Section I: Account Holder’s Information1. Write your name as it appears on your statements.

2. Write your Stash retirement account number. Here’s how to find it:

• Open your app.

• Use the account switcher to switch to Stash Retire.

• Tap to Account tab in the bottom navigation.

• Tap the Bank account account menu item.

• Find your Stash Retire account number at the bottom of the page.

3. Write your social security number.

4. Write your date of birth in the following format: MM/DD/YYYY.

Section II: Beneficiary (or Former Spouse) information

1. Write “N/A” in the Name field.

2. Leave the rest of this section blank.

Section III: Type of Distribution

Check only ONE of the checkboxes based on your situation:

If you’re withdrawing within seven days of opening your account:

1. Check the box next to Revocation.

If you’re over 59½ years old:

1. Check the box next to Roth Distribution with Exception.

If you’re under 59½ years old:

1. Check the box next to Premature.

Section V: Tax Witholding

Fill this section out the same way you filled out your investor question by following the below instructions.

1. Open your app.

2. Use the account switcher to switch to Stash Retire.

3. Tap to Account tab in the bottom navigation.

4. Tap the Investor profile questions menu item.

5. Tap the question Do you want Stash to withhold taxes from your distributions?

6. Choose either YES or NO. (If you choose YES, we will withhold 10% of any distribution.)

7. Follow the steps below based on your choice.

If you chose NO:

1. Check the box next to Do NOT withhold in the Federal Income Tax Withholding subsection AND

2. Check the box next to Do NOT withhold in the State Income Tax Withholding subsection.

If you chose YES:

1. Check the box next to Withhold in the Federal Income Tax Withholding subsection AND

2. Check the box next to Withhold in the State Income Tax Withholding subsection.

Section IV: Distribution Method

Check only ONE of the checkboxes based on your situation.

If you’re just withdrawing some of your funds:

1. Check the box next to ONE-TIME DISTRIBUTION (A).

2. Write the amount you’d like to withdraw in the field labeled Payment in the amount of (1).

If you’re withdrawing all of your funds and closing your account:

1. Check the box next to TOTAL ACCOUNT DISTRIBUTION (C).

Section VI: Delivery Instructions

1. Check the box next to Special Payment Instructions.

2. Write “ACH on File” in the empty box below Special Payment Instructions.

3. Leave the rest of this section blank.

Note: If you do not have a bank account currently linked to Stash, please let us know when you submit this form, so we can fix this section.

Section VII: Signature

1. Sign your name on the line labeled (ROTH IRA HOLDER or Beneficiary).

2. Write the date of your signature on the corresponding field labeled (DATE).

Note: You do not need to get this form notarized.
