A MORE EFFECTIVE PROCESSCase Presentation by Ross W. Nash, DDS

A patient presented missing a mandibular first premolar and was aware that her other teeth were moving but did not want to wear a removable appliance and could not afford an implant. To give her a minimally invasive option, I proposed the FirstFit Bridge System, which is manufactured using Zirlux Zirconia and would require only small inlay preparations on the healthy abutment teeth. The preparations would be prepared during the same appointment as the seating of the final restoration, making it convenient for the patient.

A bite registration and full upper and lower impressions were taken with a polyvinyl impression material. These were sent to the dental laboratory along with preoperative photographs and an x-ray showing both abutment teeth. The laboratory fabricated both the final restoration and the preparation guide. The zirconia bridge can be seen on the printed model in Figure 1.

At the delivery appointment, the guide was inserted into the patient’s mouth and the special FirstFit handpiece was used to prepare the two inlay rests with the diamond bur provided by the laboratory (Figure 2). The prepared rest seats can be seen in Figure 3 and the bridge that was bonded in place can be seen in Figure 4. A radiograph of the FirstFit bridge in place is shown in Figure 5.

By using the FirstFit System, we were able to replace a patient’s mandibular first molar in a minimally invasive way, allowing for other procedures in the future should they be needed or desired.

LEARN MORECAN I USE MY OWN HANDPIECE? No. The FirstFit handpiece has been specifically designed with the correct torque and is the only one suitable for this technique.

WHAT TYPE OF IMPRESSION DO YOU RECOMMEND? Digital impressions and Clinicians Choice have proven to be efficient solutions for the FirstFit System. Any brand of polyvinylsiloxane impression material that provides a clear impression can be used.

IS THERE ANYTHING I NEED TO DO WITH THE GUIDES BEFORE USING THEM ON A PATIENT? We recommend submerging all guides in warm water for approximately 20–40 seconds per guide, which makes the guides easier to use and flexible. You should also verify that the bur is securely locked into the FirstFit handpiece before inserting it into the guide.
