Good nutrition is beneficial at any age,increasing mental functioning, building ourimmunity, boosting energy levels, and evenleading to faster recuperation times and bettermanagement of chronic health conditions. Andnow, proper nutrition for seniors is even moreimportant than ever, with the latest findings thatpoor nutritional decisions can eventually lead tocognitive decline and dementia.

Specific research points to vitamins C (foundin citrus fruits, tomatoes, spinach, red peppers)and E (found in nut and vegetable oils, wholegrain products, spinach), antioxidants that arebeing shown to be especially effective inprotecting against Alzheimer’s disease.

Bear in mind that nutrients are mostbeneficial when consumed through actual foodsrather than supplements; however, this maysometimes be challenging for older adults as aresult of decreased caloric needs. A health careprofessional should be consulted before takingvitamin supplements.

Along with antioxidants, maintaining a low-cholesterol/low fat diet can help to prevent theonset of Alzheimer’s disease. Diets high in fat arebelieved to impair learning and memoryperformance, and elevated blood cholesterol cantriple a person’s chance of developing thedisease. High cholesterol foods, like meat anddairy products, butter and cooking oils and friedfoods, should be minimized to no more than 300mg of cholesterol per day, as recommended bythe American Heart Association.

Initial studies have also revealed a connectionbetween omega-3 fatty acids and a reducedchance of developing Alzheimer’s disease.Omega-3 fatty acids, found in fish, have anti-inflammatory properties, which are thought to

boost memory and learning performance. Fishhigh in omega-3 fatty acids include lake trout,mackerel, herring, sardines, salmon, and albacoretuna.

The real challenge may lie in encouraging anolder person to eat if suffering from a poorappetite. Often, medications or treatableconditions like heartburn, constipation or nauseacan lead to problems with appetite. Meal timesfor an older adult can also feel lonely and isolatedif a life partner has recently passed away. Sometips to help a senior who’s struggling atmealtimes include:

• Ask the person why he or she does not wantto eat (not feeling hungry, the food is cold,it doesn’t taste good, etc.)

• Provide smaller, more frequent mealsinstead of three larger meals

• Serve beverages after each meal so thesenior doesn’t feel full before finishing themeal

• Restore joy to mealtimes with colorfulfoods and a variety of textures

• Make the meal setting more attractive withfood garnishes, placemats, flowers andperhaps soft music in the background

• Try finger foods that are easy to handle oruse adaptive tools like silverware withspecially designed handles for those whohave trouble using standard utensils

Ensuring that a senior loved one is eatingenough nutritious foods and drinking sufficientfluids can be a challenge, but it is important toalways treat the senior as an adult, and never as achild. It can also be helpful to hire a home careagency to provide assistance with groceryshopping, planning and preparing meals, andoffering companionship during mealtimes.

For some 44 millionAmericans, being a family caregiver often comeswith a price: discounting one’s own health, financesand personal needs.

Rewards and Challenges of FamilyCaregiving Offering support to a loved one whoneeds help is replete with positive and rewardingexperiences such as stronger communication,relationship connections and personal satisfaction inmaking a tangible difference in a loved one’s life. Yetas enriching as caring for another can be, at timesfamily caregivers can feel overwhelmed andexhausted. If care needs increase or the length of theillness stretches into several months or years,informal caregivers are vulnerable to common stress-related conditions including headaches and sleepdeprivation.

Economic Impact of Family CaregivingUnpaid caregiving for a family member or otherloved one creates a ripple effect on personalfinances. Caregivers of older adults and adults withdisabilities often face significant financial strainfrom out-of-pocket costs and long-term effects onsavings and retirement accounts.

The money stress often stems from the difficultyof caregivers balancing work and caregiving roles.Some 60 percent of caregivers said they have had atleast one change to their employment because ofcaregiving, such as reducing their work hours, takinga leave of absence, using up vacation and personaldays, and taking a less-demanding job.

Out-of-pocket spending on goods and serviceson behalf of the care recipient adds up, too, includingmeals, transportation, medical/pharmaceuticalcopays, clothing, medical supplies, legal fees andhome modifications. Overall, financial strain onfamily caregivers leaves the caregivers at risk forreduced wages and retirement and Social Securitybenefits, and inconsistent savings for their ownfuture.

Emotional Impact of Family Caregiving TheU.S. Department of Health & Human Services tracksthe health and wellness of Americans and notes anumber of signs and symptoms of emotional stress offamily caregivers, including:

• Feeling alone, isolated or deserted by others.• Feeling overwhelmed.• Feeling worried or sad.• Feeling tired.• Losing interest in once-enjoyable activities.• Flaring with irritation or anger.Over time, caregiver stress can significantly

compromise one’s health. Among family caregivers,anxiety and depression are more likely to affectwomen than men, and left unchecked can weaken theimmune system and lead to other health problemssuch as obesity, diabetes, heart disease and stroke.Taking steps to counter the financial and emotionalstrain of caregiving helps both the caregiver andloved one enjoy better health and time together.

