Overview of Program and Contract Intent
BCC PurposeProvide enrollment, screening and diagnostic services to
low-income, underinsured or uninsured for individuals who:
• Are ages 40 or over
(May be less than 40 and symptomatic for breast disease);
• Earn less than the set income guidelines
(250% federal poverty level); or
• Are not be enrolled in Medicare Part B.
Provide patient navigation services to individuals who:
• Are ages 40 or over (or under 40 if symptomatic);
• May be covered by Medicare Part B;
• Earn less than the set income guidelines; and
• Are insured.
FY18 Proposed BCC Screening
Goal: 2,516 Women
• Federal funds - 2,000 screening slots
• State funds - 237 screening slots
• Komen funds - 279 screening slots
None of these funding streams have yet been secured for the contract period.

Excerpts from the CFY – BCC Application Guidance:
1.01 Purpose
The purpose of this Application Guidance is to provide eligible applicants
information on completion of the application for the FY18 Care for
Yourself (CFY) – Breast and Cervical Cancer Screening Program. Services
covered by this application include financial support to local boards of
health to assure the provision of breast and cervical cancer screening
and diagnostic services for eligible persons.
1.02 Project and Contract Period
Funding during the defined project period is dependent on approval of the
application, contractor performance during the fiscal year, compliance
with general and special conditions of the contract, availability of project
funds, program modifications, or any other grounds determined by IDPH
to be in IDPH’s best interest.
The contract period will be a one-year term - June 30, 2017 to June 29, 2018.
Application Guidance
Local Boards of Health are, and will be, the only
entity eligible to apply for and serve as the
contractor for the CFY - BCC contract.
A Local Public Health Agency may not apply in
response to the posting.

FY18 BCC Performance Measure
CRITERIA: The Contractor shall conduct 40% of its allotted
screening numbers by December 31, 2017.
MEASURE: At least 40% of the Contractor’s screenings are
completed on unduplicated participants by December 31, 2017,
evidenced by data submitted no later than January 31, 2018;
the Contractor will receive a $500.00 incentive payment.
Evidence of completion will be verified by the Department from
the monthly Vouching Reports. Payment of incentives will be
made with the June 2018 reimbursement.

KOMEN slots are used for 75/25 split
All Komen awarded slots are included in the total
number of screening slots allotted in the contract.
Some contracts have Komen screening slots identified
for certain counties.
Special outreach funds have been allocated to
those service areas
Calls will be scheduled to discuss this requirement.
Payments
The Contractor shall complete and submit:• a monthly claim in the grant site located in
IowaGrants within 30 days of the month of
expenditures; and
• monthly claim support documentation for outreach
activities completed and costs associated with the
activities identified.
Warrants will be made payable to the Contractor and
mailed to the Project Director listed on the Contract
Face Sheet.
Warrants may be sent to an alternative address if
requested in writing from the Contractor.

notice of award is received from the CDC and
budgets have been revised.
