ABOUT MARIE STEINMETZ, MD Dr. Marie Steinmetz is a board-certified physician in family medicine and integrative medicine. Ahead of her time in 1996, Dr. Steinmetz began studying other therapies such as acupuncture through the Helms Institute, the leading educator of acupuncture to physicians in the United States. Later that same year, she went on to study botanical medicine at Columbia University.

Dr. Steinmetz began studying Functional Medicine with Jeff Bland in the late 1990s and went on for the full certification in Functional Medicine in 2014. In January 2014, she graduated from the leading Fellowship in Integrative Medicine at the University of Arizona under Dr. Andrew Weil, the founder of Integrative Medicine. She is board certified in Integrative Medicine by the American Board of Physician Specialties.

Over her career, she has served on the acupuncture board for the Virginia State Board of Medicine, chairman of the family medicine department at Alexandria Hospital, member of the pharmacy committee at Alexandria Hospital, president of the Virginia Chapter of the Society of Medical Acupuncture, President of the Northern Virginia Chapter of the American Academy of Family Medicine and member of the executive committee of Medical Society of Northern Virginia.

Dr. Steinmetz built one of the few large, successful, and profitable Integrative and Functional Medicine Centers in the country. The center provided acupuncture and Functional Medicine and osteopathic manipulation. Other providers in the office included a Qi Gong teacher, a nutritionist, a yoga instructor and classes for the public on various therapies and subjects including the Integrative Medicine approach to heart disease, women’s health and weight-lifting. Additionally, Dr. Steinmetz contributed a monthly article to a local newspaper on Integrative health for five years.

After consulting with Inova for one year, the Steinmetz Center became part of the Inova Health System, and Dr. Steinmetz became their Medical Director for Integrative and Functional Medicine. During her time as Medical Director, Dr. Steinmetz realized there was a need for healthcare systems to have an expert to help them bring Integrative and Functional Medicine into their system. She worked on numerous projects at the system level including demonstration kitchen, perioperative committee, cancer center and the wellness committee which worked on synergy between different departments as both moderator and speaker. After leaving Inova she continued as a consultant for their highly successful Integrative and Functional Medicine Symposium at Inova.

DEVELOPING A MODEL OF INTEGRATIVE AND FUNCTIONAL MEDICINE Over the course of twenty years, Dr. Steinmetz developed a successful model to maintain quality patient care and allow patients to take advantage of their out of network benefits. This model can be modified to fit with small to large systems, depending on their strategic plan and system needs. This archetype combined the characteristics of primary care in a family practice setting and consultations in Integrative Medicine and Functional Medicine, acupuncture and manipulation. Maintaining high standards of treatment while incorporating Integrative and Functional Medicine, is what earned the respect of the physicians in the community. This resulted in Dr. Steinmetz being named multiple times in the esteemed Washingtonian Top Doctor issue voted on by her physician peers. Additional Dr. Steinmetz was voted as ‘Top Doc’ in the Washington Consumer Checkbook by patients.

OPERATIONAL MANAGEMENT DECIDING WHERE TO START. There are various capacities in which Integrative and Functional Medicine can be incorporated into a health care system or physician group. Strategizing how and where this amalgamation will occur depends on whether a department within the health care system requested this new development or if there was a direct need identified that requires full implementation. The Integrative and Function Medicine model Dr. Steinmetz created can be implemented as a consultation service, without the primary care services, or as a full solution with both primary care and consultation. The incorporation of the Integrative and Functional Medicine to a health care system will result in savings within the system, or better yet increased revenue.

For a system interested in expanding their services to Integrative Medicine, it is best to work through the inquiry process with leadership and identify areas of need and interest. Where is the greatest need or best-fit for this type of solution? For example, many hospital systems have specific contracts for the Medicare/Medicaid population. They’re often the sickest of the sick and consume a lot of healthcare resources. This might be a good area for Dr. Steinmetz to come in and look at some very basic things to help those people improve. Some of these would be of relatively low cost; such as getting a nutritionist, or measuring vitamin D. There are a number of things that we could do to improve patient care and outcome.

