The program is available at no cost to the member. The Diabetes Empowerment Educaon Program, known as DEEP, is designed to help people with pre- diabetes, diabetes, relaves and caregivers gain a beer understanding of diabetes self- management. Goals of DEEP include: To reduce diabetes complicaons and modifiable risk factors. To beer manage other chronic diseases by improving blood glucose control. To increase knowledge of diabetes and promote improved coordinaon of care with medical professionals. To increase self-management skills such as self-monitoring, healthier eang, reasonable exercise plan, stress management, and appropriate use of medicaons (prescripon and OTC). To beer manage psychosocial issues. To facilitate short- and long- term behavioral change. Taught by a cerfied instructor, the six-week program focuses on the following modules: 1. Beginning session- Understanding the human body 2. Understanding diabetes and its risk factors 3. Monitoring your body (BP, weight, BMI, waist circumference, glucose monitoring) 4. Get up and Move! The importance of physical acvity 5. Health management through sensible nutrion guidelines 6. Diabetes complicaons: Idenficaon and prevenon 7. Medicaon and medical care – Improving communicaons with your provider(s) 8. Stress and Coping - Mobilizing your family and friends. Below is a list of upcoming classes. EBD is happy to announce a new

program offered to our Medicare

Primary members with diabetes.

The program is available at no cost

to the member.

The Diabetes Empowerment

Education Program, known as DEEP,

is designed to help people with pre-

diabetes, diabetes, relatives and

caregivers gain a better

understanding of diabetes self-

management.

Goals of DEEP include:

To reduce diabetes complications and modifiable risk factors.

To better manage other chronic

diseases by improving blood glucose control.

To increase knowledge of

diabetes and promote improved coordination of care with medical professionals.

To increase self-management

skills such as self-monitoring, healthier eating, reasonable exercise plan, stress management, and appropriate use of medications (prescription and OTC).

To better manage psychosocial issues.

To facilitate short- and long-

term behavioral change.

Taught by a certified instructor, the

six-week program focuses on the

following modules:

1. Beginning session- Understanding the human body 2. Understanding diabetes and its risk factors 3. Monitoring your body (BP, weight, BMI, waist circumference, glucose monitoring) 4. Get up and Move! The importance of physical activity 5. Health management through sensible nutrition guidelines 6. Diabetes complications: Identification and prevention 7. Medication and medical care – Improving communications with your provider(s) 8. Stress and Coping - Mobilizing your family and friends.

As part of the ARBenefits wellness program,

active employees and covered spouses who

test positive for nicotine use, can still

successfully complete the program by

enrolling in Tobacco cessation through

GuidanceResources®.

Members who test positive for nicotine have

two ways to enroll in a Tobacco Cessation

program through GuidanceResources®. Each

program is offered at not extra cost to the

member.

Members can enroll into the full telephonic

program. The over-the-phone program

includes five sessions with a counselor, and

coverage for cessation aids including

prescriptions and patches.

Members who are interested in enrolling into

the telephonic program can call

GuidanceResources® at 1-877-247-4621.

An online cessation program is also available

for members. The online program consists of

a four parts module. There is no coverage for

cessation aids for members in the online

program.

To enroll into the online Tobacco Cessation

program, members can access

GuidanceResources® through their ARBenefits

Member Portal account. Follow the same

steps outlined in the online health assessment

article above.

Members who go through the online Tobacco

Cessation program need to make sure they

complete the survey at the end of the

program.

Members who test positive for nicotine must

enroll in a GuidanceResources® Tobacco

Cessation program by 10/31/18 to

successfully complete the ARBenefits

wellness program.

Diabetes Management Program Moving to Kannact

ARBenefits is excited to announce a new

management program available to members

with diabetes. Kannact is a program available,

at no cost, to all ARBenefits members.

Kannact combines real-time biometric

monitoring with one-on-one coaching to help

members engage and better navigate the day

to day challenges of living with diabetes.

Kannact does not replace a member’s

physician, but rather partners with the

provider to provide support and guidance

between visits.

On August 21, 2018, The State and Public

School Life and Health Insurance Board

approved a motion to transition the Plan’s

diabetes management program to Kannact.

Additional information will be provided to

members that currently participate in the

diabetes management program through

Active Health Management who wish to

transition to Kannact.

Kannact sends all necessary testing supplies

directly to the participants’ doorstep. The

following supplies and benefits are provided

at no cost to the member, and are fully paid

for by the Plan through an in-network claim

reimbursement. No more fighting with the

pharmacy to get your supplies!

Bluetooth Glucometer which uploads

your readings to your private Kannact

portal; no need to log your readings

anymore.

Prescribed testing strips & lancets

shipped directly to your door step

24-hour access via mobile app, text

messaging and phone

Diabetes self-management tools and

guidance through an ADEA certified

diabetes health coach

Enrollment for Kannact opens on September

4, 2018. Members can find more information

about the program and enroll by going to

www.Kannact.com/ARBenefits. Kannact can

also aid members over the phone at

1-844-279-4153.
