1.0 Section T1 – Cover Letter (Admin, Technical and Unpriced Commercial)

(a) Proposal Form

Bidder is required to confirm his understanding and agreement to the bid document by executing the Proposal Form using the same format as provided in the Administrative, Technical & Unpriced Commercial Package.

(b) Covering Letter for Administrative, Technical and Unpriced Commercial shall be sign over Rp.6000 duty stamp by authorized director.

2.0 Section T2 – No Legal Dispute Statement

4.0. Section T4 – Preliminary Work Programme

Bidder to provide a preliminary work programme showing the completion date meeting the requirement as per Schedule of WORKS - Exhibit II shall consist of the following:

(a) Schedule and Schedule Control (as described in item 7.0 of Exhibit IV - Administration Procedures) :

(i) Critical Path Network (CPN). The critical path shall be in colour

(ii) Level 4 Schedule Develop and printed in color :

- Overall Master Schedule- Engineering Schedule- Reflag/IPKA, master list and import/export Schedule for marine vessel,

equipment and materials.- Mobilisation Schedule- Procurement Schedule- Transportation Schedule- Installation Schedule

(b) Committed and anticipated workload by Bidder for the next twelve (12) months.

5.0. Section T5 – Contractor Organization and Project Management Team (PMT) for the Work

Bidder to provide:

(a) Organization Chart showing Bidder's Overall Organizational and functional groups and showing lines of authority.

(b) Functional Chart of Bidder's proposed PMT organization for the WORKS showing lines of authority, responsibility and communication; reporting; and identify the key personnel by name, title and brief job description. Refer to EXHIBIT V - CONTRACTOR’S ORGANIZATION AND PROJECT TEAM KEY PERSONNEL for the minimum requirements for PMT Key Personnel. Bidder shall also provide a brief description of its management and administrative controls for WORKS. Bidder to demonstrate the PMT covers all phase of the Project.

(c) Bidder to provide most recent resume of every PMT Key personnel. Minimum qualification requirement and number of personnel required for each key position for the various key positions is given in EXHIBIT V - CONTRACTOR’S ORGANISATION AND PROJECT TEAM KEY PERSONNEL. Bidder to list summary of key personnel for the WORKS as per Table T5A (SUMMARY OF KEY PERSONNEL FOR OVERAL PROJECT MANAGEMENT TEAM) and TABLE - T5B SUMMARY OF KEY PERSONNEL FOR TRANSPORTATION & INSTALLATION

(d) Bidder to submit individual letter of consent for each proposed key personnel (to be signed off by each proposed key personnel) to officially address their willingness to accept the job, to confirm their availability for timely mobilization to Worksite upon award and to confirm that they will be full time dedicated to the project. Sharing of the proposed key personnel with other projects is prohibited.

6.0. Section T6 – Barge Availability

Bidder shall submit the following detailed information and evidence to prove the above matter provided is meeting CONTRACT requirements.

(a) Bidder or parent Company owns the Pipe Lay Barge (PLB) - Indonesia Flag or

compliance AZAZ cabotage with evidence through the letter of undertaking. As example, Bidder to provide reflagging document as evidence that the PLB barge in the process re-flag and able to meet mob schedule or able to secure the IPKA document if the PLB is not Indonesia Flag.

(b) Bidder's proposed Pipe Lay Barge (PLB) is available to complete WORK as specified in SCOPE OF WORK and Indicative Schedule.

. 7.0. Section T7 – List of Subcontractor(s)

(a) Bidder shall list down its proposed Subcontractor(s) in the format per Table T7A.

(b) Bidder shall also ensure that priority should be given to local companies for any local manufactured.

8.0. Section T8 – Pipe Lay Barge (PLB) for Transportation and Installation.

Bidder shall demonstrate the proposed Pipe Lay Barge (PLB) must be capable to install subsea spools and pipelines re- route in 58.5m depth. Pipe Lay Barge (PLB) shall be equipped with all spreads as per Exhibit VI.

9.0 Section T9 – Execution Plan Bidder to submit the comprehensive Execution (detail as per SOW in Exhibit I) Plan for :

(i) Recovery old pipeline in dry or wet (with treated water) condition (exclude or include riser-whichever practical and cost effective) and re routing of 12” Export Pipeline from old platform to new location.

