Open Enrollment is your once-a-year opportunity to learn your benefits options and to make additions, changes or deletions to your health coverage. Any elections made during Open Enrollment are effective and irrevocable as of January 1, 2019 and will stay in effect until December 31, 2019 (unless you have a qualified change in status and make a timely corresponding change). Passive Open Enrollment – To retain the same benefits coverage for Plan Year 2019, you do not need to do anything. If no changes are made during the Open Enrollment period, we will carry forward your 2018 elections. The FHDA partners with the California Public Employees’ Retirement System (CalPERS) to provide your health and retirement benefits. Important Reminder: when adding new dependents, you must submit ALL required documentation such as Marriage Certificate, Birth Certificate or Passport, and Social Security Cards, for all dependents, to the District HR/Benefits Unit no later than 5pm, Friday, October 5, 2018 in order for benefits to be activated effective January 1, 2019. No exceptions.

2019 Benefit Rates – retirees’ monthly contributions remained the same Beginning 2019, the Mandatory Minimum Monthly Employer Contribution towards health is $47.60 or
$136.00 X 35%. This amount will be reduced from the monthly FULL CalPERS published rates, and this is applicable only for retirees who have retired from the District prior to May 17, 2012.
2019 HMO Health Plan Service Area Changes: o Blue Shield Access+: Exiting counties of Alameda, Contra Costa, Marin, San Francisco, San Mateo,
Santa Clara, Solano and Sonoma; Existing members will be defaulted to HealthNet SmartCare HMO o HealthNet SmartCare: Exiting counties of El Dorado, Placar, Sacramento, and Yolo; default varies
depending on your zip code o UnitedHealthcare Alliance (Basic Plan only): Exiting all counties in Bay Area and Other Northern CA
Regions; Existing members will be defaulted to Anthem Traditional HMO. PPO Benefits Changes in 2019
o PERS Choice and PERSCare Basic co-pays for urgent care and specialty visits will increase to $35. The co-pay to visit your primary care physician (PCP) remains at $20
PERS Select Basic PPO Plan has adopted Value-Based Insurance Design (VBID): o The new PERS Select Plan will change co-pays for primary care, specialists, and urgent care; award
credits to reduce your annual deductible; eliminate hospital tiering, so you can access any hospital in your network at one coinsurance rate; eliminate coinsurance for inpatient delivery, after enrollment in Healthy Moms program.
o PERS Select VBID offers the following:
Available in all 58 California counties
Anthem will assign a primary care physician (PCP) to each insured to coordinate your care
Increase calendar year deductible
$1,000 for individual
$2,000 for family
Earn $100 credit/individual towards deductible if you get 1) Biometric screening (tested at your doctor or at Quest Lab) 2) Flu shot 3) Non-smoking certificate 4) Second opinion for elective surgeries by calling 888-361-3944 5) ConditionCare certification for engaging with a nurse manager for disease management if
you have asthma, diabetes, COPD, or heart conditions
Office Visits reduced to $10 with primary care physician (PCP)
Urgent Care copay increase $20 to $35
Specialist Visit copay increase $20 to $35
Hospital Coinsurance decrease 30% to 20%
Inpatient Maternity delivery coinsurance waived when using Healthy Moms program
SpineZone, an online back and neck pain program, and Wisdom, a study to optimize breast cancer screening, will be available for all PPO members.
Medicare Health Plan Availability: o Only Kaiser offers both BASIC and Medicare Advantage HMO Plan o Medicare-eligible members may enroll with Kaiser, PERSCare/Choice/Select PPOs, Anthem Medicare
Preferred Plan, and UnitedHealthcare Group Medicare Advantage PPO o UnitedHealthcare (UHC) Group Medicare Advantage PPO (restricted to Medicare-eligible retirees only) -
Allows plan members to receive care at the same benefit level from any willing Medicare provider, except Kaiser, for a $10 co-pay. It covers Medicare Parts A, B, and D and the plan will cover members nation-wide in all states and U.S. territories. In addition, the plan will offer dental and vision coverage as an option. Members signing up for this dental and vision benefit will pay UHC directly for this coverage.
o Anthem Medicare Preferred PPO (restricted to Medicare-eligible retirees only) - Allows plan members to receive care at the same benefit level from all contracted Medicare provider, except Kaiser, for a $10 OV co-pay. It covers Medicare Parts A, B, and D and the plan will cover members nation-wide in all states and U.S. territories.
Offers similar coverage to UHC Group Medicare Advantage PPO, however, the plan required 10% co-insurance for Durable Medical Equipment and Diabetic Services such as glucose monitors, and test strips.
Optional dental and vision coverage are available to Medicare-eligible retirees. Members signing
up for this dental and vision benefit will pay Anthem BC directly for this coverage.
