2005,Antelope Valley Partners for Health(AVPH)completed a Community Needs Assessment funded by Kaiser Permanente. It concluded that there was a critical need for expansion of primary care, prevention and education services for the uninsured and underinsured.

2006 Antelope Valley Community Clinic Advisory Board was created. Committee members included major providers, organizations & Community members:AV Hospital, Lancaster Community Hospital, High Desert Medical Group, Kaiser Permanente, County of Los Angeles and the Cities of Lancaster.

In March 2007 we incorporated as 501 C 3 and FQHC compliant Board

In August 2007 we received a start-up grant from Kaiser Permanent for $250,000

In March 2008 we acquired Care-A-Van Mobile Clinic from VNCS/PICF

In August 2008 we received $500,000 grant from LA Care.

In January 2009 we received Ahmanson Grant for $350,000

In March 2009 we acquired Clinic in Palmdale.

In April 2009 we began construction on Phase 1 clinic in Lancaster. License in June 09

In August 2009 we opened the Lancaster Clinic

In September 2009 we submitted FQHC Look-Alike Application

In January 2010 we received $350,000 & $850,000 from County for PPP Expansion

In March 2010 we received Parsons Foundation Grant for $150,000 for construction

In March 2010 we received approval of FQHC Look-Alike

In April 2010 we began construction on 15,000 Health and Wellness Clinic

In April 2010 we signed lease on 6,500 sf Clinic in Palmdale with plans to move existing clinic in July

4. Clinic Services

Currently operate 2 Mobile Clinicsand two fixed clinic one in Palmdale and one in Lancaster.

All will be staffed by physicians and mid-level Nurse Practitioners.

Family, internal and pediatric medicine, obstetrics and gynecology. OB Clinic from AV Hospital is sub-letting at proposed Phase 2 and Palmdale

Diagnostic laboratory and imaging services on-site and through AV Hospital and Lancaster Community Hospital in-kind support.

Preventative health services including prenatal and perinatal services. Black Infant Health, Healthy Births, Save A Smile etc through Antelope Valley Partners for Health

Screening for breast and cervical cancer, well-child services, immunizations, disease management for diabetes, asthma and hypertension.

Adult and Children's Dental program

5. Care-A-Van Mobile Health Clinic in service since 1996. Sees 3,200 patients every year at 16 sites throughout the Antelope Valley 6. Care-A-Van Mobile Clinic II. Donated by County of Los Angeles. Plan is to convert to full-service Dental Clinic in 2010 7. Primary Clinics in Lancaster & Palmdale

Palmdale- 2,800 sf opened March 1, 2009. Acquired from local physician. Primarily Women and Children but have expanded to adult medicine. Temp space until we move to larger clinic in Palmdale. Seeing about 750 patients a month.

Lancaster-Arbor Grove 2,800 sf. Opened in August 2009. 7 Exam rooms. Will see pediatrics & adult medicine. About 600 patient per month. When larger Health and Wellness Center opens we will maintain this spacefor older adults and or prevention and disease management care such as our new Diabetes Education & Treatment program. The new facility will be for Womens Services including OB and Pre-natal, pediatrics and Family Medicine.

8. AVCC Lancaster Arbor Grove 2,800 sf 7 exam rooms. Adjacent to Health and Wellness Center. Will continue to use for Adult Medicine and Specialty Clinics 9. 2,800 Lancaster Clinic Completed 8.09 next to Health & Wellness Center 10. Antelope Valley Community Clinic-Palmdale 2,800 sf 6 exam rooms acquired March 1, 2009 Seeing 750 patients per month. License in June 2009 11. 20 more exam rooms. Expanding our primary care, pediatrics, OB, dental and specialty care clinics.Adjacent to existing Lancaster clinic. Store all Mobile Clinics behind building and consolidate all administrative functions plus education and outreach programs
