Who owns current SGD? Is it an SGD? Was it funded strategically? How is it supported? How will it be supported? What are future funding options? Can it grow with life out of school? An SGD users team must consider: Age of device How current system will/not follow the person What support needed for success in variety of contexts SGD Transition Planning
An SGD users team can: Provide practice for student in vocational settings with SGD Make academics relevant to potential employment/social/post-2ndary settings Provide strategies for student specifically related to their disability and SGD use Be open about the changes and challenges they will soon encounter Teach self advocacy SGD Transition Planning
An SGD users team must consider: SGD is persons paddle for their boat. Dont leave them adrift! SGD Transition Planning
Involve parents right away Gently remind them that they will not live forever Person using SGD, family, and team, must dream about future Leader must be identified to carry SGD transition plan forward Find local people who have successfully transitioned with an SGD SGD Transition Funding

Every SGD funding option at transition requires purposeful connection to resources Most successful are woven into future (e.g. DVR for future employment) Most successful are natural result of long-term assessmentand thoughtful strategic planning What if they already have an iPad as SGD? To fund for transition via DVR or employer, iPad and app may be funded. Nearly all other funders require SGD to qualify as DME Must conform to SGD criteria set by funder (most use Medicare guidelines) What if they want to fund an iPad as SGD?
