The victim was trying to prepare arrangement to fill up wheat waste in the tractor trolley with iron pipe. The tractor trolley was parked below the line of 11kv JGY Kansa feeder. As he lifted the iron pipe (

about 12 feet) ,it was touched to the live conductor of 11kv Line, he got electric shock. He was admitted to civil hospital patan but he died.( The distance beetween ground to 11 kv line is 16.5 feet .)

fuse wire by bamboo of (4 mtr and 2 mtr Joint) for making power supply "ON". While making connection with bamboo, Do fuse wire comes in induction zone of near by phase,there was a big

11 kv Vagharol-JGY feeder & 11 kv Pachakwada-AG feeder are going parallel at village kakoshi after emanating from 66 kv Kakoshi S/S . Maintenance of 11 KV Pachakwada-AG feeder was arranged by SDO and program was given to seven linestaff in supervision of shri K.N. Damor( L.I .).L/C of 11 kv Pachakwada-AG feeder was taken by Shri K.N.Damor (L.I.) at 9:30 a.m. & work was started .Victim Shri M.N. Karen (Helper)was climbed on 11 kv Vagharol-JGY feeder instead of 11 kv Pachakwada-

AG feeder at Location no-10 as wrong feeder name was written on that pole as 11 kv Pachakwada-AG feeder. So victim was climbed on wrong feeder due to wrong marking of feeder and he got

electric shock.He was shifted to JANATA hospital ,Patan for medical treatment but the victim was Died.

feeder passing just between his land was reduced to 12' to 13' . While lifting 13' to 14' long iron angle during working in land by victim Shree H.B.Mena, it accidentally touched to the live conductor of 11 kv Shilaj AG Dom feeder so he got electrocuted & met with FH accident Note-R.P.Patel had applied

broke down. At that time on the road two buffaloes of Shri Laxmanji Galabji Thakor were passing near the house of Mohanbhai Keshaji and came in contact with live LT wire and got electrocuted.

A Celling Fan found faulty in house of Chuhan Dhulaji Somaji , and A lekage current from the fan flows from fan to angle , and angle to steel shade of the house. A leakage current from the steel

shade of Chauhan Dhulaji Somaji' house flows to the steel shade of Chauhan Udaji Jivaji's gaman which is next to the chauhan dhulaji somaji's house, below which a buffalo of the owner is there. In the

gaman steel shade is also vertically put such that vertical steel shade come in the contact with the steel shade fixed above the gaman. so the leakage current was also flows to the vertical steel shade. Suddenly a buffalo in the gaman come in contact with the vertical steel shade and got electrocution.

One stay wire is provided on the LT pole and 5 ltg and street light connection are given from the said pole. out of 5 ltg. connction ,One of the service wire of shri Vaghari Dhanjibhai Sakarabhai have the damaged insulation on service wire and open part of the service touched to the GI wire of the said service . When the cow was passing betn. pole & the stay ,the stay wire was vibrated & touched to the GI Wire of demage service, so the electric current was passed in stay wire between the portion

from stay insulator to ground level and when the cow came in contact with stay wire and got electroculated .

Two parallel HT & LT line is existing. One house is constructed below the LT line. So the same house owner has proposed for LT line shifting. So the LT line conductor is opened from the pole and then LT

line supply is given by 6 mm2 P.V.C. cable with G.I wire on L.T line pole on one side. The 6 mm2 P.V.C. cable is supported on H.T pole on other side. Meanwhile due to over load , the G.I wire is short circuited & came in contact with LT PVC cable phase wire.Hence there was leakage of current in G.I

The Arial Bunch cable is provided on LT pole to prevent theft. At the time of site visit, the insulation of AB cable was found damaged i.e.phase and neutral wire was opened un authorisely. An extra PVC 2.5

mm2 cable was not connected with LT line AB cable at the time of site visit but it is assumed that it was connected with LT line AB cable and removed after occurance of accident. This 2.5 mm2 PVC

cable was digged under land and at some places hide in grass. While cutting the grass by victim, PVC cable got damaged and she came in contact with live phase wire and got electric shock. This cable was laid by house holder for theft of power hence UGVCL is not solely responsible for this electrical

accident.

Pakka Makan under Sardar Avas Yojana in 2007-2008. 11 kv line is passing over 3 to 5 feet on left side of the house & the height of house is approximately 10 feet. 11 KV line hight is approximately

13to 15feet from ground . Shree Mangalbhai Shankarbhai Senma climbed on the tarace of the house of neighborhood Shri A.M.Senma to collect the wooden pieces which was stacked on terrace of the house . By throwing the wooden pieces from Terrace to Ground & By mistake he came in contact of Live conductor & he got Electric shock & thus Non Fatal Accident occurred. Tripping came on 11 kv Karbatiya JGYfeeder at the time of accident. UGVCL has not given the lighting connection in this

house due to illegal construction of house under 11 kv line.

