Presentation

In the last decades the anomalies of placental invasion (AIP: pla-centa accreta, increta and percreta) have been object of a rising debateall over the world, because of:• the progressive increase of incidence• the high feto-maternal mobility and mortality• the possibility of a prenatal diagnosis• the need of a multidisciplinary organization of management

The most important risk factors are a previous cesarean sectionand placenta previa. For some years clinicians have led a commonstrategy for the restraint and reduction of cesarean sections rate, be-cause the negative implications of such procedure have become morefrequent and there is also a correlation between AIP and the numberof previous cesarean deliveries, that is to say the number of scars.

Another aspect emerging in literature is the possibility of a pre-partum diagnosis representing the fundamental premise for a correctmanagement and for the reduction of maternal morbility and mor-tality. In particular, Ultrasound diagnosis has made significant pro-gresses in the last 2-3 years and the utilization of 3D Dopplertechnique has permitted the exploration of more advanced diagnos-tic aspects, unknown till few years ago.

In the first trimester of pregnancy, the early evaluation of the re-lation between implantation site and previous cesarean scar is a“new” concept with which gynecologists should familiarize for amore frequent and culturally correct use in clinical practice. The earlydiagnosis, or even a very early screening in cases with anamnesticrisk, are today interesting aspects of the research. The improvementof diagnosis can justify care pathways expensive and complex andthe prompt addressing to reference centres where there is the possi-bility of ensuring an adequate multidisciplinary care.

This course has the purpose of showing the experience gained inthe last years and contributing to a better care for a pathology withhigh risk for maternal health, thanks to the presence of eminent in-ternational and national Experts.
