{ } Peritoneoscopic Implantation System for Peritoneal Dialysis Catheters Peritoneoscopic Implantation System for Peritoneal Dialysis Catheters The Y-TEC ® System for implantation of peritoneal dialysis (PD) catheters is a well-established procedure. Since 1983, over 28 articles have been published in peer-review journals or at international confer- ences. All these articles and reports (randomized and retrospective) have consistently shown supe- rior short and long-term outcomes and longevity. The Y-TEC ® System itself is composed of reusable in- struments (2.2 mm diameter endoscope, fiberoptic light guide, sterilization tray, and a fiberoptic light source) and a choice of two disposable catheter implantation pacs. The VP-112 DLX pac contains virtually all the compo- nents an interventional nephrologist needs to implant a PD catheter including the critical proprietary Y-TEC ® components. These include the patented Quill ® Guide Assembly*, small and large Dilators, the patented Cuff Implantor ® †, and the Tunnelor ® Tool, plus thirty more components. (For complete component list, see the web site.) The VP-210 STD pac is the primary PD catheter implantation kit. It contains only the critical propri- etary Y-TEC ® compo- nents listed above. These pacs can be used to implement almost all PD catheters in- cluding Flex-Neck ® , Swan-Neck™, and con- ventional PD catheters. (The Toronto-Western™ and Missouri Swan-Neck™ catheters cannot be implanted with this technique.) The Y-TEC ® instruments as well as the disposable pacs, and a brief illustration of the implantation procedure, are described on the overleaf. Y-TEC® System Instrumentation

Twenty-eight published clinical studies or reports of the Y-TEC® procedure all state comparable data indicating a reduction of catheter problems and major improvements in PD catheter longevity compared to other implantation techniques. These studies show:

• Catheter survival at 8 weeks is 95-98%. • Exit site infection frequency is 1-5%. • Tunnel infection frequency is 0-1%. • Leak frequency is 0-1%. (Most are self-sealing with additional 24 hours of dry abdomen.) • Catheter survival at 24 months is 70-84%.

By contrast, National Institute of Health (USA) data shows significantly poorer results for surgical (cut down) and guide-wire techniques.

The Y-TEC® procedure is a system of instruments and disposables to enable the interventional ne-phrologist to implant peritoneal dialysis catheters easily, accurately, and with a very high degree of confidence and success. The specially designed 2.2 mm diameter Y-TEC® scope enables the nephrolo-gist to enter the peritoneum with minimal trauma and discomfort to the patient. Also included is a fiberoptic light guide to transmit light through

the scope into the abdomen, a sterilization tray in which to sterilize the scope and light guide, and a fiberoptic light source.

ConclusionThe Y-TEC® system of catheter implantation has shown that “…peritoneoscopic placement of the Tenckhoff (catheter) is fast, safe and a cost effective method of catheter placement that yields excellent results…” and with “…little perioperative mobility, immediate availability for use without leakage or outflow obstruction and remarkably reduced inci-dence of tunnel infection.”
