Never go to a party hungry. This

will only cause you to eat more

throughout the night. Try to have

a small snack before attending so

you aren’t ravenous when you

While standing around a buffet

table, most people tend to take a

INTRODUCTION

Snoring and Sleep Apnea are common conditions in our society today. They arecaused by a narrowing or collapse of the airway during sleep. Snoring or sleep apneaappliances are devices that are worn in the mouth during sleep that are designed toreposition the lower jaw so that the airway is supported in such a way that air canflow more freely. This reduces or eliminates snoring and can treat your sleep apnea,which can lead to a more quiet, restful night’s sleep.

CAUSES OF SNORING

70-90 million people are affected by snoring. Snoring can occur when the airway becomes partially blocked during sleep. This may occur more frequently when:

� You sleep on your back� You are overweight� You have a large neck size� The tongue falls back into the throat� The tonsils and/or adenoids are large� You mouth breathe during sleep� You are overly tired� You eat or drink alcohol too late at night� You have a cold or are congested

WHAT IS SLEEP APNEA?

Sleep Apnea is estimated to affect 20 million people. Sleep Apnea occurs when the airway becomes completely blocked and air is not able to move through theairway. This is commonly called Obstructive Sleep Apnea because the airway is totally obstructed. This obstruction may occur anywhere from the area of the softpalate down to below the tongue. When the airway is obstructed, the patient willawaken many times during the night with a cough or gasping sound as the bodyattempts to restore normal breathing.

SYMPTOMS AND SIGNS

The most common symptoms and signs of Sleep Apnea:� Snoring� Non-refreshing sleep� Daytime fatigue or tiredness� History of falling asleep driving, watching TV, or reading� Awaken during sleep coughing, choking, or gasping� Morning headaches� Elevated blood pressure� Cardiovascular disease

It's a time to celebrate, decorate, shop for gifts, decorate the Christmas tree or light the menorah. Ah, the holidays. What a wonderful time of the year.

What a stressful time!The Holidays are one of the most stressfulevents of the year. The expense of buying gifts, the pressure of last minute shopping, and the

heightened expectations of family togetherness can all combine to undermine our best

intentions. Here are some suggestions can help you reduce your 'Christmas stress'.

Budgeting for the HolidaysFor many of us, the Holiday aftermath includes massive credit card

bills that can take months to clear. Christmas or Hanukkah doesn't have to

be a financial headache if you plan ahead. Stress reduction strategies include:

Work out a rough budget of expected costs as early as possible.

This includes ‘hidden’ expenses such as food bills and overseas

telephone charges.

Calculate how much disposable income you have between now

and the holidays. A certain percentage of this can be dedicated each

week (or fortnight or month) to covering your expected costs.

Don't be discouraged if the amount seems small. If you save $5, $10,

or $20 per week over a year, it can provide you with a hefty nest egg.

If your nest egg isn't enough to cover your estimated expenses,

consider recalculating your budget to a more realistic amount.

If you have trouble keeping your hands off your nest egg,

consider opening a 'Christmas Club' account with a bank or

credit union.

Shopping strategiesMake a list of all the gifts you wish to buy before you go shop-

ping. If you wait for inspiration to strike, you could be wandering

aimlessly around the shopping centre for hours.

Cross people off the list as you buy to avoid duplication

Buy a few extras, such as chocolates, just in case you forget

somebody or you have unexpected guests bearing gifts.

If possible, do your holiday shopping early – in the first week of

December or even in November. Some well-organized people do

their Christmas shopping gradually over the course of the year,

Some restaurants may be fully booked for months before Christ-

mas, so don't wait till the last minute.

RelationshipsStress, anxiety, and depression are common during the festive

season. If nothing else, reassure yourself that these feelings are

normal. Stress reduction strategies include:

Don't expect miracles. If you and certain family members bicker

all year long, you can be sure there'll be tension at gatherings.

Avoid known triggers. For example, if politics is a touchy subject

in your family, don't talk about it. If someone brings up the topic,

use distraction and quickly move on to something else to talk

about.

Use relaxation techniques, such as deep breathing or focusing on

your breath to cope with anxiety or tension.

Family members involved in after-lunch activities (such as

cricket on the back lawn) are less likely to get into arguments.

