Mount Sinai Heart Fibromuscular Dysplasia Research Program Co-PIs: Kovacic and Olin Award Period: 3/31/2016 - 3/30/2020 Awarding Body: Anonymous Philanthropic Donor Fibromuscular Dysplasia (FMD) is a non-inflammatory vascular disease of unknown etiology. This program will undertake an extensive clinical (PI: Olin) and molecular (PI: Kovacic) characterization of this disease. Using skin biopsies to generate patient-specific cell lines, we will establish the cellular and genetic profile of fibroblasts and other vascular cells from FMD patients and matched healthy controls. Our aim is to identify activated pathways and genes, and to ultimately elucidate the underlying cause of this condition. In this proposal, we will use genetic analysis to infer causal relationships between cardiometabolic traits, metabolites and clinical phenotypes for subclinical CAD disease outcomes. Our study will non-invasively quantify the entire atherosclerosis cascade including endothelial dysfunction, vascular stiffness, vascular inflammation, plaque composition and total plaque burden in patients with sleep apnea using state-of-the-art vascular imaging. Our study will provide significant mechanistic insight regarding the role sleep apnea plays in the progression of atherosclerosis and regarding the anti-atherosclerotic actions of continuous positive airway pressure therapy. Our study will identify patients at high risk for progression of atherosclerosis and those who have the greatest anti-atherosclerotic actions of sleep apnea treatment. Industry studies Role: Co-I (Total effort for all studies = 3% FTE) Awarding bodies: varies, but include Medtronic, Daiichi Sankyo, Astra Zeneca. Dr. Kovacic is a Co-Investigator on several industry funded clinical trials run through the catheterization laboratory of Mount Sinai – see summary table below. He serves in a supporting role that is largely is encompassed within his routine clinical duties. For example, a normal routine clinical duty of Dr. Kovacic is implanting cardiac valves (Transcatheter Aortic Valve Replacement – TAVR). TAVR is routine in patients deemed to be at intermediate or higher risk for surgery. A current area of clinical research is if TAVR is beneficial for patients at low surgical risk. However, for Dr. Kovacic performing the TAVR in a low risk study patient the study procedure is essentially identical to a regular clinical implant in a patient of higher risk. Neither Dr. Kovacic nor his research program receives any funding from these studies. Study Sponsor

LEDUCQ Transatlantic Network of Excellence PI: Jason Kovacic Award Period: 10/01/2013 – 12/31/2018 Awarding body: Foundation LEDUCQ (Transatlantic Networks of Excellence Program) Title: Cellular and molecular targets to promote therapeutic cardiac regeneration The rationale for this network lies in the acute need for defining the mechanisms regulating and ultimately limiting regenerative responses of the adult heart. Unraveling the complex and poorly understood processes mediating the age-dependent decline in cardiovascular stem cell efficacy and regenerative capacity is a core objective of this network. In Project 3 we will use DNA from the children and both DNA and inflammatory cell-derived RNA from caregivers to identify network models and predictors of primary prevention outcomes using genetic, genomic and molecular data from children and adults before and after lifestyle intervention. Using unique mouse models this proposal will define the function of endothelial cells in these processes and also investigate the ability of endothelial cells to change their phenotype to become smooth muscle cells, fibroblasts or other cells that drive atherosclerosis. ISSCRES0141 PI: Jason Kovacic Award Period: 04/29/2013 – 6/20/2016 Awarding Body: AstraZeneca Title: YELLOW II Study - Reduction in Coronary Yellow Plaque, Lipids and Vascular Inflammation by Aggressive Lipid Lowering This investigator initiated clinical study will assess the in vivo regression of atherosclerotic yellow plaque content and changes in plaque morphology after high-dose statin therapy by utilizing diffuse reflectance near-infrared spectroscopy (NIRS) and optical coherence tomography (OCT) in diseased coronary arteries of ischemic heart disease patients presenting for coronary angiography. Concurrently, lipid trafficking, reverse cholesterol transport and the inflammatory macrophage phenotype will be assessed in response to changes in patient lipid profiles. High-dose statin therapy (rosuvastatin 40mg daily) will be administered to all subjects (n = 80), with baseline measurements being compared to those at final follow-up after 8-12 weeks of therapy for each subject.