While you may not be able to always stop asenior from falling, you can take some steps tohelp reduce the risk. There are many reasons forfalls in the elderly, so make sure to look at thewhole picture. Use these precautions take care ofyour loved one and their home to prevent falls:

1.Begin with the bathroom. Wet surfaces (on thefloor or counters) can be very dangerous.Seniors lack the balance and reaction timeneeded to avoid a fall. While it may seem easyto step out of a shower, an unsteady seniormay slip and come crashing to the floor. Tohelp, provide non-slip mats in the bathtub andshower. Install grab bars in strategic pointsaround the bathroom. Towel bars are notsufficient because they will collapse undersomeone’s full body weight. Grab bars can beattached inside the shower stall or just abovethe bathtub to help a senior with lowering orstanding up.

2.Provide a shower seat. Seniors can be farmore secure when they are seated whenshowering.

3.Replace the shower head with a hand-heldnozzle. Seniors can become more confidentwith showering with a nozzle. The nozzle canbe easily turned away in the case of suddenwater temperature changes. In addition, it canresult in a much more thorough cleaning.Long-hoses are now available, which are fareasier to use for someone seated in the shower.

4.Tuck away extension cords. Are there anypower cords stretched across a senior’s floor?Tape them down or slide them underneath orbehind furniture.

5.Remove excess furniture. A more mobilesenior may be able to sidestep a footstool or acoffee table but not all seniors can do this.Remove unneeded furniture to give the seniormore room to maneuver and help to create asafer living environment. Also, a deep plusharmchair may look comfortable but the seniormay become trapped if he or she lacks thebody strength necessary to push up and out ofthe chair.

6.Get a cane or a walker. A doctor can bestadvise if the senior will need a mobility aid.This could be a wheelchair, a walker, a cane,or a motorized scooter. Reducing oreliminating the walker stigma that often existsmay be a little more difficult to do. However,seniors may find that using a walker increasestheir own freedom, independence, and qualityof life. Be aware that an occupational therapistneeds to train the person to use these properly.For increased convenience, choose awheelchair, walker, or scooter that cancollapse and fit easily in a car’s trunk or backseat.

7.Evaluate a senior’s footwear. Shoes need to fitwell and have non-slip soles. Shoes withVelcro straps can be easier to tighten or loosen.They also remove any risk of tripping overlong laces. Do not choose senior’s shoes basedon how easily they can be put on and taken off.

8.Keep a senior active. Whether through regularwalking or light exercising and stretching, anactive senior can remain more stable than asedentary senior. Exercising can help preventfalls by keeping stabilizing muscles strong.

Recently when I announced that I would beworking for a brand-new senior care agency inSouthern Oregon, the person that I was sharingthis with quickly and abruptly deflated myenthusiasm by saying “another SENIOR CAREagency, don’t we have enough here already?”

This comment aroused in me a series ofthought-provoking questions that familymembers must, at one time or another whenselecting the “perfect” agency to care for theirloved ones, encounter.

According to The Oregon Care PlanningCouncil and the list generated from SouthernOregon Retirement Connection, SouthernOregon, (Ashland, Central Point, Grants Pass,Klamath Falls and Medford) haveapproximately 21 home care agencies. Is that alot?

Well no, it is not. The latest US Census fromOregon states that Jackson, Josephine andKlamath Counties have an average estimated22.5% adults over 65. A little less than thequarter of the population of each of thosecounties. That, my friend, is a lot.

Questions: With so many agencies to choose from, how

do you know what is the best for your family andhow do you find them?

A statement in The Oregon Care PlanningCouncil suggested searching in the local yellowpages. A couple of things came to mind when Iread that:

1. Very few people use the yellow pagesanymore. Google seems to be the vehicle bywhich information is garnered.

2. Many agencies don’t always list in theyellow pages. This means you are limiting youroptions in finding the best available agency to fityour needs.

So, back to the question: How do you knowwhich is the best agency for you? Here are a fewideas to start with- Word of mouth. This reallydoes work. Ask around and find out what hasbeen the best for friends or family. What type ofservice did they provide? Was it companionshipand light housekeeping, or medicationmanagement and assistance with bathing andpersonal care? Are there dementia issuesinvolved?

Are they a licensed and bonded company?Please be aware that Oregon requires HomeCare Agencies to obtain licensing with the statehealth department and may be under the samestringent rules that Home Health agencies mustadhere to. Home Care agencies are required tocomplete extensive criminal background checksand training to potential caregivers, use of careplans, periodic inspections by the state healthdepartment.

Additionally, If your loved one is leaving ahospital or rehab center, the discharge planners,may suggest agencies that they trust and have ahistory with that they know will follow throughwith the best possible care for your familymember.

Okay, so now you have it: Word of mouth,referral from a medical professional or researchthe options from a trusted source.

