DescriptionThe active component of Decomit®50 mcg inhalation aerosol andDecomit® 100 mcg inhalation aerosol is beclometasone dipropionateBP, an anti-inflammatory corticosteroid. It is an environment friendlyinhaler. Decomit® HFA is a pressurized, metered-dose aerosolintended for oral inhalation only. This product does not containchlorofluorocarbons (CFCs) as propellant. It uses hydrofluoroalkane(HFA) as propellant which is environment friendly and replaced CFCsworldwide. Each actuation of Decomit® 50 HFA inhalation aerosoldelivers 50 mcg of beclometasone dipropionate and each actuation ofDecomit®100 HFA inhalation aerosol delivers 100 mcg ofbeclometasone dipropionate. Each canister provides 200 actuations. Indications and UsageDecomit® HFA is indicated in the maintenance treatment of asthma asprophylactic therapy in patients 5 years of age and older.Beclometasone dipropionate is also indicated for asthma patients whorequire systemic corticosteroid administration, where addingBeclometasone dipropionate may reduce or eliminate the need for thesystemic corticosteroids.Beclometasone dipropionate is not indicated for the relief of acutebronchospasm.Dosage

The recommended dosage of Decomit® HFA relative to CFC-basedBeclometasone dipropionate (CFC-BDP) inhalation aerosols is lowerdue to differences in delivery characteristics between the products.Recognizing that a definitive comparative therapeutic ratio betweenHFA-Beclometasone dipropionate and CFC-BDP has not beendemonstrated, any patient who is switched from CFC-BDP to HFA-BDP should be dosed appropriately, taking into account the dosingrecommendations above, and should be monitored to ensure that thedose of HFA-BDP selected is safe and efficacious. As with any inhaledcorticosteroid, physicians are advised to titrate the dose of Decomit®HFA upward or downward over time to the highest or lowest level thatmaintains proper asthma control. This is particularly important inchildren. Patients should be instructed on the proper use of theirinhaler. Patients should be advised that Decomit® HFA may have adifferent taste and inhalation sensation than that of a CFC basedDecomit®.
ContraindicationsHypersensitivity to beclometasone inhaler is a contraindication; andspecial care is necessary in patients with active or quiescentpulmonary tuberculosis.PrecautionsPatients should be instructed on the proper use of the inhalers toensure that the drug reaches the target areas within the lungs. Theyshould also be made aware that Decomit® HFA inhaler has to be usedregularly for optimum benefit. Patients should be made aware of theprophylactic nature of therapy with Decomit® inhaler and that it shouldbe taken regularly, even when they are asymptomatic.The maximum daily administration of Decomit® inhaler should notexceed 1mg (1000 mcg). Significant reduction in plasma cortisol levelshas been reported in patients who receive twice this amount.Side EffectsBeclometasone dipropionate HFA inhalation aerosol has been foundgenerally well tolerated in the extensive clinical development program.The adverse effects that occurred in a very few patients are headache,pharyngitis, URTI, rhinitis, sinusitis, pain, back pain, nausea,dysphonia etc. No patients in the clinical development programdeveloped symptomatic oropharyngeal candidiasis with HFAbeclometasone. If it occurs, it can be treated with topical antifungaltherapy whilst still continuing with the Decomit® HFA inhaler.In some patients, inhaled beclometasone dipropionate may causehoarseness or throat irritation. It may be helpful to rinse mouth withwater immediately after inhalation.As with other inhalation therapy, the potential for paradoxicalbronchospasm should be kept in mind. If it occurs, the preparationshould be discontinued immediately and alternative therapy instituted.Pharmaceutical PrecautionsStore below 30°C.
