Mediclaim on floater basis. Under this floater scheme the sum insured for the individual family members will be added and the total insured amount will be treated as sum insured for family members.
Corporate buffer to the tune of 10 lakhs. This facility will come in handy during the situation where an employee utilizes his/her sum insured for the treatment and if he / she needs an additional amount for treatment, the same can be utilized from the corporate buffer.

Collect a Medical Reimbursement Format from HR
Ensure that you fill the form completely and attach the relevant documents like original discharge summary, bills, prescriptions.

a) The Nominee's Name, DOB & Relationship should be mentioned clearly in the Nomination Form in-order to avoid discrepancies during the Claim Processing
b) Incase of any Additions/Deletions in the Nomination List provided by the Employee, kindly intimate it to the HR Department as soon as possible
Original Discharge Summary / Report from the Hospital with the following details:

Company contributes 15% of an employees monthly earned basic.

• If any Loan is pending, further loan will not be allowed except in case of emergency.when one emergency loan is pending, another emergency loan shall not be given
• Second Loan for higher amount to foreclose the previous loans shall not be allowed
