This booklet was prepared to help you understand some of our services and procedures. Pleasebring this folder and all forms with you to the hospital on the day of testing and admission. If youhave any additional questions, please speak with your physician or with any member of our staff.

Scheduling Preadmission TestingPreadmission testing (Lab work, EKGs, X-rays, etc.) can be done on a walk-in basis at thefollowing locations:

Blood WorkIf you need blood work for Preadmission Testing, PinnacleHealth offers 14 walk-in locations inCarlisle, Dillsburg, Harrisburg, Hummelstown, Lemoyne, Mechanicsburg, Middletown andMillersburg. The Day of Preadmission Testingn Please bring this folder, a photo ID and all of your insurance

cards and the papers given to you at your physician’s office.n A parent or legal guardian must accompany all minors

under 18 years of age.n Do not “fast” unless instructed to do so by your surgeon.

The last meal eaten before coming for preadmission testingshould be low in fatty foods, milk and milk products.

n Below are directions to the appropriate locations for thePreadmission test at each site. (You can find driving directionson pages 12-17):Community CampusFree parking is available in front of the main lobby. Report tothe Patient registration area located in the hospital’s mainlobby, next to the front desk. A free shuttle service is availablefrom the parking lot if needed.Harrisburg CampusFree parking is available in the PinnacleHealth patient parkinggarage, which can be accessed from either Front Streetor Second Street. Bring your parking ticket in with you to bevalidated in the patient registration area. Valet service isavailable directly in front of the Harrisburg Hospital lobby.This service is purely optional. There is no charge for ValetParking. The Valet attendant will give you a claim check andphone number to call when ready. Enter the hospitalthrough the main lobby and follow signs to the Patient Reg-istration, which is located on the first floor near the mainlobby.Fredricksen Outpatient CenterFree parking is available in front of the Fredricksen OutpatientCenter. Imaging and lab services are located on the first floorjust off the rotunda. Enter the Fredricksen Outpatient Centerby the flagpole/circular driveway.

Before Your SurgeryA member of the Preadmissions Department will call you between 11 a.m. and 8 p.m. the day prior to your scheduledsurgery. Please have a list of your medications available toreview with the preadmission nurse during your phone call. Ifyou have not been called by 7 p.m., please call (717) 230-3461.

Instructions Before Your SurgeryFasting Instructionsn For adults (greater than 12 years of age or a child weighing

110 pounds or greater): NPO (no food or water) aftermidnight the evening prior to surgery unless instructedotherwise during the pre-op phone call.

n For children (less than 12 years of age): No more than 4 oz.of clear liquids, two hours prior to arrival time to hospital.Breast-fed babies may breast feed up to three hours prior toscheduled arrival time. Formula-fed babies may be givennot more than 4 oz. of infant formula up to five hours priorto scheduled arrival time.

Other Reminders:n Please complete the enclosed medication list. Leave all of

your regular medications at home, except inhalers, unlessyour physician specifically asks you to bring them. Bringany inhalers along with you and give them to your nursewhen you arrive.

n Please remember to bring your insurance cards and photo ID.n Arrange for a friend or relative to drive you home after the

surgery or procedure. You may not drive yourself home afterreceiving anesthesia.

n Two adults may accompany children who are havingoutpatient surgery. Please don’t bring children other thanthe patient.

n PinnacleHealth does not accept responsibility for the loss ordamage to any belongings brought into the hospital exceptfor those that have been deposited in the hospital safe.Patients are urged to leave articles at home that they considerto be of personal value. Belongings that are retained at thebedside are the sole responsibility of the patient.

n Since eyeglasses, contact lenses, prosthesis and denturesmust be removed before surgery, please remember to bringprotective containers, or you may want to leave them athome. Please bring your own contact solution.

n All jewelry, including piercings and your wedding band,must be removed. If your wedding band cannot be removed,please go to a jeweler to have it removed.

n Shower or bathe and brush your teeth.n Do not wear makeup on the day of surgery.n Wear loose, comfortable clothing and shoes with flat heels.n Money, credit cards or other valuables should be left at

home. Keep no more than $5 at your bedside for newspa-pers or small convenience items.

n Do not smoke after midnight prior to your surgery or procedure.n If you are staying overnight, please leave large items like

suitcases in your car until you have been assigned a room.