Being mindful while we care for others hasbenefits for both the caregiver and the carereceiver. Many thoughtful people explored whatit means to create a culture of mindfulcaregiving in anticipation of the opening ofSouthern Oregon Friends of Hospice’s newresidential hospice, Celia’s House in HolmesPark. In 2018, before opening in May, thevalues, aspirations and actions for mindfulcaregiving were created. These values,aspirations and actions formed the basis oftraining for the volunteers and staff at Celia’sHouse in Holmes Park with the goal of creatinga calm, healing and supportive atmosphere.

"To have the humility to listen deeply andnot assume that I know" is one aspiration of themindful caregiver. Generous listening is reallyfundamental to mindful care. It requires slowingdown and paying attention to the words and thebody language of another. Feeling heard helpsthe care receiver feel valued and loved.

Mindfulness takes regular practice.Knowing one’s own mind requires that we slowdown, breathe and pay attention to our feelingsand thoughts. At Celia’s House, each new set ofvolunteers sits in silence for 3 to 5 minutes andthen shares what is on their mind or how they arefeeling. This is the practice of knowing one’sown mind. It creates the opportunity to set asideone’s personal stresses to focus on thecaregiving at hand, keeping kindness at thecenter of everything.

Four key values of Celia’s House arecompassionate caring, respectful honoring,mindful presence and inclusiveness. Fromthese values, seven fundamental aspirations ofmindful caregiving evolve including “to havean open mind and an open heart;” “to find the

inner stillness, no matter the circumstance;” and“to have the courage to enter each moment.”

To simplify the topic of mindful caregivingfurther, basic actions form a practical guide formindful caregiving. First pause and observe. AtCelia’s House we like to say “don’t dosomething, just stand there.” Ask “what mattersmost now?” Listen quietly and deeply for thewords and the body language. Validate andclarify each person's answers and then followthrough with respectful action and kind caring.

Each person who finds themselves in thiscaregiving role can certainly practice thesehabits of mindful caregiving. “To act mindfullyrecognizing my own emotions and limits” isanother important aspiration. Some caregiversfind themselves in overwhelming situations andend up getting sick or hurt themselves. Caringfor the caregiver is an important part of mindfulcare. Recognizing one’s own abilities, strengthand emotional limits, with the ability to ask forhelp is crucial for the health of both thecaregiver and the care receiver.

“Mindfulness means paying attention in aparticular way: on purpose, in the presentmoment, and nonjudgmentally.” Jon Kabat-Zinn

The words of Thich Nhat Hanh and JonKabat-Zinn illuminate why both the care giverand receiver are protected by the practice ofbeing a mindful caregiving.

Let’s talk about INSOMNIABoy, what a subject- who hasn’t struggled

with insomnia in their life?Answer: We all have! A little problem for

most and a BIG problem for so many.Here’s the situation:-We have a sleep cycle.-We have a wake cycle.When one is turned on-the other is turned

off- Insomnia can be a problem with either partof this cycle: too much wake or too little sleepdrive.Insomnia And The Brain:

Some people are biologically prone toinsomnia and seem to struggle with sleep formany years without any identifiable cause-evenwhen they follow healthy sleep advice.SLEEP ADVICE:Bedroom Setting:

A. Should be very dark and especially all theway till time to get up-watch for earlysunrises in the summer. I wear an eyemask every night.

B. Should be very quiet-watch out for streetnoises, doors closing, dogs and cats,neighbors and other occupants in thehome moving about. I personally wearhigh quality ear plugs every night.

C. ”Blue Light”. This refers to cell phones,computers, TVs and even LED lighting.End your bluelight exposure an hourbefore bedtime.

D.Cool and circulation: Should be 65degrees or lower with a light aircirculation. I’m talking ”year-round.” Mywife and I set our thermostat on A/Cthroughout the winter as well as thesummer and with fan action-“on” not“auto”

-Working at home in the evenings can makeit hard to unwind and make you feel preoccupiedwhen it comes time to sleep.

-Naps in the afternoon may be helpful forsome and throw the next person into a night ofinsomnia.

-“Catching up on sleep” Sleeping “In” canupset the sleep/wake circadian rhythm cycle.Resulting in an “insomnia night”Insomnia and Food/Drink:

-Alcohol: A sedative in the beginning, onlyto disrupt the sleep later in the night.

-Caffeine: Four or more cups per day mayresult in insomnia and it takes eight hours toclear out of your system.

-Nicotine: a stimulant that results ininsomnia when consumed too near bedtime.

-Heavy meals and spicy meals: When nearthe bedtime-one can most likely expect sleepdisruption plus some regurgitation,sour–stomach feeling.Insomnia and Anxiety:

“The Dynamic Duo”, one begets the other.Who hasn’t experienced this double trouble?You feel anxious and lose sleep and then becomemore anxious because you lost sleep, then,because you lost sleep you increase your level ofanxiety-the classic, “Catch 22”. When thishappens for many nights,(or many months) youmight start feeling anxiety, dread or panic atJUST the prospect of not sleeping. This is howanxiety and insomnia can feed each other tobecome a cycle that should be interruptedthrough Treatment.

