letter froM the Chair and exeCutiVe direCtor

2007 marked the start of HVO’s third decade of service in training professional health care workers. When HVO wasfounded, the impetus was the increasing “brain drain” as some of the best and brightest left their country of birth andsought a more lucrative life in the western hemisphere. HVO felt it was crucial to train a workforce interested in andcapable of addressing basic health care issues in their own home country.

Today, the world’s health needs still encompass some of the scourges of the past (malaria, tuberculosis, polio) but,increasingly, the needs are similar to those faced here in the US. Chronic diseases are far more common, and there isa greater need for advanced medical training. Yet, such skills are frequently lacking or non-existent.

The World Health Organization has indicated that the most crucial need in public health is the limited health work-force available in developing countries. To that end, HVO has joined forces with other organizations as a memberof the Global Health Workforce Alliance. Their agenda is that members share a vision that “all people, everywhere,shall have access to a skilled, motivated and facilitated health worker within a robust health system.” By workingtogether to address this need, it is envisioned that different organizations will cooperate and build on each other’sstrengths and skills to build “coherent national and global leadership for health workforce solutions.”

HVO continued this year to expand its sponsor base and corporate partnerships to build its membership and reachmany new health specialists. As we move forward in this new decade of health challenges, we urge our members toconsider how they might share their skills. Through each volunteer contribution, another step is taken towardsimproving access to health care for everyone.

in the form of active education. The concept of HVO Anesthesia is truly noble and I highly recommend thisprogram in Tanzania to anyone who is interested in experiencing international medicine and travel.

2007 – aYear of GrowthThis was a year of growth and expansion for HVO in many different ways.HVO entered two new countries for the first time – Cameroon and Laos. Inanother first, we initiated a dental training program in conjunction with theTanzanian Red Cross in two refugee camps in western Tanzania. Letters ofunderstanding were signed and recruitment began on four new programs inwound management. Several members of the American Society for Hema-tology, a new HVO sponsor, conducted a site assessment in Uganda.

At year’s end, the American Society of Clinical Oncology became a sponsorof HVO, positioning HVO to start developing educational programs in can-cer treatment and management in developing countries where, according tothe World Health Organization (WHO), 70% of all cancer deaths occur.

Another exciting development was the roll-out of the HVO KnowNET. Thisis a web-based platform that facilitates the interaction and exchange betweenand among volunteers, program directors, colleagues at the training sites, andHVO staff. A library of volunteer-produced educational resources is beingcollected on the HVO KnowNET. This virtual community will allow forsmoother continuity among volunteers, so they can build on and reinforceinformation provided earlier.

a Year of transitionsAfter 15 years, the USAID-funded VietnamRehabilitation Project came to a close. Over that time, the country and itswork in support of people with disabilities made remarkable strides. This wasan exciting time to be in Vietnam as it once again became an active memberof the international community and its economy grew. HVO volunteers –physicians, nurses, physical therapists – were involved in multiple rehabilita-tion-related training activities over the years, in addition to assisting in the re-vision of teaching curricula, teacher training, as well as the development ofscopes of practice and standards of care. As part of the grant, HVO mentoredthe Disability Forum, a networking and advocacy group for people with dis-abilities. Over the last couple of years, with HVO’s guidance, the group wasestablished as a local non-government organization which operates under thename IDEA (Inclusive DEvelopment Action). While the RehabilitationProject has come to a close, HVO continues to work in Vietnam through thevolunteer connections with several different institutes.

Other transitions this past year have been noted with sadness. HVO lost fourcolleagues each of whom had a significant role in the organization. Dr. GarryHough was an early supporter of HVO and a generous donor who invested inour future through the gift of a charitable trust. Dr. Edward Blair developed

and ran an orthopaedic program in Malawi for more than 10 years beginningin 1985. Much of the orthopaedic care delivered in Malawi today is a directresult of Dr. Blair’s work. Dr. A. Lee Gordon, a hand surgeon from Colorado,was instrumental in gaining support and funding for a hand surgery trainingsection at HVO. Dr. Christopher McConnachie, an orthopaedic surgeon,moved to the Transkei, a South African homeland, in 1985 with the intentionof staying for five years. During his more than 20 years in South Africa, hewitnessed the enormous political and social changes that came with the endof apartheid while he was working to improve the quality of orthopaedic carein the Eastern Cape region. Each of these colleagues will be missed.

deVelopinG new partnershipsPartnerships are critical to HVO’s ability to maximize its impact. To thatend, HVO continued to be involved in the WHO Global Initiative for Emer-gency and Essential Surgical Care with several members actively involved inresearch and outreach. The provision of surgical care is often assumed to betoo expensive and too sophisticated to be part of a basic package of healthservices in a developing country. Data show, however, that death and perma-nent disability can be avoided through simple surgical interventions follow-ing road traffic accidents, violence or war, abdominal emergencies,complications of labor and delivery, congenital abnormalities, fractures,

burns or acute infections. According to the 2002 World Health Report, theseconditions cause the loss of approximately 11% of the total lost years ofhealthy life. Injuries alone kill more than 5 million people every year.

HVO also joined the Global Health Workforce Alliance in 2007. Started in2006, this alliance will “draw together and mobilize key stakeholders involvedin global health to help countries improve the way they plan for, educate andemploy health workers.” This is an exciting new initiative that raises the pro-file of this critical issue in global health.

the futureAs we look to the future, HVO is well placed to continue its programmaticexpansion. The global shortage of health care providers combined with thefact that there are so few resources available to invest in education and sup-port of health care providers in developing countries means that HVO pro-grams continue to be needed. Highly trained and committed volunteerhealth care professionals are eager for the opportunity to be involved in proj-ects that build local capacity and contribute to strengthening the health caredelivery systems in a sustainable manner.

The Golden Apple AwardIn 2006, the HVO Board of Directors created the Golden AppleAward as a way to recognize the extraordinary contributions ofvolunteers in support of educational activities at our program

sites. These volunteers have made an exceptional gift of their time and talentand truly personify the volunteer spirit.

The 2007 honorees are: Suzanne Brown, CRNA; Glen Crawford, MD; LenaDohlman, MD, MPH; Marvin Godner, MD; Martin Hobdell, BDS, PhD, MA;Christopher Johnson, MD; William Laney, MD; John McFadden, MD;Puthucode Rajamani, MS; Wingfield Rehmus, MD, MPH.

Representative of this level of achievement is Dr. Martin Hobdell. He washonored and acknowledged for his work in dental public health at the Facultyfor Odonto-Stomatology in Vietnam and the University of Health Sciences inCambodia, where he has initiated, established, and maintained graduate levelcertificate and masters training courses in dental public health. Dr. Hobdellcreated the curriculum of the 2-year courses along with local faculty at each site,monitored the progress of the students, and co-taught the courses with otherpublic health education volunteers. The programs are continuing with recentgraduates performing research and assisting with ongoing programs that posi-tively impact the public health of their countries.
