After helping hundreds of Westchester County herniated disc sufferers eliminated their pain, Local Doctor explains how…
“Back Pain TechnologyCheats Mother Nature
Helps Herniated Discs Heal Naturally… In As Little As 25 Minutes!”At Last! Westchester County Doctor Offers Technology That Doesn’t Just End SEVERE Pain From Herniated Discs,
But Helps Your Body To Start Rebuilding Them… As Fast As Humanly Possible!
Best Part: You can check it all out for FREE if you like!
There is no doubt… technology hasmade all of our lives better… and easier. Just about every miraculous medical
cure and comfort of modern living can beattributed to amazing breakthroughs intechnology. Heck, laser eye surgery can give people
who are almost as blind as a bat perfect20/20 vision. How amazing is that? That’s why it’s no surprise that technol-
ogy might be able to solve your back andneck pain from herniated/bulging discs orsciatica…for good.
Here is why: Back and neck pain can bedevastating. And nothing is worse thangoing to doctor after doctor… trying treat-ment after treatment… spending moneyyou can’t afford… and still suffering inpain. That’s why…I bet you would love to instantly end
your pain… without any risk what-so-ever… and have it never come back forthe rest of your life? Well, that might NOT be so easy. But,
what if there was a wonderful advance-ment in technology that, with a handful ofsimple, non-invasive treatments, couldpossibly have you out of pain and on yourway to healing naturally?Better yet, what if this technology could
possibly heal your herniated/bulging discs(instead of cutting it out like surgery) soyou have the best chance to live pain-freeand do all the things you want and love todo? And what if this technology was: 3 FDA cleared!3 Proven safe and effective!3 Allows your herniated discs to
heal naturally without the risky anddangerous side effects of surgery!3 Relieves pain fast… in some cases in as little as 25 minutes!
Well, guess what? There is such tech-nology and there is a very good chance itcan help you… like it has already helpsthousands of herniated/bulging disc andsciatica sufferers all around the country…No Matter How Bad Your Pain Is, How
Long You’ve Been Suffering Or How Many Other Treatments And
Doctors Have Failed You!Don’t laugh. It’s really true. Here’s
proof…
When you have a herniated disc, com-pressive forces cause your spinal bones tocome together… basically squashing yourdisc. The space in between your bones de-creases and the soft, jello-like disc materialleaks out. This disc material that leaks out is
called a herniated disc and can cause se-vere pain, numbness and tingling in yourneck, back, arms or legs when the hernia-tion pushes into and “pinches” a nerve. So it makes sense, if you could slightly pull
the spinal bones apart… that would open upthe space, take the pressure off the disc, thedisc off the nerve and solve the problem.Right? But the only problem is… how do youdo that? For years and years, doctors havebeen trying to do it with traction. But traction has been proven ineffective
and often very painful. Why?Traction doesn’t work because when you
have a herniated or bulging disc, the area isinjured and when you try to move, your mus-cles go into spasm. Spasm is your body’sway of trying to protect you from causing fur-ther injury. That’s why traction hurts so much.As soon as the traction starts… your musclesoverprotect the area and spasm.
But the good news is: Now there istechnology that out-smarts your body’snatural spasm reaction. In other words: Itfools mother nature! This technology is called non-surgical
spinal decompression (NSSD) and here’show it works… NSSD is very different from plain trac-
tion because it has advanced computer tech-nology that actually senses your spinalmuscles. When it starts to gently pull, it im-mediately knows when your muscles startto contract and it stops pulling. As soon asyour muscles relax, it starts pulling again.
The amazing thing is: the computersenses your muscles contraction soearly… you don’t even feel what is goingon. Which means you never feel any pain!In fact, the only thing most patients feelis… RELIEF!!!
But the best part is: NSSD is able togently separate your spinal bones which,in many cases, actually sucks the herni-ated disc material back into the disc whereit belongs! This takes away your pain andallows your herniated and bulging discs toheal naturally!
And check this out: Treatments arepleasant and simple. All you have to do islie on your back, listen to your favoritemusic, read a magazine… or… take a nap!The wonderful technology does everythingwhile you simply RELAX the pain away!
To sum up NSSD:• For most patients it is 100% pain-
less…in fact many patients actually fall
asleep during treatment!• Works fast! Most patients get relief
after a handful of treatments – some after the very first!
• Is non-invasive so it does not havethe dangerous risks of surgery!• Gets to the root cause of the prob-
lem and helps it heal naturally!Isn’t modern technology wonderful?
And wouldn’t you like to… Give This Great Technology A Try…
For Free?If you would, there’s great news. There
is a Doctor located at 1241 MamaroneckAve. right here in White Plains, NY who hasinvested hundreds of thousands of dollarsin this wonderful technology and has beentreating herniated/bulging disc and sciaticapatients with incredible success. His name is Dr. Jonathan Donath DC,
MS and he is so excited about the resultshe’s been getting, he would like to share itwith as many pain sufferers as possible.
But There Is A Catch…There’s always a catch... isn’t there?
Even though this technology has helped amajority of the patients Dr. Donath hastreated… it is not a wonder cure and itdoes not help everyone.
For that reason, Dr. Donath does a com-plete examination on every potential pa-tient and only accepts you if he feels youare most likely to get the pain relief andoutcome you are looking for.
Dr. Donath’s Special Deal For You!Because the economy is so bad and Dr.
Donath wants to help you if possible, he isoffering the first 17 people who respond tothis article a free consultation and NSSDqualification examination. And if you qualify for NSSD treatments,
Dr. Donath will also give you one treatmentfor FREE! And if you do not qualify, Dr. Do-nath will help you find the best option foryou. That’s why, if you suffer with pain from
a herniated/bulging disc or sciatica youshould call 914-259-8104 right now. WhenBarbara answers the phone, simply tell heryou would like your free non-surgicalspinal decompression consultation and ex-amination. If you are one of the first 17,she will schedule you within the next fewdays. If you are in a lot of pain or alreadyscheduled for surgery, tell Barbara and shewill be sure to get you in immediately.Maybe even today.
Your consultation and examinationshould take about 60 minutes. During thistime you can get all of your questiones an-swered in a warm and friendly environment. You will know exactly what’s causing yourpain and the best possible way to solve it.Dr. Donath will explain to you all your possibleoptions along with his recomendations soyou can make the best choice for YOU.
Why Limited To 17?Dr. Donath has limited the number he
will accept right now to 17 because hedoes not believe patients should wait andthey should also get the quality personalattention they deserve. When you go for your free visit, I’m
sure you will see how unique and pleas-ant the experience is. It’s nice to betreated like a person and not a numberor based on insurance coverage. It’salso nice to know exactly what’s causingyour pain and how to relieve it as fast aspossible. Dr. Donath’s favorite cases arethe one’s that were able to cancel backsurgery after only a handful of treat-ments.Dr. Donath also loves when a herniated
disc patient that is in excruciating pain andthinks they’ve tried everything, lays downon the NSSD table, takes a 25 minutenap… and… wakes up with all their painGONE!Just imagine how it would feel to have
all your pain finally gone. Imagine going to bed and being able to
sleep the entire night through… … and waking up refreshed and ener-
gized… ready to take on the brand newday – without the pain and stiffness thathas been terrorizing you.And imagine finally knowing you
have the solution to your herniated discpain.Well, you may not have to just “imagine”
anymore… because… NSSD might startrelieving your pain… in just 25 minutes! So, if you’d like to see if you qualify and
give this amazing technology a try forfree… give Barbara a call at 914-259-8104right now. Why wait one more day in painwhen the solution may be one quick phonecall away? Call now before someone elsegets your free spot.
One More Thing It’s Important…Ever since getting this wonderful
technology, Dr. Donath’s office has beenflooded with calls. For that reason, ifwhen you call, the lines are busy or yougo to voice mail… just keep calling back. The possibility of living pain-free is wellworth the time and effort it may takeyou to get through and schedule yourappointment. 914-259-8104