Carol was visiting a friend when she decidedto share her grandson’s picture. As she looked atthe familiar picture of the mischievous four yearold climbing a fence, she noticed somethingpeculiar. Her grandson’s face was blurry and thestraight wood fence background appeared wavy.

Carol was diagnosed with Age-RelatedMacular Degeneration (AMD) and quicklylearned that she was not alone. AMD is the mostcommon cause of significant vision loss forpeople over 65 affecting more than 11 millionAmericans. Fortunately for Candice, a course oftreatment improved her vision and will mostlikely prevent future vision loss. We are luckythat with the help of advancements in the last tenyears, we have slowed the progression of AMD,a devastating disease for many patients. Twopreventable risk factors for developing thedisease are smoking and high blood pressure.Smokers have at least twice the rate ofdeveloping AMD.

Carol discovered that she could play asignificant role in her own treatment by taking asupplementation of vitamins and minerals. Thespecific daily amounts used to treat AMD are500 mg of vitamin C, 400 IU of vitamin E, 80mg of zinc, 2 mg of copper, 10 of lutein, and2mg of zeaxanthin. Copper was added to theformulations only to prevent copper deficiencyanemia from high levels of zinc intake.

She also complemented the vitamins byeating a diet rich in fruits, vegetables, andomega-3 fatty acids. These foods should be highin antioxidants such as cartenoids and beta-carotene, lutein, and zeaxanthin. Antioxidants

are substances that offer protection from free-radicals which are cell waste products that causecell damage. For most people liquid or capsulesof cod liver oil is a simple addition of omega-3antioxidant to their diet. Other sources ofomega-3 are wild fish, walnuts, and flax seeds.

Here are some examples of foods that arehigh in vitamins, zinc, and good eye healthychoices:

• Vitamin C: Oranges, grapefruit, melon,broccoli

• Vitamin E: Whole grains, olive oil,almonds, avocados, blueberries, spinachand beans

• Zinc: Oysters, meat, flounder, sole,sardines, cashews, walnuts, pumpkin seeds,beans, dairy products, and sesame seeds.

• Caretenoids: Grapes, oranges, apples,apricots, peaches, nectarines, cantaloupe,kale, collard greens, spinach, broccoli,carrots.

Simple changes can be helpful. For example,at breakfast, add blueberries to cereal, andspinach or kale to omelets. At lunch and dinner,always use whole grain breads and topsandwiches with sprouts and greens. Lightlycook vegetables, ensuring they retain theirvitamins. When dining out, ask for grilled meatsand lightly cooked vegetables. Consider tryingthe vegetarian options, a great source of eye-healthy vitamins.

Simple additions to a healthy diet will dowonders in prevention, treatment, and youroverall well-being.

Floaters, or small specks that float acrossyour vision, are a common complaint in middleaged and elderly people. These specks occur inthe vitreous humor, the clear, jelly-likesubstance in the main chamber of your eyelocated between your lens and your retina. Whenyou’re young, the vitreous is clear. As you age,the vitreous degenerates. Collagen fiberscollapse and bind together to form clumps.These clumps are what cast shadows on theretina and are the floaters you see. As thevitreous continues to collapse, it can lead to aposterior vitreous detachment or PVD. A PVD isoften associated with new floaters and flashes oflight around the periphery of your vision. Thiscan sometimes cause tears in the retina so youshould have a dilated eye exam with a retinaspecialist if you are experiencing thesesymptoms. Once your eye doctor has ruled out aretina tear, the annoying floaters can byaddressed.

Historically, the only remedy for floaterswas a vitrectomy, or removal of the vitreous inthe operating room. Side effects of thisprocedure were having to undergo anesthesia,worsening of cataracts, risk of bleeding andinfection, and rarely loss of vison. For thesereasons, most patients were advised to just “livewith” the floaters. However, floaters can bequite bothersome to some patients.

A new procedure allows a laser to treat thesefloaters in the office. The Ellex Ultra Q ReflexYAG laser was used in the first randomizedcontrolled clinical trial of YAG-vitreolysis forvitreous floaters. The study, published in JAMAOphthalmology, a major peer reviewed journal,demonstrated that treatment reduced both thesubjective symptoms and objective findings of

vitreous opacities. 53% of patients in the lasertreatment group reported significantly orcompletely improved symptoms vs 0% ofindividuals in the untreated group (P < .001). Noclinically relevant adverse events wereidentified.

YAG-Vitreolysis involves the application ofnanosecond pulses of laser light to vaporize thevitreous opacities and to sever the vitreousstrands. The floater’s collagen and hyaluroninmolecules are converted into a gas. The endresult is that the floater is removed or reduced toa size that no longer impedes vision. YAG-Vitreolysis is performed in the office so you donot have to go to the hospital. Only a mild,topical anesthetic is required. A contact lens willbe placed on your eye to better focus the laserdirectly onto the floaters. During the treatment,you will likely observe bright lights and smalldark shadows as the floaters are being vaporizedinto small gas bubbles. These gas bubblesquickly dissolve. Each treatment sessiontypically takes 15-30 minutes. More than onetreatment may be necessary to obtainsatisfactory results. Following treatment, youmay observe small black floaters at the bottomof your vision from the gas bubbles producedduring the Vitreolysis. These dissolve quickly.Rare side effects include cataract and an increasein the intraocular pressure.

