Description

The Philips IntelliVue Information Center iX (PIIC iX) is a powerful information hub that consolidates

real-time physiological waves, parameters, and alarms from IntelliVue monitors and the IntelliVue M4840

Telemetry System into a clear view of patient status. PIIC iX provides rich review applications for more

in-depth analysis of stored data. PIIC iX supports departments with a single local database or multiple care

units networked across the hospital for up to 1024 beds including connections to hospital information

systems, external time sources, paging systems, and ECG management systems.

Clinical Features

Surveillance

The Main Screen displays real-time waveforms, numerics, and alarms

for up to 32 patients on one or more displays. The following display

resolutions are supported:

Display Sectors

Notes

1. A second header row appears beneath the Name row. Icons appear in the

second row and are not obscured by alarms. A second header row does not

appear when an application is open in the bottom half of the screen and the

sectors are in the top half of the screen.

2. To determine the maximum number of sectors on a display, multiply

Max. Columns by Maximum Sectors in a Column. For example, the

maximum number of sectors for 2 waves and a second header row on a

1920x1080 Display is 3 x 7 = 21 sectors.

Surveillance features include:

• Flexible Main Screen configuration accommodates census needs.

• Main Screen patient sector features include configurable waves and

measurements, ST Maps, and icons for clinical status (DNR, Patient

Group, battery).

• Up to 12 real-time waveforms per resized patient sector.

• Up to 96 real-time waveforms per configured display.

• Each display supports one to four columns of patient views.

• Patient window displays up to 18 waves and 50 numerics.

• Enables viewing of real-time and stored data for a patient monitored

by another PIIC iX in the same clinical unit or in another unit

(overview sessions); up to 10 different overview sessions for the

same patient.

• Provides management of grouping of beds per nursing assignment.

• Touchscreen and mouse keyboard navigation harmonized with the

patient monitor user interface facilitates rapid learning.

• Day/Night Automatic Volume Adjustment allows setting an automatic

sound level adjustment at two selected times of the day.

• A Modified Early Warning Score or Single Parameter Score numeric

obtained at the bedside can be viewed in the monitoring sector or

patient window. PIIC iX does not provide trending, review, reporting,

or notification.

• Remote display setup allows a host to perform sector and display

setup from any other connected host.

Patient Sector Configurability

• Size and layout — Ability to show advanced clinical support tools

such as ST or STE Maps.1

• Waves and Numerics — Ability to change waves or adjust wave size

and change numerics directly in the patient sector.

• Alarm Limits — Ability to adjust parameter alarm limits and turn

alarms on or off in the patient sector.

• Automatic Sector Minimizing — Sectors without active monitoring

can be configured to manually or automatically minimize. Sectors

automatically restore in size when monitoring begins.

• Sector Resizing — Some patients have a higher acuity than others,

and it is beneficial to see more data for those patients. Systems can be

configured to allow manual resizing, ensuring that the smallest sectors

show at least one to two waves (as configured) and numerics for the

remaining sectors. This can also be configured to happen

automatically (for example, show an STE Map when the measurement

is turned on for a patient that may be at risk for having a heart

attack).

• Wave Choices — Any ECG, Pleth, Press, Resp, EEG, Agent, Vent,

Real-time wave

• Numerics Choices — NBP, Pulse, SpO2, Press, Resp, EEG, Agent,

Vent, Temp, SvO2, ST numerics

Measurements and Alarms

PIIC iX uses Philips industry-leading color coding to visually identify a

patient alarm and its severity on the Main Screen. In a sector that is

alarming, the sector background color is blue. Multi-level, audible alarm

tones also indicate alarms and their severity. Criteria for alarms and

recording of events can be configured on a unit-wide basis (with

password protection) and can be modified for individual patients.

Individual alarms, except red level, can be easily customized for each

patient.

Measurements and alarm features include:

• Color-coded alarm presentation and audible notification of patient

alarm limit violations and arrhythmia. Alarm levels are red, yellow, and

INOP.