You Can Save Hopeless Teeth without ImplantsBy Edward Feinberg DMD

Although implants were availablein 1993, Ms. B. did not want them.She was not a particularly goodcandidate for implants becausethe low-hanging sinus cavity wouldnot allow the placement of im-plants long enough to support den-tistry without bone grafts to raisethe level of the sinus. She had anodd pattern of tooth loss, withmissing teeth only on one side.Implants should not be placedwhen there is a poor foundation orif a patient does not want to gothrough the expense, surgery andhealing time. Ms. B. chose to re-store her mouth with the precisionattachment case. Most practition-ers are not able to offer this optionfor restorative treatment--they aremore adept with implants and theyare biased in their beliefs that fixedbridgework is always a better op-tion than removable bridgework.Not true!

The Precision Attachment Case

When there are not enough teethfor fixed bridgework, the precisionattachment case is a great restora-tive option--equal to or better thanfixed bridgework with implants.The precision attachment casehas a much longer track recordthan implants. Very few practi-tioners know much about preci-sion attachment partial denturesas demonstrated in this article--even though the concept was in-vented in 1906. Most patientsare only offered the clasp type ofremovable partial denture withvisible hooks that grip or lockonto the remaining teeth. Claspsand locking mechanisms applydestructive forces to the teeththat support the removable andusually result in the loss of theseteeth. As a result, this type of re-movable is really transitory to afull denture.

This is simply amazing! Most partialdentures that utilize attachmentscontain components that requireconstant replacement.

Saving Hopeless Teeth

In 2000, Ms. B.'s upper left bridgesupporting the removable partialdenture snapped off at the gumline.Teeth that have had root canal orbecome non-vital also becomebrittle and prone to fracture. Ms. B.did not want to remake her bridge-work, since making new bridge-work to fit an existing precisionattachment case--a difficult chal-lenge when that demands preci-sion to tolerances of tenths ofmillimeters to be successful.

Most practitioners cannot evensave teeth that are fractured at thegumline without building the teethup in some way. Every dentalschool teaches techniques that re-quire tooth structure above thegum to retain bridgework. A greatdeal of time and effort is expendedon building the teeth up or makingposts for this purpose. This ap-proach often fails. When bridge-work fails, it is a sure bet that thepost will come out with it.

However, there is a precision tech-nique with copper bands that hasbeen used in crown and bridge-

The attachmentis so precise that any male out ofthe box will fit any female with per-fection. The precision attachmentpartial denture is not locked inplace. It cannot come out duringnormal function because the pathof insertion of the partial denture isdifferent from pull of the musclesand gravity. If a stress is applied tothe partial denture, it can moveslightly to dissipate the stress, but itcannot come out. The result ishealth and longevity of the teeththat support the partial denture andpreservation of the tissues under-neath. The precision partial den-ture is worn 24 hours a day and isonly removed for hygiene pur-poses. No one can tell that it is re-movable bridgework, as the teethare custom made to hide all evi-dence of the attachment.

The precision attachment partialdenture has been made at oursince 1950 and has a track recordof lasting longer than any other typeof case (implants included)—evenon the weakest teeth imaginable. Itis rare for the males on the partialdenture to be replaced—evenafter decades of continuous wear.

Since1950, our office has made thou-sands of restorations with thistechnique. All of them are worn fora trial period with Vaseline or rub-ber prior to permanent cementa-tion and they do not fall out! Theyalso have margins that are farbelow the gum and are not visible.Because they are designed like theMason Jar cover (the best deviceever invented for food preserva-tion), decay is a rarity—even in pa-tients who are highly susceptible todecay!

We were able to make two crownsfor Ms. B. that we attached to herexisting bridge with acrylic. Thisbridge has the housing for herprecision attachment case, soadding the crowns requires a highdegree of accuracy. Ms. B.'sbridge now fit like brand-new! Ithas served her well for the pasttwelve years--supporting herpartial denture under continuousforces, under water and in an en-vironment loaded with bacteria.There have been no changes inher X-Rays during all this time--anabsolute miracle!

Mitchell Cairo, M.D., Pediatric Oncology – Maria Fareri Children’s Hospital at Westchester Medical Center

Fresh out of medical school, Cairo took aninterest in researching treatments andcures for pediatric cancer.