Taking Medicine Before SurgeryIn general, you can take all medications, including pain andacid reduction medications as well as beta-blockers, early inthe morning of surgery with a sip of water. Please note thefollowing exceptions:

Do NOT take the following medicines:n MAO inhibitors (for example Nardil and Parnate) must be

stopped two weeks prior to surgery. You must make yourprescribing physician aware that you are stopping thesemedications for surgery.

n Blood thinners including aspirin products, ibuprofen, Motrin,Plavix, NSAIDS must be stopped one week prior to surgery,unless instructed otherwise by your physician.

n Ace Inhibitors (Lisinopril, Enalapril, etc.) and Diuretics (waterpills) should not be taken the day of surgery.

Do NOT take any of the following:n Dietary Supplementsn Herbal medicationn Pondamenn Fastinn PATIENTS MUST BE OFF ALL DIET PILLS TWO WEEKS

PRIOR TO SURGERY

For patients who have diabetes: You will be instructed onhow to manage your diabetes medications during your pre-opphone call. These instructions will cover the medication youshould take the evening before and the day of surgery.

When You Arrive for Your SurgeryCommunity Campusn Follow the signs to the patient parking area. There is a shuttle

that runs every 5-10 minutes to bring you to the hospital.

n Enter the hospital building through the main entranceand unless you are instructed otherwise, please report tothe surgical waiting room.

n You will be directed to the Surgical Waiting Room. n To increase your comfort and protect your privacy,

PinnacleHealth has installed the Q-matic system. There is aQ-matic ticket dispenser immediately inside the door to theSurgical Waiting Room. Please take two tickets and have aseat in the waiting room.

n The receptionist will call you to the desk by using this number.This number will also be used to notify your family whenthey are able to join you in the pre-op area.

n You may be asked to complete a form regarding your medicalhistory and any medications you are taking.

Harrisburg Campusn Enter the hospital through the main entrance and unless you

are instructed otherwise, please report to the 3rd floorSurgical Waiting Room at your scheduled time by using thePublic Elevator D, which is dark blue in color.

n To increase your comfort and protect your privacy,PinnacleHealth has installed the Q-matic system. The Q-maticsystem resembles a computer screen/console. When you getoff the elevator and turn right, you will see the Q-matic ticketdispenser in the elevator lobby on your left. Please take a twotickets prior to entering the waiting room, and have a seat inthe waiting room.

n The receptionist will call you to the desk by using this number.This number will also be used to notify your family whenthey are able to join you throughout your surgical experience.Please limit the number of visitors you bring to the Pre-Oparea. To protect the privacy of patients, visitors in the Pre-Oparea will be limited to two per patient.

n You may be asked to complete a form regarding your medicalhistory and any medications you are presently taking.

West Shore Campusn Free Valet parking is available Monday-Friday.n Enter the hospital through the main entrance. Unless you

are instructed otherwise, please report to the patientregistration area on the first floor.

n After completing your registration you will be directed tothe Surgical Services Waiting Room on the second floor

n Please sign in at the Reception Desk in the Surgical ServicesWaiting Room.

n You may be asked to complete a form regarding your medicalhistory and medications that you are taking.

Inpatient SurgeryThe Day of Surgeryn A staff member will escort you to your room and prepare

you for surgery. You will also be interviewed by ananesthesiologist.

n After you are ready, your family will be able to be with youfor a while before your surgery.

n When it is time for your procedure or surgery, you will meetthe hospital staff who will take care of you throughout theprocedure. They will double check your identity, allergies,procedure and basic health status.

n Your family may wait for you in the Surgical ServicesWaiting Room.

n Your family will receive instructions to follow your progressthrough our tracking system message board, which also canbe accessed through the Internet or Smartphone.

Recoveryn Following a stay in the Post Anesthesia Care Unit, you will

go to a nursing floor to complete your hospital stay.n Your family member(s) will be contacted when you are

ready to be transported to your inpatient room.

