Pre-Assessment Checklist

Hire/Identify a Program Coordinator Submit resumes for Program Coordinator position. Submit contact information for all individuals involved with

this contract.

Attend Georgia Strategic Prevention System (GASPS) Orientation

Provide a list of individual(s) who attended the training.

Attend required initial training convened by (OPSP) adhering to all registration, logistical and learning assignments.

Provide list of individuals who attended the training and their role in this project.

Organizational and Initiative charts Submit an organizational chart. Submit an alcohol prevention initiative staffing chart.

Pre-Assessment Checklist

Join, establish and/or convene a substance abuse prevention/Drug-Free Community coalition.

Submit meeting minutes and signature roster along with MPR reports.

If the coalition has to be established, provide thorough description of monthly progress.

Establish and convene the Community Prevention Alliance Workgroup (CPAW). Remember the mobilizing your CPAW is an ongoing process.

Signed membership agreements from CPAW. Submit sub-workgroup meeting minutes and signature roster

along with MPR reports.

Participate in the statewide Alcohol Awareness Campaign. Submit campaign plan and relevant materials to OPSP for

approval.

Why?• Increase community awareness around alcohol• Part of larger media campaign (Did You Know?) • April is Alcohol Awareness Month• Build capacity among providers - working with various forms of media, including social media • Collect service data• Deliverable in contract

What are we doing? Develop a campaign around alcohol during February Disseminate the campaign during March/April Submit campaign design and budget to your RPS by or before February 29th (i.e. if your proposed design is ready now, submit it for approval before you produce any campaign materials). For Alcohol Awareness Campaign, providers select the community or communities in the region to target (You are NOT REQUIRED to wait for your GASPS Community Defining conference call.) Each contract requires a campaign
