We know you will enjoy reviewing this year’s annual report and reflect on the many ways in

which Humboldt Senior Resource Center impacts the lives of older adults in Humboldt County. The many pictures throughout the report express feelings of joy, love, appreciation and self worth for participants, staff and volunteers.

This year the phrase “Full Continuum of Care” was used in many presentations to describe HSRC services. With the first full year of Redwood Coast PACE we were proud to be able to say this to our community. This phrase suggests that whether you are active and independent or needing more intense care management, social and medical services — HSRC has something to offer.

Providing services also means continually improving our buildings and equipment so we can meet the needs of participants. Through the generosity of community donors, new vans were added to our transportation fleet, as well as new equipment in our central kitchen. A major upgrade of our restroom facilities was started so that clients could be better served and will continue to be completed in this next year.

As Redwood Coast PACE grows, we are continually adding new staff to our team. Our staff is truly committed to our participants and they work extremely hard each day to improve participants’ lives. And these staff could not do their job if it were not for many dedicated community volunteers who give of themselves to teach classes, serve meals, work in the kitchen, make sack lunches for children in need, serve as a board member, deliver newspapers, and their giving goes on and on.

I also would like to highlight this year a partnership we have with UC Davis, School of Nursing, to work with nurse practitioner and physician assistant students in our PACE program. We are so excited to be able to promote a very successful model for gerontology health care in a rural part of our state with the hope to encourage skilled healthcare students to choose working in the field of gerontology.

Activities Program includes Senior Firewood, flu shots, Computer Lab, tax assistance, commodity food distribution, and others.

Classes promote healthy aging, and include exercise, languages, dancing, safe driving, and more.

The “Are You OK?” telephone reassurance program and Senior Home Repair provide peace of mind and assistance to seniors.

The Nutrition Program provided more than 65,000 nutritious meals to local seniors in fiscal year 2015-16.

More than 31,000 meals were served this year at Senior Dining Centers in Arcata, Eureka, and Fortuna.

Approximately 35,000 Home Delivered Meals were provided to homebound elders from McKinleyville to Fortuna.

Dining Centers offer “more than just a meal” ... they also provide opportunities for friendship and socialization, as well as fun!

Alzheimer’s Services provides consultations, resources and support – including support groups – for families and caregivers. Books and videos are available on loan from the extensive resource library, located on the HSRC campus in Eureka.

Adult Day Health is a therapeutic day care program for those age 18 and older with chronic conditions and disabilities.

In FY 2015-16, 90 participants were served by this program, which provides nursing, therapies, activities, social services, a hot meal, and transportation to and from the Center.

Fees are paid through Medi-Cal, Veterans Administration, some health insurance plans, and donations.

The Multipurpose Senior Services Program (MSSP) is a care management program for frail elders age 65 and older who are Medi-Cal eligible and who are at risk of placement in a skilled nursing facility due to frail health.

The goal of MSSP is to help arrange services that enable clients to remain in their own homes for as long as possible with a higher quality of life. In FY 2015-16, MSSP served 107 clients.

MSSP is funded by state and federal agencies. There is no cost to MSSP clients for this service.

Redwood Coast PACE – a Program of All-inclusive Care for the Elderly – helps qualified seniors with chronic conditions by providing comprehensive healthcare and supportive services.

The goal of PACE is to help participants stay as healthy and independent as possible in their homes and community, and help them delay or avoid placement in skilled nursing facilities.

As of June 30, 2016, a total of 113 participants had enrolled in Redwood Coast PACE, with a current census of 92. Our goal is to have 135 participants by September 2017.

PACE participants benefit not only from the medical and supportive care they receive, but also from socialization with others that they meet through the program at the Day Center.

Our loyal volunteers are key to the day-to-day programs at HSRC.

Volunteer opportunities include Home Delivered Meals drivers, nutrition assistants and hosts, entertainers, clerical support, class and support group leaders, and others.

Senior News, circulation 13,000, is published monthly and focuses on community news and information of interest to those age 50 and better.

Community advertisers, writers, the Senior News Advisory board and volunteer distributors give the Senior News staff members the support they need to sustain this community resource.