Outpatient SurgeryThe Day of Surgeryn You must have someone available to drive you home.n A staff member will escort you to your room and prepare

you for surgery. You will also be interviewed by ananesthesiologist.

n After you are ready, your family will be able to be with youfor a while before your surgery.

n When it is time for your procedure or surgery, you will meetthe hospital staff who will take care of you throughout theprocedure. They will double check your identity, allergies,procedure and basic health status.

n Your family may wait for you in the Surgical ServicesWaiting Room.

n Your family will receive instructions to follow your progressthrough our tracking system, which also can be accessedthrough the Internet or Smartphone.

n After surgery, you will be taken to the Recovery Room, whichis staffed by specially trained nurses.

Recoveryn Following a short stay in the Post Anesthesia Care Unit,

you will go to the outpatient discharge area. Your family willbe contacted at this time and will be able to rejoin you.

n Your vital signs will be taken again at least once.n You will be offered something to drink.n If extended recovery time or medical care is necessary, you

may need to stay overnight.n When it is time for you to return home, the nursing staff will

give you any instructions you need. A copy of the DischargeInstruction Sheet will be provided for you to refer to athome.

n Arrange to have a responsible adult stay with you overnight.n Do not resume activities such as driving until your judgment

and coordination are back to normal. For some people, thiscan take one or two days.

n Do not drink alcohol for 24 hours following anesthesia orsedation.

n We may make a call the next working day to check on howyou are doing.

About Your Anesthesian Anesthesia is administered by anesthesiologists, who are

specially trained medical physicians, that work with nurseanesthetists and technicians.

n Your anesthesiologist is not an employee of PinnacleHealth.This means you will receive a separate bill for your anesthesiaservice that is not part of your hospital surgery charge.

n On the day of your surgery, your anesthesiologist will meetwith you to discuss your medical history and go over anytesting or lab results you may have had done recently. Afterdiscussing your options and answering your questions, ananesthetic plan will be determined.

n You will be continuously monitored and cared for through-out your surgical procedure. Our goal is to provide a safeand comfortable anesthetic experience and recovery.

Regional Anesthesia (for certain procedures):Depending on your surgical procedure, a regional anestheticsuch as a spinal, epidural, or a nerve block may be discussed asan option for pain control. The anesthesiologist will discussthe details of the procedure, including the risks and benefits. Ifyou, your anesthesiologist and your surgeon think you maybenefit from one of the above options, the procedure is typi-cally performed prior to surgery. n Spinal Anesthesia: This consists of a single dose of local

anesthesia being delivered to numb you from your ab-domen to your feet. The extent of your numbness dependson your surgical procedure. The pain control from a spinalusually lasts 4-6 hours.

n Epidural Anesthesia: This procedure involves placing acatheter in the epidural space which will deliver local anes-thesia to provide pain control over a longer period of time,usually 1-3 days.

n Nerve Block: This consists of an injection of local anestheticnear a nerve to provide pain control for during and afteryour surgery. Typically, the block lasts 4-24 hours.

Tracking Your Location During SurgeryNot knowing what your loved one is experiencing is a stressfulpart of surgery. To help update family members throughoutthe surgery process, PinnacleHealth uses an electronic trackingsystem. When you arrive in the pre-operative area, you will beassigned a badge. This badge is placed on your IV, or your chart

if you do not receive an IV, and stays with you throughout yourstay in the surgery areas. The tracking system allows our staffto identify your location at any time and provide updates toyour friends and family. These updates are posted on theelectronic message boards in our surgery waiting rooms, as wellas kiosks in our hospital main lobby and cafeterias. Your friendsand family simply watch for the number assigned to you onthe boards and use a password with the kiosks. Our trackingsystem may also be accessed through the PinnacleHealthwebsite (www.pinnaclehealth.org) and smartphones.

Your ComfortProcedures requiring use of the operating room may result inminimal discomfort to severe pain. Managing your discomfortis of primary importance to us. Our goal is to partner with youto set safe comfort goals.To do so, we will gather some information from you in thepre-op area. Some questions you may be asked include:n Do you have pain now?

• Related to your scheduled procedure?• Unrelated to your scheduled procedure?

n Using 0-10 pain scale:• Where do you place your pain?• What level do you consider tolerable/goal?• What do you do for this pain currently?