The year was 1979, the cure rate for children with different types of cancer wasabout 20 percent: It’s now 80 percent.

Cairo has played an integral role over thelast three-plus decades in developing life-saving treatments for pediatric hematology and oncology patients. He is a pioneer in the field, a premier figure inthe pediatric cancer and blood disordersubspecialty, especially research.

Last year, Maria Fareri Children’s Hospitalhired Cairo as its new Chief of PediatricHematology, Oncology and Stem CellTransplantation. He runs a new Stem and Cellular Therapy and Engineering Laboratory on the grounds of WestchesterMedical Center that will give patientsacross the East Coast access to cutting-edge treatments for forms of blood cancerand other diseases.

Cairo came to Maria Fareri Children’s Hospital from New York-Presbyterian’sMorgan Stanley Children’s Hospital, wherehe was Director of Pediatric Blood andMarrow Transplantation. He has been theprincipal or co-principal investigator of a dozenclinical research studies and a contributorto nearly 300 peer-reviewed publications.

Karen Pechman, M.D., director of physical medicine and rehabilitation –White Plains Hospital Center and Burke Rehabilitation Hospital

What would this list be without a local girlmade good? Pechman graduated from theBronx High School of Science. From there,it was on to even greater things.

In 1980, at age 22, Pechman completed an accelerated medical program at BostonUniversity. Pechman returned home to New York, where she received a traininggrant to conduct research in immunology at NYU.

There, she worked near a rehabilitationcenter, watching doctors working with a variety of patients. She saw physicians help people rehab serious injuries, shewatched in awe as they helped patients get their lives back – and Pechman wasdeeply moved.

All these years later, Pechman helping patients, including many who need prosthetics. Pechman uses cutting-edgetechniques and products, including the“myoelectric” arm. This breakthroughhand’s open and close in response to electrical signals from muscles.

She uses other innovative techniques such as electromyogram (EMG), whichidentifies nerve damage in patients withspinal-cord injuries. It was invented by oneof Pechman’s mentors, Dr. Edward Delagi,of Albert Einstein College of Medicine

Wu has performed over 800 liver and kidney transplants overthe course of his career and was a member of the four mansurgical team that performed the world’s first multi-visceralorgan transplant.

Wu has made many valuable contributions to the field oftransplantation and cancer research and may be best knownfor pioneering cavaplasty, one of the most widely used surgicaltechniques in liver transplantation. Developed during histenure at the University of Iowa Hospitals and Clinics, thecavaplasty technique shortens operative time and minimizesthe opportunity for postsurgical complications.

With this technique Wu is credited with performing theyoungest living related pediatric liver transplant (19 days old)as well as transplanting the oldest recipient with the longestsurvival and the oldest donor with the longest survival. Wu has also pioneered new concepts involving early diagnosisand combined radiation therapy in specific types of livertransplantation—treatment regimens which have become the standard of care, prompting UNOS to change its livertransplant policies.

Wu has developed and led kidney and pancreas programs in deceased and living donor adult and pediatric liver transplantat the University of Iowa and the University of Arkansaswhere in 2004 he launched the State’s first and only livertransplant program which was ranked number one in the nation for best outcomes by Healthgrades Born and raised in Mainland China, he has devoted significant resources andtime to promoting transplantation in his native country. Building a systematic withdrawal plan around a sustainable withdrawal rate is a smart effort for making your money last, but it may not

know that running out of money is not a scenario you hope to experience. If only there were guarantees… The good news is: there are.

Get Guaranteed Life-long Income

A Guaranteed Lifetime Income Annuity2 is a financial product that does, in fact, guarantee life-long income. To illustrate, imagine that

same hypothetical million-dollar portfolio. From it, you could use $842,3343 to purchase a Guaranteed Lifetime Income Annuity, which

will provide the $50,000 per year of income you had planned to live on. But, this $50k is now guaranteed for as long as you live, regardless

of market performance. No more worrying about outliving your savings. Plus, you would still have $157,663 left in your portfolio to

access as you wish, making the Guaranteed Lifetime Income Annuity significantly more efficient than a simple withdrawal strategy.