• Philips IntelliVue ST/AR multi-lead ECG arrhythmia analysis, including

Afib and end of Afib detection.

• Customizable alarm and arrhythmia settings accommodate up to 25

patient profiles for telemetry patients.

• Generates manual and scheduled laser printer reports or 2-channel

strip chart recordings on a Philips recorder.

• Uses Alert Data Integration1 to send text alarm messages to a variety

of mobile paging devices.

• Captures and stores alarms and events in an Audit Log (see Clinical

Audit).

• The Alarm Summary application displays a snapshot of a patient’s

most frequent alarms and their limits, along with trends for the major

vital signs. This can help a clinician to determine if alarm limit changes

for the patient are appropriate, based on unit policies. An Alarm

Summary report can be printed electronically or to paper, scheduled

or on demand.

ST Segment Monitoring

The ST/AR ST segment algorithm analyzes adult patient ECG leads2.

The algorithm measures ST segment elevation and depression and

generates alarms based on two contiguous leads. Both atrially paced

and non-paced beats are analyzed but not ventricularly paced or

ventricular ectopic beats. PIIC iX supports STE (ST Elevation) alarm

using STEMI (ST Elevation Myocardial Infarction) limits.

PIIC iX displays and trends the ST values (up to 12) that are calculated

and alarm at the bedside. For more information on ST Monitoring, see

QT/QTc Interval Monitoring

Philips QTc algorithm brings clarity to a critical change in adult,

pediatric, and neonatal patients at risk for prolonged QT syndrome and

Torsades de pointes. QT/QTc values are trended in the tabular and

graphical trends for historical review and in Horizon trends for

protocol management. Continuous measurements can be made from

all available leads, primary lead, or from any selected lead. Alarms are

configurable to support clinical protocols. For more information on the

Algorithm Application Note.

EASI and Hexad Reduced Lead Display and Storage

PIIC iX supports EASI-derived 12-lead and Hexad ECG reduced lead

systems. The PIIC iX review applications allow retrospective review of

the available ECG waveforms. EASI and Hexad 12-Lead ECG

waveforms are exported in XML format. Refer to the 12-Lead ECG

Monitoring Using a Reduced Lead Set Application Note.

Note: EASI derived 12-lead and Hexad ECGs and their measurements are

approximations to conventional 12-lead ECGs and should not be used for

diagnostic interpretations.

Workflow Support

PIIC iX supports the following workflows:

Patient Management

The clinical user can manage each patient from admission to transport,

transfer, and discharge, and manage the patient’s associated equipment.

Patient Transfer

Patients can be quickly and easily transferred to a new bed by taking

the IntelliVue X2 or MMS to the new location and docking to the new

monitor, or by choosing the exact bed location from the PIIC iX or at

the bedside. Equipment can be configured to stay with the patient upon

transfer. Stored data will move to the new bed.

Bedside and Central Workflow Support

PIIC iX enables remote control of IntelliVue Patient Monitors, such as

alarm limits, arrhythmia, and relearn, to accommodate centralized

nursing workflows. PIIC iX also supports workflow at the bedside

including: patient admission from hospital ADT system, equipment and

caregiver assignment, and patient transfer.

Clinical Review Applications

PIIC iX review applications can assist clinicians to evaluate patient

conditions and assess treatment status. Retrospective review

applications display historical physiological data in a variety of

interactive formats including waveforms, graphic and tabular trends,

and events. With the dual display option, data is available in a

consolidated view on one display for a clearer view of patient status.

1 Alert Data Integration is not available in all geographies. Alert Data Integration provides

supplemental information about patient alarms or events, it is not intended for primary

notification of the alarms.2 The ST/AR algorithm runs on PIIC iX for M4841 telemetry transceivers. For IntelliVue

Patient Monitors, including Wearable Patient Monitors, the algorithm runs on the

monitor, not on PIIC iX.

With the Advanced Specialty Editor1 up to 12 configured reviews total

per unit are available with the additional features Multi/12-Lead Review,

ECG Statistics, and ST Snippet Review.

