struck by the way grateful patients thank us as much for the clinical team’s compassion as for their timely diagnosis and effective treatments.

In the highly competitive FY15 FQHC grants process, our

submission presented both a compelling community needs assessment for our Montgomery County service area and a viable operating vision for MobileMed. Fortunately, we are building on our effective and efficient operational model. At our core is a patient-centered primary care model attentive to the multi-cultural and linguistic needs of our patients. We utilize flexible, adaptable approaches for care access and continuity that work for patients with limited resources, and support those patients with case management and navigation services. MobileMed also embraces community collaborations with multiple and diverse partners for patient outreach, accessibility, and care delivery.

Indeed, I remain humbled by what our volunteers, hospital partners, and community collaborators contribute to our impact, and we take our resource stewardship seriously. The new FQHC designation offers new opportunities for MobileMed to: increase capacity; stimulate program development; and extend outreach to hard-to-reach and underserved populations. As our journey continues, MobileMed’s mission remains the same as it has always been: to improve the health of low-income people who face the greatest barriers to care access.

A 77-year-old Chinese man presented at MobileMed with swollen and itchy lower extremities. We referred him for a Doppler study and he was found to have deep vein thrombosis. Shady Grove Adventist Hospital agreed to perform a procedure in which a stent was placed in his left leg. Because MobileMed caught his condition and arranged to have it treated immediately, a pulmonary embolism was avoided. He now receives essential monitoring at MobileMed’s Anti-Coagulation Clinic to reduce the risk of serious complications.

“I am very grateful for the care.” Samuel, age 60, came to our Upcounty clinic with multiple health issues. Based on a physical exam, we referred him to a general surgeon for hernia repair. Lab testing revealed that the patient was severely anemic, so we arranged for him to receive blood transfusions. With MobileMed serving as a prima-ry care medical home with enabling services, his health condition is much improved.

Here’s a sampling of the patients that we serve and their healthcare stories:

A 53-year old uninsured woman came to our Korean Clinic with advanced breast cancer. MobileMed’s case management services steered her to a crucial state program and our charity network of cancer specialists. She had a biopsy, followed by surgery and chemotherapy. MobileMed helped guide and support our patient to ensure that she received timely, critical healthcare in a compassionate and culturally appropriate manner.