In case of premature death, the Guaranteed Lifetime Income Annuity has a Cash Refund option, which returns to beneficiaries the difference

between the initial premium and whatever has been already paid out. If inflation is a concern, an Annual Increase Option4 can be added

Carbon Monoxide

Gasoline-powered generators release carbon

monoxide (CO) which can be deadly. If your

power goes out and you are using a gasoline-pow-

ered generator for energy, be sure that the genera-

tor is installed safely and far from your home.

Never use your generator, charcoal or gas grills,

gas lanterns or camping stoves indoors or in an en-

closed area, like a garage. Gas stoves can produce

carbon monoxide. Do not use your gas oven to heat

your home.

Chain Saw Safety

Before moving or cutting tree limbs, make sure

there are no wires tangled in the limbs.

If you have little or no experience using a chain

saw, hire a licensed professional to remove tree

limbs and downed trees safely from your property.

Wear eye/face protection, foot protection, hard hat,

ear protection, and chainsaw chaps.

Drinking Water

After a storm, water from public water supplies

and private wells may not be safe to drink. Water

that is dark, has an odor, or has floating pieces

should not be used. Listen to the news or your

water company to know if your water’s safe. Pri-

vate wells that are flooded or may be contaminated

should be disinfected before use.

Electrical Wires

DO NOT TOUCH downed electrical wires. Report

it to your local utility company, police or fire de-

partment. Do not turn power back on until a qual-

ified electrician has inspected the property and says

it is safe to do so. Ask him/her to check wiring, ap-

pliances, and mechanical system components like,

the furnace, water heater, and gas valves, to see if

they can be salvaged or will need to be replaced.

Fires

Prevent fires by using flashlights or battery pow-

ered lanterns, if possible. If you must use candles,

place them in safe holders, away from anything

that can burn and never leave unattended.

Flooding

Flooding can contaminate your well, making your

water unsafe. Information on what to do if your

well is contaminated.

Medications

If you have to evacuate or leave your home, be sure

to bring your medications and medical supplies

with you. You should bring enough medication and

medical supplies to last you for a few days. If you

have a question on how to store your medication,

including medication that requires refrigeration,

you should contact your pharmacist. Persons who

depend on medical supply deliveries and plan to

temporarily stay at another address – with a neigh-

bor, family member or at a shelter – should notify

their medical supply providers to have their sup-

plies delivered to the temporary location with no

lapse in their care.

Sewage safety

Severe rainstorms, floods, and even spring thaws

can put a strain on sanitary sewers and septic sys-

tems. Large volumes of storm water and ground

water entering sewage systems can flood them,

causing back-ups into basements and on to private

property. Blockages in sewer systems can also

cause back-ups. Your health may be impacted if a

sewage back-up occurs in your home. Sewage

back-ups can contaminate your private drinking

well water. It can also pollute surface water (lakes,

ponds, rivers, and streams, and reservoirs used for

drinking water).

Like so many other areas in the northeast, Westchester County was hit hard by

Hurricane Sandy. In some parts of Westchester, the damage was devastating,

and it will take several weeks to recover.

Even though Sandy has left, it's still important to remain safe during clean-up

In the aftermath of Hurricane Sandy, County Executive Robert P.

Astorino has advised homeowners and businesses needing repairs to

use caution when selecting a contractor to avoid scams and being gouged.

"Common sense is a consumer's greatest asset," said Astorino.

"Use it. People who have had their homes damaged are upset and very

vulnerable. Opportunistic individuals can and will take advantage of this."

John Gaccione, the acting director of the county's Consumer

Department, said that the county will enforce any laws against gouging.

"Taking advantage of consumers in need and raising prices without

any justification will be investigated by the department," he said.

Home improvement contractors working in Westchester must be

licensed. A licensed contractor will have the necessary contractor in-

surance. To find out if a contractor is licensed, check online at

www.westchestergov.com/consumer.

Here are other tips from the

county's Consumer Department:

If you haven't done so, call your

insurance company as soon as you

can. Many insurance companies have

existing relationships with home repair

contractors.

Watch out for contractors who are suddenly

in the area. Fly-by-night contractors will

take advantage of homeowners in need.

Don't get rushed into signing a contract

or agreeing to repairs.

Avoid contractors who demand payment in cash or want more

than one-third of the total payment before work has begun.

Do not agree to an oral contract. Get everything you can in writing.

