Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any
Pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.

Evins Funeral Home is a family business providing quality service and merchandise, because we are totally dependent upon our client families for prompt payment. Our payment terms are as follows: Advance Payment: The balance due for all funeral cost is payable at the time of the arrangement conference and prior to services rendered or delivery of merchandise. Evidence of payment and agreement between all parties must be confirmed at the
time of the arrangement conference.
Guaranteed Payments:
1) Pre-Paid Funeral Account:
When a funeral account has been pre-paid with Evins Funeral Home or any other funeral home, the pre-paid amount will constitute a guaranteed payment upon receipt of the pre-paid policy. Any amount not yet paid into the pre-paid funeral account will be a balance due.
1) Life Insurance:
A life insurance policy in an amount sufficient to cover the balance due will constitute a guaranteed payment once the status of the policy has been confirmed with the life insurance company and an assignment form has been signed by any and all beneficiaries. There is an insurance assignment charge of 5.99% of assigned amount.
Estate:
Under circumstances where the estate of the decedent must stand the burden of paying funeral expenses, we ask that a member of the family pay funeral expenses and file their claim for reimbursement with the decedent’s estate. In cases were we will assign an estate there is a fee.
Payments:
Payment in full required at time of arrangement conference
When monies are not readily available or guaranteed to us for funeral expenses, evidence of financing and payment must be given prior to the time of services. In all cases where total payment has not been received at the time of ar-rangements, a charge of 2.0% per month (24% per annum) of the unpaid balance due will be charged for each month or fraction thereof.
Note: We do not carry thirty (30) day accounts, however we do however offer special bank financing O.A.C ask for details.
As a matter of policy and for security and privacy reasons, Evins Funeral Home can only receive monies
for services from the contract signer who is the legal next of kin or power of attorney.
Cash Advances:
Cash advances items (money sent to a third party on your behalf and for your convenience) are sometimes necessary to conduct a funeral service. These charges represent a cash outlay, without a profit being realized by Evins Funeral Home. Therefore, reimbursement for such advances is requested at the time of the ar-rangement conference. The client family must mutually agree upon financial responsibility with Evins Funeral Home at the time of the arrangement conference and prior to services being rendered or merchandise delivered.

Basic Services of Funeral director, Staff and Overhead
Our basic service charge includes, but is not limited to, staff to respond to initial request for service; arrangement conference
with family or responsible party; preparation and filing of necessary certificates and permits; placement of obituary notices;
planning of funeral arrangements; and coordination of service with cemetery, crematory, veterans services, fraternal organiza-
tions or other as required. Also included in this charge are overhead expenses relative to our facility such as insurance, main-
tenance and utility expenses, secretarial and administrative cost, equipment and inventory expenses. This fee for our basic
services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already in-
cluded in our charges for direct cremation, immediate burial and forwarding or receiving of remains
Embalming
Except in certain special cases, embalming is not require by law. Embalming may be necessary, however, if you select cer-tain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. (Embalming maybe required for shipping on some common carriers).
Forwarding Remains to another funeral home Includes: Basic Services of funeral director, staff & overhead, embalming, removal from place of death, disinfecting / han-dling and professional fees. Does not include: airline or airport fees, death certificates or permits, shipping container or casket, other preparation of remains, cash advances, visitation or viewing.
Receiving of remains from another funeral home
Includes; Basic Services of funeral director, staff & overhead, transportation of remains to cemetery or crematory. Does not include: embalming, transportation from airport, other preparation, visitation of funeral ceremony.
Direct cremation: Direct Cremation with minimum cremation container provided by the purchaser Direct Cremation with container purchased from our funeral home (This cost is in addition to cost of container or casket) Direct Cremation with minimum cremation container provided by funeral home Without any ceremonies or burial rites at funeral home, graveside or elsewhere. This service includes Basic Services of funeral director, staff & overhead, Preparation, Removal within a 15 mile radius of funeral home, transportation to crematory, filing permit, minimum alternative cremation container (corrugated). If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and
can be made of materials like fiberboard or composition materials (with or without and outside covering). The containers we
provide are corrugated. Does not include Embalming, Services with Viewing, ID viewing, Disposition Permit, Sales Tax
or Certified Copies of Death Certificates.
Direct Burial: Direct Burial with casket provided by the purchaser Direct Burial with casket purchased by our funeral home (This cost is in addition to cost of casket) Direct Burial with a minimum casket provided from our funeral home
Without any ceremonies or burial rites at funeral home, graveside or elsewhere.

The rental unit casket has the appearance of fine solid oak wood grain quality, yet the “inner unit”, which is constructed of cardboard/particle board with a beige crepe interior, is removable for cremation purposes. This special unit is designed for use in our facility, any church, or for services elsewhere.
THE SURVIVOR OF THE DESEASED WHO IS HANDLING THE FUNERAL ARRANGEMENTS, OR THE RESPON-SIBLE PARTY, IS ENTITLED TO RECEIVE A COPY OF ANY AGREEMENT THAT HAS BEEN SIGNED AND PAID FOR IN FULL OR IN PART BY OR ON BEHALF OF THE DECEASED AND THAT IS IN POSSESSION OF THE FU-

WARRANTIES
Our funeral home makes no representations or warranties about the protective value of certain caskets or outer burial containers other than those made by the manufacturer. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the written warranties, if any, extended by the manufacturers thereof. The seller extends no other warranties and, specifically, no warranties of merchantability and fitness for a particular purpose.
WHO CAN GET SURVIVORS BENEFITS FROM SOCIAL SECURITY?
Here is a list of family members who usually can get benefits:
Widows and widowers age 60 or older.
Widows and widowers at any age if caring for the deceased’s child(ren) who are under age
16 or disabled.
Divorced wives and husbands age 60 or older, if married to the deceased 10 years or more.
Widows, widowers, divorced wives, and divorced husbands age 50 or older, if they are dis-
abled.
Children up to age 18.
Children age 18 - 19, if they attend elementary school or high school full time.
Children over age 18, if they became disabled before age 22.
The deceased worker’s parents age 62 or older, if they were being supported by the worker.
A SPECIAL ONE-TIME PAYMENT In addition to the monthly benefits for family members, a one-time payment of $255 can be paid to a
spouse who was living with the worker at the time of death, If there is none, it can be paid to: A spouse
who is eligible for benefits.
A child or children eligible for benefits. This payment cannot be made if there is no eligible spouse or child.
Evins Funeral Home does not accept social security survivors benefit as a form of payment.
For Veteran's And Their Spouses
All verifiable honorably discharged veterans are entitled to the following services at no additional
charge to the family: Optional burial plot for casket or urn at a national cemetery, grave marker
(available for all cemeteries), full size American flag and presidential certificate. Honors ceremony
conducted by active duty military personal and or local color guard are available at burial or memo-
rial site, ceremony includes playing of taps, flag folding ceremony and 21 gun salute (may not al-
ways be available). Spouses of veterans are entitled to burial at a national cemetery and grave
marker only. Transportation charges to national cemeteries are not included as part of funeral or
cremation packages, see general price list for cost information.