Never go to a party hungry. This
will only cause you to eat more
throughout the night. Try to have
a small snack before attending so
you aren’t ravenous when you
arrive.
While standing around a buffet
table, most people tend to take a
little bit of everything. This may
sound like a good plan, but it can
quickly add up to more calories
than you need. Your best bet is
to stick to the dishes that look
the most appetizing to you.
This way you can enjoy the
foods you like, and still leave
room for dessert.
Remember to watch your
portions. Meat should be about
the size of the palm of your
hand. Starches, including pasta,
rice, and potatoes, should be no
larger than the size of your
closed fist. Try to fill your plate
with fresh, healthy vegetables,
which will help fill you up
quicker, without adding too
many calories.
Watch out for appealing sauces,
gravies and dips. These items
tend to contain a lot of fat and
calories that will quickly add up.
Take caution with alcohol at
parties. Many holiday drinks
contain a lot of fat and calories
that will quickly add unwanted
pounds to your waistline. Stick
with a 12 oz. beer, 5 oz. glass of
wine, or a small cocktail made
with fresh juice.
Try to get in some type of exercise
the day of a holiday party. Even
if you can only find 10 free
minutes, it will be beneficial to
burn some extra calories before
you consume a lot of them.
During the holiday season,
socialize and celebrate good
times with friends and family.
Remember to stick to your
healthy eating plan so you don’t
overindulge on things that will
steer you off your path.
We wish you and your
family safe, healthy, and
happy holidays!
Healthy Holiday eatingYou have to love the holidays, filled with parties,
family gatherings, and lots and lots of food and drink!
As they say, 'tis the season. It can be hard to stick
to healthy eating habits you’ve worked so hard to
follow all year.
With so many food options to choose from,
it can be difficult to avoid temptation.
Here are some tips to help you avoid the
dreaded weight gain during the holidays.
You came to our aid during the most stressful time of life for my
family. You are like a guardian angel to us - with every task you
gave us 110 percent of your efforts and you have resolved many
issues for us, in both financial and legal matters. We have known
you for over eight years and you have always demonstrated
fairness and good judgment. In addition, you have provided
invaluable comfort, friendship and legal guidance for my family
and we are grateful for your services. We look forward to continue
to work with you and your firm.
– Beatrice Y.
I hired Ely to assist with my parents estate planning and it
was a wise decision. He is a true expert in navigating through
complex elder care law and procedures. His guidance was
invaluable and his communication excellent. I would strongly
recommend his services.

INTRODUCTION
Snoring and Sleep Apnea are common conditions in our society today. They arecaused by a narrowing or collapse of the airway during sleep. Snoring or sleep apneaappliances are devices that are worn in the mouth during sleep that are designed toreposition the lower jaw so that the airway is supported in such a way that air canflow more freely. This reduces or eliminates snoring and can treat your sleep apnea, which can lead to a more quiet, restful night’s sleep.
CAUSES OF SNORING
70-90 million people are affected by snoring. Snoring can occur when the airway becomes partially blocked during sleep. This may occur more frequently when:
You sleep on your back You are overweight You have a large neck size The tongue falls back into the throat The tonsils and/or adenoids are large You mouth breathe during sleep You are overly tired You eat or drink alcohol too late at night You have a cold or are congested
Snoring becomes more common: As we get older If you are a male After menopause If you have a small airway (may be genetic)
WHAT IS SLEEP APNEA?
Sleep Apnea is estimated to affect 20 million people. Sleep Apnea occurs when the airway becomes completely blocked and air is not able to move through theairway. This is commonly called Obstructive Sleep Apnea because the airway is totally obstructed. This obstruction may occur anywhere from the area of the softpalate down to below the tongue. When the airway is obstructed, the patient willawaken many times during the night with a cough or gasping sound as the bodyattempts to restore normal breathing.
SYMPTOMS AND SIGNS
The most common symptoms and signs of Sleep Apnea: Snoring Non-refreshing sleep Daytime fatigue or tiredness History of falling asleep driving, watching TV, or reading Awaken during sleep coughing, choking, or gasping Morning headaches Elevated blood pressure Cardiovascular disease
Other commonly occurring findings associated with Snoring and Sleep Apnea: Gastric reflux or acid reflux with indigestion Grinding of the teeth during sleep (Bruxism) Symptoms of depression, anxiety, dementia Diabetes Mood swings

It's a time to celebrate, decorate, shop for gifts, decorate the Christmas tree or light the menorah. Ah, the holidays. What a wonderful time of the year.
What a stressful time! The Holidays are one of the most stressfulevents of the year. The expense of buying gifts, the pressure of last minute shopping, and the
heightened expectations of family togetherness can all combine to undermine our best
intentions. Here are some suggestions can help you reduce your 'Christmas stress'.
Budgeting for the HolidaysFor many of us, the Holiday aftermath includes massive credit card
bills that can take months to clear. Christmas or Hanukkah doesn't have to
be a financial headache if you plan ahead. Stress reduction strategies include:
Work out a rough budget of expected costs as early as possible.
This includes ‘hidden’ expenses such as food bills and overseas
telephone charges.
Calculate how much disposable income you have between now
and the holidays. A certain percentage of this can be dedicated each
week (or fortnight or month) to covering your expected costs.
Don't be discouraged if the amount seems small. If you save $5, $10,
or $20 per week over a year, it can provide you with a hefty nest egg.
If your nest egg isn't enough to cover your estimated expenses,
consider recalculating your budget to a more realistic amount.
If you have trouble keeping your hands off your nest egg,
consider opening a 'Christmas Club' account with a bank or
credit union.
PresentsIf you have a large circle of extended family or friends to buy
gifts for, it can be very costly. You might be able to reduce the
stress and cost of Christmas for everyone if you suggest a
change in the way your family and friends give presents.
For example, you could suggest that your group:
Buy presents only for the children.
Have a Secret Santa, where everyone draws a name
out of a hat and buys a present only for that person.
Set a limit on the cost of presents for each person