Insist on a written contract that sets forth all the important details

of the job. That includes a detailed scope of work, a start date, a

completion date, cancellation clause, warranty information and a

schedule of payments.

Get at least three estimates. In reviewing the estimates, don't always

go with the lowest bidder. A higher bid may be worth it if the

contractor is the most experienced, uses better materials and

keeps to a timeline.

need to take notice of what they are paying for these items and

should contact the department if prices seem to be much higher

than normal.

Children’s Hospital at Westchester Medical Center - the

children’s hospital for the Hudson Valley and Fairfield

County – recently celebrated the 30th anniversary of its

Isaac and Naomi Kaplan Family Regional Neonatal Inten-

sive Care Unit (RNICU) with a special reunion with

RNICU graduates of all ages. Hundreds of RNICU gradu-

ates, their parents, friends and family members along with

past and current RNICU staff members attended.

Caring for the Most Fragile Patients

The fact that many of these graduates can return to the

hospital for such a special occasion is remarkable in itself.

The RNICU at Maria Fareri Children’s Hospital retains New

York State’s highest designation (Level IV) for regionalized

neonatal care and is the only unit in from New York City to

Albany, as well as Fairfiled County, Connecticut, with this

designation. As such, each day community hospitals across

the region transfer their most severe and complex cases to

the Maria Fareri Children’s Hospital’s RNICU for ad-

vanced care. The RNICU at Maria Fareri Children’s

Hospital cares for babies suffering from life-threatening

Although the holiday can be a time for great fun and entertainment amongst family and friends, unfortunately it can also be a

6. Establish a routine for starting and ending your day. Allow time for reflection in order to express appreciation

and gratitude especially for the small things that make a difference in your life.

7. Read. Read. Read. You may be surprised to see how this exercise can refresh your mind.

8. Accept the weather changes that come with this season. The Power of Nature is supreme and cannot be controlled

by human hands. Acceptance helps.

The early diagnosis of lung cancer, before symptoms

occur, provides the best chance for cure according to

Avraham Merav, MD, co-director of the Westchester Lung

Nodule Center at Phelps. Early lung cancer is most often

detected as a nodule on a CT scan. Since the increased use

of diagnostic CT scanning and the recent introduction of

CT screening for lung cancer in high-risk individuals, nod-

ules are being discovered ever more frequently. The ma-

jority of such nodules are benign, and it requires a great

deal of expertise to identify the malignant ones.

Women treated for breast cancer at Hudson Valley

Hospital Center will have more choices thanks to

a $150,000 grant the Hospital is to receive from

New York State with the help of Senator Greg Ball.

The money will help to pay for equipment that

will allow the Ashikari Breast Center at Hudson

Valley Hospital Center to perform intra-operative

radiation on women undergoing breast cancer sur-

gery. The procedure will be available to patients

starting the second week in November.

“The Ashikari Breast Center at Hudson Valley

Hospital Center offers women the best cancer care

close to home,’’ said breast cancer surgeon Dr. An-

drew Ashikari recently at a press conference at the

Hospital. “Now women who could benefit from

intra-operative radiation don’t have to travel else-

where to get it. This is a great addition to the Hos-

pital and a victory for women with breast cancer.’’

Sen. Greg Ball said he was happy to advocate

for his community in Albany.

“The stresses associated with battling breast can-

cer are overwhelming and providing world class

services locally, without the additional stress of

travel and hardship of leaving the familiarity of

their community is exactly why I couldn’t be hap-

pier to deliver this $150,000 grant to the Hudson

Valley Hospital Center. I am proud to assist in

making the Cheryl R. Lindenbaum Cancer Center

the destination of choice for cancer patients in the

Hudson Valley,” said Senator Greg Ball. "It is a

great honor to be able to deliver this money for the

hard working doctors, nurses, staff and the entire

Hudson Valley Hospital Center community, espe-

cially as we embrace the struggles of our mothers,

sisters, aunts and other beloved ones during breast

cancer awareness month."

Dee Curtis, who was diagnosed with

breast cancer in June 2011, said she had no

idea what Intra-operative radiation was or

how the treatment could help to save her

breast before speaking with Dr. Andrew

Ashikari. Back then, Hudson Valley

Hospital Center’s Cancer Center had not

yet opened so Curtis had to travel to

another Hospital to have her surgery.