Data Storage and Review

For in-hospital storage, PIIC iX can store up to 7 days of all patient

monitoring data (full disclosure waves, parameters, alarms, and events)

for each patient.

Data storage features include:

• Stores graphical and tabular trends for up to 7 days.

• Stores up to 12 waves for 24, 48, 72, 96 hours or 7 days.

• With a 12-lead ECG Full Disclosure license, IntelliVue Patient

Monitors (Release K.2) send 8 ECG waves and 8 additional waves for

display and storage on PIIC iX.

• Post-discharge, PIIC iX automatically stores data for every patient for

up to 7 days (given purging criteria, the last 24 hours of patient

monitoring data is available for 6 days post-discharge).

• Stores at least the last 24 hours of patient data/full disclosure for up

to 7 days post-discharge.

• Stores all 12-lead captures for up to 7 days post-discharge.

• From the Web, monitoring information for discharged patients is also

available for viewing for up to 7 days post-discharge.

• During periods of monitoring gaps, whether from a network issue or

Standby or Leads Off, the compressed wave and strip tile will indicate

the cause of the gap.

• Data Warehouse Connect is a licensed feature that allows patient

data, including waves, alarms, events, and trends (both admitted and

discharged) to be exported directly from surveillance PIIC iX stations

to long-term data storage. This feature is designed for clinical

research and can be used for sentinel event review for a single

patient, particularly for events that are more than 7 days after

discharge.

• Holter Export is a licensed feature that stores patient ADT and ECG

wave data (including 12-lead) in a repository, making it available for

use by the Philips Holter monitoring system. Users can choose a

selected duration, up to 96 hours.

• Wave Strip Export is a licensed feature that allows for all alarm and

saved strips to be exported as an image file (.png) to a configured

server for import to the EMR. The image can show up to 20 waves.

Clinical Audit2

PIIC iX captures and stores a log of alarms and events for up to 90

days. The log can be searched by patient or unit, by alert filters (red,

yellow, or blue INOP alarms, for example), and by action filters (silence

an alarm, for example). This log is available for display or export in

Excel format from the PIIC iX to USB. It can also be accessed from

System Configuration and saved to a USB or shared network drive.

Twelve-Lead ECG

• IntelliVue Patient Monitors, Release K.1 and later, send 8 ECG waves

at diagnostic bandwidth, allowing 12 waves to be stored at the

PIIC iX. Waves are stored according to the Full Disclosure license.

The 12-lead full disclosure purchase option (FDB) is required.

• The 12-lead order interface to the Physician Order Entry system

allows the choice of the order for export, order reconciliation, and

reimbursement.

• Signal Quality Indicator for 12-lead full disclosure, including diagnostic

12-lead, EASI, and Hexad.

• In addition to storing 12-lead ECGs, PIIC iX can capture and export a

diagnostic 12-lead ECG to a compatible Cardiology Management

System, such as TraceMasterVue and GE Muse.

Reports, Recording, and Printing

Clinical Reports

• Clinical Reports can be generated on demand or at a scheduled time.

A unit configuration (with password protection) sets the report

templates for all patients on the PIIC iX.

• Reports include: 12-Lead Capture, Alarm, Multi-Lead, Patient

Summary, QT, Review, ST, ST Snippets, Strip, Tabular Trend, ECG

Statistics, Clinical Settings, Unit Summary, and Alarm Summary. Each

report can be sent to a specified laser printer or electronic PDF

document. The availability of some reports is dependent on the

purchase option Speciality Review Editor.

• Reports can be configured to print to multiple printers (Overview

and Surveillance Station area printer, for example).

• Unicode and Roman 8 Character encoding are supported, depending

on local printing requirements.

• Strips can be configured for printing (10 seconds with 2 seconds of

pre-time focus data, for example).