Age-related macular degeneration (AMD) isthe leading cause of blindness in the U.S. forpatients over the age of 50. AMD affects thecentral retina (the macula) and can lead todifficulty focusing, blurry vision, or distortion.Most of the time, the disease manifests as the dryform of disease before signs of wet AMD develop.

In dry AMD, slowly progressive atrophy of thepigmented cell layer deep to the retina (the RPElayer) occurs. This RPE reduces the harmfuleffects of light, removes free radicals and otherharmful byproducts of retinal metabolism, andserves other important functions. As the RPE cellslose functionality, the photoreceptors (rods andcones) also degenerate, resulting in vision loss.

In wet AMD, accounting for 10% of cases,abnormal blood vessels develop under the retinaand cause fluid exudation and hemorrhage whichultimately leads to scarring and significant loss ofvision if left untreated. Dry Macular Degeneration

Although there is no proven treatment for thedry form of the disease, antioxidant vitamins havebeen shown to slow the progression of atrophy. Inthe Age Related Eye Disease Study (AREDS), alarge multicenter trial sponsored by the NationalEye Institute (NEI), Vitamins C, E, beta carotene,Zinc and copper were shown to lower the risk ofprogression by 25%. A second study, the AREDS2was conducted with a vitamin combination thatused a lower dose of zinc and replaced the betacarotene with Lutein and Zeaxanthin, because betacarotene can increase the risk of lung cancer inpatients with a history of smoking. The AREDS2formulation was effective in lowering the risk ofprogression.

Newer treatments are being studied for dryAMD including drugs that suppress inflammationknown to contribute to the disease and surgicalimplantation of stem cells or RPE cells.

Wet Macular DegenerationCurrent treatment for wet AMD involves

injections of medications into the eye that blocksspecific growth factors which contribute to theabnormal vessels growth and leakage. Thesetreatments do not cure the disease and they need tobe repeated on a regular basis to suppress diseaseactivity. While these treatments have provided aneffective means to preserve vision in manypatients, newer treatments are being studiedincluding treatments that block different molecularmediators of disease, treatments that last longer orhave higher affinities for the growth factors thatwe are trying to suppress, sustained releasedevices and gene therapy. These investigations arestill fairly early in the phases of development, andit may be many years before these newertreatments are available if they prove to beefficacious.Patient Recommendations

Dietary modifications in addition to oralantioxidants can lower risk of disease progression,therefore, patients with AMD are encouraged toeat leafy green vegetables and fruits. Alpha-3Omega fatty acids found in food sources such asfish, flax seeds, and walnuts are also beneficial.Smoking cessation is recommended as smoking isa significant risk factor for progression toadvanced AMD.

It’s important to remember that vision lossfrom macular degeneration is often preventable.Any person who experiences visual loss shouldseek immediate medical attention. Early treatmentfor wet AMD is extremely important and can makea difference in maintaining driving and/or readingvision.

Strong bones are the foundation ofmovement and quality of life. As we age bonesoften begin to weaken and increased fracturerisk becomes a very real concern. Much researchhas investigated the mechanisms behind bonehealth and many drugs have been developed,some better than others. What does naturalmedicine have to offer here? Calcium and vitamin D have a long standingreputation for supporting healthy bones. Whenlooking at modern calcium supplementation, ourprimary concern is ease of digestion andappropriate dosage. Do you have painful, arthritic joints in yourfeet? Do you suffer with foot pain that limitsyour ability to stay active or even bear weight?These are common conditions in the agingpopulation. Often patient’s are seen becausethey’re foot pain prohibits them from engagingin activities that require them to be on their feet.The common complaint is, “I want to be active,but my feet hurt too bad.” There are optionsavailable now, via advancements in medicaltechnology, which allow us to more effectivelytreat many of these ailments. The first modalityadvancement is with a device that has been usedfor centuries to address said symptoms, footorthotics. With today’s technology, we are ableto design and fabricate 3-D printed devices thatare not simply the static, hard plastic shells ofold, but light and somewhat flexible devices thathelp to both cushion and functionally supportyour foot during weight-bearing activities.These devices have proven to be very effectivein helping to mobilize people that haveotherwise been unsuccessful in findingsomething to relieve the pain in their feet. Wehave been working with a company out ofBelgium since 2016 to improve upon theefficacy of this new innovation.

The second modality in treatment of thearthritic foot and ankle is the utilization of

Platelet-rich Plasma (PRP). Although this is notnew technology, the ability to provide thetreatment has been improved by technology.Arthritic joints are one of the newer conditionsthat PRP can now readily treat. PRP involvesdrawing a small vial of the patient’s blood,utilizing a proprietary based centrifuge to spindown the blood and separate the amino acids,proteins and other healing components from theplasma. This is then loaded into a syringe forinjection into damaged ligaments, tendons andeven joints to promote cellular healing of thesestructures. Often, this is performed withultrasound guidance. Typically a procedure likethis can be performed in an office setting,requiring 15-20 minutes of your time. Althougheveryone may not be a candidate for thisprocedure, a clinical evaluation of the foot withlimited diagnostics such as on site x-ray wouldhelp determine these parameters.