STAFFING & CLINICAL TRAINING PHYSICIANS For physicians, the objective is to find a practitioner that has Integrative and Functional Medicine training, preferably with the University of Arizona, or a comparable level of education and training. With the preference being that the clinician is certified in Functional Medicine. It’s extremely beneficial to have a team member that can do acupuncture, and to have a team member who does osteopathic manipulation. These services can also be received through hiring a chiropractor, or acupuncturist. One of the most important insights we’ve had is that doctors working in this sector need to be working together instead of working as separate entities. Dr. Steinmetz has hired many providers and staff and can offer insight into hiring Integrative and Functional Medicine professionals as well as training as a team.

SUPPORT STAFF It is essential to train the staff of the practice to understand what the physicians are doing. Since they are also interacting with patients and potential clients. It’s best if they are knowledgeable about the common tests that are used, when they are used and the context surrounding them. That way the correct information and examples are being given to the patients. If people are coming from a more traditional insurance-based practice, they will need to understand the differences. There would have to be training for the staff on what is covered by insurance and what is not covered by insurance. And that can differ from insurance company to insurance company. For example, some may pay for acupuncture and some may not. So, if that model is implemented there needs to be a grid of sorts that this information could be used as a reference for the patients. Ideally any formal education or training for the supporting staff would be completed through regular office meetings which would involve two to four hours per month. These could be regular lunchtime meetings where lunch is provided. This will allow for better results through the sharing of knowledge and having the staff understanding the goals and purpose of the initiative. This type of ongoing learning is critical because they have relationships with the patients.

ADMINISTRATION MANAGEMENT RECOMMENDED POLICIES AND PROCEDURES Policies will be developed and customized procedures will be implemented to create a system that fits with the different facets of the initiative. For example, there will be instructions on functional medicine labs for the nursing staff to explain and distribute (in person or electronically) to patients. The phone staff or web site will be provided with a resource that contains answers to frequently asked questions such as “what’s the difference between Integrative and Functional Medicine?” It will be important for the staff to understand and communicate how our doctors are better qualified than any potential competitors.

PARTNERS AND VENDORS We provide vendors and other partners to accommodate the needs of an integrative and functional medicine practice. Supplement wholesalers are an example of a vendor. Supplement sales have the potential to make a financial impact on the business. There are many reasons to offer supplements to patients, first, it’s important to circumvent patients going to the drugstore where there are too many options and not necessarily the exact product and dose you recommend. Secondly, it’s important to stock supplements, such as herbs, to manage the quality that you cannot control from outside.

K&A has relationships with vendors who source supplements from different companies that have been vetted in terms of the quality of their products. It’s recommended to avoid dealing with one company as there isn’t one that can provide all of the supplements that are needed. It is ideal to purchase from wholesalers and the system can be charged competitive prices as larger retailers. Ultimately, the goal is to ensure that patients are taking the correct

prescription and that it is a quality product with high manufacturing standards.

For Functional Medicine, relationships will be with different types of labs. K&A President John Killeen has developed relationships with multiple vendors over the past 30 years that will be used a pool from which to choose the best labs, supplements, and other revenue generating products/service that are Integrative and Functional Medicine related. We want to make sure we connect with the best labs with the best track record. We have access to and experience with the vendors you will want to work with.

FACILITY MANAGEMENT SPACE AND LOGISTICS There are difference space needs depending on the modalities involved, for most a standard sized exam room, will suffice. If the physician will be conducting manipulation or acupuncture, there will be the need to have a special table and depending on the exam room there may be a need for some additional space.

It’s understood that the work flow is going to be different as Integrative and Functional Medicine are incorporated into a regular practice due mainly to nurses doing more in terms of special Functional Medicine tests. Although buildouts can be done, the goal is to use the space that is currently available in the most optimal way.