Shopping strategiesMake a list of all the gifts you wish to buy before you go shop-
ping. If you wait for inspiration to strike, you could be wandering
aimlessly around the shopping centre for hours.
Cross people off the list as you buy to avoid duplication
Buy a few extras, such as chocolates, just in case you forget
somebody or you have unexpected guests bearing gifts.
If possible, do your holiday shopping early – in the first week of
December or even in November. Some well-organized people do
their Christmas shopping gradually over the course of the year,
starting with the post-Christmas sales.
Buy your gifts by mail catalogue or over the Internet. Some com-
panies will also gift-wrap and post your presents for a small addi-
tional fee.
Tips to reduce the stress of cookingIf you are cooking lunch at home, delegate tasks. You don't need
to do everything yourself.
Consider keeping it simple – for instance, you could always
arrange for a 'buffet' lunch, where everybody brings a platter.
Make a list of food and ingredients needed. Buy as many non-
perishable food items as you can in advance – supermarkets on
Christmas Eve are generally extremely busy.
Write a Christmas Day timetable. For example, 11.30am – put
turkey in the oven.
You may need to order particular food items (such as turkeys)
from your supermarket by a certain date. Check to avoid disap-
pointment.
Book well in advance if you plan to have lunch at a restaurant.
Some restaurants may be fully booked for months before Christ-
mas, so don't wait till the last minute.
RelationshipsStress, anxiety, and depression are common during the festive
season. If nothing else, reassure yourself that these feelings are
normal. Stress reduction strategies include:
Don't expect miracles. If you and certain family members bicker
all year long, you can be sure there'll be tension at gatherings.
Avoid known triggers. For example, if politics is a touchy subject
in your family, don't talk about it. If someone brings up the topic,
use distraction and quickly move on to something else to talk
about.
Use relaxation techniques, such as deep breathing or focusing on
your breath to cope with anxiety or tension.
Family members involved in after-lunch activities (such as
cricket on the back lawn) are less likely to get into arguments.
Plan for something to do as a group after lunch if necessary.

You Can Save Hopeless Teeth without ImplantsBy Edward Feinberg DMD
ASK AN EXPERT
Although implants were availablein 1993, Ms. B. did not want them. She was not a particularly goodcandidate for implants becausethe low-hanging sinus cavity wouldnot allow the placement of im-plants long enough to support den-tistry without bone grafts to raisethe level of the sinus. She had anodd pattern of tooth loss, withmissing teeth only on one side.Implants should not be placedwhen there is a poor foundation orif a patient does not want to gothrough the expense, surgery andhealing time. Ms. B. chose to re-store her mouth with the precisionattachment case. Most practition-ers are not able to offer this optionfor restorative treatment--they aremore adept with implants and theyare biased in their beliefs that fixedbridgework is always a better op-tion than removable bridgework. Not true!
The Precision Attachment Case
When there are not enough teethfor fixed bridgework, the precisionattachment case is a great restora-tive option--equal to or better thanfixed bridgework with implants. The precision attachment casehas a much longer track recordthan implants. Very few practi-tioners know much about preci-sion attachment partial denturesas demonstrated in this article--even though the concept was in-vented in 1906. Most patientsare only offered the clasp type ofremovable partial denture withvisible hooks that grip or lockonto the remaining teeth. Claspsand locking mechanisms applydestructive forces to the teeththat support the removable andusually result in the loss of theseteeth. As a result, this type of re-movable is really transitory to afull denture.
This is simply amazing! Most partialdentures that utilize attachmentscontain components that requireconstant replacement.
Saving Hopeless Teeth
In 2000, Ms. B.'s upper left bridgesupporting the removable partialdenture snapped off at the gumline.Teeth that have had root canal orbecome non-vital also becomebrittle and prone to fracture. Ms. B.did not want to remake her bridge-work, since making new bridge-work to fit an existing precisionattachment case--a difficult chal-lenge when that demands preci-sion to tolerances of tenths ofmillimeters to be successful.
Most practitioners cannot evensave teeth that are fractured at thegumline without building the teethup in some way. Every dentalschool teaches techniques that re-quire tooth structure above thegum to retain bridgework. A greatdeal of time and effort is expendedon building the teeth up or makingposts for this purpose. This ap-proach often fails. When bridge-work fails, it is a sure bet that thepost will come out with it.
However, there is a precision tech-nique with copper bands that hasbeen used in crown and bridge-
In a precision attachment case, thefixed bridgework houses “femalereceptacles” (locks) and the partialdenture contains a male “key” thatis machined to fit precisely intothese receptacles. The attachmentis so precise that any male out ofthe box will fit any female with per-fection. The precision attachmentpartial denture is not locked inplace. It cannot come out duringnormal function because the pathof insertion of the partial denture isdifferent from pull of the musclesand gravity. If a stress is applied tothe partial denture, it can moveslightly to dissipate the stress, but itcannot come out. The result ishealth and longevity of the teeththat support the partial denture andpreservation of the tissues under-neath. The precision partial den-ture is worn 24 hours a day and isonly removed for hygiene pur-poses. No one can tell that it is re-movable bridgework, as the teethare custom made to hide all evi-dence of the attachment.
The precision attachment partialdenture has been made at oursince 1950 and has a track recordof lasting longer than any other typeof case (implants included)—evenon the weakest teeth imaginable. Itis rare for the males on the partialdenture to be replaced—evenafter decades of continuous wear.
work for over seventy years thatonly requires tooth structure belowthe gum for success. Crown andbridgework made with this tech-nique fits with perfection. Since1950, our office has made thou-sands of restorations with thistechnique. All of them are worn fora trial period with Vaseline or rub-ber prior to permanent cementa-tion and they do not fall out! Theyalso have margins that are farbelow the gum and are not visible.Because they are designed like theMason Jar cover (the best deviceever invented for food preserva-tion), decay is a rarity—even in pa-tients who are highly susceptible todecay!
We were able to make two crownsfor Ms. B. that we attached to herexisting bridge with acrylic. Thisbridge has the housing for herprecision attachment case, soadding the crowns requires a highdegree of accuracy. Ms. B.'sbridge now fit like brand-new! Ithas served her well for the pasttwelve years--supporting herpartial denture under continuousforces, under water and in an en-vironment loaded with bacteria. There have been no changes inher X-Rays during all this time--anabsolute miracle!
Precision Dentistry is Least Expensive in the Long Run
Ms. B. knows that precision den-tistry is the very best investment sheever made for her oral health, hersmile, her ability to eat anything andher peace of mind. Her dentistryhas given her far more pleasure andquality of life than any vacation, piece of jewelry or fancy car.

