Mentoring Communities

Basic guidelines per the FOA:• Mentor minimum of two communities in

Years 3 – 5– States/Cities/Counties – Preference to

communities who have not received CDC funding (Steps, ACHIEVE, PHC, CPPW)

– Tribes – mentor 50/50 in and outside tribe

• At least 10% of funds should be directed to mentoring selected communities

Basic guidelines per the FOA: • Produce at least one Implementation

Guide– State-Coordinated: One Guide for each

funded community

• Instruct mentees on using Guide

Implementation Guide

Timeline• By June 15, 2010: Submit proposed PSE

strategies to Project Officer• By July 15, 2010: CDC confirmation of strategy

for your Guide• By December 29, 2010: Submit Guide to CDC no

(with your Annual Report)• Ongoing: Revise/update Guide as needed and

submit revised Guide (if applicable) to CDC

• How are you determining the effectiveness of the proposed strategy/strategies?

Recommended Process• Refer to Implementation Guide document