“I was going to have a mastectomy, but

Dr. Ashikari said with this treatment, he

could save my breast,’’ said Curtis who

lives in Yorktown with her husband. “

I also had 1 less week of radiation, which

was a good thing because if I had to go one

more week, I wouldn’t have been able to

$413,000 to buy the intra-operative radiation

equipment because it is committed to mak-

ing the Hospital’s Cheryl R. Lindenbaum

Cancer Center the destination of choice for

cancer patients in the Hudson Valley.

“Cancer patients should not have to add

extra travel to their already hectic lives dur-

ing such a stressful time’’ said Federspiel.

“The philosophy of our Cancer Center is to

provide quality, comprehensive cancer care

close to home. It should be no different when

it comes to women who choose to have intra-

operative radiation.’’

Intra-operative radiation (IORT) allows se-

lected breast cancer patients undergoing

breast preservation surgery to receive one

dose of radiation while asleep during surgery,

compared with 6-1/2 weeks of radiation after

surgery. In higher risk patients, it is used to

boost the surgical cavity with radiation to re-

duce the post-op radiation to only 5 weeks.

Women who undergo IORT only and develop

a recurrence can then undergo a repeat

lumpectomy and whole breast radiation,

while those who get standard radiation and

develop a recurrence are recommended to

have a mastectomy. The Ashikari Breast Center

has participated in the largest international

trial of IORT and will be active in the Amer-

ican trial, which will begin soon. The IORT

procedure at HVHC will be overseen by

the Cancer Center’s Department of Radiation

Oncology under its Medical Director,

Dr. Chika Madu.

“Not everyone is a candidate for IORT, but

in many women undergoing a lumpectomy it

provides a less stressful option to traditional

whole breast radiation treatments,” said Dr.

Pond Kelemen of the Ashikari Breast Center.

The Ashikari Breast Center came to

Hudson Valley Hospital Center in July

2011 and their offices are located in the

Hospital’s Cheryl R. Lindenbaum Com-

prehensive Cancer Center, which opened in

November 2011.

Leaders in their field, The Ashikari team

has pioneered a breast reconstruction proce-

dure that has attracted women from all over

the world, most notably a nipple-sparing pro-

cedure that allows them to remove cancerous

or potentially cancerous breast tissue, mini-

mizing the trauma of a mastectomy. Many

women, including those who are cancer-free,

but have high-genetic risk and are seeking

prophylactic surgery, favor this approach.

Today many people are involved in “Medicaid Planning” with a certified elder law attorney. What does this kind of

planning mean and why do they do it? In order to become eligible for Extended Medicaid coverage, an individual

must not exceed their state’s minimum income & asset requirements1. This usually requires that a person transfer

assets out of their estate in order to protect those assets from expensive long-term care bills. Moreover, any gifts

or transfers have a 60 month look back period regarding eligibilty2.

Most people are familiar with the “Medicaid Spend-Down” scenario and how devastating this can be to most families

financially. Paying for the cost of long-term care of a loved one dollar-for-dollar is never a good option. Many families

are financially ruined because of the cost of extended care. It is essential that every person carry out their advanced

planning regarding long-term care services. Otherwise, these expenses can easily run into the hundreds of thousands of dollars.

So what medical bills can be counted toward a spend-down?

• Your own medical bills.

• Your spouse’s medical bills.

• Parent’s bills for their children’s spend down.

• Bills of a child living with you.

• Bills of a child who does not live with you, but whose medical bills you help pay for.

• Past unpaid medical bills (sometimes up to 6 years old) for yourself or any of the people named above.

• The part of any medical bill not covered by Medicare or private insurance.

• Medical expenses (including insurance premiums) paid for you by certain public programs, for example, the

Elderly Pharmaceutical Insurance Program (EPIC) and the Child Health Insurance Program (CHIP).

Despite knowing about this potentially enormous liability, many folks still do not purchase long-term care insurance because they see it as

too expensive. They just do not want to pay the premiums every year nor do they want to have to worry about if they can afford the policy if

those premiums increase sometime in the future. So instead, they decide to transfer the majority of their assets out of their estate to facilitate

eligibility for Medicaid benefits in five years.

