Sutton Adult Social Services are responsible for a wide range of social care services for people who need help and support to live independently in their own homes. This leaflet provides information about how you can access these services.
I think I need help. What should I do?If you think you need care at home you should talk to The London Borough of Sutton through First Contact, the number for them is listed at the back of this leaflet.
We will then work with you to find out the best ways of helping you and your carers. An assessment is the way we find out what your needs are. We will involve you as fully as possible in any assessment. Carers are also entitled to their own assessment, you can find out more about these in the leaflet ‘How to get help if you are looking after someone’.
Who can Sutton help?Adult Social Care Services are available to:
Residents of the London Borough of Sutton aged over 18 who need help to look after themselves. This can be due to physical or mental illness, any type of disability or sensory impairment such as being deaf, blind or hard of hearing
Carers who live in the London Borough of Sutton assessed as needing support to help them to look after another adult or dependent child
Even if you are not eligible for health, housing or social care services, staff will be able to give you advice and information about other services that can help.

How will Adult Social Services respond to my needs?

Your assessment will be carried out by a care manager. They will look at your needs and your carer’s to see what kind of help and support we can offer.
When you are assessed we will look at:Your ability to look after yourself in your home
Your emotional well-being and that of your carer
Your personal safety and that of your carer
Your cultural and religious needs
The care manager, in consultation with you and your carer, will make a decision about what social care and support services are available to help you live independently in your own home. An agreed care plan will then be produced and you will be given a copy of this.
The care manager can arrange services based on:
1 Your assessed needs; People who are eligible for social care and support services are also entitled to a Direct Payment. Direct Payments are cash payments made by Adult Social Services & Housing (ASSH) directly to people to arrange and buy their own care. This enables them to have more choice, control and independence in meeting their care and support needs.
2 The Council’s eligibility criteria for receiving services. This is in line with the national framework for adult social care called Fair Access to Care Services which ensures that everybody is judged by the same set of standards. What services can help you with care and support? Sutton Adult Social Services can provide a range of general or specialised services. They can be provided directly by the Council or by voluntary and private organisations that are funded by the Council. These include:
Accommodation Services including shared lives placements (living with a family who provide care and, or support), Supported Housing and Supported Living.
Nursing Care is available if you are unable to remain at home due to needling total care by nursing staff
Home care services that can help you with your personal care and basic everyday tasks in your own home
START – the Short Term Assessment and Reablement Team. They are homecare workers who are highly trained in enabling people to regain skills and independence.
Day services and resource centres where you can meet new people, receive care, obtain health advice or take up new learning opportunities
Respite Care to give carers a break for a few hours or longer
The Occupational Therapy team who can advise on how you can maintain your independence at home and work
Meal on Wheels who can deliver a hot meal to your home
Safecall, a community alarm response service providing help and advice 24 hours a day and a Mobile Response Service where someone will come to your home in an emergency
The Telecare Service that provides a range of monitoring sensors to help you live safely and securely at home
Extra Care Sheltered Housing providing accommodation with care and support which can help you to stay independent
Residential Care is available if it becomes unsafe for you to remain at home on your own. We will do all we can to enable you to stay in your home
Community Support Services for people with learning disabilities to enable them to develop their social living skills, to improve or maintain their health and to lead independent fulfilling lives as members of their community
Personal Assistants to support you to access employment, education, health services and leisure opportunities

Will I have to pay? You may be charged for your social care but only after an individual assessment of your ability to pay; this will be arranged before your service begins. The assessment is based on guidelines that all councils must use when charging for care services. Your charge will be a contribution to the care, not the full cost. For more details see the ‘How we Charge for Social Care Services’ leaflet.
