Personal Identifier

This is a unique code that is sent to HUD with each resident so that the resident’s

name is not sent to HUD. This code is used to ensure the information remains “De-

Identified.”

InForm – Use one Letter, for example U4321467

Not SMITH304 - this shows identifying information

Household Identifier

This is a unique code that is sent to HUD so that the resident’s last name/household name is not sent to HUD.

For elderly residents, it is fine to use the Personal Identifier number and place an “H” after it. Since most elderly residents either live alone or when two residents live in a unit, they are both adult head of households, this value is not as essential as it is to family housing (with dependents).

InForm - if the resident Personal Identifier is 123456, the Household Identifier can be 123456H.

If there are dependents int the Household, use this same number for the dependents (123456H). SfS does not currently collect data on resident under 16.

Participant Status Code

Does the resident use the services of the service coordinator?

A Participant:

• Interacts w/ the SC

• Does an annual assessment w/ SC to identifies wants, needs, interests, abilities.

• Comes to SC programs and events

It does not have to be the entire assessment or a formal assessment but a proactive approach to resident

engagement, interaction, and case management/follow up.

A Non-Participant does not participate in any services.

Did the resident Opt-Out? Do they never talk to SC/Non-Responsive?

They may come to an occasional social event - but they are not really a Participant in the SC program

Resident Needs Assessment Service Code

Did the resident receive do an assessment with the SC this past year?

If Yes, mark this as Yes.

It does not have to be the entire assessment or a formal assessment but a proactive approach to

resident engagement, interaction, and case management/follow up.

You don’t have to ask every resident every question!

You know your residents best.

Use the Data Collection and Assessment process to build relationships.

Don’t ask questions you think will alienate or offend a certain resident.

Data is vital to expanding and advancing this industry. Get what you can - but don’t make residents run when they see you!

We need your expertise in helping advancing this effort but use your best discretion when working with each resident!

This is the number of years the resident has lived in subsidized housing

OR

The number of years living at the property the resident is currently resides

The SC usually only knows the number of years a resident has lived on that

property, so that answer is acceptable.