Why should I have a lawyer prepare my will?
If you die without a Will, the law, not you, determines how your assets are divided and to whom they are
given. A poorly drafted Will may result in a sizeable chunk of your assets not ending up where you want
them to go. We are an experienced law firm that can expertly craft your will to address your long-term care,
tax, estate planning, and asset protection planning issues. It is in your best interest that planning be done
properly so as to minimize the tax burden on your estate.
Our experience and skill in the fields of Trusts, Wills, and Estates enables our firm to uniquely tailor estate
planning to your individual needs. We will review your entire estate and draft a personalized plan that will
best direct the disposition of your assets after death, whether through your Will, or by means of testamentary
substitutes (trusts, joint-asset ownership with right of survivorship, or by beneficiary designation in life
insurance policies, annuities, or individual retirement accounts).

The law firm of Ely J. Rosenzveig & Associates, P.C. practices in all aspects of elder law, including
trust and estate planning, administration, and litigation, tax counsel, asset protection and special
needs planning, Medicaid, Medicare, and Social Security disability benefits, guardianships, long term
care planning, spousal recovery claims, and Medicaid lien resolutions.
Why should I spend money hiring attorneys to do asset protection planning for me and my loved ones?
Currently, the monthly cost for nursing home care in New York ranges between $10,000-$15,000, and properly certified home health attendants (nurse aides) can cost as much
or more for round- the-clock home care. Statistically, it is likely that most people will need either or both of these care options as they grow older. It is critical that you
plan appropriately to best minimize the strain that elder care costs may have on your estate, and to help put you in the best position possible to qualify for benefits that are
largely covered by the government, under Medicare or Medicaid.
Our firm has attorneys with extensive experience in developing tax-effective action plans for our clients that help protect their assets from government imposition. We have
also been very successful in negotiating the complex bureaucratic minefield in which Medicaid is now mired. We have achieved remarkable results for our clients on a host
of Medicaid related issues, from obtaining Medicaid approvals for coverage of home care and institutional care benefits, to very accommodative resolutions of Medicaid
spousal recovery claims, where the office of Medicaid aggressively pursues the non-institutionalized (i.e., or “community”) spouse for Medicaid benefits provided.

Dr. Michael Tobias, M.D., Co-Chief of Pediatric Neurosurgery,Maria Fareri Children’s Hospital at Westchester Medical Center
The prospect of brain surgery is daunting, but it’s especially frightening when thepatient is a child. Fortunately, the pediatric neurosurgeons of Maria FareriChildren’s Hospital at Westchester Medical Center are equipped to successfully treat the youngest and smallest patients using the latest technology—including minimally invasivetechniques that can help with surgery, and sometimes eliminate the need foropen surgery.
Leading the way for a truly talented groupis Tobias. Complex neurological disordersin children such as epilepsy, brain tumors, spina bifida and hydrocephalusoften require a multi-disciplinary team of specialists to provide the most comprehensive and up-todate treatment.
For Tobias, there are two elements to providing the best patient care:surgical skill and attention to a child’s emotional well being. As ChiefResident in Neurosurgery at Albert Einstein College, Dr. Tobiassought to improve both sides of the equation by redesigning a shuntsystem used to treat children with hydrocephalus (excess fluid on thebrain) and establishing a new curriculum for neurosurgery residents topromote better communication. He feels strongly that caring for children poses unique challenges, but that’s what makes the job so rewarding. When treating children, he says, medical specialists reallyhave multiple patients to consider -- the child and his or her parents.Specialists also need to collaborate closely with the child’s pediatricianand be sensitive to the child’s emotional well being.
Tobias received both his undergraduate degree with honors and hismedical degree with honors in neurosurgery and pediatric neuro-surgery from Brown University. He completed his internship and residency in neurosurgery at Albert Einstein College of Medicine,Bronx, NY, where he participated in the establishment of a new educational curriculum for resident training in pediatric neurosurgery. He then trained as a Pediatric Neurosurgery Fellow at LeBonheur Children’s Medical Center and St. Jude Children’s Research Hospital,in the Department of Neurosurgery, University of Tennessee, Memphis,TN. Tobias has also co-authored multiple academic publications.

Dr. Roy Ashikari, M.D., FACS, Hudson Valley Hospital Center (HVHC)
Ashikari started a new journey-opening the Ashikari Breast Center at Hudson Valley Hospital’s new cancercenter in Cortlandt Manor last November. HVHC basically welcomed a legend. Chief of The Breast Serviceat Sloan Kettering during the 1970’s and 1980’s, Ashikari has been the premier senior breast surgeon inWestchester County for the past two decades. He has written the seminal research papers on Paget’s disease, Ductal (DCIS) and Lobular (LCIS) Carcinoma In-Situ, Male breast cancer, and breast cancer inpregnancy.
Ashikari has opened his new Cortlandt Manor location to better reach patients in northern Westchester, Rockland, Putnam and Orange counties. The Ashikari Breast Center is located beside the Emergency Department and consists of Ashikari, his son Andrew Ashikari, and Dr. Pond Kelemen. With Roy Ashikari,semi-retired, his son and Keleman continue to provide some of the best care in the field.
The Ashikari team has pioneered a breast reconstruction procedure that has attracted women from all overthe world, most notably a nipple-sparing procedure that allows them to remove cancerous or potentiallycancerous breast tissue, minimizing the trauma of a mastectomy. Many women, including those who arecancer-free, but have high-genetic risk and are seeking prophylactic surgery, favor this approach.
What sets the Ashikari team apart from others is their determination to use the latest and more effective techniques to eradicate cancer while fully supporting and treating the patient as a whole person.

Erik Larsen, M.D., Assistant Director of Emergency Room –White Plains Hospital
Larsen has the stuff you see on televisionshows. Think ER or Grey’s Anatomy. Butthere is a big difference between Larsenand the TV dramas. He is the real thing.
What Larsen did to help save a patientthree years ago is still the stuff of legends. With the 49-year-old mother of two clinging to life, Larsen made a quick decision that you only see on Grey’sAnatomy. He used cooling blankets recently purchased by the hospital but not tried, with no protocol established, and induce therapeutic hypothermia, tostave off brain damage that so often accompanies this kind of without-warningmassive heart attack.
And it worked. Thanks to the enterprisingLarsen, the work of his staff and EMT’s,the patient survived.
Larsen has an extensive history of progressive leadership and administrativeexperience with Emergency Medicine. He has experience in flight, disaster andwilderness medicine. Most notably, Dr. Larsen was the Medical Director at the New Orleans International Airport during Hurricane Katrina as well as theMedical Director of the NDMS RapidNeeds Assessment Team during Hurricane Wilma.

