Welcome to another issue of Altoona Regional’s Healthy Living Magazine. It was one year ago, in June 2005, that we launched this publication so we could “speak to all ages about health and wellness” and about how you and your family can achieve and maintain good health.

Our goal remains today as it was then — to showcase not only the people and services of Altoona Regional but also your friends and neighbors who have come to us for their health care needs. As I wrote then, “Theirs are the stories that will make Healthy Living Magazine entertaining as well as informative.”

In this issue are several such stories, starting with the one on Page 3 about Dennis Fasekas of Duncansville and his experience at our Wound Care and Ostomy Program. The program produces so many stories of success — many with heartwarming overtones — it was diffi cult to choose just one. But Dennis’ stood out as an illustration of exceptional staff members who treat not just wounds but people. Because wounds are often deeper than they appear.

This holistic approach is also the theme of the article on Page 8 about how our Pastoral Care

In a continuing effort to increase patient satisfaction, Altoona Regional has expanded the Altoona Hospital Campus Emergency Department by adding eight beds. The expansion will help staff see patients faster and treat and discharge them sooner. The Emergency Department now has 31 total beds and handles 45,000 visits a year — nearly 125 a day. That’s up from 28,000 in 1999.

With a growing volume of visits, Emergency Department staff frequently had to place patients in beds in hallways, where privacy was a challenge. Now, to ensure privacy, beds are separated by walls, not curtains.

Also, the ED staff has two areas for patient evaluation, along with areas for consults and behavioral health treatment and observation. And bedside registration allows the staff to follow the national trend, which is to begin treatment as soon as possible after triage.

Feedback from patients and family members, as well as the Medical Staff, has been very positive, according to R. Scott Magley, M.D., senior vice president and chairman of the Department of Emergency Medicine.

HEALING MISSION doesn’t stop with the wounds you can see

Dennis Fasekas, 50, of Duncansville, enjoys amusement park rides, hiking and camping — which is a big deal for someone who once weighed more than 600 pounds and suffered from a debilitating leg ulcer.

In the fall of 1998, Dennis reached his peak weight of 635 pounds. He could no longer work. He had high blood pressure and sleep apnea, a condition where you stop breathing for short periods during sleep. To make matters worse, the open, oozing wound on his lower left leg was not responding to any of his home remedies.

“This wound would drain so much that my sweat pants would be soaked by the end of the day,” Dennis recalled.

Little did he know that the compassionate care he’d receive at the Wound Care and Ostomy Program would literally heal all fac-ets of his life –– physical,

emotional and spiritual.

“If it weren’t for this won-derful department, I wouldn’t be where I am today. I may not even beliving. I thank God for bringing me to them,” said Dennis, his voice cracking with emotion.

A key to the program’s success is the dedication of its nursing staff and the fact that the same nurse sees a patient every time he or she visits, according to Administrative Director Ellen Schraff, a registered nurse and certifi ed wound specialist.

“Primary nursing care continues to play a large part in the successful treatment of our patients,” she said. “We have recog-nized this as the best way to meet both physical and emotional needs in our commitment to provide holistic care. And if it be-comes necessary to admit a patient to the hospital, they are seen by the same wound care staff.”

Amy Swires, R.N., was Dennis’ primary nurse. “It was amazing to watch the (psychological) changes in him,” she related. “He became more outgoing, and more of his personality emerged as we cared for him and he lost weight.”

With the wound clinic staff’s support and encourage-ment, Dennis lost 123 pounds. His other health problems resolved as the weight came off. His treatment team also included physical therapist Amy Flick, who specializes in lymphedema treatment, and medical doctors Neil Kaneshiki and Shuba Maitra, who specialize in wound care.

“I can’t say enough positive things about Drs. Kaneshiki and Maitra,” Dennis said. “They are honest and up-front, and I like that in a doctor.”

After Dennis was discharged from the program, he made sure to stay in touch with the staff.

A month after undergoing sinus surgery, Kathleen Hasselback remains infatuated with being able to breathe freely.

“I’ll be out with friends and I’ll take my fi nger, block one of my nostrils and say, ‘Listen to this!’” the 56-year-old DuBois woman shares. “And you can hear how the air moves freely in and out. It’s just great!”

Equally great is the absence of the killer sinus headaches, facial swelling and pressure she endured before her surgery.

“It feels so good not to have the headache, the pressure or the edema,” she said. “I was not a pretty person in the morning. My voice, which had gotten deeper and huskier from the constant cough and postnasal drip, is starting to clear up. My body in general is starting to feel better now that I’m not having constant sinus infections.”

