Important Information:
The Intelex injury reporting software is only available on desktop/laptop units connected to the Mount Royal University network servers (wired in connection).
It cannot be accessed via cellular devices or other WiFiconnection.
Intelex functions best via the Google Chrome internet browser.

Incident Reporting System - Intelex
Intelex is Mount Royal University’s incident / accident online reporting system.

All incidents should be reported into Intelex, by the Supervisor, within 24 hours of occurrence.
What is Intelex? Reporting timeline:
**For all life threatening emergencies, please dial (9)-9-1-1 , then Security.
**For all non-life threatening emergencies, please contact Security (5900) and call 911.
What to report into Intelex?
All incidents / accidents occurring on MRU property are required to be reported involving:
▪ Students
▪ Faculty
▪ Staff
▪ Contractors
▪ Volunteers
▪ Visitors
**Only MRU employees have access to the injury reporting
software. An employee must submit an injury report on
behalf of students, contractors, volunteers and visitors.
The witness should be noted in Intelex.

Please report any injuries and occupational illnesses
sustained during the course of your work to your
Supervisor or Manager and into Intelex within 48 hours of
occurrence.

Step 1 – Log into MyMRU, click the “Employee” tab & within the HR section click “Report an Accident”

How to report into Intelex:
For first time login, use
the forgot password
button.
Your Intelex username is
the same as your MRU
username and follow the
“forgot password”
instructions.
You will receive an email
to create a password.
Step 2: Log into Intelex

Step 3: Once you are logged into Intelex, open the
‘Report an Injury’ form

How to report into Intelex:
Follow these instructions for proper
completion and submission of the
incident report.
Step 4: Once you have accessed the ‘Report Injury’ form, fill in the form with
the injured persons details and any related injury information.
‘Log Out’ once
completed.

Employees, Contractors, Volunteers
All incidents involving employees, contractors and volunteers that are submitted into Intelex are received by
the Environmental Health and Safety department. The EH&S team follows up with the injured and the
Supervisor to conduct a thorough investigation, if necessary. Corrective action will be put in place in order to prevent future incidents.
If a WCB report is required, the Supervisor or Manager will receive an email notification from Intelex and will be required to fill out on the behalf of the injured in conjunction with an HR representative, as per the WCB

Investigation / Corrective Actions / WCB

Students and Visitors
All incidents involving students and visitors that are submitted into intelex are transferred to the Risk team.
The EH&S team may be involved at any point. If necessary, a WCB report will be filled out on the behalf of
the injured.

The Environmental Health and Safety team at Mount Royal University plays a critical role in the prevention of workplace
incidents by supporting a progressive health and safety community culture through individual education and awareness.
This is accomplished through University-wide EH&S policy and procedure implementation, workplace hazard assessment,
accident prevention education and health and safety training for employees, contractors, students and volunteers, in
accordance with health and safety legislation and best practices.
The ultimate goal of the EH&S team is to make sure everyone goes home safe and healthy everyday.
