If you have been assigned to a Health Improvement Program (HIP), read the frequentlyasked questions below.A:

What is a Health Improvement Program (HIP)?Health Improvement Programs are coaching programs that help members to understandand manage their health conditions. Those participating in Health Improvement Program arenot required to meet a performance goal such as a reduction in blood pressure or weight orcholesterol levels.

How were participants selected to be assigned to a HIP?Participants were identified for the on-going participation path based on a combination offactors which may include biometric screening data and, for PPO participants, medical andpharmacy claim data.

• Participants who have 4 or 5 markers for Metabolic Syndrome.Metabolic Syndrome is the name for a group of risk factors that raises your risk for heartdisease and other health problems, such as diabetes and stroke. You can have any oneof these risk factors by itself, but they tend to occur together. Participants who have four orfive metabolic risk factors or markers will be included in this category. In accordance withthe National Institutes of Health guidelines, the risk factors or markers for MetabolicSyndrome include:

• High triglyceride level — 150 milligrams per deciliter (mg/dL) or higher of this type of fatfound in blood

• Reduced high-density lipoprotein (HDL) cholesterol — cholesterol less than 50 mg/dLfor women and less than 40 mg/dL for men of this “good” cholesterol

• Increased blood pressure — 130/85 millimeters of mercury (mm Hg) or higher• Elevated fasting blood sugar — 100 mg/dL (5.6 mmol/L) or higher• Large waist circumference — a waistline that measures at least 35 inches for women and

40 inches for men

• Participants who are identified as likely to need significant healthcare resources or haveworsening of an existing health condition during the coming year.Predictive modeling software was used to identify those participants in higher risk categories.Condition Management services can help these participants by coordinating healthcare withtheir doctors and educating the participant on managing his or her conditions. Some of theconditions requiring care management or disease management services are:

• Cardiac• COPD• Diabetes• Chronic Kidney Disease• Asthma• Multiple chronic conditions

How are participants notified to enroll in the assigned Health Improvement Program?If you are assigned to a Health Improvement Program, you received information regarding your(HIP) assignment in January 2020 from the Chicago Benefits Office. The Health ImprovementProgram (HIP) vendors, Telligen or TCOYH, will also communicate with you by mail beginning inJanuary, 2020 to tell you about the program, answer questions and complete your enrollment.

What is the deadline for initial enrollment in my assigned Health Improvement Program?Initial enrollment for all Health Improvement Programs offered by Telligen or TCOYH must becompleted by April 30, 2020.

Where can I go to determine if I have completed my assigned participation?The active participation requirement will differ by program. Following your initial enrollmentsession, individualized coaching sessions will be agreed upon and communicated to you foreach quarter by your health coach.You can call your coach anytime you have questions aboutyour participation.

The quarterly active participation deadline dates are:

• April 1, 2020 to June 30, 2020• July 1, 2020 to September 30, 2020• October 1, 2020 to December 31, 2020

METABOLIC SYNDROME COACHING PROGRAMThe Metabolic Syndrome Coaching Program is designed to assist you in bettering your health.Telligen is the vendor providing this telephonic coaching program. With the support and guidanceof a health coach, you will learn about your risk factors and how to make progress towardimproved biometric numbers and/or lowering metabolic risks. You can also discuss goals andstrategies to better your health, plus benefit from the expertise coaching provides.

CONDITION MANAGEMENT PROGRAMThe Condition Management Coaching Program is designed to assist you in bettering yourhealth. Telligen is the vendor providing this telephonic coaching program. With the support andguidance of a health coach, you will learn about your risk factors and how to make progresstoward self-management skills regarding your health conditions. You can also discuss goals andstrategies to better your health, plus benefit from the expertise coaching provides.

TAKING CONTROL OF YOUR HEALTH (TCOYH)The TCOYH program is designed to assist you in bettering your health. The TCOYH program isrun in collaboration with the Illinois Pharmacists Association (IPhA) and the Midwest BusinessGroup on Health (MBGH). This program offers private, face to face coaching sessions witha Pharmacist to teach you how to better manage and monitor your chronic conditions and todiscuss effective medication management. You can also discuss goals and strategies to betteryour health, plus benefit from the expertise coaching provides.

What if I choose to not participate in my assigned Health Improvement Program?This is a voluntary wellness program. However, if you choose to not participate in your assignedHIP, the City of Chicago employee will incur a $50 per non-participant increase in his/her monthlyhealth care contribution.

Ongoing participation is VOLUNTARY in 2020B:

Your Ongoing Participation Path is voluntary. This wellness program is designed to helpyou achieve whatever health and wellness goals you set. Following the completion of theRealAge test, you can work toward your health goals year-round by using the Chicago LivesHealthy wellness portal or mobile app. There you’ll find all kinds of expert health and wellnessinformation, everything from articles to videos, health guides, slideshows, quizzes and more.

WELLNESS PORTAL TOOLSCheck out some of these great tools on the Wellness Portal for living your healthiest, happiestand most productive life:

TIMELINE: Your timeline will reflect RealAge tips, which are personalizedrecommendations based on the outcome of your RealAge test. Additionally, you will see highlypersonalized content based on your health and well-being along with your motivation andinterests. The more you use your timeline, the more customized it will become.

GREEN DAY TRACKER: Track your core health factors such as steps, diet, fitness, stress,sleep and more. When 8 health factors are green on a given day, you earn a green day. Use thetracker to monitor your health and lower your RealAge. Sharecare utilizes several different typesof trackers to measure progress towards earning green days: stress, relationship, blood pressure,weight, steps, smoke, sleep, glucose, fitness, diet, cholesterol, alcohol and medications. Trackingoperates through a variety of input types based on member selection and the type of informationbeing tracked. PROFILE: You can securely store and access all your biometrics and health data inyour profile. PLEASE NOTE: You must check the box under HIPAA, if you wish to retain yourbiometric information if you leave the program.

How were participants notified that their ongoing participation is voluntary?If ongoing participation is voluntary for you for the 2020 wellness program year, this informationwas sent to you in the Chicago Lives Healthy wellness program announcement letter in January2020 sent by the Chicago Benefits Office. You can also call Sharecare customer service at1-866-556-7671 to confirm your ongoing participation status.

B) If not prompted to answer this question or if you wishto connect or disconnect automatic tracking, click theheart icon in the bottom toolbar of the Sharecare app toaccess Trackers. From here, click the gear icon in the toprighthand corner, then set automatic tracking preferences.(see STAH Visual 1b-1 & 1b-2)