Currently there is no cure for arthritis or theexquisite wear and tear that we expose our feetand lower extremities to throughout our lives.There are, however, effective options, topotentially help eradicate much of the pain andother symptoms associated with theseconditions, to allocate a higher quality of life.

Medical has seemingly become a necessityof absolute. We want to have care, we need tohave care, most of us would prefer-to-havepreventative care, rather than find after monthsof pain, discomfort, and “learning to live with it”that we actually have a serious diagnosis. Withthis in mind, I’d like to share my recentexperience with you.

Recently, I decided to get a check up on theitems that were just annoyances gone on toolong. I was pleased to present my insurance card,pay my copay and schedule the follow up visitfor labs. On my lab day, I showed up fasting,paid my copay and scheduled my follow upappointment for results. By the time my followup for results had come, I felt that I had receivedgreat care by the provider, the front desk teamwas welcoming, and I was “pleased as punch” toturn it all into insurance. Whew! What a sigh ofrelief! Health care, without breaking the bank.

I want to be clear, I had at least five visitswith the provider. Once I had results of the labs,we looked at supplements, meds, etcetera fromthe pharmacy and had more scheduled visits onthe calendar. I felt that I was sleeping better,concentrating better, overall- feeling more wellthan I had before. Then, the crash.

I received my bill in the mail. I had billsfrom everywhere I had been. I felt “GPStracked” for all my medical appointments, pick-ups at the pharmacy, the GPS had even followedmy blood to the lab! Now all hands in the cookiejar! Here this wonderful insurance, of which thepremium had been paid for a year and the copaypaid at each visit, was telling each aspect of thehealthcare wheel that I had not met my

deductible, so they would pay nothing. To makematters worse, upon review of my bills, theprovider had billed me for the exact service thatthe lab had provided. Interestingly, thelaboratory was less expensive with their servicethan my provider.

Now if you are familiar with this scenario,you’re probably thinking, “Yep! That’s the wayit is.” I hear many people say that health-coverage has gone to catastrophic insurance.They are now finding ways for themselves andtheir family to find alternative, cost effective,great care that has nothing to do with aninsurance card. Some of the crowd aresuccessful in their findings. Many are strugglingsomewhere in the mess, like myself. Some arejust not getting healthcare because of it.

I would always encourage you to gethealthcare. Be sure you and your loved ones areon the road to health or staying healthy. Checkyour options, be sure to ask questions,information will help you to make the bestdecision for yourself.

I love it when people approach me withquestions about the aging process, especiallyquestions about memory loss and confusion.For this article, I’ve chosen 5 questions I getasked most frequently.

1. What is the difference between dementiaand Alzheimer’s disease?

Dementia is defined as “memory loss andconfusion significant enough to interfere withone’s daily activities.” It is an “umbrella term”,meaning that there are many causes, possibly asmany as 700 or more. Two categories ofdementia causes are “irreversible causes” and“reversible causes”. Alzheimer’s disease makesup 23 types of irreversible dementia.

2. The follow up question I am often asked isif there is any benefit from going through adiagnostic process if there is no “cure”?

I usually answer with a resounding yes! It isenlightening to know what the cause or causes ofone’s confusion and memory loss is (are);reversible causes such as depression, infection,thyroid imbalance, dehydration and many otherscan be treated, with amazing improvement incognition. And even if the cause is irreversible,understanding which areas of cognition are stillstrong helps us know how to plan accordingly.The only time I hesitate to go beyond basic labsand evaluation with the individual’s primarycare physician is when the person wouldn’tunderstand why they are being tested, and itwould just feel humiliating and demeaning.

3. What is the difference between brainchanges due to normal aging and brainchanges related to Alzheimer’s disease oranother form of dementia?

The key to this answer is in the definition ofdementia… “significant enough to interfere withone’s daily activities”. The best guide to thissubject is called “Know the 10 Signs: EarlyDetection Matters”, by the Alzheimer’sAssociation, found at alz.org.

4. If I have dementia can it get better? No. And yes. If your dementia symptoms

are caused by a progressive illness such asAlzheimer’s disease, Frontal Temporal Disease(FTD), Lewy Body dementia, or otherirreversible causes of dementia, the underlyingillness is not going to improve. That said,people find significant symptom relief in avariety of ways. See question #5.

5. What are some things I can do to make mylife with dementia more tolerable?

There are several approaches that can beexplored. Some medications help slow down theprogression. Managing pain and illness makes adifference in mental abilities and overallwellbeing. People who join our support groupsor engage in hobbies report that they feel lesslike a “patient” and more valuable as a person.People who use a reliable routine to managetheir daily commitments experience less stress, astronger sense of control and increasedindependence. Several people I know havefound diets that help them minimize theirsymptoms. Finally, almost everyone I talk withreports that counting their blessings or lookingat the half full part of the glass makes the biggestdifference in their quality of life.