Retiring at last. You have earned it by every measure. But now, you face an important transition in your personal financial plan.
You have finished accumulating retirement assets and are now beginning to distribute them as income to support your retirement
lifestyle. The choices you make now can certainly impact your long-term financial situation.
Preserving Your Assets
Building a systematic withdrawal plan around a sustainable withdrawal rate is a smart effort for making your money last, but it may not
be enough. Assume you have a retirement portfolio worth $1 million. Initially, annual withdrawals of 5% ($50,000) seem conservative.
You may be surprised to learn however, that at that rate there is only a 90% chance your money will last just 20 years.1 In other words,
one in ten people following this seemingly conservative withdrawal strategy could potentially out live their retirement nest egg later in
life.
In addition to careful consideration of withdrawal rates, retirees must be aware of the significant risk market volatility poses. Erratic
swings in equity-based investments can decimate portfolio values without warning - seriously threatening potential future income since
percentage-based withdrawals shrink as total assets are eroded.
Holding onto what you have - asset preservation – is a top priority for retirees. Though you may not know how long you’ll live, you do
know that running out of money is not a scenario you hope to experience. If only there were guarantees… The good news is: there are.
Get Guaranteed Life-long Income
A Guaranteed Lifetime Income Annuity2 is a financial product that does, in fact, guarantee life-long income. To illustrate, imagine that
same hypothetical million-dollar portfolio. From it, you could use $842,3343 to purchase a Guaranteed Lifetime Income Annuity, which
will provide the $50,000 per year of income you had planned to live on. But, this $50k is now guaranteed for as long as you live, regardless
of market performance. No more worrying about outliving your savings. Plus, you would still have $157,663 left in your portfolio to
access as you wish, making the Guaranteed Lifetime Income Annuity significantly more efficient than a simple withdrawal strategy.
In case of premature death, the Guaranteed Lifetime Income Annuity has a Cash Refund option, which returns to beneficiaries the difference
between the initial premium and whatever has been already paid out. If inflation is a concern, an Annual Increase Option4 can be added
to the annuity contract at purchase -- initial annual payments would start off smaller, but increase each year between 1 – 10% according
to the owner’s preference. Over time, the total annually adjusted payouts would potentially provide more income than their unadjusted
counterparts.

A Monte Carlo simulation analyzes probabilities and seeks to identify the number of years your profile will last, given your financial profile. The simulation is
hypothetical, does not reflect past performance, and is not a forecast of future results. Simulation results may vary with each use and over time. Each client’s results
depend on individual factors which a simulation does not account for, such as how investment decisions are implemented in reality and the costs of investing.
A simulation may not capture how asset classes fall in and out of favor over time. These factors can influence a client’s results materially.2 New York Life’s Guaranteed Lifetime Income products are issued by New York Life Insurance and Annuity Corporation, (A Delaware Corporation), a wholly

Carbon Monoxide
Gasoline-powered generators release carbon
monoxide (CO) which can be deadly. If your
power goes out and you are using a gasoline-pow-
ered generator for energy, be sure that the genera-
tor is installed safely and far from your home.
Never use your generator, charcoal or gas grills,
gas lanterns or camping stoves indoors or in an en-
closed area, like a garage. Gas stoves can produce
carbon monoxide. Do not use your gas oven to heat
your home.
Chain Saw Safety
Before moving or cutting tree limbs, make sure
there are no wires tangled in the limbs.
If you have little or no experience using a chain
saw, hire a licensed professional to remove tree
limbs and downed trees safely from your property.
Wear eye/face protection, foot protection, hard hat,
ear protection, and chainsaw chaps.
Drinking Water
After a storm, water from public water supplies
and private wells may not be safe to drink. Water
that is dark, has an odor, or has floating pieces
should not be used. Listen to the news or your
water company to know if your water’s safe. Pri-
vate wells that are flooded or may be contaminated
should be disinfected before use.
Electrical Wires
DO NOT TOUCH downed electrical wires. Report
it to your local utility company, police or fire de-
partment. Do not turn power back on until a qual-
ified electrician has inspected the property and says
it is safe to do so. Ask him/her to check wiring, ap-
pliances, and mechanical system components like,
the furnace, water heater, and gas valves, to see if
they can be salvaged or will need to be replaced.
Fires
Prevent fires by using flashlights or battery pow-
ered lanterns, if possible. If you must use candles,
place them in safe holders, away from anything
that can burn and never leave unattended.
Flooding
Flooding can contaminate your well, making your
water unsafe. Information on what to do if your
well is contaminated.
Medications
If you have to evacuate or leave your home, be sure
to bring your medications and medical supplies
with you. You should bring enough medication and
medical supplies to last you for a few days. If you
have a question on how to store your medication,
including medication that requires refrigeration,
you should contact your pharmacist. Persons who
depend on medical supply deliveries and plan to
temporarily stay at another address – with a neigh-
bor, family member or at a shelter – should notify
their medical supply providers to have their sup-
plies delivered to the temporary location with no
lapse in their care.
Sewage safety
Severe rainstorms, floods, and even spring thaws
can put a strain on sanitary sewers and septic sys-
tems. Large volumes of storm water and ground
water entering sewage systems can flood them,
causing back-ups into basements and on to private
property. Blockages in sewer systems can also
cause back-ups. Your health may be impacted if a
sewage back-up occurs in your home. Sewage

Like so many other areas in the northeast, Westchester County was hit hard by
Hurricane Sandy. In some parts of Westchester, the damage was devastating,
and it will take several weeks to recover.
Even though Sandy has left, it's still important to remain safe during clean-up
and restoration efforts. Here are important post-Hurricane safety tips. Stay safe!