TIMELINE The timeline for implementation depends on many factors, including if there is a physician in the system that can immediately start practicing the Integrative and Functional Medicine tests and procedures. Other variables include confirming what aspects of Integrative and Functional Medicine does the physician want to practice and which departments (if not all) will be part of this new development. If the starting point includes a buildout, which would mean bringing in new staff (i.e. new doctors, acupuncturists, etc.), time of completion could be a year. If there is a space that is already in place and can be used, and the practice would like to function as a consultation or a primary care practice plus consultation, the time of completion would be set at approximately six months. Again, variables such as the licensing of the doctors come in to play as there may be a viable doctor who is ready to practice but it may take three months for this physician to get a license in that jurisdiction.

MARKETING MANAGEMENT PATIENT COMMUNICATION AND MARKETING One of the first initiatives will be to examine all of the practice’s social media. This includes analyzing Google AdWords, and Google Analytics, looking at Facebook. The next step will be to begin posting on these social media avenues generate interest at all the new developments.

Internal marketing will also be an important initiative as this development continues. Possibly beginning with monthly communication that is sent to all the employees “Hey, we’re offering this service,” or an article written by one of the doctors that goes out to the hospital system. Additionally having the doctors speak in the community, because most systems are already prepared to have doctors speaking.

ABOUT INTEGRATIVE MEDICINE AND FUNCTIONAL MEDICINE INTEGRATIVE VS. FUNCTIONAL MEDICINE Integrative Medicine, has more modalities to treat and diagnose the patient than what is learned in medical school. This would include herbs, both Chinese herbs and Western herbs, and would include acupuncture, manipulation and various mind/body therapies. The goal here is to add more treatment options to the patient, many of which have less side effects than our current medication treatment plans.

Functional Medicine is really looking at the biochemical root of problems. Thinking of a tree, what is usually treated is the branches of the tree – hypertension, depression, irritable bowel. In Functional Medicine, the focus is to go deeper, to the roots of that tree. Is it due to genetics? Is it related to stress? Is it a result of eating the wrong foods? Is it the after-effect of wiping out all the good bacteria in the patient’s gut from taking too many antibiotics? So, in Functional Medicine, the perspective used is to look at the biochemistry a little deeper than what is done in conventional medicine, and then treat that root problem.

Dr. Steinmetz has been practicing both Integrative and Functional medicine for 20 years, there are many practitioners who practice one or the other. Those that only practice Functional Medicine don’t always know the herbs that would be administered and used in Integrative Medicine. These practitioners are not always aware, or may possibly be vaguely aware of hypnosis, guided imagery, and other tools that are used in Integrative Medicine. The unique perspective Dr. Steinmetz brings is that she has been practicing both concurrently for the past 20 years and has found patients get better results using both.

RETURN ON INVESTMENT The timeline for the return on investment can be determined once the specific services the healthcare system will be providing are confirmed. If the system decides to begin implementing Integrative and Functional Medicine in the cancer center then those numbers could be solidified. Or if the system comes to the conclusion that the goal is to have a center with three doctors, an acupuncturist, a chiropractor, licensed clinical social worker, and give classes, all in one year, then again, based off of this information, the return on investment can be calculated.

BENEFITS OF IMPLEMENTING INTEGRATIVE AND FUNCTIONAL MEDICINE There are a number of benefits that can be presented. If it is run efficiently, it is most certainly a positive income stream. Concepts such as supplement sales will be new to most healthcare systems but will increase the return on investment and are worth mentioning.

Additionally, this type of medicine is something patients are looking for in every area of the country. Patients are looking for systems that are more aligned, and not just practicing conventional medicine. Currently, there’s the Academic Consortium for Integrative Medicine and Health, and there are probably close to 90 members in that now. These are all large universities or hospital systems that have departments of Integrative Medicine some stronger than others.

If systems want to stay ahead, they need to offer integrative and functional medicine. One of the services being offered by JP Killeen & Associates, Inc., is helping healthcare systems to execute Integrative Medicine and Functional Medicine as part of the institution. There is now some data from the Cleveland Clinic concerning long-term cost-savings, which supports the belief that this endeavor will bring cost-savings into the healthcare system with shorter length of stay, and less re-admissions.

Lastly with the large amount of grant money available to do research and provide services there is a major opening for receiving grants once Integrative and Functional Medicine has been implemented into the system. Dr. Steinmetz would assist in assessing grants and coordinating with the departments involved.
