SAMPLE IEP

The following IEP was put together for an eight-year-old boy diagnosed with ADHD, specific learning challenges, especially reading and reading comprehension. Impulsive outbursts are also of concern. He is receiving

individual instruction to address deficits in reading and math, plus behavioral social skills training. He also receives instruction from an

occupational and speech and language therapist.
