THIS IS DYLAN AND HE IS IN THE 5TH GRADE.

Dylan has a tough time reading. He has difficulty with multi-syllable words and words containing affixes. He has no self-correct skills and reads fairly rapid, skipping over words he does not know. He is reading at Level Q and has difficulty understanding the main idea of what he reads. Because of this, he misses out on key pieces of the plot and can only recall recent passages and pages of what he is reading. He tries to read some of the more complex fantasy books, but has trouble with the comprehension.

Individualized Reading Plan for Dylan

❑This presentation will focus on each of the areas and activities where Dylan and his teacher need to work to build good reading and word decoding skills:

Independent Literacy Work Station Activity –Wonkadoodle Story and Multiple-choice Questions

▪ Allows for practice with affixes and multi-syllabic words, including nonsense words.

▪ Allows practice for recalling, sequence, and details with multiple-choice prompts.

prefixes, and suffixes to create larger words and illustrate one word they created.

Affix Word Journal/ Graphic Organizer

▪ The student will be given a Word Journal. The journal will coincide with the word wall. The student will

use the graphic organizer, to list words with the indicated prefix, suffix, or root. The student will write

words with the indicated affix and then will draw a small picture in the box at the bottom of each

column. This will indicate to the student the different parts and meaning of words. This journal will

consist of pages for each letter of the alphabet and will address the more common affixes for 5 th graders.

After the student completes the affix exercise, he should post 3 words that were constructed using the

affixes to the word wall.

Graphic Organizer for Word WorkIndependent Literacy Work Station Activity

Objective:

Learn how affixes and root words affect word meaning.

Develop an awareness of how to decipher word meaning by analyzing the affixes and root words, and looking at context.

Understand how this knowledge will help them to improve reading comprehension and spelling.

There will be a collection of words with prefixes,

suffixes and roots. The students will identify each

piece of the word and break it down to find the

meaning of each piece of the word. The words will

be multi-syllabic.

Main Idea Task Card Objective

▪ The main idea task cards are a collaborative activity for students that struggle with obtaining the main point of what they are reading.

▪ There are two sets for each card.

▪ The cards have a short informational text. They will identify the main idea in the paragraph and discuss with a partner or partners. Each reading has 2 sets. Set 2 being more difficult with more information.

● The word sort is a collaborative activity for students who have difficulty identifying and correctly reading multisyllabic words with affixes.

● Students will be given three sets of cards. Each card will have either, prefixes, suffixes, and root words.

● Students will then pair the cards, in order to create new words.

Suggested Independent Reading Texts

The Suggested Independent Reading Texts were chosen based on reading Level Q.

The main idea in these books will be made obvious to Dylan after the teacher introduces the text, and provides some introduction with observing the cover and title. However, Dylan will still be responsible for identifying the key features of the main idea and the underlying meanings he will discover through inferring.

Each book will be introduced with a short introduction by the teacher. The introduction will help Dylan understand what he is about to read and why he is about to read it.