USB Recorder

The M3176C 2-Channel USB recorder can record one or two

real-time or delayed waveforms. The 50 mm wide thermal array

recorder provides high resolution, high quality waveforms. The grid and

waveforms are printed simultaneously to assure accurate registration.

The recorder has recording capability for up to two waveforms and

three lines of annotation.

Printer

The PIIC iX printer can print chart-ready copies of stored patient data

and scheduled reports. Compatible HP printers are those that are

supported by the HP® PCL5 Universal Printer Driver, Version 5.4.

On-Line Help

Context-sensitive help screens provide detailed information on how to

use applications, optimize monitoring, and troubleshoot problems.

Information Technology (IT) Features

PIIC iX meets the IT need for a secure, standards-based solution that

fits into existing infrastructure and communicates with hospital and

clinical information systems. The PIIC iX supports the following

standard IT practices:

• High Availability using VMWare for PIIC iX Servers (Large Primary,

Physiological, and Web Servers) including support for vMotion.

• User Authentication using either customer provided Active

Directory or local authentication.

• Customer Password Policy Support using configurable passwords that

meet complexity and expiration requirements.

• Centralized deployment of updates for antivirus and security patches.

• Layer 3 networking.

• Multicast traffic is controlled in the monitoring LAN. Logical multicast

zones can be configured. The CI service runs on an elected

Surveillance PIIC iX in each zone, which eliminates the need for

multicast across WANs, VPNs, and ISPs. Monitors also use multicast

for Bed-to-Bed Overview functionality. Messages are handled within

each clinical unit by the Alarm Reflection Service of an elected

Surveillance PIIC iX.

• DNS/DHCP protocols using customer-provided DNS.

• PIIC iX is hardened using the US Department of Defense Security

Technical Implementation Guides for operating systems, SQL, .Net,

and Internet Explorer1.

Interoperability Features

The following interoperability features are available with PIIC iX:

• Consistent HL7 messaging between PIIC iX with IntelliVue

Information Center releases L.0, M.0, and N.0

• Tested and validated to the IHE Patient Care Device (PCD-01) profile

• Inbound parameters measured by the IntelliVue Patient Monitors,

including interfaced using VueLink or IntelliBridge, can be exported

via HL7 (with the exception of infusion pump parameters).

• HL7 Toolkit to assist in diagnosing HL7 issues.

• Connectivity to IntelliBridge Enterprise provides a single integration

point with hospital information systems.

• Electronic PDF report export capability, including annotated wave

strip reports in addition to HL7 vital signs export.

• Admission, discharge and transfer, as well as admission from a

Hospital Admitting System, supported at the bedside monitors.

• Additional supported ADT HL7 messages (updates, for example).

• Active Directory for user management. With the Active Directory

interface, user information can be imported directly from the Server

to PIIC iX.

• Supports a continuous patient record by implementing Automatic

Reconnect, Sync from Local Mode, and HL7 Store and Forward.

The following interfacing and export features are available with PIIC iX:

HL7 Inbound

• ADT — PIIC iX supports patient admission at the bedside from the

hospital ADT system through the IntelliBridge Enterprise (IBE).

• Labs — PIIC iX provides for inbound lab data to support Philips

ProtocolWatch Sepsis and to display other labs on IntelliVue Patient

Monitors.

HL7 Outbound

PIIC iX sends HL7 messages (compliant with IHE profiles) containing

patient monitor parameter data, external device data, alert data, and

physio calculations to the EMR system. HL7 versions 2.3 and 2.4 are

supported for Classic and Vista profiles. HL7 version 2.6 is supported

for the IHE profile. The communication transport is TCP/IP. All patient

numeric data is exported via HL7.

HL7 Toolkit

The HL7 Toolkit includes a simulator and diagnostic ability that can be

used to view HL7 service output and troubleshoot HL7 installation.

12-Lead Analysis and Export

Up to 100 conventional diagnostic 12-lead ECGs per patient can be

stored. Up to 30 captures can be locked to prevent them from being

overwritten. All stored ECGs are available for export to a cardiology

management system.

