Hello, everyone. May I have your attention for a moment please? Thank you.

The following information will help us to assist you in getting the utility assistance that you need.

At the desk, you received an envelope and application. Please complete the application, and place it in the envelope along with the following documentation:

Current Light BillCurrent Gas BillValid Identification (for every household member over 18)Social Security Cards (for all household members)Birth Certificates (for new household members)90 days proof of income.

INCOME

Now lets talk about the type of income documentation you will need to present to your Intake Specialist.

If you are employed and get paid every two weeks (bi-weekly), you will need to show 6-7 paystubs. If you get paid every week, you will need to show 12-13 paystubs. If you get paid twice a month (bi-monthly,) you will need to show 5-6 paystubs.

If you receive a utility check, you will need documentation of the amount.

If there is no income in the entire household, you will need to provide an IRS tax transcript.

If you receive financial support from another person, you will need letter of support from them describing how they are assisting you, with their address, phone number, and signature.

UTILITY BILLS

Ideally, all utilities should be in your name. However, if they are not, you must have the bill holders identification, Social Security Card, and income to complete the intake process.

If your gas is off, if your starting new service, or youre having it transferred into your name, you will need a pending account number.

Pending account numbers are only valid for 10 days.

If you need a pending account number, you can ask for information on obtaining a pending account number from the front desk.

If your lights are off, you will need a Letter 29 from the Illuminating Company. You can ask for information on obtaining a Letter 29 from the front desk.
