Access to healthcare in India, particularly in rural area is extremely challenging. Often, due to lack of access to healthcare services patients do not seek medical attention on minor symptoms until the disease has advanced to a life-threatening stage requiring more expensive and super specialist care. In fact, at the primary level the non availability of healthcare incentives people to seek medical consultation from unregistered medical practitioners and compromise with substandard medical care resulting in misdiagnosis and improper treatment.
The major challenges in Indian primary care are from due to following reasons:• Lack of medical infrastructure

The other major challenges for providing universal Healthcare:
•Although, bed density is significantly lower than the WHO guideline the under-utilisationof existing resources further compounds the problem
• Health workforce remains is inadequate and un-willing to work in rural towns and villages. The total number of doctors and nurses is lower than the WHO benchmark of 2.5 doctors per 1,000 population and most medical practitioners are concentrated in major cities.

The rapid increase has put tremendous pressure on the existing rural healthcare Infrastructure.

• Cover for out-patient spend.
• Non-communicable diseases.
•Public private partnerships sponsored social insurance programs and increasing
awareness of health insurance
• Improving the efficiency and efficacy for PHC (primary health centers) and CHC
(community health centers).

As on March 2011, there were 148,124 sub-centers, 23,887 PHCs and 4,809 CHCs.