“This technology is not a replacement for surgical skill. It’s a tool that adds to surgeon confi dence, and the patient benefi ts with a safer, more thorough sinus procedure.”

As for Ronald, he’d recommend the surgery: “I took no pain medicine afterward. I had no discomfort in my head at all. I had this surgery before and had quite a diffi cult time recovering. They are

doing something right these days, I can say that.”

And his symptoms?

“I have no trouble breathing at all; there’s no discharge, no more headaches and no more stuffed-up feeling. It’s real good. It’s 100 percent recovery,” he said. “And everyone at the surgery center was extremely courteous. I’d recommend this to everyone.”

“People seem to be afraid of peritoneal dialysis and I try to reassure them,” Dick said. “It’s really easy once you get used to it, and it helps to have a good partner like my wife.”

Another benefi t of dialysis at home is the ability to travel. Dick, who served as mayor of Martinsburg for 28 years, looks forward to vacations, especially camping adventures.

“My medical supplier delivers my dialysis equipment to me on vacation, no matter where I am,” he said.

The supplier also equips Dick with a manual dialysis bag in case anything would go wrong. In that case, though, he always has a list of nearby dialysis units he can go to for treatment.

Here are some tips from the Consumer Product Safety Commission and Safe Kids Blair County to help keep your Fourth of July holiday happy:

• Know the laws regarding fi reworks where you live or where you are vacationing during the Fourth of July. Pennsylvania, 38 other states and the District of Columbia enforce federal regulations and applicable state restrictions and allow some consumer fi reworks –– those containing no more than 50 milligrams of powder — and novelty items.

• Never allow children to play with or ignite fi reworks.

• Read and follow all warnings and instructions.

• Be sure other people are out of range before lighting fi reworks.

• Only light fi reworks on a smooth, fl at surface away from houses, dry leaves and fl ammable materials.

• Never try to relight fi reworks that have not fully functioned.

• Keep a bucket of water nearby in case of a malfunction or fi re.

“The key is to enjoy the pyrotechnics safely and leave the aerial light displays to the professionals,” says Sherry Turchetta, R.N., community education specialist with Altoona Regional and coordinator of Safe Kids Blair County.

Sherry reports that of the more than 4,300 children ages 14 and under treated in U.S. emergency rooms each year for injuries result-ing from fi reworks, most

related activities are more related activities are more frequently injured and sustain more severe injuries than bystanders,” according to R. Scott Magley, M.D., chairman of the Department of Emergency Medicine.

“Unsupervised children are 11 times more likely to be injured by fi reworks. This is because young children lack physical

Sherry noted that sparklers, while legal and considered “safe” by many, burn at a very high temperature and can ignite clothing.

“Often, children cannot understand the danger involved and can’t act appropriately in case of emergency,” she warned.

“Be realistic about the possibility of gaining weight while on vacation,” Randy says. “You can’t really avoid it, but you can defi nitely minimize it.”

Enjoy your next vacation by following Randy’s rules:

• “Winging it” won’t cut it! You need to plan. Save up your calories by watching what you eat the week before vacation.

• Book a hotel with kitchen accommodations to prepare healthy meals.

• Make smart breakfast choices like fruits, yogurts and whole grain cereals.

• Opt for menu items containing the words “steamed,” “roasted” or “broiled.”

• Avoid creamy, high-calorie salad dressings. Instead, choose olive oil and vinegar or a light dressing.

• Sample small amounts of high-calorie foods, like desserts.

• Watch your portion sizes. Try sharing a large entrée

with a friend.

• Be extra careful at buffets because it’s easy to overeat.

• If you’re starved, you tend to eat too much, so spread meals throughout the day.

Ah, summer vacation! Get ready to kick back, relax and enjoy some well-deserved time away. And as you gather items to pack your suitcase — shorts, camera, book, toothbrush — don’t forget your favorite pair of jogging shoes.

That’s right. Vacation is no excuse to forgo exercise. After all, people typically consume more, and burn less, calories each day while on vacation. Your body needs exercise to offset that annoying side effect of all-you-can-eat buffets — weight gain.

Fellowship trained in sports medicine, William A. Tyndall, M.D., Ph.D., of University Orthopedics Center, believes vacation is an excellent time to maintain, add some variety, and even increase your exercise program.

“Vacations are often planned at destinations of great beauty, such as the ocean or the mountains,” he said. “These sites provide easy distractions to sneak a program of walking, running, swimming or biking into your daily routine.”

Before boarding the plane or getting in the car, remember to have realistic expectations about exercising while traveling. There’s a good possibility that you will break from your normal fi tness routine. But instead of feeling guilty about it, or giving up altogether, fi nd alternate ways to incorporate physical activity into your vacation.