Your DischargeWhen you are discharged, you will be given written instructionsthat are specific for your procedure. If you need assistanceplanning your care after discharge, please call our Social Workand Discharge Planning Department at (717) 782-5574 between8 a.m.-4:30 p.m.If you experience ANY surgery-related problems after yourdischarge, contact your physician immediately for advice.

Patient Portal and WebsiteYou can access portions of your hospital medical record securely online. Through our Patient Portal, you can also accessHospital discharge information and Post-visit summary reports.You can securely create your Patient Portal account by goingto www.pinnaclehealth.org/patient-portal and selecting the "Request an Account" button. Surgical Site Infection PreventionA surgical site infection is an infection that happens at or closeto the part of the body where surgery was performed. It usuallyoccurs within the first few weeks after surgery, but in somecases can happen later.Symptoms of a surgical site infection include redness, painor swelling at the incision, fluid draining from the incision,fever or you feel poorly or your incision opens. Call your doctorif you think you might have an infection. It might not be aninfection, but it is important for you to let him/her know sothat they can give you the proper follow-up care.

What you can do to prevent a surgical site infection:n Do not shave near the body area where the surgery will be

performed. Shaving can cause skin irritation which can leadto an infection

n Eat nutritious foodsn Control your blood sugar leveln Stop tobacco usen Tell your surgeon if you have any type of infection or rash n Have good personal hygiene. Make sure you shower with

an antiseptic soap. You may be asked to use a special soapto shower before you come for your surgery.

Surgery CancellationOn occasions, surgeries do not take place on the day or timethey are scheduled. This can happen for a number of reasons,including, but not limited to:n Incomplete or abnormal lab results n Failure to comply with pre-operative instructions n Availability of operating rooms and hospital bedsn Physician, patient or family requestIn the event that your surgery is canceled, we will make everyattempt to notify you before you arrive at the hospital. Wethank you for your patience and understanding.

Paying for Your Care Many changes have taken place in the health insuranceindustry in recent years. Procedures and services once coveredin full are now partially covered, covered only under certaincircumstances, or, in some cases, not covered at all. Everyinsurance plan is different. Please be sure to check yourinsurance and ask questions before coming to the hospital.Many insurance plans and health maintenance organizations(HMOs) now require preadmission approval and/or a secondopinion for certain procedures. For your own peace of mind,we advise that you know the benefits of your insurance plan.n Please bring all insurance cards, photo ID, and required

forms with you when you come to the hospital forpreadmission testing. This will speed your admission andprevent billing problems.

n Anyone who requires emergency service with PinnacleHealthwill be treated, regardless of his/her ability to pay.However, patients will be expected to meet their financialresponsibilities.

n If you are scheduled for a surgical procedure and do not havehospital insurance, you will be asked to pay a portion of theestimated charges or establish a payment agreement withthe Financial Counseling Office prior to your admission.

n If you have any questions about estimated charges, pleasecall our Financial Counseling Office at (717) 230-3717.

n If you have hospital insurance, Medicare or MedicalAssistance, you will be asked to pay identified co-paymentsor deductibles upon admission.

n Charges for anesthesia equipment, medicines and nurseanesthetists will be included in your hospital bill. Youranesthesiologist will submit a separate bill to you or yourinsurance company. Feel free to ask questions abouthis or her charge as you would with your surgeon or otherprivate physician.

Patient Rights & ResponsibilitiesPinnacleHealth Hospitals recognize that patients have bothrights and responsibilities. To comply with The Joint Commission,American Osteopathic Association, American HospitalAssociation standards and with federal and state law, theHospitals accept the following statement of patient's rightsand responsibilities and will provide this statement to patientsat the time of admission. Issues raised by patients, theirfamilies or designated/legal representatives will be handledby the Patient Representative.

A Statement of the Patient’s Rights1. Patients have the right to respectful care, given by competent

personnel, which reflects consideration of their personalvalue and belief systems and which optimizes their comfortand dignity.

2. Patients have the right, upon request, to be given the nameof their attending physician, the names of all other physiciansdirectly participating in their care, and the names andfunctions of other healthcare personnel, including but notlimited to medical students, resident physicians and othertrainees, having direct contact with the patient.