1. coffee enemas are the best single method of detoxifi cation for a person with cancer? ANSWER. False. Although possessing detox ability, coffee enemas simply do not contain the
necessary nutrition to support specifi c detox pathways, which are dependent on the toxin or
toxins in question; this must be determined based on clinical interview and lab testing.
2. a macrobiotic, vegetarian or raw diet is the best dietary approaches for one with cancer?ANSWER. Regardless of the type of special food plan in question, the very best plan must be
based on the needs of the individual. A raw diet may over tax an individual’s ability to produce
natural enzymes and, a particular diet may be defi cient in one or more nutrients that are
required by the specific patient who has cancer.
3. intravenous vitamin c, as opposed to oral vitamin c, produced anticancer effects such astumor killing through by being an antioxidant? ANSWER. False. Intravenous vitamin C has
been proven to work in cancer because it is an oxidant and NOT an antioxidant.
4. hydrogen peroxide is a potential cancer killing agent? ANSWER. Yes. Hydrogen peroxide is
produced by white blood cells in the body and does have cancer cell killing effects.
5. apigenin is an herb that has proven ovarian cell cancer killing effects?ANSWER. True.
6. according to the american cancer society, green tea drinkers in east asia have a lower riskof stomach, breast and colon cancer than non-tea drinkers? ANSWER. True; however, the
amounts of active elements in green tea consumed by Americans cannot be expected to be
the same. Also, it is best to take the active ingredients of green tea when one has cancer as
opposed to drinking tea only.
7. The form of vitamin D currently available in health food stores is the type that has beenused in most medical studies for cancer prevention and treatment? ANSWER. False.
The active form of vitamin D3, known as 1, 25-D3, is the major anticancer form of vitamin D.
This is a prescription item.
8. The optimal level of vitamin D measured on blood work is between 20 and 100 mg/dl? ANSWER. False. Although the accepted clinical range of vitamin D tested on blood is between
20 and 100, optimal levels are approximately 75 and are associated with the lowest morbidity
and mortality.
9. The evidence for toxic levels of vitamin D are when blood vitamin D are high? ANSWER.
False. The level of vitamin D in the blood can be high, but this does not mean it’s toxic.
High calcium levels, known as, hypercalcemia is evidence that too much vitamin D is present.
The treatment is to remove the vitamin D until the calcium levels come down (in a few days)
and then restart vitamin D at a lower dose. Simple as that!

In the aftermath of Hurricane Sandy, County Executive Robert P.
Astorino has advised homeowners and businesses needing repairs to
use caution when selecting a contractor to avoid scams and being gouged.
"Common sense is a consumer's greatest asset," said Astorino.
"Use it. People who have had their homes damaged are upset and very
vulnerable. Opportunistic individuals can and will take advantage of this."
John Gaccione, the acting director of the county's Consumer
Department, said that the county will enforce any laws against gouging.
"Taking advantage of consumers in need and raising prices without
any justification will be investigated by the department," he said.
Home improvement contractors working in Westchester must be
licensed. A licensed contractor will have the necessary contractor in-
surance. To find out if a contractor is licensed, check online at
www.westchestergov.com/consumer.
Here are other tips from the
county's Consumer Department:
If you haven't done so, call your
insurance company as soon as you
can. Many insurance companies have
existing relationships with home repair
contractors.
Watch out for contractors who are suddenly
in the area. Fly-by-night contractors will
take advantage of homeowners in need.
Don't get rushed into signing a contract
or agreeing to repairs.
Avoid contractors who demand payment in cash or want more
than one-third of the total payment before work has begun.
Do not agree to an oral contract. Get everything you can in writing.
Insist on a written contract that sets forth all the important details
of the job. That includes a detailed scope of work, a start date, a
completion date, cancellation clause, warranty information and a
schedule of payments.
Get at least three estimates. In reviewing the estimates, don't always
go with the lowest bidder. A higher bid may be worth it if the
contractor is the most experienced, uses better materials and
keeps to a timeline.
Consumers who need to purchase essential goods and services
need to take notice of what they are paying for these items and
should contact the department if prices seem to be much higher
than normal.
The Department of Consumer Protection can be reached by e-mail
at [email protected], or by phone at (914) 995-2155.

Health Tips for Staying WellSarah Neuman Center presents:
Staying FitFor most seniors, jogging, weight lifting and kickboxing may not be healthy
options. Try these simple, fun tasks to boost your fitness, but please remember
to first check with your doctor before starting a new exercise regimen.
Be Active
* Step off the bus two blocks before your usual stop and walk the
rest of the way.
* Take a Tai Chi or Yoga class. It’s easy on the joints, improves your
balance and makes you stronger.
* Doing everyday chores, like vacuuming, mopping, dusting and
shopping, count too.
* Walk a dog; play with a cat.
Be Social
* Join a local dance class or simply invite a friend over to “cut the rug.”
* Laugh more. It reduces stress, lowers blood pressure and decreases your
risk of heart disease.
Be Mindful
* Always try to maintain good posture, whether you’re sitting on the couch
or on a chair, to keep your back strong and less susceptible to injury.

Twins, triplets and preemies, oh my! Maria Fareri
Children’s Hospital at Westchester Medical Center - the
children’s hospital for the Hudson Valley and Fairfield
County – recently celebrated the 30th anniversary of its
Isaac and Naomi Kaplan Family Regional Neonatal Inten-
sive Care Unit (RNICU) with a special reunion with
RNICU graduates of all ages. Hundreds of RNICU gradu-
ates, their parents, friends and family members along with
past and current RNICU staff members attended.
Caring for the Most Fragile Patients
The fact that many of these graduates can return to the
hospital for such a special occasion is remarkable in itself.
The RNICU at Maria Fareri Children’s Hospital retains New
York State’s highest designation (Level IV) for regionalized
neonatal care and is the only unit in from New York City to
Albany, as well as Fairfiled County, Connecticut, with this
designation. As such, each day community hospitals across
the region transfer their most severe and complex cases to
the Maria Fareri Children’s Hospital’s RNICU for ad-
vanced care. The RNICU at Maria Fareri Children’s
Hospital cares for babies suffering from life-threatening
complications associated with extreme prematurity as well as
full-term babies with cardiac, neurological, pulmonary and
other birth defects.
“We care for the region’s tiniest and sickest newborns and
are among the top two hospitals in New York State in
terms of case severity,” explained Edmund LaGamma,
M.D., Chief, Newborn Medicine, Maria Fareri Children’s
Hospital at Westchester Medical Center. “Despite this case
severity, our outcomes are among the best in the nation.”
30 Years of Excellence
The Regional Neonatal Intensive Care Unit (RNICU)
was established at Westchester Medical Center in 1982
with 20 beds. The unit grew to become the regional re-
source for advanced neonatal intensive care and today, the
RNICU at Maria Fareri Children’s Hospital regularly cares
for over 50 newborns daily. More than 25 neonatologists,
experts in the care of critically ill newborns, work at the
RNICU or partner with community hospitals network on
advanced neonatal care.
Oh,Baby! RNICU Facts:
• Since opening, the RNICU at Westchester Medical Center has cared for over 20,000 newborns.• The smallest weight of newborns cared for at the RNICU can be less than 1 pound with some babies born before the 24th week of pregnancy (less than 6 months). • The RNICU draws from a catchment area of 23,000 births at its affiliate hospitals and admits a subset of about 700 newborns that are sick, small, surgical, chromosomal, or biochemical disorders of those newborns each year.• Clinicians of the RNICU are thought leaders, publishing over 25 peer-reviewed publications annually.
• Its academic program is the 4th largest fellowship training program in the country.

