HOW TO OBTAIN YOUR MEDICAL CANNABIS CARD GATHER MATERIALS You will need the following: • Electronic copy of a valid US government-issued photo ID and proof of Maryland address • Electronic copy of a clear, recent photo • Last four digits of the patient’s Social Security Number • Please note: In lieu of a recent photo, Hospice patients may submit a letter from the Attending of Hospice Record (AOHR) provider on hospice letterhead REGISTER ONLINE • Visit the MMCC Registry website and click the “Create Account” link under the “Patients” heading • Complete the “Before You Begin” section to review and confirm that you have the required documents and information ready for upload • Complete the online application form, uploading the requisite files VERIFY EMAIL • After submitting your online application, you will receive a verification email • Click the link supplied in the verification email to verify your email address • You will receive another email after MMCC has reviewed your application, indicating it has been approved NEXT STEP Once approved by the MMCC, you can do the following: • Select a Caregiver if applicable by logging into the MMCC Patient Registry • Visit a registered provider to obtain a valid written certification • Purchase a Patient ID card-$50 (optional) www.HerbologyDispensary.com 1 2 3 4