Having said that, there still remains the danger of a claim during the transfer period. Often, attorneys recommend that the Medicaid Planning

involve the purchase of a temporary long-term care policy with a five year benefit period in order to “bridge” the liability of the 60 month look

back period as per the Deficit Reduction Omnibus Reconciliation Act 2005. Once the look back period has expired, the client can cancel their

coverage and avoid paying any future premiums. This strategy however still costs money, typically $30,000 to $50,0003.

The following planning takes this concept to the next level…

What if the client has the means to purchase a Single Premium Universal Life Insurance policy with a Long-Term Care Rider instead of the

traditional LTCI policy in order to “bridge” the gap? These contracts are often referred to as “Linked” policies because during the 60 month

look back period the client not only has adequate long-term care insurance, but also a guaranteed life insurance death benefit that is higher

than the original deposit.

And what if the policy can be cancelled for a 100% return of premium after the 60 month look back period with no surrender charges? Sounds

too good to be true? Well, two carriers currently offer linked products with guaranteed return or premium riders4. Thus, the client can buy the

insurance and keep it in force until such time as they no longer need it and their assets are fully protected. This strategy completes the entire

leveraged transaction at no cost to the insured.

EXAMPLE: John is a 68 year old male who purchases a single premium $100,000 Linked Universal Life Insurance policy with a

Long-Term Care Rider to insure his liability while he waits for his assets to become Medicaid ineligible in 60 months. The $100,000

single premium buys him $190,000 of life insurance and $398,000 of long-term care coverage from day one. Assuming there are

no claims, at the end of five years, he can surrender the contract and receive 100% of his premium back with no surrender charges.

The transaction is essentially a 60 month FREE loan with some very significant tax-free insurance benefits5.

As a board-certified internist who has been in practice for over 13 years, I spend much of my time attempting to alleviate the pain andsuffering of my patients. As a clinician, one of the biggest challenges I face is helping my patients deal with musculoskeletal andneuropathic pain syndromes—those all-too-common, yet frequently debilitating ailments caused by bad backs, pinched nerves,arthritis, and the like. Unfortunately, much of what I have to offer my patients boils down to the following: take a pain pill, stretch ifyou can, wait, and hope for the best. After a failed trial of “conservative” therapy a call might go out to the orthopedic surgeon forassistance; a truly satisfactory result is not common. The simple fact is that physicians are not well trained to deal with musculoskeletalpain, and patients can suffer as a result.

Thankfully, my views on this topic have been turned completely upside down since I havepersonally discovered Dr. Jonathan Donath’soutstanding practice. After beginning a vigorous exercise regimen I started to experience awful neck and upper back pain, accompaniedby numbness and tingling down my left arm and fingers. For weeks I tried to nurse the pain with non steroidal anti-inflammatory medication,but the pain persisted and got to the point that I was almost unable to work, sleep, or most importantly, play guitar. I wasliterally intears from the excruciating pain. A neurosurgeon diagnosed a herniated disc and pinched nerve in my neck and offered to decompressit with an operation. In desperation I called Dr. Donath instead.

My visit to Dr. Donath’s office was an outstanding experience. He spent about an hour with me and combined multiple modalities to fixthe herniated disc in my neck: deep massage, active release techniques, traditional manipulation, cold therapy, and finally spinaldecompression. After that first hour with Dr. Donath my pain had decreased from a 10/10 down to a manageable 4/10, and I was ableto comfortably move my head and neck. That very night I was able to play guitar again. Two days later I went back for another series oftreatments and the pain decreased even further. After about five days the pain was absolutely gone. Unbelievable! Since then he hasused the same wonderful techniques to help me overcome painful bouts of sciatica as well.

My own patients and family members have become “believers” in Dr. Donath’s abilities as well--- the results he achieves are real andlasting. He is my go to guy for the majority of musculoskeletal and neuropathic pain syndromes that I now realize are truly treatable.

I am a pediatrician and in my work, I have to bend down a lot and I’m constantly lifting kids. I usually have one to three bad flare upsof low back pain every year. When my back would "go out", I could barely walk to the bathroom and would have to lie down in bed for afew days. I have tried everything: doing nothing and "toughing it out", muscle relaxants, physical therapy, chiropractic, therapeuticmassage, and even painful steroid injections. Nothing worked.