3. Patients have the right to every consideration of privacyconcerning their medical care program. Case discussion,consultation, examination and treatment are consideredconfidential and should be conducted discreetly, givingreasonable visual and auditory privacy when possible. Thisincludes the right, if requested, to have someone present whilephysical examination and treatment are being performed, aslong as it does not interfere with the performance of treatment.A. Patients have the right to request a room transfer if

another patient or a visitor in the room is unreasonablydisturbing the patient and another room, equally suitablefor the patient's care needs, is available.

4. Patients have the right to have all information, includingrecords, pertaining to their medical care treated asconfidential, except as otherwise provided by law or thirdparty contractual arrangements. Patients have the rightto have their medical records read only by individuals directlyinvolved in their care, by individuals monitoring the qualityof their care or by individuals authorized by law or regulation.

5. Patients have the right to know what hospital rules andregulations apply to their conduct as a patient.

6. Patients have the right to expect emergency procedures tobe implemented without unnecessary delay.

7. Patients have the right to good quality care and highprofessional standards that are continually maintained andreviewed.

8. Patients have the right to full information in layman’s terms,concerning diagnosis, treatment and prognosis, includinginformation about alternative treatments and possiblecomplications. When it is not medically advisable that suchinformation be given patients, the information shall begiven on their behalf to their next of kin, designated/legalrepresentative or other appropriate person.

9. Except for emergencies, the physician must obtain thenecessary informed consent prior to the start of anyprocedures or treatments, or both. Examples of proceduresor treatments requiring informed consent are surgicalprocedures including the administration of anesthesia;administering radiation, chemotherapy and bloodtransfusions; inserting a surgical device or appliance; andadministering an experimental medication, using anexperimental device or using an approved medication ordevice in an experimental manner.

10. Patients or a legally responsible party, in the event thepatient is unable to give informed consent, have the right tobe advised when a physician is considering the patient aspart of a medical care research program or donor program.Also, patients or a legally responsible party must giveinformed consent prior to the actual participation in such aprogram. Patients or a legally responsible party may, at anytime, refuse to continue in any such program to which theyhave previously given informed consent.

11. Patients have the right to request or refuse any drugs, treat-ment or procedure offered by the Hospitals, to the extentpermitted by law. A physician shall inform the patient of themedical consequences of the patient’s refusal of any drugs,treatment or procedure. A patient’s right to request anydrugs, treatment or procedure must not be construed as amechanism to demand the provision of treatment or servicesdeemed medically unnecessary or inappropriate.

12. Patients have the right to assistance in obtaining consultationwith another physician at their request and their own expense.

13. Patients have the right to medical and nursing services with-out discrimination based upon race, color, religion, gender,sexual preference, age, disability, national origin, ancestry orsource of payment.A. Patients who are disabled have the right to expect

reasonable and equal access to the facilities, services andprograms of the Hospitals.

14. Patients have the right to be communicated with in a mannerthat is clear, concise and understandable. If the patient doesnot speak English or is hearing impaired, the patient shouldhave access, where possible, to an interpreter or auxiliary aids.

15. The Hospitals shall provide the patient or patient’s designee,upon request, access to all information contained in theirmedical records, unless access is specifically restricted by theattending physician for medical reasons. Access to informationshall be within a reasonable time frame and the Hospitalsshall actively seek to meet the request as quickly as its recordkeeping system permits.

16. Patients have the right to expect good managementtechniques to be implemented within the Hospitals, theavoidance of unnecessary delays, and when possible, theavoidance of personal discomfort through effective painmanagement.

17. Patients have the right to be transferred to another facility, ifmedically permissible, when the Hospitals cannot provide thetype of care indicated because of a conflict with the Hospitals’mission or the incapacity to meet the patient’s needs orrequests. Patients may be transferred to another facility onlyafter they or their designees or legal representatives havereceived complete information and an explanation concerningthe needs for and alternatives to such a transfer. The transfermust first be accepted by the receiving institution.

18. Patients have the right to examine and receive a detailedexplanation of their bill.

19. Patients have the right to full information and counselingon the availability of known financial resources for theirhealthcare.