Although the holiday can be a time for great fun and entertainment amongst family and friends, unfortunately it can also be a
time of sadness for many who have lost loved ones during this time or for others who are reminiscing about their loved ones.
These special tips are bite- sized nuggets to consider whether your sadness is due to bereavement or due to physical separation
of your loved one beyond your control.
1. Plan pamper moments. Plan something special for yourself such as lunch out with a special friend or family
member. Other activity may be a visit to the hair or nail salon or any other activity that may help to add to a
meaningful day.
2. Listen to music that enlivens your spirit and strengthen positive emotions.
3. Steer your mind towards positive thoughts or experiences. This good habit will keep your immune system strong.
4. If you are un-employed, find an organization of interest and volunteer your time. Your self-giving attitude may
reward you with a job at the same organization should a vacancy becomes available and you are qualified for it.
5. Eat nutritiously and exercise as often as possible. This will increase your energy and ward off the fatigue of
boredom or sadness.
6. Establish a routine for starting and ending your day. Allow time for reflection in order to express appreciation
and gratitude especially for the small things that make a difference in your life.
7. Read. Read. Read. You may be surprised to see how this exercise can refresh your mind.
8. Accept the weather changes that come with this season. The Power of Nature is supreme and cannot be controlled
by human hands. Acceptance helps.
9. Share your joy with others. A smile goes a long way to can brighten someone’s day and usually leaves you
feeling better also!
10. Practice deep breathing and relaxation. This often recommended activity is still rarely taken. Try it and note
the benefits it yields, before long you will see, your sadness eventually passes away!
If you are experiencing deep sadness and unable to cope, please reach out to someone; a good neighbor, a friend,
religious organization, support group or your local helpline. Someone will be there to help you.

The early diagnosis of lung cancer, before symptoms
occur, provides the best chance for cure according to
Avraham Merav, MD, co-director of the Westchester Lung
Nodule Center at Phelps. Early lung cancer is most often
detected as a nodule on a CT scan. Since the increased use
of diagnostic CT scanning and the recent introduction of
CT screening for lung cancer in high-risk individuals, nod-
ules are being discovered ever more frequently. The ma-
jority of such nodules are benign, and it requires a great
deal of expertise to identify the malignant ones.
Dr. Merav and
Dr. Rocco Lafaro,
with more than 60
years of combined thoracic surgical
experience, joined to establish the Westchester Lung
Nodule Center at Phelps, providing comprehensive
evaluation for patients with recently diagnosed lung
nodules and offering low-dose CT scan screenings for
individuals who are at high risk for developing lung
cancer, particularly those with a history of smoking.
Each case is thoroughly reviewed by the Center’s
select team of specialists who are board certified
in Radiology, Pulmonology, Pathology, Oncology
and Thoracic Surgery. Every attempt is made to
determine if the nodule is benign or malignant with
the least invasive methods available. Quite often,
only observation is recommended. For those patients
who do require a surgical intervention, the Center
offers the benefit of an experienced team who perform
the majority of their procedures using minimally
invasive thoracoscopic (VATS) techniques.
Patients are seen within 48 hours of their call for
an appointment. Ongoing communication is maintained
with each patient’s primary care physician or other
referring physicians.
Phelps distinguished itself in 2009 by becoming the
first hospital in Westchester to report its thoracic surgery
cases to the National Database of The Society of Thoracic
Surgeons. Continued reporting assures our patients that
Phelps’ results are in line with the national experience.

Women treated for breast cancer at Hudson Valley
Hospital Center will have more choices thanks to
a $150,000 grant the Hospital is to receive from
New York State with the help of Senator Greg Ball.
The money will help to pay for equipment that
will allow the Ashikari Breast Center at Hudson
Valley Hospital Center to perform intra-operative
radiation on women undergoing breast cancer sur-
gery. The procedure will be available to patients
starting the second week in November.
“The Ashikari Breast Center at Hudson Valley
Hospital Center offers women the best cancer care
close to home,’’ said breast cancer surgeon Dr. An-
drew Ashikari recently at a press conference at the
Hospital. “Now women who could benefit from
intra-operative radiation don’t have to travel else-
where to get it. This is a great addition to the Hos-
pital and a victory for women with breast cancer.’’
Sen. Greg Ball said he was happy to advocate
for his community in Albany.
“The stresses associated with battling breast can-
cer are overwhelming and providing world class
services locally, without the additional stress of
travel and hardship of leaving the familiarity of
their community is exactly why I couldn’t be hap-
pier to deliver this $150,000 grant to the Hudson
Valley Hospital Center. I am proud to assist in
making the Cheryl R. Lindenbaum Cancer Center
the destination of choice for cancer patients in the
Hudson Valley,” said Senator Greg Ball. "It is a
great honor to be able to deliver this money for the
hard working doctors, nurses, staff and the entire
Hudson Valley Hospital Center community, espe-
cially as we embrace the struggles of our mothers,
sisters, aunts and other beloved ones during breast
cancer awareness month."
Dee Curtis, who was diagnosed with
breast cancer in June 2011, said she had no
idea what Intra-operative radiation was or
how the treatment could help to save her
breast before speaking with Dr. Andrew
Ashikari. Back then, Hudson Valley
Hospital Center’s Cancer Center had not
yet opened so Curtis had to travel to
another Hospital to have her surgery.
“I was going to have a mastectomy, but
Dr. Ashikari said with this treatment, he
could save my breast,’’ said Curtis who
lives in Yorktown with her husband. “
I also had 1 less week of radiation, which
was a good thing because if I had to go one
more week, I wouldn’t have been able to
stay in work. I was just that tired.’’continued on next page