In One fuse of main switch direct wire is inserted , While victims were taking bath using submersible pump. Due to sudden power cut , one of victim had checked their distribution board and found loose connection of looping arrangement. He tried to solve and get shocked. Another victim while trying to help him , he also got shock. After emergency medical treatment in Gandhinagar civil hospital,they both are declared dead.

S/Dn office was not informed regarding this incidence, They were informed by the police station with asking opinion whether the accident can be occurred or not?.No any evidence was found on dtd 06.09.11,regarding electrical accident at the time of Site Punchnama.As per Information provided by shri Mansukhbhai Budhabhai Rajput, the said victim shri Monaji Pakaji Vaniya was driver of the Hyva DUMPER.Due to negligency of the victim, he had put the dumper on auto hydraulic mode without verifying any site situation and started to clean the back side of the dumper. Meanwhile Dumper moved up and might have been touched to the overhead HT line(Ambli feeder) and thus the accident might have been taken place.

While replacing of faulty transformer of 11 KV Khodam AG feeder at village Phinchod victim climbed on transformer centre to hang the chain pulley block and his right hand touched to Y phase and spark occurs.Before climbing on transformer centre Prajapati S.M.village helper of Phinchod village has AB switch cut off to dead the line of that section, Victim has thrown ring wire to check this feeder is live or dead but at that time no any spark was seen then he has touched an earthig rod to R,Y and B phase for earthing purpose then climbed on transformer centre to hang chain pulley block and touched to Y phase and got shocked and fallen down. The leakage current reason is under investigation & it will be resubmitted after division report.


On dtd 1.10.2011 feeder maintenance carried out and restringing of loose conductor done for proper clearance. In line staff meeting awarness is given to avoid accident to line staff.

During fault attending work of 11kv Anganwada JGY feeder , which is isolated at location 34/3 & 34/3/40/41.At the same time on pole no-34/3/42 the pin insulator was found doubtful like puncture. The victim was climb on the same pole for the inspection of pin insulator & victim got electric shock & suddenly fallown on ground.On details investigation it is came to the knowledge that one other 11kv Jampur feeder was passing near 11kv Anganwada JGY feeder.The AG connection of Patel Anadabhai Ramabhai was given from AG T/C of 11kv Jampur feeder. Also the lighting connection of same party was given from 11kv Anganwada JGY feeder, which is so near from AG connection.Due to power failure of 11kv Anganwada JGY feeder party has got power by connecting private wire with AG supply & due to this return power was came on 11kv Anganwada JGY feeder.

As per massage from outsider on dt:- 30.11.11 jointly inspected the site by Gambhoi s/dn & Bhiloda-1 s/dn. A non fatal accident occurred to outsider human namely Chauhan Bhavanaben Balaji at village: Motipura on dtd: 29.11.11. However, the actual reason yet to be pointed out. During site visit there was no any person came forwarded for eye witness.But, as per joint site investigation report of Gambhoi sdn & Bhiloda-1 sdn at the farm of shri. Chauhan Bhuptaji Galaji, one 11kv pole exists. There is a spark observed on top conductor of that pole near to top pin insulator and also burnt skin observed on earthing wire below V cross arm. It concluded that, there might be try to attempt of suicide by victim unauthorizely through climbing on that pole. Also detail investigation report of Police is awaited till today.

On Date 29/01/2012 ( Sunday ),The Delmal village helper was going to attend the complaint of Ag connection in R/O Shri Rabari Virambhai Gandabhai ( Conn.No 21805/00364/0 ) of village Delmal ( Preparing new DO jumpers ) Which is on 11 KV Limbja feeder & at the time of accident the single

phase power supply was ON . He went the site of Ag connection & dismentalled all the three DOs of transformer centre & for disconnect the line he sent the outsider man to cut off the line by

dismentalling the DOs of this 11 KV tap line which is about 10 poles away from this trans centre & with out conformation that the DOs are dismentalled or not He prepared all the three out going jumpers complately with binnding & when he prepared to repaired the incomming DO jumpers , He got the

electrical shock & fell down from the Transformer DO angle & got the injuries on his back side




Inform to linestaff to avoid this type of mistake in future & use safety equipments while working on line & line must be done dead by earthing on both the side

As per press note on 24-02-2012 about fatal accident to shri Thakor Babuji Manaji, this office has visited site on 24-02-2012 at 10:30 am and taken statement of victim's brother shri Thakor Kalaji Manaji. Victim had no any electrical connection given by U.G.V.C.L. but he was using power from

residencial connection of Shri Thakor Dhanaji Manaji by yellow colour's private flexible 1/18 wires of about 25 meter. As per Thakor kalaji Manaji's statement, "Victim and his wife had quarrel with

neighbour on 23-02-2012. connected with male plug connector. Single phase motor have female plug connector. while the single phase motor was on and victim was removing the male and female connector to switch off the motor

directly and at that time victim touched the live male contact of the wire and got electric shock and died.