Every second of every day an older adult falls.Last year, more than one in four older adultsreported falling and more than 28,000 older adultsdied as a result of falls—that’s 74 older adults everyday. It is estimated that every 20 min an older adultdies from a fall in the United States. There aresimple steps you can take to prevent falls anddecrease fall risks. Throughout the home

• Remove throw rugs so you do not trip on them. • Replace or remove carpet that is torn or has

turned-up edges. Avoid thick carpet. Shoes maycatch on these and cause you to stumble or fall.

• Move furniture or other things that may blockpathways.

• Be sure you have good lighting throughout yourhome.

• Use night lights or leave some lights on in thehouse to help you see at night or when youcome home in the evening.

• Use switches that glow in the dark, so they canbe seen more easily.

• Keep electrical cords and small things out ofyour path.

• Use your cane or walker rather than usingfurniture to give you support when walking.

Stairs• Mount sturdy handrails to help with going up

and down stairs. They should extend beyond thetop and bottom stair.

• Improve the visibility on your stairs. Have goodlighting on the stairs. Non-skid surfaces can beapplied to wood stairs to prevent sliding.

• Paint a bright colored line on the edge of eachstep so they are more easily seen, especially ifyou have poor vision.

In the bathroom • Place non-skid decals or a mat in the tub or

shower. • Install grab bars around the toilet and in the

shower or bathtub. Towel bars are to holdtowels, and they will break if you use them asgrab bars.

• Use a tub seat and an elevated toilet seat.

• Leave the bathroom door unlocked so it can beopened if you do fall.

In the bedroom • Avoid wearing long nightgowns or robes. These

can cause you to trip. • Avoid wearing loose shoes that cause you to

scuff or shuffle your feet as you walk. Wearshoes or slippers that fit well and stay securelyon your feet.

In the kitchen • Have commonly used items at counter level or

within easy reach. • Do not climb or reach to high shelves. • If you use a step stool, use a stable step stool

with a handrail. Other tips

• Be careful that you do not trip over your pet. Beaware of where your pet is when you aremoving around.

• Use caution when sitting down. Before sittingdown on a chair, make sure the backs of yourlegs are touching the seat of the chair behindyou.

• Keep a telephone close by or consider carryinga portable phone.

• Take your time. Get in the habit of moving atspeeds that are safe for your energy level andability. Do not rush to answer the phone or door.

• Ask for help when getting up from bed, a chairor the toilet if you feel at all shaky, weak, dizzyor lightheaded.

• Exercise regularly. Exercise improves yourbalance and coordination. Consider joiningclasses for balance.

• Have your vision checked at least once a yearby an eye doctor.For more information contact Siskiyou

Inflammation? What is it? Inflammation is your immune system

reacting, like when you have gotten a bee stingand you swell up around it, or you sprain yourankle and it swells for a few days. Inflammationcauses the swelling, which brings cells andsubstances to heal what is hurt. This is a good thing. So how can things gowrong?

Your immune system works very hard. It isconstantly looking for and neutralizing foreignsubstances so they don’t harm your body. That isgood. However, there can be too much of a goodthing.

We live in a very toxic world. Every day,toxic chemicals get into our bodies through air,water, food, even common household andpersonal care products. Much of our foodcontains pesticides, herbicides, dyes,preservatives, additives, artificial colorings,flavorings and sweeteners. Our bodies react tothese toxins by producing inflammation.

Even “natural” foods we have trusted causeproblems. The shape of your jawbone and gums willnaturally change over time, this means yourdentures will not fit as well as when they werefirst made. A reline can adapt the fitting surfaceof your dentures to accommodate the changingshape of your mouth, so your dentures will fitwell and will feel more comfortable.

To accommodate the changes in your gumstructure as your dentures become unsuitable forchewing as they loosen and slide around, a hardor soft reline may be necessary. Relines are anideal way to repair the fit of your dentureswithout having to do a complete new fittingfrom scratch.

Relines usually can be done in the same dayat your local denturist office. Although, somesoft relines are a two day process.

The difference between a hard reline and asoft reline. A hard reline is done with the sameacrylic material your dentures were first madewith. If you find that your dentures are not ascomfortable as you would like and you aregetting sore spots, then a soft reline may be whatyou need. A soft reline is done with a somewhatpliable material and will last about a year to ayear and a half before needing replaced. Thesoft material is generally more comfortable thanthe hard acrylic the dentures were first madewith. So, for the sensitive denture wears a softreline may be best for you.

If your dentures have not been serviced in along time and you are experiencing severalproblems. You may be thinking its time for areline or even a new denture but, your gums arered and swollen. The denturist may suggest atemporary reline or tissue conditioner whileyour gums are healing. If you had a reline or anew denture made with your gums in poorcondition, that would simply lead to moreproblems once your denture was relined or youhad a new denture made. So, this type of relinewould be best for you while your gums healedby keeping them tight and comfortable.