The following features provide continuous data:

Trend Upload

Up to 8 hours of numeric data collected on an IntelliVue Patient

Monitor (Release K.1 or later) not connected to the PIIC iX is

automatically uploaded once the monitor can reconnect. Vital signs

(such as HR, SpO2, ABP and NBP) collected while the monitor was off

the network are uploaded to the PIIC iX and stored and available for

retrospective review.

HL7 Store and Forward

HL7 Store and Forward permits off-network storage of historical trend

data and/or real-time data in order to forward that data to the EMR in

separate HL7 configuration modes/ports when EMR connection is

restored. Four hours of messages can be stored.

Distributed HL7 and Alert Data Integration

HL7 Service and Alert Data Integration (Paging) can be configured to

be Distributed or Centralized. When configured as Distributed, the

HL7 or ADI Service runs on each PIIC iX with each PIIC iX connecting

directly to configured clients. When Centralized, the services run on

the gateway Server and each PIIC iX sends information to that server

which sends the information to configured clients.

Auto-Reconnect and Settings Synchronization

Database Synchronization and Auto Reconnect permit systems not

currently connected to the primary server to make changes to patient

demographic settings as well as admissions, discharges, and transfers.

When the connection to the primary server is restored, local changes

are automatically synchronized back to the primary server and patient

data that was collected locally will be available to view in the

retrospective applications.

PIIC iX Web

PIIC iX Web is a licensed web application that provides viewing of near

real-time waves, parameters, alarms, and retrospective data for patients

across care units on PIIC Classic or PIIC iX using a Web client browser.

Additionally, it provides access to retrospective review data using iPad

and Android tablets. Web communication can be secured using HTTPS

and to require user authentication. Available as a single patient or

multi-patient option, PIIC iX Web permits access to:

• Cardiac Review

PIIC iX Mobility

PIIC iX Mobility Service is a licensed option that permits iOS and

Android devices to view near-real time PIIC iX patient waves,

numerics, and alarms using the IntelliVue Mobile Caregiver application.

IntelliVue Mobile Caregiver is a free application available on iTunes or

Google Play Store. Once a configured mobile client user with

appropriate PIIC iX Clinical Unit access is authenticated by the PIIC iX

Mobility Service, the user can view a list of all beds in the topology,

current vital signs for selected beds, and near real-time wave data for

individual beds. The following mobile devices are supported:

Centralized Licensing

With centralized licensing for the Enterprise and Small Network

PIIC iX, customers can purchase a central pool of monitored bed and

option licenses and distribute them across different surveillance

stations. Centralized licensing allows the flexibility of moving licenses in

the future, based upon changes in units and monitoring practices, and

not being tied to the physical hardware. This requires downtime and

trained personnel.

With software licensing and Philips Remote support, the Philips

Customer Care Solutions Center knows what system, options and

release levels customers have installed, which is helpful when

diagnosing system issues.

User Configuration

Monitoring controls, display formats, alarm response, and patient data

presentation can be configured to user preferences with simple System

Configuration tools without interrupting patient monitoring. Unit-wide

configurations can be made in password-protected applications but can

be modified for individual patients. System configurations can be

archived for quick restoration after software updates.

The PIIC iX M3290B Application Software complies with requirements of the Council Directive 93/42/EEC (Medical Device Directive). It carries

CE-marking accordingly. The processing unit, Philips-supplied displays, and Philips-supplied Printer comply with IEC 60950-1, CISPR 22 Level A,

and CISPR 24. UPS comply with IEC 60950-1, EN 62040-1, and EN 62040-2. The processing unit, Philips-supplied displays, Philips-supplied UPS,

and Philips-supplied Printer carry CE-marking to Council Directive 2006/95/EC (European Low Voltage Directive) and Council Directive

2004/108/EC (EMC Directive). These are not suitable for installation in the Patient Care Vicinity (Patient Environment). The Philips Recorders

carry CE-marking to Council Directive 93/42/EEC (Medical Device Directive).
