On behalf of the board of directors and staff of Health Promotion Council (HPC), I am honored to share
our 2008-2009 achievements in this annual report. Last year was a year of solid achievement and
opportunity for HPC. Despite the uncertainty of the economy and the shifting political environment,
HPC’s impact remains constant, and our commitment to serving diverse communities is unwavering.
One of our most significant achievements over the last year was expanding HPC’s geographic reach
across the Commonwealth of Pennsylvania, touching nearly 60 counties. Expansion enables us to
assist even more organizations in building their capacity to implement targeted chronic disease
risk reduction strategies. We have also continued to convene and build alliances of multi-sector
groups, such as the Southeastern PA (seven county) Regional Tobacco Coalition, six Regional
Advisory Councils and the Philadelphia Urban Food and Fitness Alliance, to influence policies
and practices that address broad-based public health issues like tobacco control and prevention,
childhood obesity, food access and built environment issues. HPC works particularly to address
these issues in low wealth communities.
Join us in looking back on some of our accomplishments—none of which would be possible without
the commitment and support of our staff, board, clients, partners and friends. Bringing together
the right people in the right roles helps ensure that we continue to advance our mission and im-
prove our effectiveness. We thank everyone who has contributed to our mission and success. As
we continue in 2010, we look forward to another productive year filled with opportunities to work
together to make our diverse communities healthier and stronger.

� HPC’s nutrition educators visited 20
Philadelphia schools, reaching 14,500
students throughout the city and pro-
vided additional educational resources
to 500 principals, teachers and nurses.
� HPC’s Southeastern PA Regional Tobacco
Project reached over 1,000 youth and
adult tobacco users, including nearly 90
pregnant women, who participated in
SEPA cessation programming, and suc-
cessfully quit using tobacco.
� HPC’s WISE SNAC (Wellness Initiative
for the School Environment: Smart
Nutrition and Activity Collaborative)
promoted messages and opportunities
for healthy eating and physical activity
to 18,000 children in 25 elementary
schools in three Montgomery county
school districts. The program also
received the 2008 Nemours Vision
Award, which recognizes exemplary
programs in child health promotion and
disease prevention.

� Kathleen Sebelius, 21st Secretary of
Health and Human Services, recog-
nized the Philadelphia MCAN Project
as a national model for managing
childhood asthma during her visit to
Thurgood Marshall Elementary, one of
16 schools in Philadelphia that part-
ners with the Merck Childhood Asthma
Network, Inc. (MCAN).
� HPC’s Child Asthma Link Line received
the U.S. Environmental Protection Agen-
cy’s National Exemplary Award for its
leadership and commitment to compre-
hensive asthma care. The Link Line is a
free, community-based call center that in-
tegrates and helps coordinate medical ap-
pointments, asthma education and other
services for families affected by asthma.
CHRONIC DISEASE RISK REDUCTION CHRONIC DISEASE PREVENTION & MANAGEMENT COMMUNITY & ORGANIZATIONAL CAPACITY BUILDING � HPC’s City Harvest community garden-
ers, in partnership with the Pennsylvania
Horticultural Society, donated 16,000
pounds of food to local cupboards.
� HPC's BE A BRIDGE provided training to
over 700 professionals, representing 283
organizations across the state, to help
them develop integrated population-
specific tobacco and chronic disease-
related programs and interventions.
� W.K. Kellogg Foundation selected Phila-
delphia Urban Food and Fitness Alliance
(PUFFA) Grow and Sow initiative to lead
its Food & Community Program effort.
The $1.2 million national grant is one
of nine awarded by the foundation for
community-led projects that work to
transform food systems and the built and
natural environment locally. The grant
extended the work PUFFA began in 2007
under the W.K. Kellogg Foundation’s
earlier Food & Fitness program.
� Staff from HPC’s Child and Adult Care
Food Program trained over 70 emer-
gency housing employees in nutrition
and food safety.
� HPC provided cooking demonstrations,
tastings and nutrition education to more
than 3,000 people last year at various
community gardens and food cupboards.
� HPC’s Community Healthcare Inter-
preter Training program, modeled after
the nationally recognized Cross Cultural
Health Care Program in Seattle, WA,
trained 98 bilingual individuals to work
as medical interpreters in health care
settings throughout Pennsylvania.

On behalf of the board of directors and staff of Health Promotion Council (HPC), I am honored to share
our 2008-2009 achievements in this annual report. Last year was a year of solid achievement and
opportunity for HPC. Despite the uncertainty of the economy and the shifting political environment,
HPC’s impact remains constant, and our commitment to serving diverse communities is unwavering.
One of our most significant achievements over the last year was expanding HPC’s geographic reach
across the Commonwealth of Pennsylvania, touching nearly 60 counties. Expansion enables us to
assist even more organizations in building their capacity to implement targeted chronic disease
risk reduction strategies. We have also continued to convene and build alliances of multi-sector
groups, such as the Southeastern PA (seven county) Regional Tobacco Coalition, six Regional
Advisory Councils and the Philadelphia Urban Food and Fitness Alliance, to influence policies
and practices that address broad-based public health issues like tobacco control and prevention,
childhood obesity, food access and built environment issues. HPC works particularly to address
these issues in low wealth communities.
Join us in looking back on some of our accomplishments—none of which would be possible without
the commitment and support of our staff, board, clients, partners and friends. Bringing together
the right people in the right roles helps ensure that we continue to advance our mission and im-
prove our effectiveness. We thank everyone who has contributed to our mission and success. As
we continue in 2010, we look forward to another productive year filled with opportunities to work
together to make our diverse communities healthier and stronger.
