transform Adult Social Care – Greater no. of people – Increased choice & control

• Salford City Council & NHS Salford are both currently rated excellent – Not achieved in isolation– Involvement of Service Users and Carers– Partnership working– Focus on Outcomes for Salford People– Use of Support Plans rather than Care Plans

to strive for excellence and value for money across all services delivered in Salford.

To raise the standards across the market place in terms of Care Planning, Nutrition and Administration of Medication in Care Homes registered in Salford.

• New innovative approach to improving performance across the market

• Multi disciplinary approach & involvement• Joint working – together we have a common goal of

delivering the best Outcomes for Salford people• Procurement role is to use the commercial drivers of

the business relationship to deliver the strategic aims of CH&SC and Salford City Council

Engagement and Delivery • Initiative is a starting point to review and improve the

services offered, specifically in terms of Care Planning, Nutrition & Medication

• We all know that performance improvements are about ownership, leadership and management

• Engagement is the key to understanding & agreeing common aims & strategic objectives

• Almost 90% representation of those care homes invited

• 51 invited guests & 17 representatives from SCC & Salford PCT

• Demonstrates commitment, enthusiasm & determination to excel

• Providers recognised the initiative provided real business opportunities

When we have succeeded • All Care Homes Registered in Salford will be meeting

or exceeding the Key National Minimum Standards ratings for Care Planning, Nutrition and Medication

• These improvements will not be achieved in isolation they will have positive benefits across the range of performance standards you are measured against

• Continue to strive to have all Care Homes in Salford classified as 3* Excellent rated providers by CQC

Key Messages• Several years ago the market was relatively immature,

customers were constrained by capacity. Increasingly we have capacity, flexibility and choice.

• Service Users & their advocates will be increasingly demanding

• Exciting time to be involved, every provider large & small has a part to play

• Going forward we are very clear choice will be on the basis of quality and value for money

• Programme is all about results which you need to deliver
