Objective
• Record Vaccination data for a medical case using the new US2014 form
• Familiarise the standard Required Vaccines by Age
• Manage Vaccine Availability• Validate the US2014 form• Understand the updates of the US2014 form
on the list of Vaccination medical services in the Summary tab of Individual Details page
Prerequisites
• Individual is a registered case with a Medical activity
• Individual is a USRAP (refugee) or US Immigrant (self-payer) case
• Individual is assigned or scheduled for Health Assessment
Process Overview
• Find an Individual• Record the individual’s Vaccination in the
Health Assessment Form– Vaccine history (if any)– Vaccine Doses provided by IOM– Reason for Blank Waiver (if any)
• Validate the Health Assessment Form
Rules
• Vaccines provided by IOM during the initial exam is recorded under the Vaccine Given By Panel Site column
• Additional vaccines, prior to re-medical, are recorded under the Additional Vaccine Given by IOM columns
• Not routinely available vaccines can be configured per mission
• Not age appropriate vaccines are based on a standard list of required vaccines by age

Sample Medical Profile – USRAP 2014
• Standard Medical Form: US2014• Health Assessment Core Services:– PE IOM– X-Ray Chest (only for ages >= 15 yr)– TST (only for ages >= 2 yr and ages < 15 yr)– Syphilis Screening (only for ages >= 15 yr)– MMR (only for ages 1 – 64 yr)– DTP (only for ages >= 2 mos and ages < 7 yr)– Hep B Vaccine (for 2 mos and above)
Any field in the Contraindication to Vaccination section can be checked only if there is at least one vaccine with Reason for Blanket Waiver = C

Pregnant must be selected only if Pregnancy, current is Yes in the History tab

Form-Service Mapping
• The following fields from the US2014 form are mapped to a Vaccination medical service:– Vaccine Given by Panel Site Date– Additional Vaccine Given by IOM Date 1– Additional Vaccine Given by IOM Date 2
• All 3 date fields populate the Completion Date of the Vaccination medical service
• If all 3 date fields are populated, the Health Assessment of the individual will show 3 instances of the vaccination medical service
• ND is the Not Done checkbox field that sets the status of a core Vaccination medical service to Not Done
Helpful Hints
• Appointments are not required to start recording results
• System updates the Medical Services in the Summary tab of the Individual Details page based on updates made in the US2014 form (including the Status)
• Set All No/Negative/Normal function can populate most of the data for you, but it is still best to do QA/QC