Hudson Valley Hospital President John
Federspiel said the Hospital chose to use the
grant money and invest an additional
$413,000 to buy the intra-operative radiation
equipment because it is committed to mak-
ing the Hospital’s Cheryl R. Lindenbaum
Cancer Center the destination of choice for
cancer patients in the Hudson Valley.
“Cancer patients should not have to add
extra travel to their already hectic lives dur-
ing such a stressful time’’ said Federspiel.
“The philosophy of our Cancer Center is to
provide quality, comprehensive cancer care
close to home. It should be no different when
it comes to women who choose to have intra-
operative radiation.’’
Intra-operative radiation (IORT) allows se-
lected breast cancer patients undergoing
breast preservation surgery to receive one
dose of radiation while asleep during surgery,
compared with 6-1/2 weeks of radiation after
surgery. In higher risk patients, it is used to
boost the surgical cavity with radiation to re-
duce the post-op radiation to only 5 weeks.
Women who undergo IORT only and develop
a recurrence can then undergo a repeat
lumpectomy and whole breast radiation,
while those who get standard radiation and
develop a recurrence are recommended to
have a mastectomy. The Ashikari Breast Center
Healthcare News - Westchester December, 2012 Page 27
has participated in the largest international
trial of IORT and will be active in the Amer-
ican trial, which will begin soon. The IORT
procedure at HVHC will be overseen by
the Cancer Center’s Department of Radiation
Oncology under its Medical Director,
Dr. Chika Madu.
“Not everyone is a candidate for IORT, but
in many women undergoing a lumpectomy it
provides a less stressful option to traditional
whole breast radiation treatments,” said Dr.
Pond Kelemen of the Ashikari Breast Center.
The Ashikari Breast Center came to
Hudson Valley Hospital Center in July
2011 and their offices are located in the
Hospital’s Cheryl R. Lindenbaum Com-
prehensive Cancer Center, which opened in
November 2011.
Leaders in their field, The Ashikari team
has pioneered a breast reconstruction proce-
dure that has attracted women from all over
the world, most notably a nipple-sparing pro-
cedure that allows them to remove cancerous
or potentially cancerous breast tissue, mini-
mizing the trauma of a mastectomy. Many
women, including those who are cancer-free,
but have high-genetic risk and are seeking
prophylactic surgery, favor this approach.

Today many people are involved in “Medicaid Planning” with a certified elder law attorney. What does this kind of
planning mean and why do they do it? In order to become eligible for Extended Medicaid coverage, an individual
must not exceed their state’s minimum income & asset requirements1. This usually requires that a person transfer
assets out of their estate in order to protect those assets from expensive long-term care bills. Moreover, any gifts
or transfers have a 60 month look back period regarding eligibilty2.
Most people are familiar with the “Medicaid Spend-Down” scenario and how devastating this can be to most families
financially. Paying for the cost of long-term care of a loved one dollar-for-dollar is never a good option. Many families
are financially ruined because of the cost of extended care. It is essential that every person carry out their advanced
planning regarding long-term care services. Otherwise, these expenses can easily run into the hundreds of thousands of dollars.
So what medical bills can be counted toward a spend-down?
• Your own medical bills.
• Your spouse’s medical bills.
• Parent’s bills for their children’s spend down.
• Bills of a child living with you.
• Bills of a child who does not live with you, but whose medical bills you help pay for.
• Past unpaid medical bills (sometimes up to 6 years old) for yourself or any of the people named above.
• The part of any medical bill not covered by Medicare or private insurance.
• Medical expenses (including insurance premiums) paid for you by certain public programs, for example, the
Elderly Pharmaceutical Insurance Program (EPIC) and the Child Health Insurance Program (CHIP).
Despite knowing about this potentially enormous liability, many folks still do not purchase long-term care insurance because they see it as
too expensive. They just do not want to pay the premiums every year nor do they want to have to worry about if they can afford the policy if
those premiums increase sometime in the future. So instead, they decide to transfer the majority of their assets out of their estate to facilitate
eligibility for Medicaid benefits in five years.
Having said that, there still remains the danger of a claim during the transfer period. Often, attorneys recommend that the Medicaid Planning
involve the purchase of a temporary long-term care policy with a five year benefit period in order to “bridge” the liability of the 60 month look
back period as per the Deficit Reduction Omnibus Reconciliation Act 2005. Once the look back period has expired, the client can cancel their
coverage and avoid paying any future premiums. This strategy however still costs money, typically $30,000 to $50,0003.
The following planning takes this concept to the next level…
What if the client has the means to purchase a Single Premium Universal Life Insurance policy with a Long-Term Care Rider instead of the
traditional LTCI policy in order to “bridge” the gap? These contracts are often referred to as “Linked” policies because during the 60 month
look back period the client not only has adequate long-term care insurance, but also a guaranteed life insurance death benefit that is higher
than the original deposit.
And what if the policy can be cancelled for a 100% return of premium after the 60 month look back period with no surrender charges? Sounds
too good to be true? Well, two carriers currently offer linked products with guaranteed return or premium riders4. Thus, the client can buy the
insurance and keep it in force until such time as they no longer need it and their assets are fully protected. This strategy completes the entire
leveraged transaction at no cost to the insured.
EXAMPLE: John is a 68 year old male who purchases a single premium $100,000 Linked Universal Life Insurance policy with a
Long-Term Care Rider to insure his liability while he waits for his assets to become Medicaid ineligible in 60 months. The $100,000
single premium buys him $190,000 of life insurance and $398,000 of long-term care coverage from day one. Assuming there are
no claims, at the end of five years, he can surrender the contract and receive 100% of his premium back with no surrender charges.
The transaction is essentially a 60 month FREE loan with some very significant tax-free insurance benefits5.

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain andsuffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal andneuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves, arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch ifyou can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon forassistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletalpain, and patients can suffer as a result.
Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’soutstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompaniedby numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication, but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally intears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompressit with an operation. In desperation I called Dr. Donath instead.
My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fixthe herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinaldecompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was ableto comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series oftreatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he hasused the same wonderful techniques to help me overcome painful bouts of sciatica as well.
My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real andlasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare upsof low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for afew days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeuticmassage, and even painful steroid injections. Nothing worked.
When I moved to Westchester, I met Dr. Jonathan Donath. In just a couple of sessions with Dr. Donath, the pain completely disappeared. If it hadn't happened to me, I wouldn't have believed it. During my last flare up, I was expecting to be on my back for 2-3 days and inpain for 2 weeks. Instead, after one session with Dr. Donath, I was able to walk; two more sessions and I was fine. I could walk, benddown and I had almost no pain. Since that last back episode, Dr. Donath has helped me with my tendonitis in my right hand andwith my chronic knee problems. I recommend Dr. Donath to everyone with back, muscle or joint pain. Even if you have not had reliefwith other treatments and other Chiropractors, give Dr. Donath a chance. I did and he changed my life.
Eugene Dinkevich MD, Mount Vernon, NY
It’s my great pleasure to write about Westchester Spinal Decompression Center. I really don’t have words to express my feelings aboutthe care I got in last 3 months. I was almost handicapped, living with terrible pain all the time, crippling my day to day life. I am a medicaldoctor and work in a hospital with all sorts of amazing physicians, but after seeing a physical therapist, getting epidural steroids injections, many orthopedists and neurosurgeons, finally my destination lead me to see Dr Jonathan Donath. With great skepticism, I lied downfor spinal decompression therapy. It was unbelievable that after only two initial sessions, my radiating pain down my leg had almostdisappeared. Gradually, I started getting better, started doing exercises that they gave me midway through the treatment. Now, I amdoing all my daily activities without any pain.
I highly recommend Dr Jonathan and his colleague Dr Kavita for any problems related to the musculoskeletal system, you will definitelyfeel at home and will get better.
