INTERNATIONAL HEALTHCARE PLAN GUIDELINES TO THE DIRECT SETTLEMENT NETWORK (DSN) in the Middle East When visiting the medical providers within the network, please follow the procedures listed below:
Present your Goodhealth DSN card to the receptionist prior to the consultation Sign the treatment voucher / claim form to acknowledge receipt of the treatment
following the consultation Settle any co -insurance, excess payments or non-covered items (please see below
list) with the clinic prior to leaving. NON-COVERED ITEMS The following list is a list of the major exclusions from Goodhealth insurance policies. Please note this is not an exhaustive list and does not substitute the policy wording. In case of any doubt, please contact us for clarification. 1. Any Co-payment or excess applicable. (As shown on the DSN card) 2. Physiotherapy in excess of five sessions per condition. (Unless pre-approved by us) 3. Alternative Treatments (including but not limited to Chiropractic treatment, Osteopathy, Homeopathy & Acupuncture) 4. All outpatient services under Major Medical module and Dental, Maternity, Wellness, Vaccinations and Optical benefits under all other modules are not available via Direct Billing 5. Pre-existing conditions i.e. any medical conditions which existed before the Date of Entry, including related, secondary or associated complaints. 6. Any treatment received after the 12-month period from the “valid from” date. (As shown on the DSN card) 7. Chronic conditions (unless pre-approved by us) - i.e. conditions that cannot by cured by proven methods of conventional medicine (including but not limited to Hypertension, Diabetes, Hypercholesterolemia). 8. Birth injuries, Congenital Anomalies, genetic deformities or diseases, Hereditary Medical Conditions with symptoms present at birth. 9. Infertility and Birth control. 10. Sexually-transmitted diseases 11. Self inflicted injuries/ alcohol or substance abuse/ sleep apnea or other sleep disorders. 12. Rehabilitation programme (unless pre-approved by us) 13. Treatment for Psychiatric complaints / conditions (unless pre-approved by us) 14. Costs which are above Reasonable and Customary charges with respect to the city / area of your clinic.

15. Telephone, fax or copying charges. 16. Cost of providing medical documentation or completing a claim form. 17. Treatment for refractive errors and sight defects. 18. Dietary supplements, vitamins, minerals, dietician charges. 19. Routine check-ups, screening tests, travel advices. 20. Use of Ambulance Services (unless pre-approved by us). Please be reminded to call the numbers mentioned on the back of your Membership Card in case of emergency. 21. Any course of treatment where the combined cost for lab & radiology exceeds USD 200.00 (unless pre-approved by us) IMPORTANT NOTES I. The direct settlement network is not to be used to obtain any treatment, which falls outside the scope of your policy. Abuse of the network facility and Membership Card may result in the suspension or immediate cancellation of your policy. Please refer to your Application Form and the Policy Wording for further details regarding the suspension & cancellation procedures. II. Should you require treatment as an in-patient or day-patient, please contact Goodhealth’s Claims Services or use the numbers mentioned on the back of your Membership Card. Medical Services requiring Pre-authorisation The following is a list of the most commonly occurring Medical Services that require pre-authorization. Please note however that this is not an exhaustive list and if in doubt contact us for clarification.
Physiotherapy in excess of five (5) sessions per condition. • MRI, CT Scans, Scopies, and major investigative and diagnostic procedures. • Any second and subsequent medical opinion / s from any medical practitioner for the same medical condition. • Any out-patient course of treatment where cost for lab & radiology together exceeds USD 200.00 or equivalent in local currency. • Pharmacy: Medications exceeding USD 200.00 or equivalent for Acute conditions & All medications for chronic conditions. • All Day-patient and In-patient treatments.