20. Patients have the right to expect that the healthcare facilitywill provide a mechanism whereby they are informedupon discharge of their continuing healthcare requirementsfollowing discharge and the means for meeting thoserequirements.

21. Patients cannot be denied the right of access to an individualor agency who or which is authorized to act on their behalf,to assert or protect the rights set forth in this policy.

22. Patients have the right to be informed of their rights duringthe admission process or at the earliest possible moment inthe course of their hospitalization.

23. Patients have the right, in collaboration with their physician, tomake decisions involving their healthcare and to participatein care planning and treatment. This right applies to the legalguardians of neonates, children and adolescents unless thelaw provides otherwise.

24. Patients have the right to make decisions regarding thewithholding of resuscitative services or the foregoing orwithdrawal of life-sustaining treatment within the limits ofthe law and the policies of the Hospitals.

25. Patients have the right to participate in the consideration ofethical issues surrounding their care, within the frameworkestablished by the Hospitals.

26. Patients have the right to formulate “advance medicaldirectives” or to appoint a surrogate to make healthcaredecisions on their behalf. These decisions will be honored bythe Hospitals and its healthcare professionals within the limitsof the law and the Hospitals’ mission, values and policies.A. Patients are not required to have or to complete an

“advance medical directive” in order to receive care andtreatment in the Hospitals.

27. A patient’s rights may be exercised, under certaincircumstances permitted by law, by their designee or legalrepresentative or by their next of kin. Although the patienthas the right to participate in their care and treatment tothe fullest extent possible, there are circumstances where thepatient may not be able to participate. Examples include, butare not limited to, patients who have been adjudicated aslegally incapacitated; patients who are medically incapableof understanding and consenting to treatment; and minorswho are not emancipated.

28. Patients, their designees or legal representatives have theright, without recrimination, to present grievances regardingtheir care and treatment and to have those grievancespromptly addressed by the Hospitals.A. Patients, their designees or legal representatives also have

the right, without recrimination, to present grievances tothe Pennsylvania Department of Health and to theKeystone Peer Review Organization and shall be informedof the telephone numbers and addresses of these agencies.

29. Patients have the right to have a family member orrepresentative of their choice and their own physician notifiedpromptly of their admission to the Hospitals.

30. Patients have the right to receive care in a safe setting.

31. Patients have the right to be free from all forms of abuse orharassment.

32. Patients have the right to be free from seclusion and restraintsof any form that are not medically necessary or are used asa means of coercion, discipline, convenience or retaliation bystaff.

A Statement of the Patient’s Responsibilities1. Patients or their designees or legal representatives must

provide accurate and complete information about presentconditions, past illnesses and injuries, prior hospitalizations,medications, existence of “advance medical directives” andother information relating to the patient’s health conditionrequested by the Hospitals or by the patient’s physicians whichis necessary to treat the patient.

2. Patients must inform the staff of the Hospitals if they donot clearly understand a contemplated course of action andwhat is expected of the patient.

3. Patients must cooperate with all hospital personnel andask questions if directions and/or procedures are not clearlyunderstood.

4. Patients must help the physicians, nurses and allied medicalpersonnel in their efforts to care for the patient by followinginstructions and medical orders.

5. Patients are responsible for providing the Hospitals with a copyof their “advanced medical directives,” when applicable.

6. A patient’s designee or legal representative must be availableto hospital personnel to participate in the patient’s course oftreatment if the patient is unable to effectively communicatewith the staff of the Hospitals and with the patient’s physicians.

7. Patients must be considerate of other patients and of theHospitals’ personnel by assisting in the control of noiseand visitors in their room and by observing the Hospitals’smoking policy.A. Patients must also be considerate of the Hospitals’ property

and property belonging to other patients.8. Patients are responsible for paying all or a portion of their

hospital bill unless the patient’s third party payer is solelyresponsible for payment of the hospital bill based upon acontract with the Hospitals or as otherwise required by law.

9. Patients must not take any drug that has not been prescribedby a physician and administered by the Hospitals’ staff.A. Patients must not possess or consume alcoholic beverages

or prohibited substances.
