GatorGradCare dental benefit services are offered at the following UF College of Dentistry locations:

Eligibilityl Graduate assistants enrolled in GatorGradCare are eligible for these

services one time per year.l Dependents are not eligible for these services.

New patients may receive the following services at UF College of Dentistry, subject to a benefit limit of $330 per calendar year*:l A complete dental and oral health exam which includes

screenings for caries, oral cancer, temporomandibular joint (TMJ) issues, and periodontal (gum) disease*. We will also perform a general medical and dental history, and blood pressure reading.

l Complete x-rays (when needed).l If you have no signs of gum disease, we will complete a cleaning (a

second appointment might be necessary).

Note: If you are diagnosed with gum disease, a $100 credit will be applied to your gum treatment and the rest of the cost will be out-of-pocket.

Returning patients** may receive the following services at UF College of Dentistry, subject to a benefit limit of $330 per calendar year*:l A complete dental and oral health exam which includes

screenings for caries, oral cancer, temporomandibular joint (TMJ issues, and periodontal (gum) disease. We will also perform a general medical and dental history, and blood pressure reading.

l Bite wing x-rays.l Cleaning or gum disease maintenance.

Please arrive 30 minutes before your first appointment to allow time for paperwork and be sure to bring your GatorGradCare Insurance Card with you to each appointment.

After your examIf you need additional care that isn’t covered by GatorGradCare, and choose to have your treatment in our dental center, the cost for services will be an out-of-pocket expense for you.
