Best practices of infection prevention and control provide safety in the clinical environment and treatment protocols for patients and dental profes-sionals. Exposure to poor water quality can pose a health risk for people and conflicts with uni-versally accepted infection prevention protocols. Noted most recently, in 2011 was the fatal case of an 82-year-old otherwise healthy woman who de-veloped Legionnaire’s disease after a dental visit1. The goal of effective dental waterline treatment is to reduce the number of microorganisms present in the water, thereby helping to break the chain of infection.

Dental unit waterline contami-nation was first reported in 19632. Research has shown microbial counts can reach <200,000 CFU/mL within 5 days after installa-tion of new dental unit water-lines3 and contamination levels

of up to 1 million CFU/mL of dental unit water have been documented4,5. In 1995, the American Dental Association issued a statement encouraging improvement in the design of dental equipment to offer delivery of on the outgoing water quality lev-els used in non-surgical dental treatment with 200 CFU/mL or fewer6. Based on standards for potable drinking water by the Environmental Protection Agency, the American Public Health Association, and the American Water Works Association, the Centers for Disease Control and Prevention guide-lines (2003) state the number of bacteria in water used as a coolant/irrigant for nonsurgical dental procedures should be less than 500 CFU/mL7.

The following options are available to address the biofilm with its resident microorganisms and opti-mize dental unit water quality:

Point of use filters placed at the end of each wa-terline often have pores too large to effectively trap bacteria, as well as slowing the flow of water in the tubing which contributes to biofilm growth, and they provide additional surface area for microbial growth. There is an ongoing expense of filter re-placement every 7 to 10 days.

Chemical agents available commercially are de-signed to inactivate and remove biofilms or deter attachment of biofilm in new or cleaned systems. Daily compliance with tablets, along with monthly shocking, and quarterly monitoring are key to these technique sensitive protocols. Of concern, residue from undissolved tablets is a potential source of

costly repairs to handpieces.

One innovative waterline disinfection cartridge system, available for municipal or bottled water, of-fers a continual slow release of iodinated resin. As water flows through the cartridge it pulls elemental iodine from the resin into the water stream. From there the iodine interacts, killing bacteria in the water. The cartridge is effective for one-year after being installed making compliance effortless. This simple system is FDA and EPA cleared to provide water under 200 CFU/mL with absolutely no test-ing requirement. With no protein attached to the iodine, it poses no risk for allergies and any “un-used” iodine evaporates into the air. Unlike hazard-ous heavy metal based cartridges, the used iodine cartridge can be simply disposed of into the trash.

Effective dental unit waterline maintenance is a key component of an infection control program. Crite-ria for choosing a dental unit waterline treatment system includes ability to control microorganisms and biofilm at required standards, product and labor costs, safety to equipment and the environ-ment, and ultimately compliance.

6. RECYCLING • In our facilities' throughout the world,

we have programs to recycle aluminum,glass, computers, electronic equipment,printers, and toner cartridges.

– Team Schein Members are encouraged to bring in oldcellphones, un-needed cellphonechargers, as well as cellphone and electronic cables to be safely recycled.

– Separate waste containers have beenplaced at each desk in our buildingsto allow for the separation of trash and paper.

– Corrugated cardboard accumulatedfrom packing materials are collectedduring the day and evenings by ourhousekeeping staff and deposited into an on-site compactor.

– Our lamp recycling program reducesthe negative impact associated withthe disposal of mercury-containingfluorescent lamps.

– At distribution centers, we areespecially diligent about recycling allmaterials. Nearly 50,000 metric tons of materials were recycled at U.S.distribution centers.

7. eMSDS • In March 2013 Henry Schein unveiled

a new option on our website wherecustomers can retrieve their MSDS sheets and save them to their hard drive.

– They can search for MSDS by orderdate range, invoice number, ordernumber, item number ormanufacturer item number.

– A pilot group of 10,000 accounts areno longer receiving printed MSDS with their orders and instead arereceiving messages on their paperwork to go to their onlineaccount and retrieve their MSDS.

– As a result, we are saving over 15,000pieces of paper a week. We expect to have over 70,000 accountsconverted to eMSDS by year’s endand expect to suppress printing ofover 2 million pieces of paper a yearwhile giving our customers an easierand greener way to find and save thedocuments they need.