• Go hiking in the mountains and enjoy the view.

• Go swimming at your hotel or in the ocean. Swimming is great for the heart and lungs and easy on the joints.

• Get off the beach blanket and play Frisbee or volleyball with your family.

• Go for a brisk walk on the beach. Walking or running on sand generally burns more calories than on smooth pavement.

• Steer clear of cabs, and take a walking or bicycle tour of the area. The more places you can get to by foot, the better, and that includes taking the stairs instead of the elevator.

Regular exercise has been shown to help control weight, cholesterol, blood pressure and blood sugar. It’s also a natural antidepressant and can help you generally feel better. In order to reap those rewards, Dr. Tyndall believes you need to develop good habits at home and take them with you anywhere you go, including vacation.

“I encourage you to incorporate exercise into your daily routine,” he said. “So, if you exercise at home, then it should be continued on vacation.”

While ministering, Pastor TM may listen to, talk with or pray with patients, family or staff; don a Jewish prayer shawl, or distri-bute communion from a chalice.

For Pastor TM, the Pastoral Care director, and the fi ve full-time and nine part-time chaplains on staff, it may be a tray table in a patient’s room or some available space on a trauma bay bed where the altar is set and work continues on healing the whole patient — body, mind and spirit.

“We want to be where the person is,” Pastor TM said. “The question we ask, both literally and fi guratively, physically and spiritually, is how are we ministering to the person right now where they are.”

When you’re lying on your back looking up, Pastor TM said, you have a very different perspective than when you’re standing next to a bed.

“When you come into the hospital, a lot of things can happen to you,” Pastor TM explained. “You may sacrifi ce your privacy, or your dignity, but the one thing you always have is your spirit. No one can ever take that away. That’s the part to which we minister.”

Defi ning the line between religion and spirituality, two unique concepts, Pastor TM said, is the strength of her department.

“Spirituality and religion are two very different things,” she explained. “Everyone has a spirit, but not everyone is religious. This department is equipped to minister to the spiritual side.”

Before the November 2004 merger of the hospitals, Pastor TM admitted, neither Pastoral Care department was designed to address a broad range of spiritual needs. Bon Secours-Holy Family Hospital was sponsored by a national Catholic health care organization and offered predominantly Catholic rites and services, while Altoona Hospital employed only one full-time Catholic priest and one part-time layperson, and simply could not address the volume of patient and staff needs.

But with a unifi ed staff — the fi rst within the merged system — and a plan to spiritually minister rather than be religious representatives, Pastoral Care crossed denominational lines to serve any religion or faith tradition — Catholic, Protestant, Jewish, Muslim, Hindu, Buddhist and more.

Now, along with the Catholic Mass, optional daily Eucharist and Prayer Partners, Pastoral Care is breaking new ground by offering an Interfaith

When you think about it, a hospital room or emergency room bed and a combat zone aren’t all that different from one another.

Each can be a scene of inspiration and hope and incredible sorrow, and all are often places where one’s spirituality or religious faith is invoked in an attempt to fi nd peace or make sense of a traumatic situation.

Pastor Terryann Talbot-Moses (known as Pastor “TM”) uses the metaphor to describe the work of Altoona Regional’s Pastoral Care staff.

“I like to think about the image of the hood of an army jeep covered with a cloth and transformed into an altar for a battlefi eld

service or Mass,” Pastor TM explains. “It didn’t matter that they weren’t in a building designed and built for worship; they worked with what they had and ministered to the soldiers right where they were.”

Rather than let high-calorie, high-fat foods be the rain that ruins your picnic — and your waistline — Laura Vasbinder, health system registered dietitian, says some subtle planning can provide healthier options that nobody’s taste buds will notice.

“The important thing to remember is that all foods can fi t into a healthier picnic,” Laura said. “Enjoy yourself, but don’t completely abandon your general healthy eating guidelines.”

Instead of chips, try fat-free pretzels and a low-fat dip, a dried fruit and nut mix, cheese and crackers, or celery sticks with peanut butter. Laura suggests making fruit salads with strawberries, peaches, apples or grapes, and a low-calorie vegetable dip to add more vitamins and nutrients to a picnic.For the entrée, step away from hamburgers and hotdogs. Grilled chicken or pork, fi sh such as salmon, halibut or trout, and grilled veggies with olive oil are excellent substitutes. Grilled corn on the cob can be a welcome and nutritious alternative. Picnicking offers a high risk for food-borne illness. Follow this checklist to help ensure your picnic doesn’t end with a trip to the emergency room.