(ii) 12” Subsea Spools installation at Platform side.(iii) Pipelines Cleaning, Gauging, Filling, Hydro test, Dewatering, Vacuum Drying and

Nitrogen Packing for 12” x 110km Export pipeline upon completion of subsea spool tie-in.

(iv) Caliper Pigging and Intelligent Pigging for 12” x 110km export pipeline.(v) Return all surplus to Lamongan Supply Base (LSB)

10.0. Section T10 – Quality Assurance / Quality Control

Bidder shall implement fully the Quality Assurance requirements as referred to in EXHIBIT VIII – QUALITY ASSURANCE.

11.0 Section T11 – Health, Safety and Environment Program

COMPANY wish to emphasis that the Bidder's HSE Program will be given significant consideration in the overall contract evaluation. Bidder is advised to provide accurate, complete and detailed information on safety as requested. COMPANY at its sole discretion, may at anytime conduct site visits to verify responses. Bidder may be disqualified from participating in this bid as well as future bids if inconsistencies are found.

Bidder to submit the following:

(a) Proposed HSE Plan for the entire project duration. Bidder shall refer to the HSE template/checklist as described in the EXHIBIT IX – Health, Safety and Environment Requirements, APPENDIX IX-I HSE PLAN MAJOR CONTRACTS.

12.0 Section T12 – Relevant Work Experience

Bidder shall provide:

(a) Bidder has performed similar scopes of work and proven track record with at least (as tabled in Table T12)

(i) Experience in managing and executing offshore transportation and installation work. Minimum two (2) similar projects each/both for Pipeline and Subsea Spool in the last seven (7) years.

13.0 Section T13 – Unpriced Local Content Form and Statement Letter of Local Content Commitment

Bidder shall submit commitment letter to achieve a minimum of thirty five percent (35%) of local content for the performance and completion of the WORKS. Bidder shall submit the following:

(a) Completed Local Content calculation as per format in Table T13. Bidder shall complete the committed local content for each cost component, WITHOUT INDICATING THE COST in STAGE-1 BID PROPOSAL.

(b) Bidder shall submit supporting certificate and/or information required to support the local content achievement for Goods.

(c) Bidder is strongly encouraged to maximize local content for the performance and completion of the work. Bidder is required to complete and submit the Statement Letter of Local Content Commitment as provided below.

17.0 Section T17 - Tender Clarification Form

Bidders that require clarifications of the Bid Documents shall notify the Tender Committee in writing (as per Proposal Clarification Form – Table 17) by fax. Any requests received orally will not be entertained. All such requests shall clearly state Bidder’s name and correspondence reference number. These should be addressed to Tender Committee

Copy of all clarification of the bid documents must also be submitted via e-mails at [email protected] on top of official submission in writing via fax. Native file for the clarification form to be submitted together on top of official submitted forms

All clarification requests from Bidders must be submitted in accordance with the requirements stipulated in Item 2.3 of the ITB.

Bidder shall include all bid clarifications and addendums issued to Bidders before Bid Closing Date in this section.

Written copies of the Tender Committee’s response to general requests (including an explanation of the request, without identifying the source of the inquiry), will be send to the Bidders that have received Bid Documents.

All Data which COMPANY has prepared or supplied to Bidder and all Intellectual Property (“IP”) Rights therein shall remain the property of the COMPANY.

18.0 Section T18 - Acknowledgement Letter Form

Upon receiving a copy of the Bid Documents, Bidder is requested to confirm in writing within two (2) working days to the Tender Committee at the address stated in Instruction to Bidders of its intention to submit a Proposal meeting the requirements of this Bid Document by signing and returning the Acknowledgement Letter as per format below.

If Bidder decides not to submit a Proposal, Bidder shall provide the Acknowledgement Letter and state the reason(s) for declining. Bidder shall return all Bid Documents to the Tender Committee with the letter at the address stated in Instruction to Bidders.

If Bidder subsequently decides to withdraw from or not to participate in the Bid process after previously returning the Acknowledgement Letter confirming its intention to bid, Bidder shall promptly notify the Tender Committee at the address stated in Instruction to Bidders in writing of such decision and shall return with such notice all Bid Documents to the Tender Committee, no later than two (2) working days from the notice of the decision.