For a longer lasting soft reline there is amaterial called Novus soft denture liner, thatstays pliable longer than other soft liners. Novussoft denture liner provides retention withcomfort for a more natural bond between yourgums and dentures. Novus soft liner is based onPolyphosophazene (PNF) technology. CBDA, cannabidiolic acid, is the second mostcommon phytocannabinoid produced in the cannabisdrug-variety plant — the most common being THC— and the most common phytocannabinoid in thefiber variety plant (hemp). CBDA, found in the rawcannabis flower, is converted to CBD when heated.CBD-rich plants were quite rare in the United Statesuntil around 2009, when reports of CBD’stherapeutic value started to appear in the media.Cannabis growers began testing their plants for CBDand made efforts to stabilize CBD-rich varieties. You have worked your entire life to get tothis moment, there are four areas everyoneshould look at when they retire.

First area is Protecting Your Health,making sure that your doctors, hospital stays,surgery's, prescriptions, and critical illness arecovered. Most people will qualify for Medicare,but Medicare won't pay for everything, oneterrible event can wipe out everything you haveworked your entire life for. As we age our healthdoesn't typically get better, so having goodhealth coverage is important. There are manyoptions available to you to ensure that you areprotected.

The second area of concern is PlanningYour Estate. Making sure your final expensesare covered, your spouse is supported when youpass, debt is paid off, gifting to family, friends,or charities. When we pass away, there will bean income loss to the surviving spouse alongwith expenses to cover your burial or cremation.There can be unpaid debt that can put thesurviving spouse at risk financially. Through lifeinsurance you can ensure that your death doesn'timpact your spouse or family negativelyfinancially.

The third area is Long Term Care. If you have IRA's or 401K'sthese monies can be transferred into an annuitywith no tax issues. As your retirement accountshave grown on a tax deferred basis so does anannuity. An annuity can be used as savingsvehicle or it can be used as an income for life. Inmost cases after the first year you can take up to10% of your annuity out a year without anypenalties. In most cases an annuity will avoidprobate, upon the annuitants death whoever islisted as the beneficiary or beneficiaries hasimmediate access to the monies without penalty.

Many people assume that hearing loss is anatural part of aging. Therefore it is often assumedthat at least a mild or moderate hearingimpairment can be ignored; many think it'scertainly no big deal.

Yet a study released in January of 2018 carriedout by a team at the French National Institute forHealth and Medical Research (INSERM), led byProf. Hélène Amieva, found:… [A]n increasedrisk of disability and dementia in those withhearing loss, and, in men only, an increased risk ofdepression. These associations were not found inthe participants using hearing aids.

Leading Dr Clare Walton, Research Managerat Alzheimer’s Society, to conclude: “Thisstudyadds to a growing body of researchsuggesting that hearing loss is a risk factor fordementia.”(https://www.alzheimers.org.uk/news/2018-05-02/study-confirms-link-between-hearing-loss-anddementia-older-adults)

The vast majority of scientists in the fieldagree that a lack of social interaction caused byhearing impairment contributes to cognitivedecline. (http://www.hopkinsmedicine.org/news/media/releases/hearing_loss_and_dementia_linked_in_study).

The link between hearing loss and dementia isnot yet clearly understood, but one thing is clear;an untreated hearing impairment worsens theeffects of cognitive decline. And even thosealready diagnosed with dementia function betterwhen their hearing impairment is treated. In fact,some symptoms which may be attributed todementia may merely be the effects ofmiscommunication and confusion caused by anuntreated hearing impairment.

It's also important to note that cognitivedecline is not just dementia. The ability to processand understand speech, the ability to focus on theperson you're trying to hear and ignorebackground noise – the biggest complaint most

people with hearing impairment have – these arebrain processes and are affected by cognitivedecline.

Earlier research, such as the 1999 report bythe National Council on Aging (NCOA) entitledThe Consequences of Untreated Hearing Loss inOlder Persons, found that adults with untreatedhearing impairment were more likely toexperience the following: embarrassment, fatigue,irritability, tension and stress, anger, avoidance ofsocial activities, withdrawal from socialsituations, depression, negativism, danger topersonal safety, rejection by others, reducedgeneral health, loneliness, social isolation, lessalertness to the environment, impaired memory,less adaptability to learning new tasks, paranoia,reduced coping skills, and reduced overallpsychological health.

This study shows that even those things whichmay be attributed to the “natural process of aging”suchas “reduced general health”, “fatigue” and“impaired memory” will more likely and moresignificantly impact those with untreated hearingimpairment.

The psychological and emotional effects ofuntreated hearing impairment can be devastatingto a person’s quality of life. It can rob us of thevery thing many of us value the most, ourrelationships. So, if you or someone you know hasa hearing impairment, the sooner that impairmentis treated the greater the benefit and the easier theadjustment will be. Take action beforepsychological and emotional damage has beendone, before relationships have been damaged,before the brain’s ability to understand sound in anormal way has been irrevocably damaged.
