Tele-medicine in Intensive Care
Prof. Dr. P. Roekaerts
3
3
IC – Telemedicine is the use of an off-site
command center in which an intensive care team
is connected with patients in distant ICUs to
provide intensive care through real-time audio,
video and electronic means
What is IC – Telemedicine?
4
4
Why IC – Telemedicine?
Improve access to intensivists’ quality care remote areas
poor countries : humanitarian commitment
demographics: more elderly sick patients and a shortage
of medical/nursing IC teams
cost containment
education
advice
co-management (a second pair of eyes)
extend into departments that are designed to provide an
immediate response to patients
5 The future of ICU care
5
• Ageing society
• Ageing workforce
• Foreseeable bottleneck
in health care JAMA, December 6, 2000 – Vol 284, No 21
6
Earlier intensivist management
Coordinated timely usage of performance
information
Higher rates of adherence to best practices
Shorter alarm response times
More frequent interdisciplinary rounds
6
Benefits of IC – Telemedicine related to :

The literature
A Multicenter Study of ICU Telemedicine Reengineering of Adult Critical Care Chest 2014; 115(3):500-507
“ICU telemedicine interventions, specifically interventions that
increase early intensivist case involvement, improve adherence
to ICU best practices, reduce response times to alarms, and
encourage the use of performance data, were associated with
lower mortality and length of stay”

The Thalea project
Univ.-Prof. Dr. G. Marx, FRCA
11 The project idea
Problem:
Patient data in Patient Data
Management System (PDMS)
Large data amounts,
no overview possible
Different PDMS do not
communicate
Solution:
Central „Cockpit“
to bundle data
Expert monitors patients
11
the relevant information,
at the right time,
in one place
12 The Thalea project
Goal:
save lives!
Mean:
creating a technologically advanced cockpit for remote support and advise for various ICUs
This will lead to: Earlier detection of deterioration, earlier intervention and
more lives saved
Pattern recognition
Tracking workflows thus improving compliance
Standardization of ICU-registry
12
13 The Thalea system
13
14 The Thalea system
What is the main focus?
Compatibility with patient data management systems already in
use in ICUs
High level of interoperability
Scalability
Manufacturer independency
Enabling remote consultation regionally with telemedicine
solutions
Early detection of ICU patients at increased risk
Effective resource utilization and real-time resource
management
14
Innovation Procurement = PCP + PPI 15
PCP to steer the development of solutions towards concrete public sector needs, whilst comparing/validating alternative solution approaches from various vendors
PPI to act as launching customer / early adopter / first buyer of innovative commercial end-solutions newly arriving on the market
Open market consultation
Thalea PCP-process
Prior Information Notice

16
2012 Thalea consortium established
November 2013: Project start
December 2013 - March 2014: Open market consultation
March 2014 – September 2014: Preparation call for tender
November 2014 – June 2015: PCP phase 0: Call for tender
June - September 2015: PCP Phase 1: Selected tenderers develop a solution design
September 2015 – July 2016: PCP Phase 2: Top-3 solution-designs develop into prototype
August – November 2016: PCP Phase 3: Top-3 prototypes develop into pre-series model
Thalea II / PPI Phase 1: deployment of commercial product
Thalea project timeline 17
23 interested companies from 5
different countries
52% are e-specified companies
25% are PDMS manufacturers
82% of companies are familiar with data-safety and
security
Strong interest in forming of consortia
Results market consultation 18
Prior information notice launched
Functional description designed
First call for tender launched in September 2014
Terminated due to administrative issues
Second call for tender launched in March 2015
Open procedure with bidding competition
Timeline has to be adapted
Call for Tender 19
After termination of first call for tender the timeline had
to be adapted
The timeline was extremely challenging
The Thalea consortium managed it
Successful project and great results