Wash your hands with soap and clean water before handling food, especially raw meats or poultry, and always after using the bathroom, changing diapers or touching pets.

Pack food, especially raw meats, in separate containers to prevent cross-contamination. Pack meats at the bottom of the cooler to prevent juices from dripping onto other foods.

Use separate utensils and plates for raw meats and cooked meats.

Be sure to keep hot foods hot and cold foods cold until ready to eat.

Carry your cooler in the air-conditioned car — not the trunk. At the picnic site, put it in the shade with the lid closed. Remove food just before serving.

Serve food quickly and return food (including leftovers) to cooler within one hour.

Be sure to cook meats until they’re hot all the way through, the

meat is no longer pink, and juices run clear.

“Try skipping the bun “Try skipping the bun altogether, or at least substituting whole-grain buns for white, or trading them both for a whole-grain pita or wrap.”

Macaroni salad and potato salad are staple picnic side dishes but not the healthiest choices.

“Beans are really healthy and very good for you, until you add bacon and brown sugar,” Laura said. “Even salads can be fattening and high in calories unless you substitute olive oil, canola oil or balsamic vinaigrette for the mayonnaise and thick, high-fat dressings.”

If a crunchy accompaniment is what you crave, Laura suggests substituting the baked version for regular chips, or a more radical alternative like low-calorie, low-carb baked tortilla chips and salsa, which is loaded with vitamin C and cancer- preventing lycopene.

What you drink can make a huge impact. Sweet, sugary high-calorie sodas can increase thirst, which leads to drinking more soda, which leads to more calories. An alcoholic beverage or two can lead to dehydration as well as pack on the calories.

“Water with fruit juice or just a squeeze of lemon is very refreshing and is what your body wants,” Laura said. “If soda is a must, go with diet.”

Operation Safety Net-Altoona and Partnering for Health Services combined their missions earlier this year so they could better serve the working uninsured of our region.

Prior to the merger of the two city hospitals in November 2004, Operation Safety Net was operated by Bon Secours-Holy Family Hospital, and Partnering for Health Services was operated by Altoona Hospital. They consolidated at 501 Howard Ave., Building B, Suite 204, Altoona, under the Partnering for Health Services name.

Dr. DelBaggio received her medical degree from the University of Pittsburgh School of Medicine and performed her three-year pediatric residency at Eastern Virginia Medical School/Children’s Hospital of the King’s Daughters in Norfolk. She is board certifi ed in pediatrics and is a fellow of the American Academy of Pediatrics.

Some children experience a cough and runny nose, but these are usually very mild and do not cause signifi cant discomfort.

The fever and vomiting usually subside in two days, but the diarrhea can last from four to eight days. The length of the illness, from the start of symptoms to when the diarrhea ends and appetite is back to normal, is roughly 14 days.

How important is the rotavirus vaccine and who should receive it?

Because of the frequency and amount of diarrhea, the most common complication of rotavirus is dehydration, which can occur very rapidly and can potentially be life-threatening.

Rotavirus attacks the intestinal lining and prevents food from being digested and absorbed properly. As a result, the stool contains excess water and electrolytes, or minerals that help keep the body’s fl uid levels in balance. If electrolytes are lost in large amounts, a child can suffer from chemical imbalances,

The Food and Drug Administration has approved a vaccine to fi ght rotavirus, the most common cause of severe diarrhea in infants and children. How do people get rotavirus and what are the symptoms?

The virus can live on human hands for about four hours and on nonliving objects for several days. Rotavirus has often been found on toys, water fountains, toilet handles, doorknobs, telephone receivers and surfaces in daycares and hospitals.

Symptoms typically begin within 48 hours of exposure but can appear anywhere between four to seven days after exposure. Rotavirus begins with a low to moderate fever. Vomiting, stomach cramps and diarrhea will then occur. The diarrhea is usually very watery and can sometimes be mistaken for urine in the diaper.

In most cases, rotavirus is harmless, but due to its high incidence and its potential for serious complications, it’s very important for young children to get the vaccine. Careful hygiene and proper hand-washing, especially in hospitals and daycare centers, are also very important.

The vaccine, called RotaTeq, is given orally in three doses. It is recommended for children 6 weeks to 32 weeks of age. The fi rst dose is given at 6 to 12 weeks. The next doses are given at four- to 10-week intervals, with the last dose being given before 32 weeks.

As with any vaccine, side effects are possible, but the benefi ts of preventing rotavirus and its potential complications outweigh the risks. Children should not get the vaccine if they have weak immune systems or disorders of the blood or lymphatic system, are taking immunosuppressive

medications that prevent immune

system activity, or are receiving

blood products.
